Abstract

Hydronephrosis occurs due to a disorder in the urinary tract that causes the flow of urine from the urinary tract to be obstructed, causing the dilation of the channels in the kidneys. The main symptoms that hydronephrosis patients commonly feel are pain in the lower back, nausea, and fatigue. Deep breath relaxation therapy, as a non-pharmacological therapy, has the effect of reducing pain. This study aims to determine the effect of deep breath relaxation techniques in patients with hydronephrosis and pain complaints. The method used in this study was a case study of one patient who was given an intervention for three days. The pain intensity scale was measured using the Numeric Rating Scale (NRS). This study showed a significant reduction in pain intensity in hydronephrosis patients from a scale of 6 to a scale of 3, where the deep breath relaxation technique intervention was given for three consecutive days for three sets per day in patients. Based on the results of the study, the researcher suggests the use of deep breath relaxation techniques as a non-pharmacological therapy in hydronephrosis patients with pain complaints in clinical practice, which can be provided by health workers, especially nurses or as patient-independent therapy because deep breath relaxation techniques are easy to do therapy and do not require additional tools and materials to do so.

Abstract

Anxiety in patients undergoing Transurethral Resection of the Prostate (TURP) surgery can have a significant impact on the physiological and psychological aspects of the patient. Anxiety in preoperative patients can result in surgery being postponed or canceled. One effort that can be made to reduce anxiety levels in preoperative TURP patients is to provide SEFT therapy. During tapping, there is an increase in the process of neurotransmitter signal travel, which reduces the regulation of the hypothalamic-pituitary-adrenal Axis (HPA axis), thereby reducing the production of stress hormones. This research method uses a case study, and pre-and post-implementation measurements are carried out. The sample used was one person. The research sample was given treatment, namely SEFT therapy, to reduce anxiety levels. The results of SEFT therapy were observed using the State-Trait Anxiety Inventory (STAI) measuring instrument. There was a decrease in anxiety levels before and after treatment; namely, the STAI score decreased to 31 (moderate anxiety). SEFT therapy can reduce anxiety because the tapping mechanism in SEFT therapy has the function of releasing endorphin hormones. Endorphin hormones help reduce psychological symptoms such as anxiety.

Abstract

Objective: Patients with grade III open fractures treated with OREF may experience postoperative pain, which can hinder mobilization and prolong their hospital stay. Non-pharmacological interventions such as cryotherapy are needed to help control pain safely. Clinical nursing practice does not have enough case studies on the effectiveness of cryotherapy in reducing postoperative pain. Objective: To describe the application of cryotherapyfor pain reduction in a patient with a grade III open femoral fracture following OREF in a case study. Case Report: This case study involved one male patient with postoperative pain following femoral fracture surgery who received cryotherapy in accordance with the Standard Operating Procedure (SOP) for cold therapy at Seruni Ward, dr. Soebandi Regional Hospital, Jember. For three days, the therapy was given once daily for 15 minutes. The researcher assessed the intensity of pain using the Numeric Rating Scale (NRS) before and after each intervention session. Result: Pain scores decreased gradually from 6 to 4 on the first day, 5 to 3 on the second day, and 4 to 2 on the third day. In addition, facial grimacing and protective behavior showed gradual improvement over the three-day intervention period. Conclusion: Cryotherapy was effective in reducing postoperative pain and can be utilized as a safe and practical non-pharmacological nursing intervention.INTISARILatar belakang: Nyeri pasca operasi pada pasien fraktur femur OREF, terutama fraktur terbuka grade III, dapat menghambat mobilisasi dan memperpanjang rawat inap. Intervensi nonfarmakologis seperti cryotherapy diperlukan untuk membantu mengontrol nyeri secara aman. Studi kasus mengenai efektivitas pemberian cryotherapy terhadap penurunan nyeri pasca operasi masih terbatas dalam praktik keperawatan klinis Tujuan: Mendeskripsikan penerapan cryotherapy terhadap penurunan nyeri pada pasien fraktur femur sinistra terbuka grade III pasca OREF dalam studi kasus. Laporan kasus: Studi kasus ini melibatkan satu pasien laki-laki dengan nyeri pasca operasi fraktur femur yang diberikan terapi cryotherapy sesuai dengan Standar Prosedur Operasional (SPO) tindakan terapi dingin di Ruang Seruni RSD dr. Soebandi Jember, yaitu selama 15 menit, satu kali sehari, selama tiga hari berturut-turut. Penilaian tingkat nyeri dilakukan oleh peneliti menggunakan Numeric Rating Scale (NRS) melalui pengukuran sebelum (pretest) dan sesudah (posttest) pemberian intervensi pada setiap sesi terapi. Hasil: Skala nyeri menurun bertahap dari 6 menjadi 4 pada hari pertama, 5 menjadi 3 pada hari kedua, dan 4 menjadi 2 pada hari ketiga. Selain itu, ekspresi meringis dan sikap protektif menunjukkan perbaikan bertahap selama tiga hari intervensi. Simpulan: Cryotherapy efektif menurunkan nyeri pasca operasi dan dapat digunakan sebagai intervensi keperawatan nonfarmakologis yang aman dan praktis.

