Abstract

Backgrounds and purpose: The road map of the National Health Insurance Scheme (JKN) aims to ensure that 75% of patients are satisfied with the quality of health facilities. This study was designed to determine differences in patients’ satisfaction in general, as well as at each individual first-level health facility (FKTP) and based on dimensions of quality. Methods: Study was a cross-sectional sample survey with the respondents of 148 JKN patients selected consecutively from 4 types of FKTP. A total of 4 health centers, 3 government clinics, 4 private clinics and 4 general practitioners randomly selected from 11 health centers, 7 government clinics, 13 private clinics and 70 general practitioners in the city of Denpasar. Data were collected through interviews over March-April 2015 in each health facility using questionnaires by trained interviewers. Data were analyzed by univariate, bivariate and multivariate. Results: A total of 69,59% of JKN patients said that they were satisfied with the quality of FKTP services. This figure was lower than the target of JKN i.e. 75%. The highest satisfaction (90,00%) was in the general practitioners while lowest was in the private clinics (47,46%). Based on the dimensions of quality, JKN patients’ satisfaction in the four FKTPs was significantly different (p=0,008) in physical and empathic dimensions (p=0,013). In the multivariate analysis, variables found related to the satisfaction of the respondents were the FKTP of general practitioners (adjusted PR=1,91; 95%CI: 1,05-3,47) and health centers (adjusted PR=1,76; 95%CI: 1,04-2,97). Conclusion: In general, the patients’ satisfaction rate was still lower than the target of JKN and when viewed per care facilities, the highest satisfaction was found in FKTP of general practitioners and health centers.

Abstract

Background and purpose: Employees satisfaction can improve the work performance of employees that can create a positive situation in the organizational environment. Previous studies have shown that leadership, motivation and compensation related to employee satisfaction but has not been able to explain the association of leadership, motivation and compensation together with employee satisfaction. This study aims to determine the relationship of leadership, motivation, compensation and employees satisfaction in primary health centers, Denpasar. Methods: This study was cross-sectional survey among all employees consist of 39 employees in Puskesmas II Denpasar Selatan and 36 employees in Puskesmas III Denpasar Utara. Data were collected using a questionnaire and analyzed using logistic regression to examine the association between leadership, motivation, compensation and employees satisfaction in primary health centers, Denpasar. Results: Most employees perceived lack in leadership of the head of primary health centers (52.00%), had a high motivation to work (56.00%), received adequate compensation (60.00%) and satisfied with their work (52.00%). Multivariate analysis showed that two variables had significant association, that were leadership with OR=7.28 (95%CI:=2.17-24.46) and motivation with OR=4.31 (95%CI: 1.29-14.39). Conclusion: Factors associated to the employees satisfaction in primary health centers were leadership and motivation. Aspects of leader behavior to motivate employees and extrinsic motivation primarily on working conditions need to be improved to create employees satisfaction.

Abstract

Background and purpose: Medical nutrition therapy at Sanglah Hospital, Denpasar consists of diet prescription, food labelling, food portion, and food serving. Nutrition assessment has been carried out based on the minimum service standards of Sanglah Hospital Denpasar in 2013, but agreement assessment of energy and protein contents has never been performed in the provision of medical nutrition therapy. The study was conducted to determine the compatibility of energy and protein contents of the diet prescription until the food is served in hospitalized patients who received medical nutrition therapy at Sanglah Hospital Denpasar. Methods: This is a cross-sectional study involving 50 samples of food served with prescribed diet by clinical nutrition specialists at Sanglah Hospital selected by consecutive sampling. Data were collected using observation sheets filled by surveyors and analyzed using Bland Altman Test. Results: There was compatibility of the contents of energy and protein in the provision of medical nutrition therapy of diet prescription with food labelling (energy r=0.43; p=0.04, protein r=0.48; p=0.02), food labelling with food portion (energy and protein r=1.00; p=0.00), food portion with the food serving (energy r=0.48; p=0.03, protein r=0.50; p=0.01), and from diet prescription until the food is served (energy r=0.52; p=0.04, protein r=0.57; p=0.02). Although there is a strong agreement, there is mean value difference which is clinically acceptable. Conclusion: Energy and protein content in the prescribed diet until the food is served at Sanglah Hospital were compatible. The medical nutrition therapy is appropriate and should be evaluated continuously to improve the quality of services at the tertiary level.

