Abstract

Diabetes Mellitus (DM) is a chronic disease that needs a sustainable self-care. Low-level knowledgeof self-care can worsen health condition and cause stress. DSME/S is an ongoing process tofacilitate knowledge, skill, and abilities required to perform self-care. This research aimed to analyzethe effect of DSME/S on stress in patients with type 2 DM. The design was quasi-experimental withrandomized control group pretest-posttest design. The sample size was 30 respondents which wererandomly divided into 15 respondents as the intervention group and 15 respondents as the controlgroup. DSME/S was conducted once per week for 6 weeks with 150 minutes for each encounter.The measurement of stress was measured by using Diabetes Distress Scale (DDS). The data wereanalyzed by using dependent t test and independent t test with significance level of 0.05. The resultof dependent t test revealed a significant difference of stress between pretest and posttest both inthe intervention group (p = 0.000) and in the control group (p = 0.000). Furthermore, independent ttest showed a significant difference between intervention group and control group (p = 0.001) sothat there was a significant effect of DSME/S on stress in patients with type 2 Diabetes Mellitus.Keywords : stress, DSME/S, diabetes mellitus

Abstract

Diabetic wound very easily causes complications in the form of infection caused by bacterialinvasion, and hyperglycemia condition boosts the growth of bacteria. One of the bacteria thatcan cause infection in diabetic wound is Staphylococcus aureus. Diabetic wound care can bedone with honey. Honey contains antibacterial, antioxidant and hydrogen peroxide that can killharmful germs. The purpose of this research was to identify the effect of wound care with honeyon Staphylococcus aureus colonization in diabetic wound. The research design used was a preexperimentwith one group pretest and posttest design. Sample was taken using consecutivesampling with 7 respondents. Data analysis applied dependent t-test and obtained p value of0.000 (p value <α = 0.05); it can be concluded that there is an effect of wound care with honeyon Staphylococcus aureus bacterial colonization in diabetic wounds of patients with DiabetesMellitus in work area of Public Health Center of Rambipuji Jember. The suggestion from thisstudy is to apply honey as a wound care agent because it has antibacterial activity that can helpprevent infection and accelerate wound healing.Keywords: wound care, diabetic wound, honey, Staphylococcus aureus

Abstract

Oxygenation is human major basic need for maintaining body cell metabolism, survival and activities of various body organs. Respiratory system and cardiovascular system change throughout the aging process. Decreased respiratory function will affect the fulfillment of the needs of oxygenation in the elderly. Diaphragmatic breathing exercise is one of methods to maintain and improve respiratory functions in the elderly. This research aimed to determine the effect of diaphragmatic breathing exercise on respiratory functions (RR and PEFR) in the elderly. This research is a pre-experimental research with one group pretest and posttest design. Sampling technique used purposive sampling with 14 elderly given intervention of diaphragmatic breathing exercise once a day for 14 days. Abstract

Age is a factor that can influence sleep quality. Sleep quality decreases with aging and the complaintsof sleep become common among elderly. This study aimed to analyze the effect of laughter therapy onsleep quality of elderly in Long-Term Care Jember. The design of study was randomized control grouppretest-posttest design. The sample was 30 respondents, 15 respondents is intervention group and 15respondents is control group. Sleep quality was measured by using Pittsburgh Sleep Quality Index(PSQI). The data were analyzed by t test, Wilcoxon test, and Mann-Whitney test with significance levelof 0.05. The result revealed a significant difference of sleep quality before and after laughter therapy inthe treatment group (p: 0.001), and there was no significant difference between pretest and posttest inthe control group (p: 0.082). Mann-Whitney test showed a significant difference of sleep qualitybetween the treatment group and the control group after laughter therapy (p: 0.000). The percentageof elderly sleep quality before laughter therapy in the treatment group that is bad 100%, while afterlaughter therapy that is either equal to 100% which means there is an increase in the quality of sleepof 100%. Percentage of quality sleep before therapy in the control group of poor by 100%, while after abad sleep therapy quality percentage is 100%, which means no increase in the quality of sleep in thecontrol group It can be concluded that there was a significant effect of laughter therapy on sleepquality of elderly in Long-Term Care Jember. Laughter therapy can be applied to improve sleep qualityin elderly people.Keywords: sleep quality, elderly, laughter therapy

Abstract

Pathological change experienced by patients with type 2 diabetes mellitus is impaired peripheral blood circulation, especially in the lower limbs and feet. Peripheral blood circulation can be examined by measuring Ankle Brachial Index (ABI). Therapeutic exercise walking is one of physical activities which can improve blood circulation. This research goal was to analyze the effect of therapeutic exercise walking on peripheral blood circulation in patients with type 2 diabetes mellitus. The research design was randomized control group pretest-posttest design. The sample size was 15 respondents as control group and 15 respondents as treatment group. Data was analyzed by using dependent t-test and independent t-test with significant level of 0.05. This result showed a significant increase of ABI after therapeutic exercise walking in the treatment group (p= 0.001). However there was a significant decrease of ABI between pretest and posttest in control group (p=0.010).Independent t-test indicated a significant difference of ABI between treatment group and control group (p=0.000). So, it can be concluded that there is an influence of therapeutic exercise walking on the peripheral blood circulation in patients with type 2 diabetes mellitus. Nurses should apply therapeutic exercise walking to improve peripheral blood circulation in patients with type 2 diabetes mellitus. Abstract

