Abstract

Pasien hemodialisis harus menjalankan berbagai pengobatan untuk mengurangi gejala yang dirasakannya. Dalam menjalankan pengobatan tersebut, pasien hemodialisis perlu menjalankan self-management. Berbagai penelitian menunjukan bahwa banyak pasien hemodialisis yang tidak patuh dalam menjalankan self-management. Literasi kesehatan merupakan salah satu faktor penting yang menentukan kepatuhan pasien dalam pengobatan. Penelitian ini merupakan penelitian deskriptif untuk mengidentifikasi literasi kesehatan pasien hemodialisis. Tekhnik sampel yang digunakan adalah Concecutive sampling dengan jumlah 129 orang. Kriteria inklusi Pasien hemodialisis yang memiliki kesadaran penuh disertai tanda-tanda vital stabil. Kuesioner yang digunakan dalam penelitian ini dikembangkan dari HLS-EU-Q47 pada pasien dialisis yang dikembangkan oleh Martin et.al. (2012). Hasil penelitian ini menunjukan bahwa sebagian besar sebagian besar responden memiliki literasi kesehatan yang cukup yaitu 96 orang (74,4%) dan 33 orang memiliki literasi yang baik (25,6%).  Item yang dirasakan cukup mudah oleh responden adalah mencari informasi kesehatan di semua ruang lingkup, memahami informasi dalam pencegahan penyakit, menilai informasi kesehatan di semua lingkup, serta menggunakan informasi dalam promosi kesehatan. Kesimpulan hasil penelitian ini adalah sebagian besar pasien hemdoialisis memiliki literasi kesehatan yang cukup, tetapi masih ada beberapa item yang dirasakan masih sulit untuk dilakukan pasien. Perawat diharapkan dapat membantu untuk meningkatkan literasi pasien dengan memberikan edukasi, motivasi dan evaluasi secara menyeluruh dan berkesinambungan.

Abstract

Health beliefs among family members of diabetic patients are important factor of the diabetic preventing behaviours. This study aimed to describe family’s beliefs toward diabetes mellitus prevention based on the Health Belief Model and its’ contributing factors. This corelational study involved 72 peoples that purposively sampled from 34 diabetic patients who live in Jatinangor Public Health Center Working area. A questionnaire developed by the research team consisted of 37 items and 6 domains: perceived susceptibility, severity, benefits, barriers, cues to action, and self efficacy was used to collect the data. The collected data were analyzed descriptively and inferentially. The results showed that most of respondents (77.78%) positively believed on diabetes mellitus (DM) susceptibility, severity (59.72%), benefits of DM prevention (84.77%), and cues to action (76.4%). It was also found that most of respondents negatively believed on the prevention barriers (84%) and self-efficacy (77.78%). Female and actively involved in patient care were found as the contributing factors of the respondents’ beliefs related to DM prevention (p<0.05). To conclude, the DM patients’ family members believed that they are vulnerable to have DM and DM preventions are beneficial. Female and being actively involved in patient care potentially increase the family members’ beliefs toward DM prevention. So, it is important for the healthcare professionals to maximize the family members’ involvement in DM patient care program.

