Abstract

ABSTRACTThe role of exercise as a strategy for prevention, management and therapy in cardiovascular disease has been well described, but in some studies, it has been suggested that there is an increase in biomarkers in cardiac damage or cardiac troponin (cTn) after intensive, high-intensity exercise in healthy individuals. Several studies have shown significant increases in cardiac troponins after different types of exercise. The latest meta-analysis, showing that high-sensitivity cardiac troponin (hs-cTn) increases in about 83% of individuals after long and intensive exercise. The current pathophysiology of hs-cTn is not well understood. Several hypotheses have been proposed, such as transmembrane leakage from cytoplasmic free cTnT and cTnI or decreased troponin clearance from plasma, both caused by overloading of free radicals, myocardial stretching, elevated core temperature, or alteration of pH. Further research is needed with a full prospective study to evaluate the underlying pathophysiology of enhancing high sentivity cardiac troponin (hs-cTn) is an effective strategy for preventing or limiting cardiac injury and sport exercise safe for heart. Keywords: cardiac Troponin (cTn), high sensitivity cardiac troponin (hs-cTn), high intensity intensive exercise

Abstract

Diabetic ulcers are one of the most severe complications of diabetes in people with diabetes mellitus. When examined through the International Classification of Function (ICF) approach, there are several problems associated with diabetic ulcers, including: the presence of pain, limited movement, open wounds, loss of sensory sensation or motor ability. There are some modalities can be used to help these problems, namely by using laser and therapeutic approaches such as the application of proprioceptive neuromuscular facilitation (PNF). The method of this paper is an article review by using secondary data taken from research journals which are related to the effectiveness of the use of low power laser therapy and proprioceptive neuromuscular facilitation in diabetic ulcer patients. The primary effect of laser usage on cell is increasing oxidative metabolism to produce more ATP, which ultimately leads to the normalization of cell function, pain relief, and wound healing. PNF is a method of enhancing neuromuscular movement and facilitation to facilitate muscle contraction. This method has the primary goal of achieving individual needs, and thus can perform functional movements, through facilitation, inhibition, strengthening and relaxation of muscle groups. Based on the literature review, it can be concluded that: (1) Low power laser therapy is effective in repairing wounds in patients with grade 2 diabetic ulcers, (2) Proprioceptive Neuromuscular Facilitation (PNF) is effective in improving sensory and motor neuronal conduction associated with ankle movement in grade 2 diabetic ulcers, and (3) The combination of low power laser therapy and Proprioceptive Neuromuscular Facilitation is effective in improving the limitations of motion and function of grade 2 diabetic ulcers.

Abstract

ABSTRACTExercise may improve cognitive function and slow the onset of neurodegeneration, but the underlying mechanism had not yet known certainly. The purpose of this paper is to know the mechanism of exercise in improving cognitive function in terms of biomolecular aspects. The method of this paper is literature study of articles about the benefits of exercise in improving cognitive function in terms of biomolecular aspects. Cognitive function is a function related to perception, learning process, concentration, memory and problem solving. Exercise is useful for improving cognitive function mediated by a neurotropin-brain brain-derived neurotrophic factor (BDNF). BDNF has important role in neural plasticity, nerve excitability, and especially in learning process and memory improving. Exercise can improve cognitive function by inducing increased BDNF expression that occurs through the mechanism of epigenetic regulatory change. The change in epigenetic regulation caused by exercise through DNA methylation, which increase the expression of BDNF mRNA in the hippocampus. The change in epigenetic regulation caused by exercise, which also increase the expression of BDNF mRNA is a histone modification. The increased expression of mRNA in the hippocampus is not only increase synapse plasticity in the hippocampus area but also synapse transmission, which lead to the increasing of molecules expression related to learning and memory. Keywords: Sport, Brain-derived neurotrophic factor (BDNF), Epigenetic.

Abstract

Aging is an inevitable process in human. It causes deterioration of many organs in human bodies such as the brain. The most common pathological aging condition in the brain is Alzheimer’s disease. This disease is the leading cause of deficits in short term memory, praxis, visuospatial, and executive function. It can impair cognitive function in older adults. A lot of intervention has been studied to treat Alzheimer’s disease. One of them is the aerobic exercise. Some studies find aerobic exercise increases the anti-aging protein, klotho protein. Klotho is a systemic-biomarkers that has an important role in sustaining cognitive function and brain integrity. The level of plasma klotho protein is increased by doing aerobic exercise moderately for 12 weeks. The increase of klotho protein improves memory formation and hippocampus function. In Alzheimer’s disease, klotho protein has the ability to lower the formation of neurofibrillary tangles. Hence, aerobic exercise is an essential intervention to improve cognitive function in Alzheimer’s disease patient.

Abstract

Background: Tai Chi and many other Mind-body therapies have been very popular over the last few decades as useful tools for reducing stress and improving health. These methods are non-invasive and inexpensive. There are several reports showing that Tai Chi (TC) can regulate the host's immune system in many illnesses. This review summarizes the Tai Chi’s influence on breast cancer (BC) patients. Survivors of breast cancer often experience symptoms that reduce their quality of life during and after treatment, such as cognitive dysfunction, fatigue, sleep disturbances, depression, pain, and weight gain. The proinflammatory biomarkers like TNF-a, IL-1, IL-6 which activated through the central nervous system were believed inducing fatigue, pain , and other complications that caused hidarance on life activity. This becomes a serious problem as the number of survivors increases. The results of most studies show that Tai Chi reduced fatigue, pain, plasma levels of TNF-a and overall quality of life in breast cancer patients. Objective: To identify and evaluate the effect of TC training on inflammatory biomarkers, especially TNF-a, pain, and cancer-related fatigue (CRF) in breast cancer patients. Methods: Journal searching processes were performed through five electronic databases (ProQuest, Sciencedirect, PubMed, Google Scholar, and Sage Publications) and restricted from 2012 to 2022. The search strategy on the electronic databases was using terms like 'tai ji', 'tai chi', 'tumor necrosis factor-alpha', 'Interleukin-6', 'fatigue', 'pain', 'breast cancer', ‘biomarker’, 'TNF', 'TNF-a', and 'TNF-alpha'. The limitation of the study was only using academic journals and limited to Tai Chi training as an intervention. Reviewers obtained as many as 1.557 research journals. Then the reviewers analyzed the journal's contents obtained based on the eligibility of the topics, indexes or journal ratings through the Scimagojr website with journal criteria that met Q1 and Q2 quality, sample size, results from each journal, and limitations that occurred. Result: 15 journals were chosen to be reviewed and discussed about the intervention of Tai Chi, research methodology and study results using pain, fatigue and TNF-a biomarker. Conclusion: Tai Chi seems to be effective to reduce pain, cancer-related fatigue, and improves quality of life for breast cancer patients. accepts to TNF-a level plasma seems to be no different between usual care dan Tai chi practice. Moreover, it requires rigorous methodologies and randomized controlled trials to provide more reliable evidence.
