Abstract

Oxidative stress as a consequence of chronic alcoholic intake causes lesions of liver tissue. The objective of this study was to identifythe antioxidant effect of ethanol extract derived from Balinese purple sweet potato on the liver tissue in mice after chronic consumptionof alcohol. Subjects of this study were 32 adults male Swiss mice (12–14 weeks) divided into 4 groups with control group post-testonly design. Post-treatment of blood and liver samples were collected from each group of 8 mice. The MDA liver was quantified withthiobarbituric acid reactive substances (TBARS) method. Samples were collected from control group of 8 mice without sweet potatoextract and alcohol, 8 mice with 1 mg extract of Balinese purple sweet potato, 8 mice with 0.8 gram alcohol, and 8 mice with combineof alcohol and extract of Balinese purple sweet potato, each treatment was carried out every day for 4 weeks. The results showed asignificant increase of serum SGOT, SGPT and liver MDA level, after chronic consumption of alcohol (p = 0.00). The decrease of serumSGOT, SGPT and liver level MDA was significant in the group with extract of Balinese purple sweet potato (p = 0.00). The researchersconcluded that the extract of Balinese purple sweet potato had an antioxidant effect in mice which consumed alcohol chronically. Basedon this study it is concluded that extract of Balinese purple sweet potato has a protective effect on the liver cell in mice which consumedalcohol chronically.

Abstract

Transfusi darah merupakan proses pemberian darah atau komponen darah dari donor ke penerima, transfusi darah yang sesuai memainkan peran penting dalam layanan kesehatan modern. Reaksi transfusi didefinisikan sebagai kejadian apa pun yang terjadi sebagai akibat dari pemberian produk darah, dapat selama atau setelah pemberian produk tersebut. Gambaran gejala reaksi transfusi adalah parameter penting dalam menentukan suatu kejadian reaksi transfusi. Penelitian ini dilakukan untuk mengetahui data prevalensi dari reaksi transfusi dan karakteristik penderita reaksi transfusi berdasarkan usia, jenis kelamin, produk darah dan tingkat keparahan dari gejala reaksi transfusi di RSUP Sanglah Denpasar Bulan Januari 2018 hingga Juni 2019. Penelitian ini bersifat retrospektif dengan menggunakan data sekunder formulir monitoring transfusi pasien reaksi transfusi berupa usia, jenis kelamin, produk darah yang ditransfusi, dan gejala reaksi transfusi di RSUP Sanglah Denpasar bulan Januari 2018 hingga Juni 2019. Terkumpul sebanyak 88 sampel yang memenuhi kriteria inklusi dan eksklusi. Jumlah total pasien yang menerima transfusi darah di Rumah Sakit Sanglah dari Januari 2018 hingga Juni 2019 adalah 21.918. Ada 111 (0,5%) kasus reaksi transfusi dengan rentang usia terbanyak yaitu 19-29 tahun 17 kasus (19,3%) dan kelompokiterendah yaitu rentang usiai 50-59 tahun 13 kasus (14,8%). Reaksi transfusi lebih banyak terjadi pada laki - laki dengan jumlah 47 kasus (53,4%), dan produk darah yang paling tinggi angka penggunaannya yaitu packed red blood cell (PRC) dengan jumlah penggunaan 72/28.405 (0,0025%) mengakibatkan terjadinya reaksi transfusi, sedangkan terendah fresh frozen plasma (FFP) dengan kasus terjarang 1/2185 transfusi (0,0004%). Berdasarkan usia dan gejala reaksi, maka tingkat keparahan gejala reaksi transfusi darah terbanyak yaitu reaksi transfusi sedang dengan jumlah 42 kasus (47,7%). Abstract

