Abstract

Background: Patients with fractures often experience significant pain. Complementary therapies are considered helpful in reducing pain levels in those suffering from fractures. The aim of this systematic review is to explore the impact of complementary therapy on pain reduction in fracture patients. Purpose: To  assess  the  impact  of complementary therapy affects fracture patients' levels of pain. Method: A systematic review approach in line with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Articles were gathered from online databases including Google Scholar, PubMed, and ScienceDirect, with full-text publications in English from 2012 to 2023. Among these, four were published in China and two in Iran. Quality evaluation was conducted using the Joanna Briggs Institute (JBI) Critical Appraisal Skills Program. The severity of pain in the six studies was measured using the standard Visual Analogue Scale (VAS). Results: The review included six articles, which demonstrated that complementary therapies can reduce pain in fracture patients. Various complementary intervention techniques were utilized in the studies, including foot intervention, reflexology, auricular acupressure, and elbow intervention therapy. Before and after the interventions, the mean pain intensity scores were 5.72 ± 0.97 in the intervention group and 5.66 ± 0.92 in the control group. Conclusion: Complementary intervention therapy can significantly reduce pain in patients with fractures. However, additional research is needed to confirm these findings and explore other complementary therapy methods.

Abstract

Background: Post-stroke patient depression can result in delays in stroke recovery and healing. The degree of severity of stroke will cause neurological deficits which can become stressors that will affect individual coping mechanisms.Objective: This research aims to analyze factors associated with non-hemorrhagic post-stroke depression.Methods: Analytical observational with a cross-sectional approach. The research was conducted at the Neurology Clinic of RSUD dr. R. Goeteng Taroenadibrata Purbalingga. The population is 435 post-stroke non-hemorrhagic patients. The sample is 104 respondents with the sampling technique used consecutive sampling. The instruments used were the Brief COPE, NIHSS, and PHQ-9. Bivariate analysis used the Kendall-Tau correlation test.Results: The majority of respondents were elderly, male, primary school education level/equivalent, married, live with husband/wife and children, left hemisphere lesion location, had stroke duration >2 years, did not experience recurrent strokes, hypertension, had problem-focused coping and mild stroke severity with varying degrees of depression. Age, gender, marital status, living together, location of the lesion, duration of stroke, stroke frequency, comorbidities and coping mechanisms were not significantly related (p>0.05) while education level and stroke severity was significantly associated with post-stroke depression (p<0,05).Conclusions: There is a significant relationship between education level and severity of stroke with post-stroke depression. Nurses can also motivate patients to always take medication regularly, increase patient knowledge about stroke and depression, or implement non-pharmacological therapy such as Range of Motion (ROM) so that they can help reduce the severity of a stroke and prevent or reduce the occurrence of depression after non-hemorrhagic stroke.

Abstract

Background: Stroke remains the most common cause of death and disability. Stroke may suffer from several neurological disorders, including hemiplegia, communication disorders, cognitive disorders, and visual-spatial cognitive disorders.Objective: The purpose of this study was to look at how strength training affected stroke patients' muscle strength.Methods: A systematic search for this review used PubMed, ProQuest, Science Direct, Springer Open, Directory of Open Access Journal (DOAJ), and Wiley Online Library using the keywords “strength training (strength training OR strength exercise)” AND “muscle strength” AND “stroke” AND “randomized controlled trial” NOT (systematic review OR meta-analytic). Journals are appraised using the Critical Appraisal Skills Programme (CASP) JBI 2020 the synthesis method used is narrative synthesis. To analyze the agreement between reviewers using the Interclass Correlation Coefficients (ICC) test, with the ICC result being good (r = 0.79; 95% CI, 0.53, 0.92) against the quality assessment of the selected articleResults: 15 articles fully reviewed from 2018-2023. Muscle strengthening exercises are the most effective method for enhancing muscle strength in stroke patients. The research results show that strength training tends to increase the patient’s muscle strength if it is done from the start of a stroke and is carried out for 4-6 weeks of training.Conclusion: The results emphasize the value of strength training regimens for stroke victims in order to avert life-threatening impairment

Abstract

Introduction: The problems that post-stroke patients still face after being hospitalized are weakness in one side of the body (hemiparesis), speech disorders (dysarthria/aphasia), difficulty swallowing (dysphagia) and psychological disorders such as post-stroke depression and the risk of recurrent attacks. The purpose of the service is to improve the knowledge and skills of partners about: early detection of stroke, post-stroke care, post-stroke physical exercise, post-stroke speech therapy, post-stroke swallowing therapy, post-stroke psychological management. The method carried out is training with lectures, discussions and demonstrations, the media used are modules and videos. The participants were elderly Posyandu cadres and families who took care of family members after a stroke totaling 19 people. The success of the training was evaluated by comparing pre and posttest scores and assessing the participants' ability to demonstrate the actions taught. The results obtained by the knowledge of early detection, psychological management and post-stroke care of participants were 100% improved. Skills about early detection of stroke, ROM exercises, swallowing exercises and speech therapy 89.47% of participants improved. Conclusion: The training is effective in improving the knowledge and skills of cadres and families in caring for post-stroke patients. Key word: post-stroke, training, cadres, family, care

