Abstract

BACKGROUND: One type of thalassemia known as transfusion-dependent thalassemia (TDT) requires a lifelong blood transfusion. Iron overload from prolonged blood transfusions causes nutritional disorders and several organs impairment, including the testicles. In contrast to an orchidometer, ultrasound shows more details of the testicles. This study aimed to compare testicular volumes based on ultrasound and orchidometer measurements and correlate testicular volume with nutritional status in TDT patients.METHODS: A cross-sectional study including 27 male thalassemia subjects who underwent routine check-ups was conducted. Measurements of testicular volume using an orchidometer and ultrasound, and body mass index (BMI) was performed. Ultrasound measurement of testicular volume was calculated using the Lambert Formula, while the orchidometer was performed by matching the bead next to the testicle.RESULTS: Most subjects were underweight (63%) or normal weight (37%). However, no significant correlation between BMI and testicular volume was found. As measured using an orchidometer, the mean volume of the right and left testes were 9.22±4.92 mL and 8.67±4.45 mL, respectively. Ultrasound measurement showed the mean values of the right and left testicles were 7.68±3.96 mL and 7.09±4.03 mL, respectively. Testicular measurements obtained using ultrasound showed a significant difference (p<0.05) when compared with the orchidometer. The testicle measured by ultrasound was smaller, which was more accurate. However, the comparison between the right and the left testicular volumes showed no significance.CONCLUSION: Thalassemia patients are mainly underweight, and their nutritional status have no relationship with testicular volume. Ultrasound, as compared to an orchidometer, is better for measuring testicular volume.KEYWORDS: thalassemia, testicle, orchidometer, ultrasound, body mass index

Abstract

Scalp tumors are quite rare, only 1-2% are malignant and have a tendency for intracranial extension and invasion. This extension can lead to various neurological complications, such as cancer pain, increased intracranial pressure (ICP), and other neurological deficits. A 37-year-old woman presented with decreased consciousness since 2 days earlier. The patient had a history of lump on the top of her head since 4 years, repeated 3 times after resection without previous treatment. On the third recurrence, the tumor was enlarged until it some part of the tumor can be seen depressed against the skull and this part was painful. The pain worsened throughout the entire head accompanied by visual field disturbance of right inferior homonymous quadranopia, right central facial nerve paresis, and right hemiparesis. Contrast head CT scan revealed lobulated heterogeneous lesion in the parietooccipital region, extending to the extraaxial and intracerebral, accompanied by vasogenic edema with destruction of the parietal and occipital bones. The result of histopathology is trichilemmal carcinoma. The patient received morphine drip therapy 10mg/24 hours for the cancer pain, which was then replaced with its patch equivalent dose, dexamethasone 2x10mg IV tapering off gradually according to the clinical condition, gabapentin 2x300mg, and wholebrain radiotherapy (WBRT) 30 Gy in 10 sessions. The patient was discharged in conscious condition and her pain resolved to mild pain. Keywords: cancer pain, increased intracranial pressure, intracranial invasion, scalp tumor

Abstract

Centrifugal pump is one of the most important components that is widely used by industry and functions to move liquids in the form of water or other liquid. One of the things that often happens on centrifugal pumps is the occurrence of high vibrations and abnormal sounds. High vibration in the pump is something that should be anticipated because it can cause damage to the bearings, seals, and shafts. If the pump continues to operate in this condition, it will also cause damage to other pump components and cause the pump to stop operating. Therefore, the condition of the pump must be monitored regularly to ensure that the pump can operate properly. One of the most frequently used methods to find out where high vibrations come from is to use the aptitude analyst application that will produce a unique vibrational spectrum. Vibration analysis is done by observing the amplitude and frequency of the vibration signal. Vibration spectrum analysis can detect pump damage without having to disassemble the equipment and can provide early warning before major damage occurs and takes a large repair cost. The results showed that the existence of high vibration at the 11-PA-7131A pump was caused by the parallel misalignment that occurred at the pump shaft and motor. This is indicated by the high amplitude of vibration on orders 1x, 2x and followed by the presence of amplitude on orders 3x. Vibration data retrieval after the alignment process on the pump shaft and motor also proves that there is a very significant change in vibration amplitude on orders 1x, 2x, and on order 3x. Overall vibration also experienced a very significant decrease so that the pump can be operated normally.