Introduction: AIDS is a collection of certain clinical conditions that are the outcome of infection by HIV. Currently, the problem of HIV / AIDS is a big problem that threatens many countries around the world, one of them is Indonesia. HIV / AIDS is much affected by the productive age population and more affected by men than women. The taxi driver job has a lot of time on the highway. Taxi drivers will be vulnerable to HIV transmission when performing high-risk behaviors against its HIV transmission. This study aims to find out the description of HIV antibody examination on taxi drivers in Kelurahan Kuta Kabupaten Badung. Method: The type of research used is descriptive research. This research was conducted on May 2017. The sampling of the research was done at UPT. Puskesmas Kuta 1 and the examination was conducted at Health Analyst Laboratory of STIKes Wira Medika Bali. The population used in this study are all taxi drivers who are in Kelurahan Kuta Kabupaten Badung and the samples are 33 drivers. The method used in this research is Rapid Test / Immunochromatography method. The data have been analyzed and presented in tabular form. Result: The results of this study were all samples of taxi drivers showed non-reactive results against HIV antibodies. Discussion: This means no HIV antibodies found in taxi drivers serum.

Abstract

Background: Diabetic retinopathy is one of the most common microvascular complications of diabetes mellitus. A prevalence study could contribute in explaining the pattern and giving the perspective of how a community deals with diabetes and its complications.Methods: This study was conducted to determine the prevalence of diabetic retinopathy in patients with type 2 diabetes mellitus at the Diabetic Center of Sanglah General Hospital. This study used a descriptive cross-sectional study using secondary data from medical records of patients with diabetes mellitus type 2.Results: 1070 patients with type 2 diabetes mellitus obtained from January – December 2014. 6.92% patients have suffered from retinopathy, and 93.08% have not suffered from retinopathy. Distributions of patients who suffered from retinopathy were mostly from the age group of 50-59 years old.Conclusion: Only 6.92 % of type 2 diabetes mellitus patients in Sanglah Hospital had suffered from the complication of diabetic retinopathy in the year 2014.

Abstract

Background: Aplastic anemia is a rare disease of anemia which characterized by pancytopenia due to spinal cord hypoplasia. This can occur in people of any age, race, and gender with different degree of severity, with incidence rate is two until three times higher in Asia than the West. This study aims to determine the characteristic of aplastic anemia based on age, gender, laboratory examination, and severity in Sanglah Hospital.Methods: This research is a cross sectional descriptive study with sources of data were medical records of aplastic anemia patients at Sanglah Hospital in the last 2 years, 2017-2018. Distribution of research variables is gender, age, laboratory examination, and severity.Results:The result showed that 11 data were 6 (55%) male and 5 female (45%), with the mean age was 48 ± 14 years. The highest incidence occurred in the age range of 26-60 years old with 7 patients (64%). In this study the average hemoglobin level obtained was 9.94 ± 1.46 g/dL with range between 6.65 to 12.49 g/dL. The average number of leukocytes was 11.91 ± 24.52 x 109 L with range of leukocytes is 1.99-84.58 x 109/L. The average number of absolute Neutrophils and percent was 1.53 ± 1.52 x 109/L and 25.99 ± 25.79% with range between 0.06 – 4.53 x 109/L and 2.3 - 70.83%. The average platelet count was 24.77 ± 11.35 x 109/L with range between 4.25 – 42.58 x 109/L. The highest cases were nonsevere aplastic anemia (NSAA) with 7 people (64%).Conclusion: The value of laboratory parameters for complete blood examination (leukocytes, platelets, hemoglobin, and neutrophils) in patients with aplastic anemia at Sanglah General Hospital is still very low, so it is a challenge for clinicians to carry out treatment of aplastic anemia and management related complications.