Abstract

Pasien gagal ginjal kronik harus menjalankan manjemen diri diantaranya hemodialisis, pengobatan, pembatasan cairan dan diet. Angka morbiditas dan mortalitas pada pasien hemodialisis akan meningkat apabila tidak menjalankan manajemen diri dengan baik. Penelitian ini dilakukan untuk mengidentifikasi kepatuhan pasien hemodialisis dalam menjalankan manajemen diri. Penelitian ini merupakan penelitian deskriptif yang dilakukan kepada pasien di Unit Hemodialisis di salah satu rumah sakit terbesar di Jawa Barat. Teknik pengambilan sampel yang digunakan adalah consecutive sampling dengan jumlah responden 129 orang. Teknik pengumpulan data dilakukan dengan kuesioner kepatuhan menjalankan manajemen diri pada pasien hemodialisis diadaptasi dari kuesioner End Stage Renal Disease Adherence. Data dianalisis dengan menggunakan distribusi frekuensi berupa frekuensi, persentase, dan mean. Sebagian besar responden tidak patuh dalam menjalankan manajemen diri 92 orang dan patuh sebanyak 28,7% yaitu 37 orang. Kepatuhan pasien dalam menjalankan hemodialisis sesuai jadwal sudah baik dengan rata-rata skor 271,3. Kepatuhan pasien hemodialisis masih kurang dalam membatasi asupan cairan dengan rata-rata skor 120, makanan dengan rata-rata skor 147, dan pengobatan dengan rata-rata skor 133).  Tenaga kesehatan diharapkan dapat memberikan dukungan kepada pasien dengan memberikan edukasi, konseling dan promosi kesehatan dengan menggunakan berbagai media termasuk media sosial terkait pentingnya pengontrolan cairan dan makanan. Kata kunci: Hemodialisis, Kepatuhan manajemen diri Abstract Compliance with running self-management on hemodializing patients Patients with chronic kidney failure must carry out self-management including hemodialysis, treatment, fluid and dietary restrictions. The morbidity and mortality rates in hemodialysis patients will increase if they do not carry out self-management properly. This study was conducted to identify the compliance of hemodialysis patients in carrying out self-management. This research was a descriptive study conducted on patients at the Hemodialysis Unit in one of the largest hospitals in West Java. The sampling technique used was consecutive sampling with the number of respondents 129 people. Data collection techniques carried out by compliance questionnaire method of running self management in hemodialysis patients adapted from the End Stage Renal Disease Adherence questionnaire. Data were analyzed using frequency distributions in the form of frequency, percentage, and mean. Most of the respondents were not obedient in carrying out self-management as many as 71.3%, 92 people and obedient as many as 28.7%, 37 people. Patient compliance in conducting hemodialysis schedule has been good with mean 271.3. Compliance with hemodialysis patients was still lacking in limiting fluid intake with mean 120, food with mean 147, and treatment with mean 133. Health workers are expected to be able to provide support to patients by providing education, counseling and health promotion by using various media including social media related to the importance of controlling fluids and food that must be carried out by hemodialysis patients. Keywords: Adherance, Hemodialysis, Self-Management

Abstract

The management of Diabetic Foot Ulcer (DFU) infections has undergone many developments. Some of therapies used, namely infrared, cadexomer iodine, and zinc cream as single therapies that have been proven effective. However, the combinations of these therapy is not yet known whether it’s more effective or not than single or double therapy. It’s important to know the outcome of combination of these therapys on the wound healing process. This case report was applied to DM Type 2 patient with DFU score 47 from Bates-Jansen Wound Assessment Tool (BJWAT) and hyperglycemic condition (355 mg/dL). Wound treatment was carried out using the TIME Concept and applying 3M principles which were repeated in 4 home visits for 11 days. The result showed wound improvement as indicated by decrease in BJWAT score from 47 to 38. The improvement in this case was supported by the strength of each therapy which is combined in the wound healing process compared to single or double therapy. The client's wound care has the potential to address bacterial colonization and prevent biofilm formation, maintain wound moisture, and autolysis of necrotic tissue in a faster time. However, there are several complicating factors of client’s wound healing process, namely hyperglycemia, non-adherence of insulin injection, infection, hypertension, and hyperurisemia. The combination of these three therapies was shown to wound healing in terms of wound depth, type and amount of necrotic tissue, peripheral edema, granulation, and epithelialization and accelerate wound repair compared to single or double combinations.

