Abstract

Heart failure is a clinical syndrome indicated by the lack of ability of the heart to supply enough blood to the peripheral tissues to fulfill the body's metabolic demands. In some cases, heart failure is usually accompanied by other pathological conditions where the process occurs at the same time as other diseases called comorbidities. In addition to the underlying disease, this disease is also a trigger, a risk factor, and a complication that further aggravates the condition of heart failure. Comorbidities are commonly found in elderly heart failure patients, resulting from the failure of several organ functions. However, the distribution of these comorbids varies in each age group. Uncontrolled hypertension causes complications of heart failure which is also a comorbid of heart failure itself. This study aims to determine the distribution of the incidence of heart failure patients with comorbid hypertension at Ulin Hospital Banjarmasin. This study used a retrospective descriptive research design with amethod total sampling by taking medical record data of heart failure patients who underwent outpatient treatment at Ulin Hospital Banjarmasin for the time range of February 2020-July 2020. From 188 heart failure patients during the time range of February 2020-July 2020, obtained as many as 84 (44.68%) patients with heart failure accompanied by hypertension. Based on the results of echocardiography, the most classifications were diastolic heart failure in 47 patients (55.95%), systolic heart failure in 21 patients (25%), and combination systolic diastolic heart failure in 16 patients (19.05%). The highest prevalence was in men 59 (70.24%). Conclusion: In patients with heart failure accompanied by comorbid hypertension, the most common finding is left ventricular diastolic heart failure, which predominantly affects male patients

Abstract

Abstract: Risk stratification of Coronavirus disease 2019 (COVID-19) patients is very important to determine patient care and prognosis. The reliable parameters include C-reactive protein (CRP), lactate dehydrogenase (LDH), and neutrophil-lymphocyte ratio (NLR). This study aimed to determine the relationship between CRP, LDH, and NLR levels with the severity of COVID-19 patients. An observational study with a cross-sectional approach was conducted at Ulin Hospital Banjarmasin on 91 eligible subjects. A logistic regression test was conducted to determine the relationship between variables and the severity of COVID-19. The results showed that there was no significant relationship between CRP levels and the severity of COVID-19 (prevalence odds ratio (POR) 0.85, 95% confidence interval (CI 95%) 0.34-2.09, p=0.72). There was a significant difference between the mean LDH levels in severe COVID-19 compared to non-severe COVID-19 (1006 + 601.77 vs 543.5 + 480.41, p<0.001), with POR 4.23, 95% CI 95% 1.56-11.45, p=0.005; there was also a significant difference between the mean NLR values in severe COVID-19 compared to non-severe COVID-19 (9.01 + 6.32 vs 5.38 + 5.03, p=0.002) with POR of 12.21 CI 95% 3.80-39.31, p<0.001. This study showed that LDH levels and NLR values were important predictors of COVID-19 severity.

Abstract

Background: The 2018 Riskesdas report revealed that the incidence of chronic kidney disease (CKD) affects 0.38% of Indonesia’s population. Anemia frequently complicates CKD, especially in patients undergoing regular hemodialysis. Reticulocyte hemoglobin equivalent (RET-He) assesses hemoglobin content in reticulocytes, indicating iron availability for erythropoiesis in the bone marrow. CKD significantly impacts patients' socio-economic status, heightening morbidity and mortality while diminishing quality of life. The relationship between RET-He and the quality of life in CKD patients on routine hemodialysis remains unclear. Objective: To investigate the correlation between RET-He and quality of life in CKD patients undergoing routine hemodialysis. Methods: A cross-sectional observational analytical study with data from medical records of CKD patients receiving routine hemodialysis between October 1st and October 30th, 2021. The Spearman correlation test was used for analysis. Results: This study included 92 patients, consisting of 40 males and 52 females, with a median age of 50 years and RET-He values at 31. Quality of life metrics included physical function (800, range: 0-1000), physical limitations (300, range: 0-400), body pain (175, range: 75-255), general health (475, range: 225-600), vitality (320, range: 200-400), social function (175, range: 75-200), emotional limitations (300, range: 100-300), and mental health (380, range: 160-500). The correlation analysis revealed no significant relationships: physical function (p=0.359), physical limitations (p=0.813), body pain (p=0.373), general health (p=0.547), vitality (p=0.616), social function (p=0.828), emotional limitations (p=0.482), and mental health (p=0.136). Conclusion: There is no significant correlation between RET-He levels and the quality of life in CKD patients undergoing regular hemodialysis.