Abstract

Introduction: The development of dental tourism has grown globally. Dental tourism is a search for dental and oral health care in other countries. Bali has the potential to develop the dental tourism industry. The purpose of this study was to determine the knowledge and attitudes of foreign tourists in Bali towards dental tourism. Method: A cross-sectional descriptive study has been carried out with a sample of 98 foreign tourists at International Departure Terminal I Gusti Ngurah Rai Airport. Respondents were given questionnaires with closed questions to evaluate their knowledge and attitudes towards dental tourism. The data were obtained then analyzed descriptively and displayed in the form of a frequency distribution table. Results: As many as 46.9% of respondents had heard of dental tourism, while only 15% knew of dental tourism services in Bali. The dominant source of information is by word of mouth. Most respondents only knew two factors driving dental tourism. Respondents knew all the treatment options available in the questionnaire. Respondent’s attitude towards dental tourism shows uncertainty. As many as 68.4% of respondents said they were not interested in dental tourism, of which 76.2% said they were satisfied with care in their home countries. Conclusion: In general, foreign tourist’s knowledge of dental tourism was low. The general attitude of tourists to dental tourism shows uncertainty and interest in low dental tourism. It is recommended to increase promotion effectively and creatively by utilizing internet to support the potential of dental tourism. Latar Belakang: Perkembangan dental tourism telah berkembang secara global. Dental tourism adalah perilaku pencarian perawatan kesehatan gigi dan mulut di negara lain. Bali sangat berpotensi untuk mengembangkan industri dental tourism. Tujuan dari penelitian ini adalah untuk mengetahui pengetahuan dan sikap wisatawan asing di Bali terhadap dental tourism. Metode: Telah dilakukan penelitian deskriptif cross-sectional dengan sampel 98 wisatawan asing di Terminal Keberangkatan Internasional Bandar Udara I Gusti Ngurah Rai. Responden diberikan kuesioner dengan pertanyaan tertutup untuk mengevaluasi pengetahuan dan sikap mereka terhadap dental tourism. Data yang diperoleh kemudian dianalisis secara deskriptif dan ditampilkan dalam bentuk tabel distribusi frekuensi. Hasil: Sebanyak 46,9% responden pernah mendengar mengenai dental tourism, sementara hanya 15% yang mengetahui layanan dental tourism di Bali. Sumber informasi dominan dari mulut ke mulut. Sebagian besar responden hanya mengetahui dua faktor pendorong dental tourism. Responden telah mengetahui seluruh pilihan perawatan yang umum dilakukan pada dental tourism. Sikap responden terhadap dental tourism menunjukan ketidakyakinan. Sebanyak 68,4% responden mengaku tidak tertarik dengan dental tourism, dimana 76,2% diantaranya mengaku puas dengan perawatan di negara asal mereka. Kesimpulan: Secara umum, pengetahuan wisatawan asing terhadap dental tourism rendah. Sikap secara umum wisatawan terhadap dental tourism menunjukan ketidakyakinan dan ketertarikan terhadap dental tourism rendah. Disarankan untuk meningkatkan promosi secara efektif dan kreatif dengan memanfaatkan media internet untuk mendukung potensi dental tourism.

Abstract

An integrated primary care using a life course approach for preventing and controlling NCDs in Indonesia is the right policy decision. It strives to be holistic, addressing NCDs with a long-term vision across the critical stages where modifications can be made to minimise risks of developing NCDs. This policy direction is inherently complex mandating multi-sectoral and/or inter-sectoral approaches, as well as multi-disciplinary teams. Additionally, existing evidence suggests the intricate connections between health, social, economic, and environmental across the lifespan7. Taking a life course approach also means the governments must extend the NCD prevention and control measures beyond health sector alone. This means that effective implementation of an integrated primary care using a life course approach require long-term strategies to strengthening multi-sectoral partnerships to promote a life course approach across all government sectors and community systems.

Abstract

Women infected with HIV can transmit HIV from mother to child during pregnancy, childbirth and lactation. Implementation of programs to prevent HIV transmission from mother to child (PPIA) in Bangli not been implemented optimally. The study was conducted to determine the chances of program implementation PPIA, faced viewed from the perspective of users and service providers. The study is a qualitative case study approach. Collecting data with in-depth interviews using interview guide to users and service providers. Informants were selected purposively as many as 18 people of 10 pregnant women, two counselors, two laboratory workers, 2 heads of health centers, 1 holder of program P2, and 1 Sekretais KPA Bangli regency. Data collection was conducted April-May 2016 in Puskesmas Losses Tembuku I and II health center in Bangli regency. Analysis of data using thematic analysis. Opportunities from service users say that husbands and health professionals strongly support pregnant women to take the test. Service providers revealed no support from the public, especially health workers and community leaders in collecting pregnant women.Opportunities users is the support of their husbands and health professionals, while opportunities are providers of community support and cooperation within the team

Abstract

Since 2014, there has been an increase in funds for primary health care (PHC) coming from the National Health Insurance program capitation funds and the Health Operational Assistance Fund. This study aimed to explore the effect of this budget increase on the health care services at PHC. The case study used a qualitative approach and interviews from 19 PHC health workers with the highest and lowest budget. Data were analyzed using thematic analysis and supported by quantitative data. The positive impact of the increasing PHC budget was felt by PHC staff due to the addition of operational equipment and incentives provided. Nevertheless, there was also a negative impact such as feeling overwhelmed due to an increase in the quantity of PHC activities and additional administrative affairs. It also triggered a negative interaction between staff due to the number of incentives received. The regulation on the use of budget empowers PHC to better arrange the schedule of activities and manage human resources. However, these regulations are considered restrictive and the administrative flow of funds is too long that hinder the optimal use of the budget.