Abstract

Gagal ginjal terminal yang dialami pasien hemodialisis dapat menimbulkan berbagai perubahan dalam kehidupannya. Persepsi yang positif terhadap penyakit dapat membantu pasien hemodialisis dalam menerima keadaannya dan meningkatkan motivasi untuk menjalankan berbagai tindakan pengobatan. Apabila pasien hemodialisis memiliki persepsi yang negatif terhadap penyakit, maka akan cenderung mudah mengalami berbagai masalah psikologis. Penelitian ini dilakukan untuk mengetahui persepsi terhadap penyakit pada pasien hemodialisis di Bandung. Penelitian ini merupakan penelitian deskriptif kuantitatif yang dilakukan kepada pasien hemodialisis di salah satu Rumah Sakit di Bandung. Teknik sample yang digunakan adalah consecutive sampling sebanyak 126 orang. Kriteria inklusi dalam penelitian ini adalah pasien gagal ginjal yang menjalani hemodialisis dan memiliki tanda-tanda vital yang stabil. Instrumen yang digunakan untuk mengukur persepsi terhadap penyakit adalah kuesioner persepsi penyakit singkat (Brief-IPQ) yang dikembangkan oleh Broadbant, et.al. tahun 2005, dan sudah dilakukan back translate ke dalam bahasa Indonesia. Hasil penelitian menunjukkan bahwa sebagian besar pasien hemodialisis memiliki persepsi terhadap penyakit yang negatif (50,4%). Sebagian besar pasien merasakan berbagai dampak penyakit terhadap kehidupannya dan mengalami perubahan secara emosional semenjak mengalami gagal ginjal terminal. Persepsi terhadap penyakit yang negatif pada pasien hemodialisis dapat mempengaruhi kualitas hidup, angka kesakitan dan capaian pengobatan yang dijalaninya. Oleh karena itu, dibutuhkan dukungan keluarga dan sosial. Petugas kesehatan khususnya perawat diharapkan dapat memberikan edukasi dan konseling pada pasien hemodialisis untuk meningkatkan persepsi pasien terhadap penyakit.

Abstract

This research aims to examine further how families are involved in family-based self-management education for hypertension sufferers. The method used is a scoping review with PCC (Population, Context & Concept) components. Population is "hypertension", Context is "self-management" Concept is "family-based oriented". This scoping review is aimed at answering the question "what and what forms of family-based self-management education for hypertension sufferers, compliance with treatment plans and providing care support for patients." The results of this research obtained 369 articles through searches using several databases consisting of PubMed, Science Direct and CINAHL, resulting in 5 articles analyzed in the research. The conclusion of this research is that mapping self-management interventions for hypertensive patients involving families plays an important role in improving self-management in hypertensive sufferers. Abstract

This study aims to map the influence of mobile applications as self-management support in management of diabetic feet. This research uses a scoping review method by entering keywords into the CINAHL, Pubmed, and Scopus databases. The research results show that mobile applications are effectived in increasing knowledge, self-care behavior, self-efficacy, compliance with self-care foot care, eating behavior, reducing wound/ulcer recurrence rates, and cost-effectiveness. The effectiveness of mobile applications can be used as a strategy in providing interventions to support self-management in the management of diabetic feet. The use of mobile applications has an impact on improving independent foot care management as well as increasing knowledge of diabetic foot care so that it has benefits in controlling glycemia, increasing adherence to diet, and increasing physical exercise. In conclusion, Mobile application-based based programs had the potential to improve self-management in diabetic foot patients. Keywords: Diabetic Foot, Mobile Applications, Self-Management

Abstract

Decision making for taking screening is an important component in preventing cervical cancer, but still many women who have not yet done it due to a lack of knowledge and decision-making ability.  Development of intervention tailored to the stages of decision-making based on the Precaution  Adoption  Process  Model,  such as training,  is able to improve decision-making ability to do the screening. This study aimed to determine the differences in the effect of training on decision-making for cervical cancer screening based on PAPM in the UPT Griya Antapani Health Center Bandung. This quasi-experimental study with two group pre-test post-test design involved  84  women aged  30-49  years who were chosen by purposive and proportional cluster sampling from two villages divided into  42  respondents each. Group  A received training with PowerPoint media,  leaflets,  videos, and WhatsApp while group  B received training with PowerPoint media, leaflets, and videos. The data of decision-making was collected using the  Precaution Adoption Process Model checklist:  before,  after training, and  15  days after training then analyzed using  Friedman test and  Mann  Whitney test.  The results showed that there were differences in decision-making in each group before and after training with p <0.001 but Mann Whitney test showed that there was no difference in decision-making between groups after training (p = 1,000 and p = 0.316). This depicted that any method and media can be used to change decision-making. Providing training and counseling is needed according to the stages of decision-making taking into account the factors that influence it.  Keywords: Cervical Cancer, Decision-making, Screening, the Precaution Adoption Process Model