Abstract

Abstract: Chronic Kidney Disease (CKD) is defined as kidney damage or a decrease in the gromerular filtration rate that occurs for 3 months or more, manifesting as kidney damage and requiring kidney replacement therapy, namely hemodialysis. CKD sufferers often experience an energy imbalance due to anorexia which increases energy needs while energy reserves are depleted. Muscle mass levels function as bone moving organs. Muscle mass will be measured using the BIA (Bioimpedance Analysis). Low muscle mass is often associated with low quality of life. Quality of life is an assessment of several aspects of life. Hemodialysis therapy will affect quality of life including lack of energy, fatigue and stress. Quality of life can be measured using the SF-36 questionnaire. This study was to regulate the correlation among muscle mass levels and the level of quality of life of routine hemodialysis patients at RSUD Ulin Banjarmasin. The research method uses analytical observational with a cross-sectional approach. The technique for taking subjects was simple random sampling of 60 subjects. Data collection was carried out by direct measurements and interviews of muscle mass levels and quality of life. Data analysis used the Komolgorov-Smirnov normality test and continued with test called Pearson correlation test. The results of data analysis showed a strong significant correlation (p=0.000, r=0.706). The results of the study concluded that muscle mass levels had a significant correlation with the level of quality of life of hemodialysis patients at RSUD Ulin Banjarmasin. Keywords: Muscle mass, quality of life, hemodialysis, BIA (Bioimpedance analysis), questionnaire QOL Short Form-36. Abstrak: Penyakit ginjal kronis (PGK) didefinisikan sebagai kerusakan ginjal atau penurunan laju filtrasi gromerulus yang terjadi selama 3 bulan atau lebih bermanifestasi sebagai kerusakan ginjal dan memerlukan terapi pengganti ginjal yaitu hemodialisis. Penderita PGK sering mengalami ketidakseimbangan energi akibat anoreksia yang meningkatkan kebutuhan energi sementara asupan energi cadangan habis. Kadar massa otot berfungsi sebagai organ penggerak tulang. Massa otot dapat diukur salah satunya dengan menggunakan alat BIA (Bioimpedance analysis). Rendahnya massa otot kerap dihubungkan dengan rendahnya kualitas hidup. Kualitas hidup merupakan penilaian beberapa aspek dalam kehidupan. Terapi hemodialisis akan mempengaruhi kualitas hidup meliputi kekurangan energi, kelelahan, dan stress. Kualitas hidup dapat diukur salah satunya menggunakan kuesioner SF-36. Tujuan penelitian ini adalah untuk mengetahui korelasi kadar massa otot dan kualitas hidup pasien hemodialisis rutin di RSUD Ulin Banjarmasin. Metode penelitian menggunakan observasional analitik dengan pendekatan cross-sectional. Teknik pengambilan subjek dengan simple random sampling sebanyak 60 subjek. Pengambilan data dilakukan dengan pengukuran dan wawancara langsung kadar massa otot dan kualitas hidup. Analisis data menggunakan uji normalitas Komolgorov-Smirnov dan dilanjutkan dengan uji korelasi Pearson. Hasil analisis data terdapat korelasi bermakna kuat (p=0,000, r=0,706). Hasil penelitian menyimpulkan kadar massa otot terdapat korelasi bermakna dengan kualitas hidup pasien hemodialis di RSUD Ulin Banjarmasin. Kata-kata kunci: Massa otot, kualitas hidup, hemodialisis, BIA (bioimpedance analysis), kuesioner QOL Short form-36.

