Abstract

Background: Indonesia applies decentralization policy in health sector. However, there are Health Minimum Service Standards (HMSS) that must be met at provincial and district level. In district level, there are 12 indicators that must be achieved include of maternal and child health with the coverage must be 100%. Nevertheless, there are various problems in achieving HMSS both quantity and quality aspect. This study aimed to determine the problems that were happened and determine alternative solutions in the implementation of HMSS especially in maternal and child sector.Methods: This research was done in three steps. The first step was conducted by Focus Group Discussion (FGD) with participant from many stakeholders in Central Java and Yogyakarta. This step aims to identify problems in the implementation of HMSS and develop the alternative solution. Second stage was developing the conceptual framework.Results: Based on the research, there are many problems in quantitative and qualitative aspects related to healthcare services. Program planning and budgeting is the main key to achievement HMSS. In second and third steps, an integrated planning and budgeting framework between multisector was obtained including problem identification, problem cause analysis, multisector identification, setting solutions and programs, planning and budgeting, and monitoring and evaluation. So that, there were no overlapping programs, less optimal, and lack of supervision and guidance.Conclusions: Integrated planning and budgeting was alternative solution to solve the implementation problem in HMSS especially in maternal and child aspect. However, strong commitment and monitoring was needed between stakeholder.

Abstract

Background: The prevalence of active smokers in Indonesia remains high. Global Adults Tobacco Survey (GATS) on 2011 stated that Indonesia has the highest number of active smokers (67% male and 2.7% female). National Health Indicator Survey 2016 showed the number of male adolescent who smoke was 54.8%. The aim of this study was to examine the smoking levels of junior high school students in Semarang City in order to arrange improvement in behavioral changes through identifying the patterns of adolescent’s smoking behavior.Methods: This study used cross sectional study design. Samples were selected with simple random sampling technique, and 160 smokers who were junior high school students were obtained from 16 sub-districts in Semarang City. Data was analyzed using univariate and bivariate analysis.Results: The result showed that the majority of the respondents were light smokers (90.0%) who smoke <5 cigarettes/day. The chi square test showed that pocket money to buy cigarettes (p=0.011) and the social interaction pattern (p=0.026) have correlation to students’ smoking levels.Conclusion: Most students start to smoke at the age of less than 12 years old with light smoking degree. It is correlated with the pocket money to buy cigarettes and the social interaction pattern.

Abstract

Background: Personalized medicine is one of the health treatment approaches that has developed in the last ten years. This treatment is specific because it considers the genomics of patients and requires a complete database or medical record covering clinical and genomic data. Nevertheless, there are ethical and legal challenges in the implementation of electronic medical records in personalized medicine.Objective: This study aimed  to determine the ethical and legal challenges in the use of electronic medical records in personalized medicine based on previous literature.Methods: The method used in this study was literature review with sources from journals, books, and articles on electronic medical records, personalized medicine, as well as legal and ethical aspects related both domestically and abroad. Results: Ethical and legal challenges that can occurred in the use of electronic medical records in the personalized medicine era include security and ownership of data, legal responsibility, genomic discrimination, and changes in the relationship between doctors and patients.Conclusion: Indonesia does not yet have specific regulations regarding data security, data ownership, and data sharing in the health sector. Further regulations regarding the use of electronic medical records in the personalized era are needed so that their implementation does not conflict with the ethics and laws that apply in Indonesia

Abstract

Immunisation is one of the health programs which considered cost-effective and able to prevent millions of deaths and disability which caused by diseases which could be prevented by immunisation. For the past years, the coverage of Basic Complete Immunisation has been increasing in several areas in Central Java. However, there were still some community groups that believed immunisation was harmful for their children and they did not allow their children to be vaccinated. This condition could cause a new case of diseases which usually can be prevented by immunisation, such as Polio, Diphtheria, Tetanus, Hepatitis, and many more. Therefore, this program held to refresh the cadres knowledge about immunisation also provide more knowledge for pregnant mother about immunisation so they agree to let their children vaccinated after they born until they complete the basic immunisation. This community service heldin January – February 2020 in Sendangagung Village, Pamotan, Rembang District. The participants were health cadres and pregnant woman in Sendangagung Village, Pamotan, Rembang District. This event started with a mini pre-test, explanation about immunisation, and discussion. Participants stated that this event increase and refresh their knowledge about immunisation. Moreover, participants also get a booklet about the immunisation so the health cadres might use this booklet as their education media to educate other pregnant women and toddler’s parents about immunisation.

