Abstract

Medical records are documentation of identity, examination, treatment, actions, and services that have been provided to patients. Health services during pregnancy must be documented in the mother's card, medical record, mother's cohort, and mother and child health book. The maternal cohort is a collection of data collected from the mother's card. The documentation must be filled in completely because it affects further contact if needed and in certain circumstances, it is required for a medical audit. This study aims to determine the completeness of filling out the mother's cohort form at the MCH polyclinic at the Poris Plawad Health Center in 2021. This study used a quantitative descriptive method. Determining the number of samples using the slovin formula, so that a sample of 68 maternal cohort data was obtained. The results of the research from 68 maternal cohort data obtained an average of completeness of identification (77%), important reports (62%), and procedures for documentation (49%). Factors that influence incompleteness are the absence of an SOP for Filling in the Maternal Cohort, no updating of guidelines, no recording of medical record numbers, less communicative staff, no column for filling in authentication, and double working forms. Suggestions from researchers are to update the guidelines, create SOP Filling in the Cohort of Mothers and carry out outreach, update forms to add authentication records, conduct training/seminars to increase midwifery insights, and devise a workable system of more than 1 person for recording health data in the system.

Abstract

Timely provision of outpatient medical records results in quality medical record services. The time for providing outpatient records according to minimum service standards is ≤ 10 minutes. This study aimed to determine the length of waiting time for outpatient medical records at dr. Mintohardjo Hospital. The method in this research uses a descriptive method with a quantitative approach. The sampling technique used accidental sampling with a sample size based on an estimated proportion of 106 medical records. The research results showed that 14 medical records (13.2%) were on time and 92 medical records (86.8%) were not on time, most of which were not on time, namely > 31-40 minutes, 31 medical records (29.2%). In identifying the causes of delays in providing outpatient medical records, researchers used the 5M factors. Man factor of occurrence of misfile, there are medical records that have not been returned from the polyclinic or inpatient room, and there is a lack of knowledge and education. The money factor has not yet been budgeted for repairing the Roll O Pack. The material factor for printing loan receipts and medical records is not automatic in the SIMRS application, and only 1 printer machine is available out of 7 computers, so the number of printers needs to be increased. Machine factor occurs Roll O Pack damage and interference with software from downtime (the time when a system, application, and service cannot be accessed/does not function) in the SIMRS application. The method factor is that there is no SOP for providing outpatient medical records.

Abstract

The impact of pending claims can affect insurance service activities, becoming hampered and officers' workloads increasing because they have to revise pending claims that are sent. This study aimed to assess the relationship between insurance participants' knowledge of the occurrence of pending claims: a study on insurance managed by PT. Multiniaga Intermedia Proteksi. This study was an observational study of cross-sectional design. A total of 107 research subjects participated in this study. The results of this study show that the claim submission process at PT. Multiniaga Intermedia Proteksi had 107 claims submitted to insurance, and it was known that the results of paid claims submissions were 87 submissions (81.3%), and pending submissions were 20 submissions (18.7%). The results of participants' level of knowledge regarding submitting claims were obtained from 107 respondents. The average value of participants' knowledge was 89.15, with a median of 86.67. The highest value was 86.67. The lowest score obtained by respondents was 60, and the highest score was 100 as the results of knowledge testing on the results of claim status using the t-test, obtained with a probability value (Sig.) 0.196 > 0.1, indicating that knowledge has no significant effect on submitting claims.

Abstract

The basic cause of death is used as an evaluation of a health service, knowing the general health status, reporting mortality and prevention of deadly diseases and preparing for medical needs in the future. The purpose of this research is to know the description of the accuracy of the coding accuracy of the cause of death on the perinatal case death certificate at RSIA Tiara. The research method used in this research is a descriptive mix method where a mix of quantitative and qualitative is by describing and explaining the accuracy of the UCoD coding on medical certificates causing death in perinatal cases. The population in this study were all medical records of babies who died in 2020, 2021 and 2022, totaling 32 medical records. The sample used in this study was a saturated sample where all populations were sampled, namely 32 medical records. The results of the presentation of the accuracy of the coding on the medical certification cause of death were as much as 69% while those that were not correct were as many as 31%. It is preferable to code all diagnoses in the prenatal case death certificate using the guideline and rules P1,P2,P3,P4 to give the correct code. Factors causing inaccuracy in codification of the underlying cause of death on the death certificate, the doctor's writing on the infant's medical resume in the final diagnosis section using cursive letters creates obstacles for the coding officer in determining the correct code. There is no special SPO for coding prenatal cases. Lack of human resources with a background in medical records.

