Abstract

Background: Muscle cramps are a common complaint experienced by patients with chronic kidney disease undergoing hemodialysis and can reduce their comfort and quality of life. One potentially effective non-pharmacological intervention is massage combined with lavender-scented oil, which has physical and psychological relaxation effects. Purpose: To describe the effect of massage with lavender-scented oil on reducing muscle cramps in hemodialysis patients. Method: This study used a descriptive design with a case study method in two hemodialysis patients experiencing muscle cramps in the Hemodialysis Ward at PKU Muhammadiyah Bantul Hospital. Muscle cramps were measured using the Cramp Questionnaire Chart before and after the intervention. The intervention, which consisted of massage using lavender-scented oil for approximately 30 minutes, was conducted in two sessions for each respondent. Results: There was a decrease in muscle cramp scores in both respondents after the intervention. Mr. T experienced a decrease in muscle cramp scores from 9 (severe cramps) to 7 at the first session and from 7 to 6 at the second session. Meanwhile, Mr. M's muscle cramp score decreased from 9 to 8 at the first session and from 8 to 7 at the second session. Conclusion: Massage with lavender oil showed a synergistic effect in reducing muscle cramps in hemodialysis patients.   Keywords: Hemodialysis; Lavender Aromatherapy; Massage Therap; Muscle Cramps.   Pendahuluan: Kram otot merupakan salah satu keluhan yang sering dialami pasien gagal ginjal kronik yang menjalani hemodialisis dan dapat menurunkan kenyamanan serta kualitas hidup pasien Salah satu intervensi nonfarmakologis yang berpotensi efektif adalah pemberian massage yang dikombinasikan dengan minyak beraroma lavender, yang memiliki efek relaksasi fisik dan psikologis. Tujuan: Untuk mendeskripsikan efek pemberian massage dengan minyak beraroma lavender terhadap penurunan tingkat kram otot pada pasien hemodialisis. Metode: Penelitian ini menggunakan desain deskriptif dengan metode studi kasus pada dua pasien hemodialisis yang mengalami kram otot di Ruang Hemodialisa RSU PKU Muhammadiyah Bantul. Pengukuran kram otot dilakukan menggunakan Cramp Questionnaire Chart sebelum dan sesudah intervensi. Intervensi berupa massage menggunakan minyak beraroma lavender selama kurang lebih 30 menit dilakukan sebanyak dua kali pertemuan pada masing-masing responden. Hasil: Adanya penurunan skor kram otot pada kedua responden setelah diberikan intervensi. Tn. T terjadi penurunan skor kram otot dari 9 (kram berat) menjadi 7 pada pertemuan pertama dan dari 7 menjadi 6 pada pertemuan kedua. Sedangkan Tn. M dengan skor kram otot menurun dari 9 menjadi 8 pada pertemuan pertama dan dari 8 menjadi 7 pada pertemuan kedua. Simpulan: Pemberian massage dengan minyak beraroma lavender menunjukkan efek sinergis dalam menurunkan kram otot pada pasien hemodialisis.   Kata Kunci: Aroma Lavender; Hemodialisis; Kram Otot; Massage.

Abstract

Latar Belakang: Supraventrikular takikardi (SVT) adalah kegawatdaruratan kardiovaskular yang butuh penanganan cepat untuk mencegah terjadinya ketidakstabilan hemodinamik. Modifikasi Valsalva Manuver (MVM) direkomendasikan sebagai terapi nonfarmakologis lini pertama pada pasien SVT stabil karena memiliki tingkat keberhasilan kardioversi yang lebih tinggi dibandingkan manuver vagal lainnya. Namun, bukti terkait pengaruh MVM terhadap perubahan tekanan darah masih terbatas. Metode:Penelitian ini menggunakan desain literature review dengan penelusuran artikel pada database Scopus, PubMed, dan Semantic Scholar. Artikel penelitian berbahasa Indonesia dan Inggris yang dipublikasikan pada tahun 2019–2025 dan membahas MVM pada pasien SVT dianalisis secara deskriptif dan disintesis secara naratif. Hasil:Hasil telaah menunjukkan bahwa MVM lebih efektif dibandingkan standar Valsalva manuver, teknik vagal lainnya, maupun perawatan konvensional, dengan tingkat keberhasilan kardioversi berkisar antara 25%–67,5%. Kombinasi MVM dengan adenosin menunjukkan peningkatan keberhasilan kardioversi. Perubahan tekanan darah sistolik dan diastolik ditemukan setelah intervensi MVM, namun umumnya tidak signifikan secara statistik dan bersifat sementara. Kesimpulan:Modifikasi Valsalva Manuver efektif dan aman sebagai terapi nonfarmakologis lini pertama pada pasien SVT stabil, dengan pengaruh minimal terhadap perubahan tekanan darah.