Abstract

Introduction: Central obesity is condition there is an accumulation of intraabdominal and subcutaneous fat that occurs in the abdominal area which can cause insulin resistance so that glucose is difficult to enter the cell and causes blood glucose levels to increase in Type 2 Diabetes Mellitus. Waist circumference is one indicator used for measure central obesity. This study aims to determine the relationship between central obesity and HbA1c levels.Methods: This study was observational analytic with a cross sectional approach. The study population was adults aged 30-50 years in the Batursari, Bitera Village, Gianyar. Data taken included age, sex, occupation, waist circumference, BMI, HbA1c levels in 50 respondents taken by purposive sampling method. The relationship between central obesity and HbA1c levels was analyzed using Pearson correlation test with a significance level of 0.05.Result: The majority of the sample were central obesity based on waist circumference (60%). The number of respondents with HbA1c level ≥ 6.5% by 30%. Pearson Product Moment correlation test between central obesity and HbA1c level with p = 0.001 and Pearson Correlation r = 0.456.Conclusion: There is a significant positive relationship between central obesity and HbA1c levelsLatar Belakang: Obesitas sentral adalah suatu keadaan dimana adanya akumulasi lemak intraabdominal dan subkutan yang terjadi di daerah abdomen yang dapat menyebabkan resistensi insulin sehingga glukosa sulit untuk memasuki sel dan menyebabkan kadar glukosa darah meningkat pada penyakit Diabetes Mellitus Tipe 2. Lingkar pinggang merupakan salah satu indikator yang digunakan untuk mengukur obesitas sentral. Penelitian ini bertujuan untuk mengetahui tentang hubungan antara obesitas sentral dengan kadar HbA1c.Metode: Penelitian ini bersifat observasional analitik dengan pendekatan cross sectional. Populasi penelitian adalah penduduk dewasa usia 30-50 tahun di Lingkungan Batursari, Desa Bitera, Gianyar. Data yang diambil meliputi usia, jenis kelamin, pekerjaan, lingkar pinggang, IMT, kadar HbA1c pada 50 responden yang diambil dengan metode purposive sampling. Hubungan antara obesitas sentral dengan kadar HbA1c dianalisis dengan menggunakan uji korelasi Pearson dengan derajat kemaknaan 0,05.Hasil: Hasil penelitian menunjukkan bahwa sebagian besar mengalami obesitas sentral berdasarkan lingkar pinggang (60%). Jumlah responden dengan kadar HbA1c ≥ 6,5% sebesar 30%. Uji korelasi Pearson Product Moment antara obesitas sentral dengan kadar HbA1c dengan nilai p = 0,001 dan nilai Pearson Correlation r = 0,456.Simpulan: Terdapat hubungan positif bermakna antara obesitas sentral dengan kadar HbA1c.

Abstract

Aqueous extract of purple sweet potato tubers have antioxidant potential in animal and human vascular endothelial, through increased expression of superoxide dismutase. This study aims to prove the comparison between ethanol extract and aqueous extract of purple sweet potato tubers increasing the expression of superoxide dismutase -2, superoxide dismutase -3, and endothelial nitric oxide synthase in human vascular endothelium. This study was an experimental study of the vascular endothelial cells which underwent oxidative stress in vitro, which is protected by ethanol extract or aqueous extract of purple sweet potato tuber at various concentrations. Observation of the superoxide dismutase -2, superoxide dismutase -3, and endothelial nitric oxide synthase in endothelial cells is done using the method immunohistochemistry with monoclonal antibodies. The results showed an increase in superoxide dismutase - 2, superoxide dismutase -3, and endothelial nitric oxide synthase was significantly (p < 0.05) in endothelium protected by ethanol extract or water extract of purple sweet potato tubers. The ethanol extract only showed its effect at concentration of 1.5625 µg/ml for superoxide dismutase -2 and superoxide dismutase -3. The effect of aqueous extract were at a concentration of 1.5625–3.125µg/ml. From these results it can be concluded that the ethanol extract or aqueous extract of purple sweet potato tubers can protect endothelial cells from oxidative stress through increased expression of superoxide dismutase -2 and superoxide dismutase -3. The ethanol extract requires much lower concentrations to show its effect compared with aqueous extracts. Keywords: Aqueous extract or ethanol  extract of purple sweet potato, Superoxide dismutase, Human umbilical vein endothelial cells.

