Abstract

Abstract

Background: Occupational accidents remain a major challenge in the mining industry because mining operations involve complex activities and high-risk working conditions. To prevent accidents and improve safety performance, PT X implemented the Safety Friendly Program, which includes safety observations, safety inspections, and management engagement activities. An evaluation of the program is needed to determine whether its implementation is accompanied by an improvement in occupational safety performance. Purpose: To analyze the effectiveness of the Safety Friendly Program in reducing the Accident Frequency Rate (AFR) at PT X. Method: A quantitative approach with a time-series comparative design. Secondary data were obtained from the Health, Safety, and Environment Department of PT X from July 2025 to March 2026. The data included safety observations, safety inspections, management engagement activities, and AFR. Data were analyzed descriptively using IBM SPSS Statistics version 25 and presented in tables and trend analyses to compare changes across the study periods. Results: The Safety Friendly Program was implemented consistently throughout the study period. Safety observations reached 513 activities in the third quarter of 2025, 512 activities in the fourth quarter of 2025, and 494 activities in the first quarter of 2026. Safety inspection findings increased from 279 in the fourth quarter of 2025 to 290 in the first quarter of 2026. The number of workers involved in management engagement activities also increased from 293 in the third quarter of 2025 to 309 in the first quarter of 2026. During the same period, AFR decreased from 10.57 in the third quarter of 2025 to 8.02 in the fourth quarter of 2025 and further declined to 3.07 in the first quarter of 2026. Overall, AFR decreased by 70.96%. Conclusion: The consistent implementation of the Safety Friendly Program was accompanied by a substantial decline in AFR during the study period. Safety observations, safety inspections, and management engagement activities may support accident prevention and strengthen safety practices in mining operations. However, this descriptive trend should not be interpreted as evidence of a causal relationship. Continued implementation and periodic evaluation are recommended to improve occupational safety performance.

Abstract

Background: Groundhandling workers are workers who are exposed to heat for quite a long time. A work environment that exceeds tolerance limits can cause health problems such as dehydration and fatigue. Lestari (2016) in his research stated that a hot work environment that exceeds the Threshold Limit Value (TLV) can increase the risk of dehydration. Purpose: This research focuses on fatigue and the risk factors of fatigue, especially those caused by dehydration, without disregarding other risk factors among ground handling workers at Soekarno-Hatta Airport. Method: The method used in this research is an observational analytic approach with a cross-sectional design. The sampling technique employed random sampling with a sample size of 219 respondents consisting of ground handling workers working both inside buildings and on the apron. The measuring instruments used were specific gravity urine tests to determine hydration status and IFRC questionnaires to assess workers' physical fatigue status. Result: From the existing data it was found that the majority of respondenst experienced mild fatigue, namely 36,5% of the 219 respondents and others experienced severe fatigue 63,5%. With hydration status, most workers have good hydration status (euhydration), namely 70,3% and and some others experienced dehydration, namely 29,7%. The result of the analysis between hydration status and level fatigue showed 35,4% of respondents who werw dehydrated experienced severe fatigue, while 37% of respondents who were euhydrated/normohydrated experirienced severe fatgue. The results of the analysis test obtained was no relationship between hydratin status and fatigue level. The OR (Odd Ratio) shows a result or 0,932 meaning that respondents withs dehyration are protective factor of 0,93 times agains severe fatigue. Conclusion: The conclusion of this research is that good hydraton status can prevent fatigue in workers, especially those who work with direct heat exposure.

Abstract

The informal sector/SMEs (Small and Medium Enterprises) play a significant role in Indonesia's economy. However, this sector often faces various occupational safety and health (OSH) risks. An OSH management system can help SMEs manage OSH risks and improve their business performance. This study aims to examine the factors that contribute to the successful implementation of occupational safety and health (OSH) management systems in the informal sector/SMEs (Small and Medium Enterprises). The study employs a systematic literature review methodology following PRISMA (Preferred Reporting Items for Systematic Review and Meta-analysis). The study was conducted in April 2024, searching for relevant articles from reputable sources such as Google Scholar, ProQuest, and ScienceDirect. The review encompassed 16 relevant articles and revealed that the factors contributing to the successful implementation of OSH programs can be categorized into two main factors: external and internal factors. External factors play a crucial role in enabling the successful implementation of OSH management systems in the informal sector/SMEs. These factors encompass the supportive roles of government, the private sector, and various organizations. This study on successful OSH management system implementation in the informal sector/SMEs highlights key factors for policymakers, practitioners, and researchers. Policymakers can support implementation through financial aid and clear regulations. Practitioners should consider these factors when assisting SMEs, tailoring strategies for each business. Researchers can contribute by developing more.

Abstract

Incident recurrence indicates that organizational learning has not fully succeeded in breaking the chain of failure. This study aims to analyze the relationship between the quality of corrective actions, as measured by the Hierarchy of Controls, and incident recurrence, while accounting for incident characteristics as control variables, at PT XYZ (mining contractor) during 2023–2025. This quantitative study with a retrospective correlational design analyzed 124 incident investigation reports. Critical factors are classified using two schemes presented side by side, namely the company's internal taxonomy and the Human Factors Analysis and Classification System (HFACS). Analyses were conducted at univariate, bivariate (Fisher's Exact Test and Pearson Chi-Square), and multivariate (multiple logistic regression) levels. The reliability of corrective action scoring was validated through inter-rater agreement (weighted Kappa = 0.722; substantial). Results show a high recurrence rate under both schemes (65.3% under HFACS; 72.6% under the internal scheme). Critical Factor was the most dominant predictor (OR = 7.02; p < 0.001), followed by Incident Classification (OR = 8.91; p = 0.003), while Corrective Action Quality was not significant (p = 0.556), consistently across both schemes. Applying the HFACS taxonomy refined the dominant Human Error category (80.6%) into Violation (38.7%) and Error (41.1%), revealing a diversity of failure types useful for improving investigation. The study concludes that the effectiveness of recurrence prevention is determined more by the accuracy of root-cause diagnosis during investigation than by the technical level of corrective actions alone.