Abstract

ST-Elevation Myocardial Infarction (STEMI) is a cardiovascular emergency that is highly time-dependent; delays in reperfusion are closely associated with increased complications and mortality. In this context, nurses play a strategic role as timekeepers throughout the STEMI care continuum. This narrative review aimed to summarize recent evidence regarding nursing interventions in STEMI management. The method used was a narrative review with literature searches conducted in PubMed, Scopus, and ScienceDirect databases for the period 2020–2025 using the keywords “Nursing Intervention” and “Management STEMI.” Included articles were original research studies with free full-text/open-access availability. Of the 1,663 articles identified, 189 articles were reviewed in full text, and 8 articles met the inclusion criteria for analysis. The findings indicate that core nursing interventions include rapid chest pain triage and electrocardiogram acquisition, close hemodynamic monitoring, timely administration of antithrombotic therapy accompanied by contraindication screening and bleeding risk monitoring, activation of the STEMI code, interprofessional coordination and intra-hospital transfer, preparation of the catheterization laboratory, and structured handover. In conclusion, the standardization of nursing interventions with an emphasis on accelerating reperfusion has the potential to improve the quality and safety of STEMI care. Continuous time audits and prospective studies are needed to strengthen the evidence base for clinical implementation.

Abstract

Latar Belakang: Supraventrikular takikardia (SVT) merupakan gangguan irama jantung yang sering dijumpai dan memerlukan penanganan cepat. Vagal manuver direkomendasikan sebagai terapi nonfarmakologis lini pertama pada pasien dengan kondisi hemodinamik stabil. Meskipun efektivitasnya telah banyak dilaporkan, pemetaan perkembangan penelitian vagal manuver pada SVT secara global masih terbatas. Tujuan: Penelitian ini bertujuan untuk menganalisis tren dan peta penelitian mengenai vagal manuver pada pasien SVT dengan pendekatan bibliometrik. Metode: Penelitian ini merupakan studi bibliometrik berbasis database Scopus dengan rentang publikasi tahun 1969–2025. Pencarian dilakukan menggunakan kata kunci vagal maneuver, valsava maneuver, carotid sinus massage, dan supraventricular tachycardia dengan analisis data menggunakan perangkat lunak VOSviewer Hasil: Jumlah publikasi meningkat signifikan sejak tahun 2019-2025. Amerika Serikat menjadi negara dengan kontribusi publikasi terbesar. Topik yang paling dominan adalah supraventricular tachycardia dan Valsalva maneuver. Kesimpulan: Penelitian tentang vagal manuver pada SVT menunjukkan tren peningkatan yang signifikan, khususnya dalam lima tahun terakhir. Namun, masih terdapat kesenjangan penelitian di negara berkembang. Hasil ini diharapkan dapat menjadi dasar pengembangan riset lanjutan dan penguatan praktik berbasis bukti. Abstract