Diabetes distress is an emotional problem that is directly related to the burden and worriesresulted from living with chronic disease. Stress has an impact on self-care behaviour whichaffects metabolic control. This study aimed to analyze the correlation between diabetesdistress and self-care behaviour in people with type 2 diabetes mellitus.This research appliedan observational analytic design with cross sectional approach. A total of 66 respondents wereenrolled in this study by using quota sampling technique. Data collection was conducted byadministering questionnaires of Diabetes Distress Scale (DDS) and Summary of Diabetes SelfCare Activities (SDSCA). Data were analyzed by using Spearman test with significance level of0.05. The result showed that the mean value of diabetes distress and self-care behaviour was2.16 and 3.97 respectively. The p value was 0.000 (p <0.05) with the correlation (r) of -0.629.There was significant correlation between diabetes distress and self-care behaviour. Thecorrelation was strong and negative which means the higher the level of diabetes distress thelower the performance of self-care behaviour. This study suggests the importance of assessingexperienced distress to optimize self-care behaviour in type 2 diabetes patients.Keywords: type 2 diabetes mellitus, diabetes distress, self-care behaviour

Abstract

Chronic obstructive pulmonary disease (COPD) is a lung disease characterized by obstruction to airflow that is persistent and progressive. Shortness of breath and low PEFR is major problem in COPD. Shortness of breath during sleep can increase reticular activating system and release norepinephrine which cause waking state and sleep disorder. The purpose of this study was to analyze the correlation between PEFR and sleep quality in COPD patients.The study design was correlation analytic with cross-sectional approach involving 53 patients as a sample. Data were collected by using peak flow meter to measure PEFR and PSQI questionnaire to measure quality of sleep. Data were analyzed by using Spearman rank test with significance level of 0.05. The result showed that the average value of PEFR was 41.09% and more than half of respondents (71.7%) had PEFR value in red zone category. Mean value of sleep quality was 18.62 and all of respondents had sleep quality in poor category. The p value was 0.000 (p <α) with r value = -0,876. It indicates that there was significant correlation between PEFR and sleep quality in COPD patients. Nurses should provide appropriate intervention to improve PEFR so that COPD patients can have better sleep quality.   Keywords: PEFR, sleep quality, COPD

Abstract

Diabetes Mellitus (DM) is a chronic disease that requires a continous self-care. A low level ofknowledge about self-care can worsen health condition and cause diabetes distress. The continuingstress can cause hyperglycemia that lead to complications of DM. DSME/S is a structured healtheducation that facilitates DM patients in implementing and maintaining their behavior for sustainableself-care. This research aimed to analyze the effect of DSME/S on diabetes distress in patients withtype 2 DM. This research was quasi experimental with non randomized control group pretestposttest design. Thirty respondents in this study were divided into 15 respondents as theintervention group and 15 respondents as the control group by consequtive sampling technique.DSME/S was conducted in 6 sessions: 4 sessions in the hospital and 2 sessions at respondent'shouse. The data were analyzed by using dependent t test and independent t test with significancelevel of 0.05. The result of dependent t test revealed a significant difference of diabetes distressbetween pretest and posttest both in the intervention group (p=0.001) and in the control group(p=0.046). Furthermore, independent t test showed a significant difference between interventiongroup and control group (p=0.001). The decrease of diabetes distress was higher in the interventiongroup than control group. This result indicates that there was a significant effect of DSME/S ondiabetes distress in patients with type 2 DM. Nurses are expected to apply DSME/S as a dischargeplanning to reduce diabetes distress in patients with type 2 DM.Keywords: diabetes distress, DSME/S, Diabetes Mellitus

Abstract

Airway obstruction is an important physiological disease in asthma clients that will inhibits the flow of air during inspiration and expiration, so that lung ventilation is not optimal. This situation makes clients lose base ability to reach normal airflow during breathing, especially when expiratory, which leads to reduction in peak expiratory flow. Non-pharmacological therapies can be used as a supplementary or complementary pharmacological therapy, one of which is progressive muscle relaxation. The objective of this research was to analyze the effect of progressive muscle relaxation on peak expiratory flow of clients with bronchial asthma. Respondents in this research were 11 people. Data analysis used dependent t-test with CI 95% (α=0,05). The results of data analysis showed the results of p value = 0,000 (p<0,05). The conclusion of this research is that there is a significant effect of progressive muscle relaxation on peak expiratory flow of clients with bronchial asthma. This research is expected to be applied and administered in health care, especially in the rehabilitation of clients with asthma to improve respiratory function optimally.   Keywords: progressive muscle relaxation, asthma, peak expiratory flow