Abstract

Ketidaktepatan kode diagnosis akan berdampak pada informasi pelaporan morbiditas dan mortalitas yang tidak akurat dan tarif pelayanan di RS tidak sesuai. Tujuan penelitian ini yaitu mengidentifikasi SPO terkait pengkodean diagnosis penyakit pada pasien rawat jalan di Rumah Sakit Qadr, menganalisis ketepatan pengkodean diagnosis penyakit hipertensi pasien rawat jalan di RS Qadr dan mengidentifikasi faktor-faktor penghambat yang dapat mempengaruhi ketepatan pengkodean diagnosis penyakit hipertensi pasien rawat jalan di RS Qadr. Metode penelitian yang digunakan adalah metode deskriptif dengan pendekatan kuantitatif. Populasi penelitian berjumlah 219, dengan sampel sebanyak 69 rekam medis. Penentuan sample penelitian diambil menggunakan rumus Slovin. SPO pemberian kode sudah ada dan sudah berjalan. Hasil penelitian diperoleh 60 kode (86,96%) rekam medis yang tepat kodenya dan 9 kode (13,04%) rekam medis yang tidak tepat. Kesimpulan penelitian terdapat hambatan dalam pengkodean penyakit yaitu faktor man, material dan method. Faktor man yaitu pendidikan koder yang tidak sesuai. Faktor material yaitu tulisan dokter yang kurang jelas dan faktor method yaitu SPO pengodean yang kurang spesifik. Saran kepada petugas koding agar lebih teliti dan spesifik dalam memberikan kode penyakit, lebih memperhatikan kode penyakit yang dipilih apabila terdapat ada inclusion atau exclusion term dibawah kode yang dipilih terutama pada kode kategori penyakit hipertensi dan komplikasinya.

Abstract

Badan Penyelenggara Jaminan Sosial atau disingkat BPJS melaksanakan program yaitu Jaminan Kesehatan Nasional atau disebut JKN. Dengan pembiayaannya menggunakan sitem Casemix yang berdasarkan Tarif INA-CBGs. Dalam Penelitian ini bertujuan untuk mengetahui adanya perbedaan tarif antara tarif rumah sakit dengan tarif INA-CBGs kasus Sectio Caesarea. Pengambilan sampel menggunakan teknik sampel jenuh sehingga didapatkan jumlah sampel 106 pasien. Instrumen dalam penelitian ini dengan observasi & daftar tilik. Uji statistik dalam menganalisis data menggunakan uji wilcoxon & uji korelasi regresi. Penelitian ini menunjukkan, terdapat adanya perbedaan tarif antara tarif rumah sakit dengan rata–rata sebesar Rp 7.938,303 sedangkan rata-rata  tarif INA-CBGs sebesar Rp. 4.328.693 dengan nilai (Sig. 0,001) <0,05. Hal yang dapat disimpulkan dalam penelitian ini adalah yang dimana Ho ditolak dikarenakan adanya perbedaan rata–rata tarif antara tarif INA-CBGs dengan tarif rumah sakit. Serta upaya yang rumah sakit lakukan dalam menanggulangi kerugian dengan menerapkan subsidi silang yang dimana unit pelayanan yang memperoleh selisih positif memberi subsidi ke unit yang mengalami selisih negatif.