Abstract

Abstract: Hemodialysis (HD) is one of the treatments for chronic kidney disease (CKD). CKD patients can experience increased permeability at the glomerular level. This can cause a decrease in albumin levels and handgrip strength (HGS). This study aimed to determine the relationship between albumin levels and handgrip strength in patients undergoing routine hemodialysis at RSUD Ulin Banjarmasin. This study type is analytical observational with a cross-sectional design. Subjects were selected by a simple random sampling method. Albumin levels were measured in the clinical pathology laboratory, while HGS was measured using a Camry electronic hand dynamometer model EH101. The research subjects were patients aged 18-60 years who underwent routine hemodialysis twice a week for at least 3 months. The research results from 72 subjects showed that the female gender group, age 51-60 years, and HD duration of 3-36 months were the most common. The proportion of normal albumin levels was 81%, while hypoalbuminemia was 19%. The proportion of normal HGS was 17%, while 83% were weak. The Fisher's Exact test results showed no relationship (p=0.439) between albumin levels and handgrip strength in patients undergoing hemodialysis. Keywords: albumin levels, handgrip strength, hemodialysis Abstrak: Hemodialisis (HD) merupakan salah satu terapi terapi penyakit ginjal kronik (PGK). Pasien PGK dapat megalami peningkatan permeabilitas di glomerulus. Hal itu dapat menyebabkan penurunan kadar abumin dan kekuatan otot genggaman. Tujuan penelitian ini yaitu untuk mengetahui hubungan kadar albumin dan kekuatan otot genggaman pada pasien yang menjalani hemodialisis rutin di RSUD Ulin Banjarmasin. Jenis penelitian ini berupa analitik observasional dengan rancangan cross sectional. Pengambilan sampel dilakukan dengan simple random sampling. Kadar albumin diukur di laboratorium patologi klinik, sedangkan kekuatan otot genggaman diukur menggunakan camry electronic hand dynamometer model EH101. Subjek penelitian adalah pasien berusia 18-60 tahun yang menjalani hemodialis rutin 2 kali dalam seminggu minimal 3 bulan. Hasil penelitian dari 72 subjek menunjukkan bahwa kelompok jenis kelamin perempuan, usia 51-60 tahun, dan lama HD 3-36 bulan paling banyak ditemukan. Proporsi kadar albumin normal sebesar 81%, sedangkan hipoalbuminemia sebesar 19%. Proporsi kekuatan otot genggaman normal sebesar 17%, sedangkan lemah sebesar 83%. Hasil uji Fisher’s exact menunjukkan bahwa tidak terdapat hubungan (p=0.439) kadar albumin dan kekuatan otot genggaman pada pasien yang menjalani hemodialisis. Kata-kata kunci: albumin, kekuatan otot genggaman, hemodialisis

Abstract

COVID-19 mortality is much higher in members of the population with multiple co-morbidities. Anemia is independently associated with all-cause mortality in patients hospitalized with COVID-19. C-Reactive Protein (CRP) levels are increased in patients with renal failure and are an independent predictor of cardiovascular mortality. This study used an observational case-control method. Data were taken consecutively using medical records of patients who died as a case group and lived as a control group with chronic kidney disease who underwent routine HD confirmed with COVID-19 at Ulin General Hospital, Banjarmasin. The number of subjects in this study was 92 patients. High CRP levels (>30 mg/L) significantly increased the risk of death by 4 times (OR 4.330; 95% CI 1.800-10.416; p = 0.001). Low hemoglobin levels (<10 g/dl) did not affect mortality in routine hemodialysis patients infected with COVID-19 (OR 0.643; 95% CI 0.281-1.471; p = 0.294). An increased CRP in COVID-19 patients is thought to indicate extensive inflammation in the infected lungs, then the condition of CKD patients undergoing routine hemodialysis with compromised immunity increases the risk of severity and mortality in this population. There is an effect of CRP levels on death from COVID-19 in Chronic Kidney Disease patients undergoing routine hemodialysis at Ulin General Hospital, Banjarmasin.