ABSTRACT The Six Minute Walk Test (6MWT) is widely used as a simple and practical measurement tool to assess patients' functional abilities, but the variations in its use and implementation in various cardiac rehabilitation contexts have not been comprehensively mapped. Acute coronary syndrome (ACS) is the leading cause of cardiovascular morbidity and mortality worldwide, making the recovery of functional capacity a primary focus in cardiac rehabilitation. This study aims to map the scientific evidence regarding the use of the 6MWT as an indicator of functional capacity recovery in post-ACS patients across various cardiac rehabilitation models. This study uses a scoping review approach based on the Arksey O’Malley (2005) framework and the PRISMA-ScR (2018) guidelines. Literature search was conducted on the Scopus, PubMed, Cochrane Library, and Google Scholar databases for the period 2015–2025. Inclusion criteria included quantitative studies on adult post ACS patients using the 6MWT in cardiac rehabilitation. A total of 10 studies met the criteria and were thematically analysed using the PCC framework.  The analysis results show that the 6MWT is consistently used as the primary indicator of functional capacity recovery, with a significant increase in distance covered after cardiac rehabilitation. Three main themes were identified, namely 6MWT as the Primary Indicator of Functional Recovery, the Effectiveness of Cardiac Rehabilitation Models on 6MWT Outcomes, and the Relationship between 6MWT and Clinical and Psychosocial Outcomes. The study was dominated by China, followed by Iran, South Korea, Germany, and Indonesia. The 6MWT is a valid, practical, and versatile measurement tool for assessing post ACS patient recovery in various cardiac rehabilitation contexts. The integration of the 6MWT in clinical practice has the potential to enhance rehabilitation monitoring and support the prevention of long-term complications. Keywords: Functional Capacity, Cardiac Rehabilitation, Acute Coronary Syndrome, Six-Minute Walk Test, Scoping Review.  ABSTRAK Six Minute Walk Test (6MWT) banyak digunakan sebagai alat ukur sederhana dan praktis untuk menilai kemampuan fungsional pasien, namun variasi penggunaan dan implementasinya dalam berbagai konteks rehabilitasi jantung belum terpetakan secara komprehensif. Sindrom koroner akut (SKA) merupakan penyebab utama morbiditas dan mortalitas kardiovaskular di dunia, sehingga pemulihan kapasitas fungsional menjadi fokus utama dalam rehabilitasi jantung. Penelitian ini bertujuan untuk memetakan bukti ilmiah mengenai penggunaan 6MWT sebagai indikator pemulihan kapasitas fungsional pada pasien pasca SKA dalam berbagai model rehabilitasi jantung. Penelitian ini menggunakan pendekatan scoping review berdasarkan kerangka Arksey O’Malley (2005) dan panduan PRISMA-ScR (2018). Penelusuran literatur dilakukan pada database Scopus, PubMed, Cochrane Library, dan Google Scholar untuk periode 2015–2025. Kriteria inklusi mencakup studi kuantitatif pada pasien dewasa pasca SKA yang menggunakan 6MWT dalam rehabilitasi jantung. Sebanyak 10 studi memenuhi kriteria dan dianalisis secara tematik menggunakan kerangka PCC. Hasil analisis menunjukkan bahwa 6MWT secara konsisten digunakan sebagai indikator utama pemulihan kapasitas fungsional, dengan peningkatan jarak tempuh yang signifikan setelah rehabilitasi jantung. Tiga tema utama teridentifikasi, yaitu 6MWT sebagai Indikator Utama Pemulihan Fungsional, Efektivitas Model Rehabilitasi Jantung terhadap Hasil 6MWT, Hubungan 6MWT dengan Outcome Klinis dan Psikososial. Studi didominasi oleh negara Tiongkok, diikuti Iran, Korea Selatan, Jerman, dan Indonesia.  6MWT merupakan alat ukur yang valid, praktis, dan serbaguna dalam menilai pemulihan pasien pasca SKA di berbagai konteks rehabilitasi jantung. Integrasi 6MWT dalam praktik klinis berpotensi meningkatkan pemantauan rehabilitasi dan mendukung pencegahan komplikasi jangka panjang. Kata Kunci: Kapasitas Fungsional, Rehabilitasi Jantung, Sindrom Koroner Akut, Six-Minute Walk Test, Scoping Review.

