Abstract

Background: Speech nerve disorders due to stroke cause slurred speech. Lee Silverman Loud Voice Therapy can be applied using the Swanson technique to help stimulate the speech muscles. Purpose: To determine the effect of "a" vowel phonation training based on Lee Silverman Loud Voice Therapy on speech intelligibility in ischemic stroke patients. Method: This quasi-experimental study used a pretest-posttest non-equivalent control group approach, conducted from September to November 2024 at Dr. Achmad Mochtar Regional General Hospital, Bukittinggi. A purposive sampling method was used to select 22 participants, who were divided into intervention and control groups. Data were analyzed using a paired t-test with α ≤ 0.05. Results: In the intervention group, the average pretest score was 15 and the posttest score was 4.82. In the control group, the average pretest score was 16.91 and the posttest score was 16.45. A decrease in the average score indicates an improvement. The analysis showed an effect of "a" vowel phonation training with p = 0.001 (p ≤ 0.05). Conclusion: There is an effect of "a" vowel phonation training based on the Lee Silverman Voice Treatment Loud on speech intelligibility in ischemic stroke patients. A decrease in the mean speech intelligibility score indicates an increase in speech intelligibility. A lower score indicates better speech intelligibility. Suggestion: Future researchers should conduct further research with a larger sample size and implement the full training procedure, as well as considering other variables, such as severity, initial treatment time, stroke phase, other rehabilitation provided by healthcare facilities, comorbidities, family and friend support, and motivation.   Keywords: Ischemic Stroke; Lee Silverman Voice Treatment Loud; Speech Clarity; Vocal Phonation.   Pendahuluan: Gangguan saraf bicara akibat stroke menyebabkan ketidakjelasan bicara. lee silverman voice treatment loud dapat diterapkan dengan caring Swanson untuk membantu menstimulasi otot bicara. Tujuan: Untuk mengetahui pengaruh latihan fonasi vokal “a” berdasarkan lee silverman voice treatment loud terhadap kejelasan bicara pasien stroke iskemik. Metode: Penelitian quasi-eksperimen menggunakan pendekatan pretest posttest nonequivalent control group, dilakukan pada September-November 2024 di RSUD Dr. Achmad Mochtar Bukittinggi. Sampel diambil menggunakan purposive sampling dengan jumlah 22 partisipan yang terbagi dalam kelompok intervensi dan kontrol. Data dianalisis dengan T-Test (Paired-Test) α≤ 0.05. Hasil: Pada kelompok intervensi rata-rata ketika pre-test 15 dan post-test yaitu 4.82. Pada kelompok kontrol ketika pre-test yaitu 16.91 dan post-test yaitu 16.45. Nilai rata-rata yang menurun, menunjukkan perubahan yang membaik. Analisis menunjukkan terdapat pengaruh latihan fonasi vokal “a” dengan p = 0.001 (p ≤ 0.05). Simpulan: Terdapat pengaruh latihan fonasi vokal “a” berdasarkan Lee Silverman Voice Treatment Loud terhadap kejelasan bicara pasien stroke iskemik. Penurunan rata-rata menunjukkan nilai kejelasan yang membaik. Semakin turun nilai, maka semakin baik kejelasan bicara yang dialami. Saran: Bagi peneliti selanjutnya, dapat melakukan penelitian lebih lanjut dengan sampel yang lebih besar dan melakukan seluruh prosedur latihan serta memperhatikan variabel lain, seperti tingkat keparahan, waktu penanganan awal, fase stroke, rehabilitasi lain yang didapat dari faskes, penyakit komorbid, dukungan keluarga dan teman serta motivasi.   Kata Kunci: Fonasi Vokal; Kejelasan Bicara; Lee Silverman Voice Treatment Loud; Stroke Iskemik.