Abstract

Kebijakan pusat pelayanan kesehatan masyarakat (puskesmas) sebagai Badan Layanan Umum Daerah (BLUD) diimplentasikan untuk meningkatkan kualitas pelayanan kesehatan dasar. Kebijakan BLUD puskesmas telah diterapkan di Kabupaten Gianyar sejak tahun 2010 dan berlaku pada puskesmas perawatan maupun nonperawatan. Pelaksanaan BLUD puskesmas tidak selalu meningkatkan mutu layanan. Penelitian ini bertujuan mengetahui gambaran mutu pelayanan puskesmas perawatan yang berstatus BLUD di Kabupaten Gianyar. Penelitian ini merupakan penelitian potong lintang dengan pendekatan kuantitatif dan kualitatif. Tempat dan waktu penelitian ini dilakukan di Kabupaten Gianyar, Agustus hingga Desember 2013. Data kuantitatif dikumpulkan melalui penyebaran kuesioner kepada 105 pengguna layanan di empat puskesmas yang dipilih dengan cara multistage random sampling. Data kualitatif dikumpulkan melalui wawancara mendalam kepada 13 penyedia pelayanan kesehatan yang dipilih secara purposive sampling. Data kuantitatif dianalisis secara deskriptif dan data kualitatif dianalisis dengan analisis tematik. Dari hasil penelitian, pelayanan di puskesmas perawatan berstatus BLUD di Gianyar dinilai kurang memuaskan karena keterbatasan peralatan medis dan kurangnya tenaga yang kompeten dalam pengelolaan keuangan. Pelatihan pengelolaan keuangan pada staf puskesmas dan perekrutan tenaga berlatar belakang akuntansi penting untuk dilakukan. The policy of primary health care as local public service agencies (BLUD) was established to improve the quality of basic health care services. The public service agencies primary health care policy has been implemented to all primary health care in Gianyar district since 2010. The implementation does not always improving health service quality. This research was aimed to overview the quality of services in primary health care with general services agency status in Gianyar district. This research was a crosssectional study with mixed of quantitative and qualitative approaches.This research was conducted in Gianyar between August and December 2013. The quantitative data was collected through questionaire survey to 105 patients in four primary health care who were chosen with multistage random sampling technique. The qualitative data was collected through in-depth interviews to 13 health care providers in primary health care who were chosen with pusposive sampling. The quantitative data was analysed descriptively and the qualitative data was analysed using thematic analysis. The result of the study was primary health care quality in BLUD puskesmas with inpatient services was perceived as poor due to the limited availability of medical equipment and lack of staff who major in financial management. Training on financial management and recruitment of staff with accounting background should be conducted.

Abstract

Pekerja seks perempuan (PSP) merupakan kelompok yang termarginalkan secara sosial dan memiliki kerentanan yang tinggi terhadap masalah kesehatan. Upaya perluasan Jaminan Kesehatan Nasional (JKN) pada PSP masih terbatas sehingga penting dilakukan untuk mendukung pencapaian universal health coverage. Penelitian ini bertujuan untuk memperoleh gambaran mengenai akses JKN pada PSP di Denpasar. Penelitian ini merupakan studi kualitatif. Data dikumpulkan melalui wawancara mendalam terhadap 15 orang PSP dan empat orang mucikari di Denpasar pada Agustus hingga Oktober 2014. Hasil wawancara diolah dengan analisis tematik. Kerangka analisis yang digunakan adalah The Health Access Livelihood Framework. Kepemilikan JKN pada PSP di Denpasar masih rendah, meskipun sebagian PSP memiliki kemauan untuk menjadi peserta JKN dan memiliki kemampuan membayar iuran JKN. Faktor penghambat akses JKN pada PSP adalah rendahnya pengetahuan mengenai prosedur pendaftaran dan portabilitas JKN, kekhawatiran keberlanjutan pembayaran iuran, persepsi buruk mengenai kualitas layanan yang akan diterima jika menggunakan JKN, ketidaklengkapan administrasi kependudukan serta kebijakan yang mengharuskan peserta bukan penerima bantuan iuran (Non-PBI) Mandiri untuk mendaftarkan seluruh anggota keluarga. Akses JKN pada PSP terhambat oleh faktor-faktor individual, layanan dan kebijakan yang perlu diatasi untuk meningkatkan cakupan JKN pada PSP. Female sex workers (FSW) is marginalized social group having a high vulnerability of health problems. Effort to expand national health insurance on FSW is still limited, so it is necessarily performed in order to support the achievement of universal health coverage. This study aimed to obtain the depiction of the insurance access among FSW in Denpasar. This study was qualitative. Data was collected through in-depth interview of 15 FSW and four pimps in Denpasar from August to October 2014. The interview result was analyzed using thematic analysis. The analysis framework used was The Health Access Livelihood Framework. The insurance ownership among FSW in Denpasar was low, even though some FSW were willing to be participants and afford to pay the premium. Factors inhibiting the insurance access were the lack of knowledge regarding registration procedures and portability, fear of premium payment sustainability, negative perceptions of quality of services that would be received if using the insurance, incomplete population administration and policy requiring independent non-premium support receiver participants to register all of their family members. The insurance access among FSW was hindered by individual, service and policy factors that need to be conquered to increase the insurance coverage among FSW.