Abstract

Diabetes mellitus merupakan masalah kesehatan serius, tidak dapat diobati, banyak menimbulkan komplikasi, dan memiliki banyak faktor penyebab. Karenanya, pencegahan merupakan strategi terbaik untuk mengatasi permasalahan terkait diabetes. Kader kesehatan merupakan garda terdepan dalam upaya pencegahan penyakit diabetes. Sayangnya kebanyakan kader belum cakap dalam mengenali faktor risiko maupun mengidentifikasi tingkat risiko diabetes. Sehingga upaya peningkatan pengetahuan dan kemampuan kader dalam kedua hal tersebut menjadi sangat penting. Kegiatan pengabdian ini ditujukan untuk meningkatkan pengetahuan dan kemampuan kader kesehatan dalam mengenali dan mengidentifikasi tingkat risiko diabetes. Untuk itu, kami mengadakan pendidikan dan pelatihan kader kesehatan terkait diabetes, faktor risiko dan upaya pencegahannya, serta simulasi skrining tingkat risiko diabetes menggunakan FINDRISC diabetes assessment tool. Tim juga melakukan mediasi baik dengan pihak Puskesmas maupun kleurahan di wilayah setempat. Kegiatan dijalankan di salah satu keluraran di Wilayah Kabupaten Bandung, Jawa Barat dengan melibatkan 88 kader kesehatan. Kader berpartisipasi aktif selama kegiatan, mengajukan pertanyaan, berbagi pengalaman, dan berdiskusi terkait materi yang disampaikan. Berdasarkan hasil evaluasi, ditemukan adanya peningkatan skor pengetahuan sebesar 15% setelah pemberian materi (75% vs. 60%). Dari simulasi skrining tingkat risiko diabetes, peserta menyatakan bahwa pengisian formulir tidak terlalu sulit kecuali pada bagian pengisian indeks masa tubuh. Dari simulasi ini ditemukan juga bahwa setengah (50,72%) kader mengalami peningkatan risiko (peluang terkena diabetes sebesar 4%). Kegiatan pengabdian ini efektif meningkatkan pemahaman dan kemampuan kader dalam mengenal dan menilai tingkat risiko diabetes. Kegiatan serupa perlu secara berkala dijalankan untuk upgrading maupun tindak lanjut temuan terkait tingkat risiko diabetes pada kader.

Abstract

Background: Patients with heart failure (HF) often experience cognitive impairment, which negatively affects their quality of life. An effective screening tool is essential for nurses and healthcare professionals to assess cognitive function as part of HF management. Although many instruments exist, none are specifically designed for patients with HF. Objective: This study aimed to map the instruments for screening cognitive function in patients with HF. Design: A scoping review. Data Sources: Articles published between 2019 and 2023 were searched in PubMed, ScienceDirect, and Google Scholar, with the last search conducted on 27 January 2024. Review Methods: The review followed the scoping review framework by Arksey and O’Malley and adhered to PRISMA guidelines for scoping reviews. Results: Of the 21 articles meeting inclusion criteria, six cognitive function screening instruments were used across various cognitive domains, effectively identifying cognitive impairment in both inpatient and outpatient HF settings. The Montreal Cognitive Assessment (MoCA) was the most frequently used tool, covering a broad range of cognitive domains. MoCA showed high efficacy with a kappa coefficient of 0.82, Cronbach’s alpha reliability of 0.75, sensitivity of 90%, and specificity of 87%. Conclusion: Instruments like MoCA, Mini-Cog, and TICS-m show promise for assessing cognitive function in patients with HF, each with specific strengths and limitations. MoCA is notable for its comprehensive coverage despite being time-consuming and having language barriers. Further research is needed to revalidate and improve the existing instruments. It is crucial for nurses and healthcare professionals to integrate these tools into regular patient management, highlighting the need for continued research in their application.