Abstract

Introduction: Hypertension is an abnormal increase in blood pressure that can cause persistent symptoms in target organs. Uncontrolled hypertension can lead to complications such as heart disease, kidney failure, and stroke. Nonpharmacological therapies that can be done to reduce blood pressure are slow deep breathing and music therapy. Purpose: To determine the effect of a combination of slow deep breathing therapy and music therapy on lowering blood pressure in the elderly with hypertension. Methods: The research design used a case study method by providing slow deep breathing and music therapy interventions for 7 days to 2 case study subjects who experienced hypertension. Data collection using observation sheets and standard operating procedures (SOP) slow deep breathing and music therapy. Blood pressure measurement tools use a digital Sphygmomanometer and data analysis using descriptive analysis. Discussion: The implementation results showed that after the combination of slow deep breathing therapy and music therapy for 7 days there was a decrease in blood pressure in both case study subjects. The decrease in blood pressure in case study subject I on the first day after therapy was 198/103 mmHg and after 7 days of therapy it became 178/101 mmHg. In case study subject II, on the first day of intervention, blood pressure was 163/97 mmHg and after intervention for 7 days it became 152/95 mmHg. Conclusion: Combination therapy of slow deep breathing and music therapy can reduce blood pressure in elderly people suffering from hypertension.

Abstract

Introduction: Meningiomas are a type of brain tumor that develops from the meninges (lining of the brain, that is, the membranes surrounding the brain and spinal cord. Another problem that occurs due to meningiomas and very often occurs is memory impairment. This nursing care is given with the aim of reducing memory impairment in meningioma sufferers with improved memory. Purpose: To determine the effect of the application of short-term memory exercises on memory impairment in meningioma patients. Result: The patient has been stiff for 1 month, speaks indirectly, often barks, irritability and headaches such as pulling. Results of MCT-Scan Head examination with contrast, effect: Vasogenic edema extensive in the bilateral frontotemporoparietal lobe, no picture of enhancing mass. Blood pressure: 109/65mmHg. Pulse; 90x/min, Temperature; 36.2oC, Respiration; 20x/min, SpO2; 99%, Weight; 53kg, Height; 150cm. Conclusion: The implementation of nursing given to Mrs. S with Chordoid Meningioma obtained very significant results in improving short-term memory and decreasing acute pain after being given memory training interventions and deep breath relaxation. Improvement in short -term memory has begun to be seen at the first intervention as well as acute pain reduced from 6 to 4.

Abstract

ABSTRACT Chronic pain represents a significant global health issue, adversely affecting the quality of life for countless individuals. Various methods have been developed for pain management, including hypnosis and virtual reality (VR). Hypnosis has been shown to effectively reduce pain perception, while VR provides an immersive experience that can divert attention from pain. The integration of these two approaches may offer innovative solutions for chronic pain management; however, empirical evidence supporting this combination remains limited. This systematic review aims to explore the effectiveness of the combination of hypnosis and VR in reducing pain, as well as to identify the evidence supporting the use of these interventions in clinical practice. This study employs a literature review approach following the PRISMA guidelines. The systematic review utilizes the PICO framework. The databases searched include Google Scholar, PubMed, and Scopus, using keywords such as Virtual Reality, Hypnosis, and Pain. The findings indicate that the combination of hypnosis and virtual reality (VR) significantly reduces pain in patients with various conditions, including postoperative and oncological cases. This intervention not only diminishes pain perception but also enhances patient satisfaction and quality of life. Evidence suggests that this approach can serve as a safe and effective alternative in chronic pain management. The integration of hypnosis and virtual reality (VR) demonstrates substantial potential in alleviating chronic pain. This review supports the utilization of these innovative methods as effective strategies in pain management within clinical practice. Keywords: Virtual Reality, Hypnosis, Pain.  ABSTRAK Nyeri kronis adalah masalah kesehatan global yang signifikan, mempengaruhi kualitas hidup banyak individu. Berbagai metode telah dikembangkan untuk manajemen nyeri, termasuk hipnosis dan virtual reality (VR). Hipnosis terbukti efektif dalam mengurangi persepsi nyeri, sementara VR menawarkan pengalaman imersif yang dapat mengalihkan perhatian dari nyeri. Mengintegrasikan kedua pendekatan ini dapat memberikan solusi inovatif dalam pengelolaan nyeri kronis, namun bukti empiris yang mendukung kombinasi ini masih terbatas. Tinjauan sistematis ini bertujuan untuk mengeksplorasi efektivitas kombinasi hipnosis dan VR dalam mengurangi nyeri, serta mengidentifikasi bukti yang mendukung penggunaan intervensi ini dalam praktik klinis. Artikel ini merupakan tinjauan sistematis dengan pendekatan PRISMA. Tinjauan sistematis menggunakan PICO. Database penelusuran yang digunakan adalah Google Scholar, PubMed, Scopus dengan kata kunci Virtual Reality, Hypnosis, Pain. Hasil berbagai penelitian menunjukkan bahwa kombinasi hipnosis dan virtual reality (VR) secara signifikan mengurangi nyeri pada pasien dengan berbagai kondisi, termasuk pascaoperasi dan onkologi. Intervensi ini tidak hanya mengurangi persepsi nyeri, tetapi juga meningkatkan kepuasan pasien dan kualitas hidup. Bukti menunjukkan bahwa pendekatan ini menjadikan alternatif yang efektif serta aman dalam manajemen nyeri kronis. Integrasi hipnosis dan virtual reality (VR) menunjukkan potensi besar untuk mengurangi nyeri kronis. Tinjauan ini mendukung penggunaan metode inovatif ini sebagai pendekatan efektif dalam manajemen nyeri di praktik klinis. Kata Kunci: Virtual Reality, Hipnosis, Nyeri.