Abstract

Background: Type 2 diabetes mellitus (T2DM) is a chronic metabolic disorder that results from a decrease in insulin sensitivity, which results in persistent high blood sugar levels. Benson relaxation therapy is a complementary treatment that can be used in addition to medication. The use of this in nursing care is not well documented in case study. Ojective: This research aims to examine how Benson relaxation therapy affects the reduction of blood sugar levels in individuals diagnosed with type 2 diabetes. Case report: A 51-year-old female with a type 2 diabetes diagnosis was hospitalized at the RSD Dr. Soebandi, primarily reporting feelings of weakness and nausea. The lab report showed that a blood glucose measurement of 389 mg/dl, an HbA1c level of 11, 6%, and glucosuria (+3) at 300 mg/dl were all within the range. Benson relaxation therapy was administered daily for 10 to 15 minutes to manage her hyperglycemia over the course of three days. The effectiveness of this method was assessed by analyzing the patient's blood glucose levels before and after treatment sessions throughout the treatment period. Results: During the time of the intervention, the glucose levels in the blood dropped from 389 mg/dl to 380 mg/dl on day one, from 215 mg/dl to 205 mg/dl on day two, and from 185 mg/dl to 180 mg/dl on day three. Conclusion: Benson relaxation therapy is a nursing technique that is proven to be safe and effective in managing blood sugar levels.INTISARILatar belakang: Diabetes melitus tipe 2 (T2DM) merupakan sebuah kondisi metabolik yang bersifat kronis, di mana kadar gula darah tetap meningkat disebabkan oleh berkurangnya respons tubuh terhadap insulin. Selain cara pengobatan, terapi relaksasi Benson bisa dijadikan sebagai metode tambahan. Namun, masih sangat jarang temuan kasus yang menjelaskan penggunaan terapi ini dalam konteks studi kasus keperawatan. Tujuan: Tujuan dari studi kasus ini adalah untuk meneliti dampak terapi relaksasi Benson terhadap penurunan kadar gula darah pada pasien diabetes tipe 2. Laporan kasus: Seorang wanita berusia 51 tahun yang didiagnosis menderita diabetes mellitus tipe 2 dirawat di RSD Dr. Soebandi dengan keluhan utama kelemahan dan mual. Hasil tes laboratorium menunjukkan kadar glukosa darah acak sebesar 389 mg/dl, kadar HbA1c sebesar 11,6%, dan glukosuria (+3) pada tingkat 300 mg/dl. Selama tiga hari, dia mendapatkan penanganan hiperglikemia dengan terapi relaksasi Benson, yang dilakukan sekali sehari selama 10–15 menit. Keefektifan intervensi dinilai dengan melakukan perbandingan terhadap perubahan kadar glukosa darah pasien sebelum dan sesudah terapi selama periode perawatan. Hasil: Selama periode intervensi, kadar glukosa darah turun dari 389 mg/dl menjadi 380 mg/dl pada hari pertama, dari 215 mg/dl menjadi 205 mg/dl pada hari kedua, dan dari 185 mg/dl menjadi 180 mg/dl pada hari ketiga. Simpulan: Terapi relaksasi Benson merupakan intervensi keperawatan yang efektif dan berbasis bukti, yang dengan aman membantu pengendalian kadar glukosa darah.