Abstract

Penggunaan zat adiktif seperti rokok, tembakau, dan alkohol masih menjadi masalah kesehatan di Sumatera Barat. Berdasarkan Survei Kesehatan Indonesia (SKI) 2023, dari 28.384 responden tercatat 24,8% merupakan perokok aktif, dengan 21,3% merokok setiap hari. Sementara itu, perilaku mengunyah tembakau hanya ditemukan pada 0,5% responden setiap hari, dan 97,4% tidak pernah melakukannya. Konsumsi alkohol tercatat sangat rendah, hanya 0,6% yang pernah mengonsumsi, sedangkan 99,4% tidak pernah. Kegiatan pengabdian masyarakat ini merupakan kelanjutan dari program tahun 2024 “Tungku Nan Tigo Sajarangan, Tali Nan Tigo Sapilin” yang berfokus pada pencegahan penyalahgunaan narkoba melalui komunitas RAJA BENAR di Bukittinggi. Tujuan kegiatan ini adalah memberdayakan komunitas RAJA BENAR yang telah terbentuk melalui pelaksanaan roadshow edukasi dan pembentukan Duta Gen-Berani sebagai upaya pencegahan penggunaan zat adiktif pada remaja di SMAN 4 Bukittinggi. Metode pelaksanaan meliputi analisis situasi dan advokasi mitra, edukasi melalui roadshow, penerapan teknologi dengan pembentukan Duta Gen-Berani serta posko Gen-Berani, pendampingan dan evaluasi menggunakan pengukuran pengetahuan pretest dan posttest. Tahapan kegiatan terdiri dari persiapan (koordinasi dan advokasi mitra), pelaksanaan roadshow pada 3 September 2025, diikuti 51 peserta yang terdiri dari siswa, kepala sekolah, guru, perwakilan Dinas Kesehatan, Polresta Bukittinggi, LKAAM, serta tim PkM UPN Bukittinggi. Rangkaian kegiatan mencakup pretest, pemaparan materi, diskusi panel, pelantikan Duta Gen-Berani yang disahkan melalui SK kepala sekolah, post-test, serta pendokumentasian testimoni. Evaluasi menunjukkan adanya peningkatan pengetahuan peserta dan testimoni positif dari siswa maupun pemangku kepentingan. Kegiatan ini menegaskan pentingnya pemberdayaan komunitas dan peran duta remaja dalam pencegahan penggunaan zat adiktif.

Abstract

Background: Patients undergoing Transurethral Resection of the Prostate frequently experience postoperative pain and anxiety during the early recovery phase. Non-pharmacological nursing interventions may help improve patient comfort and psychological stability during this period. Objective: This study aimed to examine the effect of the finger-holding technique on pain and anxiety among patients after Transurethral Resection of the Prostate. Methods: A quantitative quasi-experimental study with a nonequivalent control group design was conducted at Siti Fatimah Hospital Palembang from July to September 2025. Thirty-four postoperative patients were recruited using purposive sampling and divided into an intervention group (n=17) and a control group (n=17). The intervention group received the finger-holding relaxation technique twice daily for three days in addition to standard postoperative care, while the control group received standard care only. Pain intensity was measured using the Numeric Rating Scale, and anxiety levels were assessed using the Generalized Anxiety Disorder-7 scale. Data were analyzed using descriptive statistics and comparative tests with a significance level of p<0.05. Results: Both groups experienced reductions in pain and anxiety after the treatment period. The intervention group demonstrated a greater reduction in mean pain score from 6.47 to 2.06 compared with the control group, which decreased from 7.00 to 4.76. Anxiety scores in the intervention group decreased from 11.88 to 4.24, whereas the control group decreased from 11.53 to 7.18. Statistical analysis showed significant improvements in both groups, with a stronger effect observed in the intervention group. Conclusion: The finger-holding technique effectively reduces pain and anxiety in patients after Transurethral Resection of the Prostate and may serve as a simple complementary nursing intervention to improve postoperative comfort.

Abstract

Chronic Obstructive Pulmonary Disease (COPD) is a lung disease which has a chronic obstruction in airway that can interefere normal respiration led to death. Patients with COPD usually show symptoms such as dyspnea that are most frustrating in patients with COPD that can trigger the appearance of tightness and decreased saturation. The semi-fowler position is a position that can be chosen by patients who have difficulty breathing and in COPD patients is expected to optimize lung development so that oxygen saturation becomes increased. Given the many factors that contribute to oxygen saturation in patients, it is important to control the factors that affect oxygen saturation in patients who get a semi-fowler position. In addition, given the high prevalence of COPD, it is important for hospitals to identify factors that affect the length of day of treatment of patients with COPD. Identification of these factors can help health workers to plan treatment programs to plan the length of the day of treatment of patients who enter the hospital. The study aimed to identify factors that influence oxygen saturation and the length of days of treatment in COPD patients who get semi-fowler positions at Hermina Ciputat Hospital. Data collection was carried out from January 2020 to October 2021 using the cross-sectional method, namely by taking data at once at the same time through medical record data on 84 inpatients. The analysis test used in this study is the T-test and subsequently multivariate analysis is carried out using linear regression tests. Based on the analysis test, the factors that affect oxygen saturation are gender, smoking history, and initial saturation value. While the factors that affect the length of treatment are the history of smokers and the history of comorbid. Respondents who had a history of smoking would experience a decrease in oxygen saturation by 0.3888. Preventive interventions through smoking-related health education need to be done to reduce the severity in patients with COPD who are hospitalized.