Abstract

Hipotermia merupakan komplikasi pasca anestesi spinal akibat gangguan termoregulasi yang berisiko menimbulkan komplikasi bila tidak ditangani. Studi kasus ini bertujuan untuk menggambarkan perubahan suhu tubuh pasien setelah pemberian intervensi selimut konvensional dengan cairan infus Ringer Laktat tidak dihangatkan dan selimut konvensional dengan cairan infus Ringer Laktat hangat. Penelitian ini menggunakan desain pre-test dan post-test dengan pendekatan studi kasus pada enam pasien pascaoperasi dengan anestesi spinal yang mengalami hipotermia di ruang pemulihan RSU PKU Muhammadiyah Bantul pada tanggal 20–25 Oktober 2025. Subjek dibagi menjadi dua kelompok, masing-masing terdiri atas tiga pasien. Data dikumpulkan melalui pengukuran suhu tubuh menggunakan termometer digital aksila sebelum dan sesudah intervensi selama 15 menit serta observasi kondisi pasien. Analisis data dilakukan secara deskriptif dengan membandingkan perubahan suhu tubuh sebelum dan sesudah intervensi pada masing-masing kelompok. Hasil studi menunjukkan bahwa seluruh pasien mengalami peningkatan suhu tubuh setelah intervensi. Peningkatan suhu tubuh pada kelompok selimut konvensional dengan cairan infus Ringer Laktat hangat cenderung lebih besar dan lebih stabil dibandingkan kelompok selimut konvensional dengan cairan infus Ringer Laktat tidak dihangatkan. Kedua intervensi berkontribusi dalam pencegahan hipotermia pasca anestesi spinal, namun kombinasi selimut konvensional dan cairan infus hangat memberikan peningkatan dan stabilitas suhu tubuh yang lebih baik.

Abstract

Cardiac Rehabilitation (CR) is a structured, multidisciplinary program for managing cardiovascular disease that includes education, physical exercise, risk factor modification, and psychosocial support for heart patients, including those who have experienced Acute Coronary Syndrome (ACS).  This study aims to analyze global research trends and examine cardiac rehabilitation from 1964 to 2025. A bibliometric analysis was conducted on publications in the Scopus database, The search strategy:  in this review was based on the following terms: ‘Cardiac Rehabilitation’ and its alternative keywords (‘Acute Coronary syndrome*’, "Cardiac Rehabilitation. These terms were combined using the Boolean operator ‘AND’ to generate a broad range of results. using visualization, co-occurrence, and trend analysis through VOSviewer software (version 1.6.20). The United States made the largest contribution. The most frequently used keywords were "cardiac rehabilitation," "rehabilitation," and "secondary prevention." The most significant publication increases in 2025 (102 publications), 2021 (101 publications), and 2024 (94 publications). The results show fluctuations in research activity, with a significant increase from 2021 to 2025. Further research should focus on developing technology-based rehabilitation models that are accessible from home and affordable, particularly in low-income countries. As telemedicine, which is still seldom discussed and published.

Abstract

Advanced cancer patients often experience multidimensional suffering known as "Total Pain," which includes physical, psychological, social, and spiritual distress. While spiritual interventions are recognized as core components of holistic palliative care, a comprehensive global mapping of research trends over the last five decades remains limited. This study aims to explore global publication trends, identify influential countries and authors, and map key thematic clusters in spiritual intervention research for cancer patients from 1973 to 2025. A quantitative bibliometric design was used. Bibliographic data of 1,121 documents were extracted from the Scopus database following PRISMA 2020 protocols. Data analysis evaluated publication performance and geographic distribution. VOSviewer software was used for keyword co-occurrence and network visualization. Publication growth showed three phases: initiation, acceleration, and maturity (post-2011). The United States dominates global publications (42.5%), followed by Canada (7.4%) and the UK (6.4%). A significant rise in research from Iran and China indicates a shift toward de-westernization in spiritual care. The most influential study by Zimmermann et al. (2014) emphasized early palliative care. Four main thematic clusters were identified: spiritual well-being, quality of life, structured interventions (eg, Dignity Therapy), and death anxiety mitigation. Spiritual intervention research has evolved into a high-level health science. Integrating structured spiritual protocols into early palliative care is essential to address existential distress in terminal cancer patients.