Abstract

Background: The systemic lupus erythematosus (SLE) is an autoimmune disease with various clinical manifestations. SLE indicated by the presence of autoantibodies to the nucleus components and immune complex deposits. Prevalence ratio of SLE is higher in women than in men, but male patients seem to have a higher frequency of symptoms and more aggressive disease. So, this case study aims to evaluate A seventeen-year-old boy patient with lupus nephritis as a complication of Systemic Lupus Erythematosus (SLE) based on laboratory evaluation. Case Description: A seventeen-year-old male patients referred to the emergency department in Sanglah General Hospital with swelling on both eyelids and legs for two weeks before admission. The patient has high blood pressure, discolouration of urine and significant weight gain about 14 kilograms in two weeks due to swelling. Several examinations performed and the results were indicating anaemia, proteinuria, hematuria, hypoalbuminemia, high BUN and creatinine level, hyperkalemia, hyperphosphatemia and high titer of ANA-IF and anti-ds-DNA level. The patient diagnosed with lupus nephritis and treated with antihypertension and diuretic. Conclusion. The most common complication of SLE is lupus nephritis, and its prevalence varies significantly by region, race and tribe. Although overall improvements in the treatment of SLE patients have increased in kidney survival rates, the prognosis remains unsatisfactory. Early detection and diagnosis should be done so that the patient’s condition worsens not occur because the late diagnosis of LN can increase the risk of End-Stage Renal Disease.Keywords: Autoimmune Disease, Lupus Nephritis, Systemic Lupus Erythematosus.

Abstract

Dengue virus belongs to the genus Flavivirus that can be transmitted to humans by the bite of the Aedes aegypti mosquito. Dengue virus infection has a high mortality and morbidity rates almost throughout the world. Diagnosis has been classified by degree of clinical manifestation according to WHO 1997, confirmed through clinical signs and symptoms and laboratory tests The aim of this study was to examine the relationship between leukocyte count, lymphocyte count, hematocrit and platelet count with a severity degree of dengue virus infections, according to WHO criteria in 1997 in adult patients admitted to the General Hospital Sanglah 2015. This study was cross-sectional analytic study on 90 patients with dengue infection using secondary data. Samples are adult patients with dengue infection aged > 12 years in the medical records contained the results of the leukocyte count, lymphocyte count, hematocrit and platelet count were treated at Sanglah Hospital in Bali on the period January 1 to December 31, 2015. The results showed that there is no significant relationship between the number of leukocytes with the lowest degree of severe dengue infection (p = 0.354; r = -0.099), there is no significant relationship between low lymphocyte count with severity of dengue infection (p = 0.636, r = -0.051), there is no significant relationship between hematocrit value with the highest degree of severe dengue infection (p = 0.293; r = 0.112), and obtained negative relationship weak significant between platelet counts with the lowest degree of severe dengue infection (p = 0.009; r = -, 0275). Our results suggest that effort on early prevention and detection of dengue infection is important to reduce morbidity and mortality cause by Dengue Infection. Abstract

Background. Chronic diseases have become one of the world's public health problems in recent decades, and mortality rate are projected to continue to rise. The diagnosis and severity of CKD is based on the results of renal function tests recommended by KDIGO is Glomerulus Filtration Rate (GFR) patients. A simple and inexpensive method is to use endogenous filtration markers to calculate GFR such Creatinine Clearance Test (CCT) or e-GFR CKD EPI formula. Accurate calculation of GFR by CCT and e-LFG CKD-EPI are important to avoid misdiagnosis of CKD. This study aimed to determine if there is any difference in the results between these two methods. Method. This study used analytical observational cross-sectional research method. Samples were selected from the population based on inclusion criteria having complete variability data and no exclusion criteria. Analyzed by SPSS software version 28 to check normality using Kolgomorov-Smirnov test, significance test with Mann Whitney and Spearman to check correlation. Results. The results of the study of 1257 samples, showing that the data are not normally distributed (p = 0.000), had a significant difference between CCT and e-GFR CKD EPI (p = 0.000) and the correlation is linear and moderate correlation strength (r=0.403) (p=0.000). Conclusion. Differences in kidney function test results using CCT and e-GFR CKD-EPI methods in patients who carried out laboratory examinations at the clinical pathology laboratory at Sanglah Hospital, Bali during April 2020–April 2021, was found that there were differences in the results of CCT and e-GFR CKD EPI.