Abstract

Introduction: Stroke can cause various kinds of complications. One of the complications of stroke is dysphagia. Therefore, dysphagia management is necessary for stroke patients with dysphagia. Dysphagia management can be done by screening stroke patients with dysphagia and carrying out swallowing therapy with shaker exercise and effortful swallow. Purpose: This case study aims to evaluate the effect of shaker exercise and effortful swallowing on changes in the degree of dysphagia in patients with stroke. Methods: The method used is case study analysis based on nursing intervention with description analysis. The sample used was a dysphagia patient with a stroke. The instrument used to measure the degree of dysphagia is the Gugging Swallowing Screen (GUSS). Discussion: After the shaker exercise and effortful swallow intervention for 5 consecutive days in the morning and evening, there was a change in the degree of dysphagia from initially getting 4 which means a severe degree of dysphagia with a high risk of aspiration to 18 which means mild dysphagia with a low risk of aspiration. Conclusion: Shaker exercise and effortful swallowing can improve the degree of dysphagia in stroke patients.

Abstract

Background: Haemodialysis is often accompanied by shivering, especially in patients with catheter access, which can reduce comfort, impair treatment efficacy, increase the risk of complications and contribute to inadequate dialysis. Addressing this issue through non-pharmacological means, such as a warming gown, offers a potentially effective, safe, and economical solution to improve patient outcomes. Purpose: To develop a warming gown as an innovation to reduce the incidence of shivering in chronic kidney disease patients undergoing haemodialysis with a haemodialysis catheter. Methods: This study employed a two-stage Research and Development design. In the first stage, a reusable, adaptive warming gown for HD catheter patients was developed and validated (S-CVI/Ave = 0.99). Second, a quasi-experiment was conducted with 60 patients recruited through total sampling. Subsequently, patients were randomly allocated to either the intervention group (warming gown) or the control group (blanket) using computer-generated randomisation based on their identification numbers. Shivering (Crossley and Mahajan scale) and body temperature (digital thermometer) were measured at 0, 15, 30, 60, and 120 minutes. Data were analysed using Wilcoxon, Friedman, and Bonferroni-corrected repeated Mann–Whitney tests. Results: The intervention group showed a significant reduction in shivering levels from 2.63 ± 1.27 to 0.37 ± 0.49 (p < 0.001) and an increase in body temperature from 36.36 ± 0.52 to 36.84 ± 0.29 (p < 0.001). In contrast, the control group showed no significant changes (p > 0.05). Conclusions: The warming gown was proven effective in reducing shivering and increasing body temperature in haemodialysis patients, offering advantages in comfort, safety, and cost efficiency.

Abstract

Ulkus kaki merupakan salah satu komplikasi utama diabetes melitus yang memerlukan penatalaksanaan tepat, terutama pasca operasi debridemen. Salah satu masalah utama pasca debridemen adalah infeksi, sehingga pencegahan dilakukan melalui perawatan luka. Penelitian ini bertujuan menganalisis perbandingan perawatan luka dengan madu + Nacl 0,9% dan Nacl 0,9% terhadap penyembuhan luka pada pasien diabetes mellitus tipe II post operasi debridement. Metode penelitian ini menggunakan comperative study menggunakan pendekatan asuhan keperawatan pasien dengan diabetes mellitus melalui perawatan luka diabetic menggunakan madu + Nacl 0,9% dan Nacl 0,9%. Hasil yang di dapatkan perawatan luka dengan madu + Nacl 0,9% lebih baik dalam mengurangi tingkat eksudat lebih cepat menumbuhkan jaringan granulasi yang baru dibandingkan perawatan luka yang hanya menggunakan NaCl 0,9%. Perawatan luka dengan menggunakan madu + NaCl 0,9% selama 2 hari berturut-turut terbukti mampu menurunkan kadar eksudat dan menumbuhkan jaringan granulasi baru lebih cepat dibandingkan perawatan luka hanya menggunakan NaCl 0,9%.