Abstract

Indonesia targeting that at least 70% of the population vaccinated and formed the herd immunity. However, there are several challenges in reaching that target, such as the anti-vaccine movements, disinformation or hoaxes, fear of the Adverse Event Following Immunization (AEFI), and fear of needles. Based on the preliminary study, the area of Bandarharjo Public Health Centre was in the lowest 3rd place related to the COVID-19 vaccination coverage in Semarang. This was due to lack of education to the community regarding the vaccination, the high resistance to vaccination due to concerns about AEFI, and so on. Health cadres are one of the main actors in the supporting the health programs in PHC, therefore the role of health cadres need to be optimized in education and management of AEFI related to COVID-19 vaccination. This community service program was carried out offline in 30 health cadres in the working area of Bandarharjo PHC. The activity began with a pre-test, continued with the presentation of the topic, discussion session, and ends with post-test. The results of the pre-test and post-test showed that there was an increase in the knowledge of cadres between before the presentation and after the presentation session. Health cadres also received booklets containing general information about COVID-18 vaccination and also related to AEFI. Booklets given to cadres not only to increase their knowledge but also can be used to educate the community. Hopefully, with the increase in knowledge and understanding of cadres, the community will be easily get the right information. The health cadres commitment to educate the community regarding COVID-19 vaccination also hoped to increase the community knowledge related to the vaccination and also the willingness of community to receive the COVID-19 vaccination, especially in the working area of Bandarharjo PHC, Semarang City, Central Java Province.

Abstract

Sistem rujukan online merupakan inovasi BPJS Kesehatan dalam mendigitalkan sistem rujukan berjenjang. Sistem rujukan online diharapkan dapat meningkatkan kepuasan dan kenyamanan peserta dalam mendapatkan layanan rujukan. Namun belum ada evaluasi dalam pengukuran kepuasan pada sistem rujukan online hingga saat ini, khususnya di Kota Semarang. Penelitian ini bertujuan untuk mengevaluasi pelaksanaan sistem rujukan online dengan menganalisis faktor-faktor yang berhubungan dengan kepuasan peserta BPJS Kesehatan di Kota Semarang. Penelitian ini merupakan penelitian kuantitatif dengan pendekatan cross-sectional yang melibatkan 100 responden yang dipilih dengan teknik accidental sampling. Hasil penelitian menunjukkan bahwa persentase responden yang merasa puas hanya 54%. Hal ini disebabkan oleh masih belum optimalnya efisiensi administrasi dan efektifitas sistem rujukan online dalam mengurangi antrian. Selain itu ada hubungan antara harapan kinerja, harapan upaya, kondisi fasilitasi, kualitas informasi, dan kepuasan pengguna terhadap penerapan sistem rujukan online di BPJS Kesehatan Kota Semarang dengan p-value <0,05. Hasil penelitian menyarankan kepada BPJS Kesehatan untuk menentukan indikator kepuasan peserta terhadap penerapan sistem rujukan online dan mengintegrasikan sistem rujukan online dengan sistem registrasi di fasilitas kesehatan menengah dan tersier di Kota Semarang. Ada hubungan antara harapan kinerja, harapan upaya, kondisi fasilitasi, kualitas informasi, dan kepuasan pengguna terhadap penerapan sistem rujukan online di BPJS Kesehatan Kota Semarang dengan p-value <0,05. Hasil penelitian menyarankan kepada BPJS Kesehatan untuk menentukan indikator kepuasan peserta terhadap penerapan sistem rujukan online dan mengintegrasikan sistem rujukan online dengan sistem registrasi di fasilitas kesehatan menengah dan tersier di Kota Semarang. Abstract

Public Health Center as a health facility provides access to health for individuals and groups. One of the innovations of the Semarang City Health Office in expanding community access to health services is the "5G" Public Health Center, one of which is the Simplicity Service with public health center service hours added so that the public health center is open until evening (07.00-17.00 WIB). The addition of Public Health Center service hours at the Pudakpayung Public Health Center was considered ineffective because patients who visited during the addition of service hours did not reach 10 people and there were still many obstacles in the management of human resources, financing, and the operational system of the Public Health Center. The purpose of this study was to evaluate the implementation of the program in addition to Public Health Center service hours at the Pudakpayung Public Health Center with an approach of 4 health functions and 3 health goals supported by 6 building blocks. This research method was qualitative with in-depth interviews. The respondents were Head of Public Health Center, Financial Manager and Service Manager of Pudakpayung Public Health Center, Head of Health Services in Semarang City Health Office and patients. The results of this study indicate that there were problems with inadequate resources, insufficient funds, the form of services provided is not the same, there was no specific SOP that regulates the addition of Public Health Center service hours, and monitoring evaluation has not been done by Public Health Center or Semarang City Public Health Office. The socialization has been carried out, but the number of morning patients is still much more than the number of patients in the afternoon. This is a challenge for the Public Health so that it can shift patients who visit in the morning to the afternoon.