Abstract

Quality of life is a key outcome in palliative care for patients with chronic kidney disease (CKD), especially in advanced stages and patients undergoing dialysis. Although publications on QoL in palliative care for CKD continue to increase, the global knowledge landscape remains scattered and has not been comprehensively mapped through a bibliometric approach. Objective to map publication performance, the geographical distribution of country contributions, and the dynamics of research themes related to QoL in palliative care for CKD through bibliometric analysis. A bibliometric study was conducted using the Scopus database (Elsevier) with the range 10 years . The search strategy combined the keywords palliative care/support, CKD/ESRD/dialysis, and QoL/health-related QoL. The initial search yielded 1,077 documents, which were then narrowed down to 681 publications (last 10 years), 654 publications (Medicine and Nursing fields), and finally 561 documents (article and review types) for analysis. Descriptive analysis was used for annual publication trends (2016–2026) and country contributions, while keyword co-occurrence mapping, overlay maps, and density maps were performed using VOSviewer version 1.6.18. Publication production showed an increasing trend from 37 documents (2016) to a peak of 69 documents (2025), with 2026 recording 1 document that was likely temporary. Country contributions were dominated by the United States (198 documents), followed by the United Kingdom (92), Australia (74), and Canada (45). The co-occurrence map places the terms population and service context as central nodes (human/humans, male, aged, article) and shows major clusters reflecting (1) CKD themes and management related to dialysis/hemodialysis as well as palliative and conservative care, (2) comorbidities, symptoms, and clinical indicators related to disease burden (e.g., fatigue, pruritus, anemia, hypertension, sepsis, depression, creatinine, acute kidney failure), and (3) hospital service context and study design (hospitalization, observational/retrospective study). The overlay map indicates a strengthening of the themes of service outcomes and follow-up (follow-up, clinical outcome, hospitalization) as well as an increased focus on patient-centered approaches and psychosocial support (patient-centered care, nurse, interpersonal communication, qualitative research). The density map confirms the largest hotspots in the terms of population (male, aged) and kidney service/therapy themes (palliative care, dialysis/hemodialysis, CKD) that are interconnected with comorbidities and service outcomes. QoL research in CKD palliative care is growing consistently and is dominated by countries with established research ecosystems, with the main themes still strongly centered on the clinical and service context (dialysis/hemodialysis, CKD, comorbidities, inpatient outcomes). However, there are indications of a strengthening research direction towards patient-centered approaches, communication, caregivers, and qualitative methods. Future studies need to strengthen the measurement of QoL outcomes that are meaningful to patients and families, expand the geographical context, and evaluate palliative-nephrology integration models that can tangibly improve QoL.

Abstract

Palliative care for patients with kidney failure is increasingly recognized as an important approach to improving quality of life through symptom control, psychosocial and spiritual support, and strengthening shared decision-making, yet the global knowledge landscape remains fragmented and incompletely mapped. Mapping publication performance, geographic distribution of country contributions, and the dynamics of research themes related to palliative care in kidney failure patients through bibliometric analysis. A bibliometric study was conducted using the Scopus database spanning the last 10 years. The search strategy combined the keywords palliative care and kidney failure. From 4,730 initial findings, a gradual narrowing was performed to obtain 2,397 documents meeting the criteria, consisting of 1,974 research articles and 423 review articles. Descriptive analysis was used for annual publication trends and country contributions, while VOSviewer version 1.6.18 was used to map keyword co-occurrence, overlay maps, and density maps. Keyword mapping places demographic and clinical terms as central nodes, with primary clusters reflecting acute conditions and severity indicators including acute kidney injury and respiratory infectious complications, chronic kidney disease and dialysis therapy clusters linked to themes of palliative care, terminal care, quality of life, psychology, and conservative care, and geriatric clusters and service outcomes such as hospitalization, comorbidities, risk factors, length of stay, and mortality. The overlay map demonstrates a shift in themes from patient support and quality of life to a stronger focus on clinical outcomes, comorbidities, and infectious and respiratory contexts in the more recent period, while the density map highlights the largest hotspots in the elderly population and the kidney disorder theme networked with hospitalization outcomes.Palliative care research in patients with kidney failure is dominated by clinical themes and service outcomes, while patient-centered themes such as quality of life, psychology, and terminal and conservative care emerge as important but relatively less prominent.