Abstract

Latar Belakang: Rak penyimpanan adalah tempat untuk menyimpan arsip atau rekam medis. Pentingnya menyiapkan rak penyimpanan rekam medis yang memadai, untuk mengurangi penumpukan rekam medis baru, mempersingkat waktu petugas rekam medis dalam menyiapkan rekam medis jika sewaktu-waktu diperlukan untuk menjaga kualitas pelayanan. Tujuan: Tujuan dari penelitian ini adalah bagaimana kebutuhan rak penyimpanan rekam medis aktif untuk tahun 2022-2026 di RS Patria IKKT. Metode: Metode penelitian yang digunakan adalah metode deskriptif dengan pendekatan kuantitatif, dalam penelitian ini mendeskripsikan kebutuhan rak penyimpanan rekam medis di ruang arsip di Rumah Sakit IKKT Patria. Hasil: Prosedur telah dilakukan sesuai dengan (SPO) Penyimpanan Rekam Medis, untuk tahun 2022 – 2026 membutuhkan 41 rak penyimpanan rekam medis, saat ini baru tersedia 20 rak penyimpanan rekam medis, kekurangannya adalah penambahan 21 rak. Luas ruangan tidak mencukupi untuk penambahan 21 rak, saat ini ruang penyimpanan yang tersedia hanya seluas 50 m² dan telah digunakan 49,82 m², ruang penyimpanan rekam medis dan ruang unit kerja rekam medis saat ini masih menyatu dan tidak terbatas. Kesimpulan: Untuk menambah rekam medis konvensional berbasis kertas, perlu ditambah satu ruangan lagi dengan luas 40,42 m². Sebaiknya penyusutan rekam medis tidak aktif dilakukan secara rutin sehingga tersedia rak penyimpanan rekam medis aktif untuk rekam medis pasien baru.

Abstract

This research aims to determine the relationship between student academic abilities and learning outcomes in final year students of the Medical Records and Health Information Study Program at one of the universities in Jakarta. The research method used is quantitative analysis using the independent T-test where researchers describe the relationship between students' academic abilities and learning outcomes. Based on the research results, it was found that the average age of students was 22 years with the majority of students being female, namely 77.8%. The description of academic ability is 88.9% good and 11.1% not good. The average learning outcome description is 3.51 with the largest learning outcome being 3.97 and the smallest being 2.66. Based on the research results, it was found that there was a significant relationship between learning outcomes and academic abilities (p-value <0.002). Where respondents whose academic abilities are good have a higher learning outcome score of 3.54 compared to respondents whose academic abilities are not good, namely 3.25. So, it can be seen that there is a quite significant correlation between academic ability and learning outcomes.

Abstract

Patient satisfaction is a factor that can be used as a reference in determining the success of a service program. Patient satisfaction will arise when it is supported by good quality service. There are five dimensional aspects known as service quality (SERVQUAL), reliability, responsiveness, assurance, empathy, and tangible. These five dimensions influence patient satisfaction. This research aims to provide an overview of the level of satisfaction of outpatient BPJS patients at Hermina Bitung Hospital, Tanggerang Regency in 2023. This research uses a descriptive method with a quantitative approach with the data that has been collected being processed using SPSS (statistical packagen for the social sciences) then the results analyzed by measuring the average percentage for each patient criterion and a sample size of 76 respondents, from this research it can be concluded that patient satisfaction through the reliability dimension is 56.58%, the responsiveness dimension is 55.26%, the assurance dimension was 53.95%, the empathy dimension was 56.58%, while the direct evidence (tangible) was 51.32%. Thus, the researchers concluded that the number of BPJS patients in outpatient registration services was 52.63%. This result still does not reach the minimum service standard set by the Indonesian Ministry of Health in 2008, which is ≥90%.

Abstract

Shrinkage is the reduction of medical records on storage shelves by moving inactive medical records to a different room from the active shelf seen from the last patient visit. The purpose of this study was to determine the implementation of shrinkage at the Kembangan Regional General Hospital. This research method uses a qualitative descriptive method by means of interviews and observations in the medical records unit. So it can be concluded from the results of the study that the Kembangan Regional General Hospital already has SOP related to shrinkage which is divided into SOP sorting, media transfer and destruction. In the implementation of shrinkage, officers carry out the process of sorting, transferring and transferring media every day. But officers do not conduct an assessment based on use value. The destruction process is carried out when the number of medical records has reached 2 tons by using a third party to make medical records in the form of pulp. The main obstacles to the implementation of shrinkage are officers who do not have a decree on duties and teams so that officers often delay the implementation of shrinkage, paper quality that makes it difficult for officers to see medical record sheets in the past year and scan machines that heat up quickly due to continuous use. Therefore, the Kembangan Regional General Hospital should complete the SOP related to shrinkage, make a decree of duties and conduct an assessment stage so that the implementation of shrinkage is carried out properly, and add a scan machine so that the implementation of media transfer is not delayed.