Abstract

Indonesia has proceeded ASI eksklusif program to create well nutrition generation. The program is held in health services facility. Puskesmas Jati holds the program as GERSASI (Gerakan Ibu Sayang Bayi Wajib ASI). The research objective was to describe and analyze the implementation of the effectiveness of the Exclusive Breastfeeding (ASI) program called GERSASI in Puskemas Jati, Kudus Regency. The objective was to analyze achievement aspect, integration, and adaptation. Research This was descriptive qualitative research, and data collected was done by indepth interview. The research was held on July-August 2020. The informants of the research consist of key informant and triangulation informant. The GERSASI’s Program target were pregnant mothers and breastfeeding mothers in Jati kulon village. GERSASI program performs socialization in Jati Kulon village in March 2017. The consensus was done by Jati Kulon authority to Jati Kulon villagers. The infrastructure of GERSASI Program were monitoring books, stickers, and certificates. Accompaniment was done by health cadres who were breastfeeding motivators. The health cadres were chosen because they were considered to be closed to the community. The effectiveness of the GERSASI Program is still deemed ineffective. There were still several aspects that need to be improved so that the GERSASI Program can achieve its objectives.

Abstract

Medical Record is one indicator in determining the quality of health services in hospitals. However, filling medical records at Ungaran District Hospital has not yet reached the minimum service standard that has been set. This showed that the work productivity of doctors was still not optimal. In theory, supervision, work factors and contributions are extrinsic factors of motivation related to productivity. Thus, this study aimed to study the relationship between supervision, work requirements and assessment with the productivity of doctors in filling out medical record documents. This was quantitative research with cross-sectional study design. Questionnaires were given to 34 respondents with inclusion criteria of doctors who were directly involved with patients in the inpatient service. Data analyzed using univariate and bivariate analysis methods with the Chi-Square Test statistic. The results showed that 52.9% of respondents had low productivity. Based on statistical analysis test the independent variables include supervision (Pvalue = 0,000), working conditions (Pvalue = 0.041) and income (Pvalue= 0.005) showed the results that there was a relationship between supervision, working conditions and income with the productivity of doctors in filling out medical record documents. It is suggested to the hospital management to be able to improve the structure of the hospital and to provide a clear job description of the supervisor responsible for carrying out medical records filling by the doctor, improve the quality of printing medical record forms so that they are easy to read and to recalculate the proportion of income received by the doctor.

Abstract

Background: Data collection on the home visit in Semarang City must be completed in April 2019, but there were 17 public health centers have not reached 100% in June 2019. The public health center must developed strategies to improve employee performance to optimize data collection on the home visit by applying organizational factors. This study aims to analyzed organizational factors on the performance of PIS-PK employee in data collection on a home visit in Semarang City.Method: This was a qualitative research with a descriptive approach using the method of in-depth interview purposive sampling. This study was conducted in June-July 2019. The Coordinator and PIS-PK employee was the main informant. Triangulation informants in this research were regional cadres and head of the public health center, and the coordinator of PIS-PK in Semarang City. The analyzed factors were resources, supervision, leadership, organization structure, and work design.Results: The results of this research such as the resources provided in the data collection on home visits were completed. The supervision such as giving direction, guidance, monitoring and evaluating during the implementation was given well. The head of the public health center and the PIS-PK coordinator’s leadership in making decisions and solving problems showed well. The organizational structure and design of the work were poor, there was still an overlapping division of tasks so the performance did not optimal.Conclusions and Recommendation: Resources, supervision, leadership, organizational structure and work design are factors that support the employee performance. The performance of PIS-PK employee is not optimal because there were still overlapping divisions of tasks. It’s better if the public health center in Semarang City makes a division of job desk and the design of the implementation activities, so it can be more structured and monitored.Keywords: organizational factors, data collection on home visits, data collection employee.