Abstract

Abstract: Hemodialysis is a therapy for patients with kidney disease, both acute and chronic, which is used as a substitute for decreased kidney function. Body mass index (BMI) is a simple measuring tool to see the nutritional status of a person, both men and women. Blood pressure is the process when blood is distributed by the heart throughout the body. The research aims to determine whether there is a relationship between body mass index and blood pressure in patients undergoing routine hemodialysis at Ulin Regional Hospital, Banjarmasin. The method in this research is analytical observational with a cross sectional research design with the Pearson test. This study used patient subjects undergoing routine hemodialysis at the hemodialysis installation at Ulin Banjarmasin Regional Hospital in November 2023 using a simple random sampling technique according to the inclusion and exclusion criteria. Primary data was collected by measuring height, weight and blood pressure and secondary data in the form of numbers. Patient medical records obtained from the hemodialysis installation at Ulin Banjarmasin Regional Hospital. This research uses correlative data analysis techniques which begin with a normality test using the Kolmogorov-Smirnov technique because the number of subjects in this research was 50 people with a subject distribution of 65 people. The results of this study obtained a correlation value for body mass index and blood pressure with a value of p = 0.000 and a value of r = 0.06. Keywords: hemodialysis, chronic renal failure, acute renal failure, body mass index, blood pressure. Abstrak: Hemodialisis merupakan terapi untuk pasien penyakit ginjal baik yang akut maupun kronik yang digunakan sebagai pengganti fungsi ginjal yang menurun. Indeks massa tubuh (IMT) ialah alat ukur sederhana untuk melihat status gizi seseorang baik laki-laki maupun perempuan. Tekanan darah merupakan proses ketika darah dialirkan oleh jantung keseluruh  tubuh. Penelitian memiliki tujuan untuk mengetahui apakah ada hubungan antara indeks massa tubuh dan tekanan darah pada pasien yang menjalani hemodialisis rutin di RSUD Ulin Banjarmasin. Metode pada penelitian ini yaitu observasional analitik dengan desain penelitian cross sectional dengan uji pearsone. Penelitian ini menggunakan subjek pasien yang menjalani hemodialisis rutin di instalansi hemodialisis RSUD Ulin Banjarmasin pada November 2023 dengan menggunakan teknik simple random sampling sesuai kriteria inklusi dan eksklusi. Pengambilan data primer dilakukan dengan melakukan pengukuran tinggi badan, berat badan serta tekanan darah dan data sekunder berupa nomor rekam medik pasien yang didapatkan dari instalasi hemodialisis RSUD Ulin Banjarmasin. Penelitian ini menggunakan teknik analisis data korelatif yang diawali dengan uji normalitas dengan teknik kolmogorov- Smirnov dikarenakan jumlah subjek dalam penelitian ini sebanyak 50 orang dengan distribusi subjek sebanyak 65 orang. Hasil penelitian ini didapatkan nilai korelasi indeks massa tubuh dan tekanan darah dengan nilai p = 0,000 dan nilai r = 0,06. Kata-kata kunci: hemodialisis, gagal ginjal kronik, gagal ginjal akut, indeks massa tubuh, tekanan darah