Abstract

Background: Meningiomas can induce inflammation in their tumorigenesis process, thereby linking inflammation with clinical symptoms, peritumoral edema, and recurrence of meningiomas. Easily accessible and cost-effective inflammatory markers include the neutrophillymphocyte ratio (NLR) and monocyte-lymphocyte ratio (MLR). This study aims to investigate the relationship between peripheral inflammation markers, specifically the NLR and MLR, and their association with clinical symptoms, peritumoral edema, and meningioma recurrence.Method: A retrospective cohort study was conducted at Cipto Mangunkusumo National General Hospital from January 2016 to December 2019, utilizing a consecutive non-probability sampling method. Inclusion criteria were patients aged 18 years or older with meningiomas(gradesI-III), first surgery. Peripheral inflammatory markers were derived from differential blood counts, peritumoral edema data from radiological reports, and other data from medical records. The cut-off valuesfor NLR and MLR were determined to be 2.415 and 0.295, respectively. Bivariate analyses using ChiSquare and Mann-Whitney tests were followed by multivariate logistic regression analysis. Results: 173 patients were eligible for analysis. Of these, 27 had preoperative CT scans, 126 had MRIs, and 20 had no preoperative radiology data. Clinical and recurrence analyses were performed on all 173 patients, with radiology and tumor size analyses conducted on subsets of 153 and 126 patients, respectively. The majority of meningiomasin thisstudy were grade I, found in 94.2% of subjects, with the remainder being grade II and III. Higher NLR and MLR values were significantly associated with headaches (p 0.001). Elevated NLR and MLR were also correlated with peritumoral edema (p 0.001). MLR was independently associated with recurrence, with an adjusted odds ratio (aOR) of 12.647 (95% CI 2.355–67.919); p = 0.003.Conclusion: NLR and MLR as peripheral inflammatory markers demonstrated higher median values in meningioma patients with headaches and peritumoral edema. Additionally, inflammation in meningiomas was associated with the occurrence of recurrence.

Abstract

Three years into the pandemic, the World Health Organization revoked the COVID-19 public health emergency of international concern on May 5, 2023. This decision sparked debate, notably around the possibility of a surge in cases due to the SARS-CoV-2 mutations. To manage this transition, the Indonesian government enacted stringent controls on case numbers. This case series study provided an overview of COVID-19 case trends in Indonesia following the revocation of public health emergency of international concern status by the World Health Organization. Data were collected for 5 weeks after the statement (May 6–June 10, 2023) from the COVID-19 Task Force’s official online platform of the Indonesian Ministry of Health. The trends were monitored in daily confirmed, active, recovered, and death cases, and analyzed the data using Microsoft Excel and Stata 16. The findings indicated a positive trend for Indonesia, with decreased daily confirmed cases (89.42%) and active cases (44.16%). Recovered cases accounted for 97.47%, higher than the global average (96%). Unfortunately, the death rate (2.38%) exceeded the global statistic (1%). These results highlighted the need for sustained vigilance, enhancement of the 3T strategy (testing, tracing, and treatment), and wider vaccination coverage. It remains critical to uphold the 3M protocols—mask-wearing, physical distancing, and hand hygiene—to prevent a potential rebound in cases, even without the public health emergency of international concern status,as the situation transitions toward endemic COVID-19.

Abstract

Enhancing firm value is a primary objective for every business entity as it reflects sound financial performance and attracts investor interest. However, understanding the internal financial factors that consistently influence firm value, particularly in the telecommunications sector, remains limited. This study aims to analyze and examine the effect of profitability, leverage, and liquidity on firm value in the telecommunications sub-sector listed on the Indonesia Stock Exchange (IDX) during the 2020–2023 period. A quantitative method was employed with descriptive and associative approaches, using secondary data from annual financial reports obtained from the official IDX website. The research sample was selected through purposive sampling, involving 20 companies over four years, resulting in 80 observation units. Data analysis was conducted using multiple linear regression with the aid of SPSS version 25 software. The results show that profitability has a positive and significant effect on firm value, leverage has a significant negative effect, while liquidity does not have a significant effect. These findings suggest that in the telecommunications sector, profitability and capital structure play a more crucial role in enhancing firm value compared to liquidity. The implications point to the need for management to focus more on strategies to increase profitability and manage capital structure to maximize firm value.

Abstract

Introduction : Multiple sclerosis (MS) is a potentially disabling inflammatory demyelination of the central nervous system. The demyelination process will end up with the destruction of neurons that leads to the decrease of brain volume. Brain atrophy may cause more severe disability and affect the quality of life of MS patients, who are mostly at a young age. Objective : Our study aims to assess the brain atrophy among Indonesian MS patients and the association between with the degree of disability. Material and methods : A cross-sectional study included 28 MS patients. To determine the brain atrophy, we compared 11 healthy control group to the MS group. Head MRI was performed using 1.5T MRI and the brain volume was processed with Freesurfer type 6.0 automatic software. Result : The white matter (WM) and gray matter (GM) volume of MS patients was significantly lower compared to normal control with 78.6% GM atrophy and 67.9% WM atrophy. EDSS score is significantly associated with WM atrophy but not with GM atrophy. Factors related to WM atrophy is age, age of onset, and subtype of MS. A number of lesion sites were found greater in subjects with GM and WM atrophy. Discussion : The mechanisms of brain atrophy in MS involve inflammatory processes and neurodegeneration. Various factors, including lesion volume impact atrophy rates. Brain atrophy had correlation with EDSS scores. Conclusion : Brain atrophy was common in MS patients and significantly associated with the level of disability and number of lesion sites.