Abstract

Abstract: This research focuses on the relationship between depression and quality of life in hemodialysis patients at Ulin Regional Hospital, Banjarmasin. Chronic renal failure (CKD) is a progressive and irreversible failure in kidney function that affects various aspects of metabolism, fluid balance and electrolytes. Depression, as an ongoing emotional condition, affects a person's thoughts, emotions and behavior holistically. Deep mood generally reflects feelings of loss of hope. This study used the PHQ-9 to measure depressive symptoms and the QOL-SF36 to assess quality of life in hemodialysis patients. The sample for this study consisted of patients undergoing hemodialysis at the Ulin Regional Hospital, Banjarmasin. The research method was used by analytical observational with a cross sectional approach. The results showed a significant relationship between depression and quality of life (p = 0.04), with a weak negative correlation of -0.261. This confirms the correlation between depression and decreased quality of life in hemodialysis patients at Ulin Hospital, Banjarmasin. The conclusions of this study indicate that depression in these patients has an impact on reducing their quality of life. Keywords: depression, quality of life, hemodialysis, PHQ-9, QOL-SF-36.  Abstrak: Penelitian ini fokus pada hubungan antara depresi dan kualitas hidup pada pasien hemodialisis di RSUD Ulin Banjarmasin. Gagal ginjal kronik (GGK) merupakan kegagalan progresif dan tak terbalik pada fungsi ginjal yang mempengaruhi berbagai aspek metabolisme, keseimbangan cairan, dan elektrolit. Depresi, sebagai kondisi emosional yang berkelanjutan, memengaruhi pikiran, emosi, dan perilaku seseorang secara menyeluruh. Mood dalam umumnya mencerminkan perasaan kehilangan harapan. Studi ini menggunakan PHQ-9 untuk mengukur gejala depresi dan QOL-SF36 untuk menilai kualitas hidup pada pasien hemodialisis. Sampel studi ini terdiri dari pasien yang menjalani hemodialisis di instalasi RSUD Ulin Banjarmasin. Metode penelitian yang digunakan adalah observasional analitik dengan desain pendekatan cross sectional. Hasil penelitian menunjukkan signifikansi hubungan antara depresi dan kualitas hidup (p = 0,04), dengan korelasi negatif lemah sebesar -0,261. Ini menegaskan adanya korelasi antara depresi dan penurunan kualitas hidup pada pasien hemodialisis di RSUD Ulin Banjarmasin. Kesimpulan studi ini mengindikasikan bahwa depresi pada pasien tersebut berdampak pada menurunnya kualitas hidup mereka. Kata-kata kunci: depresi, kualitas hidup, hemodialisis, PHQ-9, QOL-SF-36

Abstract

Background: COVID-19 is a caused a pandemic since early January 2020. Sequential Organ Failure Assessment (SOFA) score can assess organ dysfunction in the state of Sepsis MODS. Aim: This study aims to determine the relationship between Blood Sugar Levels, C-Reactive Protein (CRP), and Total Lymphocyte Count (TLC) with SOFA score in COVID-19 patients with Sepsis. Methods: An observational study using the cross-sectional method was carried out in 2020-2021 at Ulin Hospital Banjarmasin with a total sample of 87 participants. Results: Based on the Spearman test, there were a positive correlation between Random Blood Sugar levels and SOFA with the scores r = 0.778 (p = 0.000); significant negative correlation between TLC and SOFA with the score r = -0.409 (p = 0.000); and no significant correlation between CRP and SOFA with the score r = -0.077 (p = 0.507). The multivariate test showed that the correlation value for each Random Blood Sugar with a SOFA score is R=0.664 (p=0.000), and the Total Lymphocyte Count with a SOFA score is R=-0.270 (p=0.001). The statistical analysis model revealed that Blood Sugar Levels and Total Lymphocyte Count could predict SOFA scores of as much as 60.9%. Conclusion: Blood sugar levels can be a marker of poor prognosis in COVID-19 patients with Sepsis. Hyperglycemia can be caused by pancreatic cell damage due to SARS-CoV2 invasion, acute insulin resistance, and inflammation caused by Sepsis and COVID-19. Total Lymphocyte Count is one of the many factors contributing to organ dysfunction caused by Sepsis in COVID-19. Keywords: COVID-19, Sepsis, SOFA Score, Prognosis

Abstract

Garcinia forbesii King, an endemic plant from Sumatra and Kalimantan, is traditionally used for treating skin infections and inflammation. Its potential use as a raw material for medicines needs to be supported by safety tests. The purpose of this study is to assess the acute toxicity of its 70% leaf extract in male Wistar rats, evaluating clinical symptoms, body weight changes, organ indices, and the lethal dose (LD50). The study administered extract doses orally at 5 (G2), 50 (G3), 300 (G4), and 2,000 (G5) mg/kg BW, monitoring toxicity signs over 14 days. Symptoms such as piloerection, respiratory distress, and soft feces were noted. Significant weight loss was observed at doses of 2,000 mg/kg BW. There was no significant effect on the kidney index, but the liver and heart indices exhibited significantly lower changes compared to the control. The LD50 value was estimated to be between 2,000-5,000 mg/kg BW. Further studies are recommended to assess histopathological effects and identify harmful chemicals in the plant.
