Abstract

Tekanan darah tinggi dan penyakit ginjal adalah dua masalah yang saling terkait karena tekanan darah tinggi yang tidak terkendali dapat menyebabkan kegagalan ginjal atau sebaliknya, kegagalan ginjal menyebabkan tekanan darah tinggi. Selain terapi farmakologis, terapi non-farmakologis juga dapat dilakukan untuk dapat membantu mengatasi masalah tekanan darah tinggi pada pasien, salah satunya adalah dengan menggunakan terapi relaksasi murottal. Penelitian ini menggunakan metode studi kasus. Dari hasil penerapan terapi murottal Al-Quran Surah Ar Rahman pada Ibu S. selama 3 hari, terlihat penurunan tekanan darah pada setiap intervensi. Pada hari pertama intervensi nilai tekanan darah turun dari 160/100 mmHg menjadi 140/90 mmHg dengan MAP 120 mmHg turun menjadi 107 mmHg. Pada hari keuda intervensi nilai tekanan darah turun dari 150/100 mmHg menjadi 135/90 mmHg dengan MAP 117 mmHg turun menjadi 105 mmHg. Pada hari ketiga intervensi nilai tekanan darah turun dari 145/95 mmHg menjadi 130/85 mmHg dengan MAP 112 mmHg turun menjadi 100 mmHg. Penurunan tekanan darah terjadi karena mekanisme murottal Al-Quran dalam tubuh, yaitu akan mengaktifkan dampak psikologis positif sebagai terapi relaksasi, yang akan merangsang sensasi relaksasi yang dihasilkan oleh murottal Al-Quran. Ketika otak diberi stimulus berupa suara, dan suara tersebut sebanding dengan frekuensi alami, sel akan beresonansi dan kemudian menjadi aktif serta memberikan sinyal ke kelenjar pituitari. Selanjutnya, tubuh akan melepaskan hormon endorfin. Kondisi ini akan membuat tubuh rileks sehingga akan terjadi penurunan epinefrin dan tekanan darah. Berdasarkan hasil penelitian, terapi murottal terbukti memberikan pengaruh terhadap penurunan tekanan darah pasien. Hasil penelitian ini diharapkan dapat menjadi landasan pengembangan manajemen perawatan pasien Chronic Kidney Disease (CKD) dengan tekanan darah tinggi dan memberikan kontribusi terhadap kualifikasi pelayanan keperawatan yang lebih tinggi.

Abstract

The government issued a health protocol to reduce the spread of COVID-19 and the community must obey it. The purpose of this study is to determine community compliance with health protocols and the factors that influence them. This study is a literature review, using PubMed, ProQuest, ScienceDirect, and Google Scholar. There were 10 articles that discussed 9 health protocols and 11 factors that influenced adherence. Communities have different levels of compliance with health protocols. The majority of people comply with health protocols by wearing masks. Factors that influence include education level, occupation, place of residence, health status, obtaining information from official sources, knowledge, attitudes, motivation, culture, environment, and family support. A focus on providing health education is an important strategy for increasing adherence in the community.Keywords: compliance; COVID-19; health protocol ABSTRAK Pemerintah mengeluarkan protokol kesehatan untuk mengurangi penyebaran COVID-19 dan harus dipatuhi masyarakat. Tujuan dari studi ini adalah untuk mengetahui kepatuhan masyarakat terkait protokol kesehatan dan faktor-faktor yang mempengaruhinya. Studi ini merupakan literature review, menggunakan PubMed, ProQuest, ScienceDirect, dan Google Scholar. Diperoleh 10 artikel yang membahas 9 protokol kesehatan dan 11 faktor-faktor yang mempengaruhi kepatuhan. Masyarakat memiliki tingkat kepatuhan yang berbeda-beda terkait protokol kesehatan. Mayoritas masyarakat mematuhi protokol kesehatan dengan menggunakan masker. Faktor yang mempengaruhi antara lain tingkat pendidikan, pekerjaan, tempat tinggal, status kesehatan, memperoleh informasi dari sumber resmi, pengetahuan, sikap, motivasi, budaya, lingkungan, dan dukungan keluarga. Fokus pada pemberian pendidikan kesehatan merupakan strategi penting untuk meningkatkan kepatuhan di masyarakat.Kata kunci: kepatuhan; COVID-19; protokol kesehatan

Abstract

Pendahuluan: Kanker merupakan sekelompok besar penyakit yang dimulai di jaringan tubuh yang mengalami pertumbuhan abnormal. Kanker kolorektal menjadi kasus kanker terbanyak ketiga di seluruh dunia. Efek dari adanya neoplasma, konsekuensi pembedahan, kemoterapi atau radioterapi, adanya nyeri, penggunaan opioid, dan kondisi psikologis lain yang dialami pasien kanker dapat menyebabkan munculnya gangguan tidur hingga perubahan hemodinamik. Penderita kanker harus memiliki kualitas tidur yang baik untuk mempertahankan kondisi dan daya tahan tubuh secara optimal. Terapi komplementer dapat digunakan sebagai terapi pendukung dalam meningkatkan kualitas tidur pada pasien kanker seperti foot massage dan aromaterapi. Tujuan: Menganalisis penerapan foot massage dan aromaterapi terhadap kualitas tidur dan hemodinamik pada pasien kanker kolon di RSD dr. Soebandi Jember. Metode: Jenis penelitian ini adalah studi kasus yang dilakukan pada seorang pasien kanker kolon yang mengalami masalah gangguan tidur dan hemodinamik dengan memberikan intervensi foot massage dan aromaterapi. Foot massage dilakukan sebanyak 7 kali dan aromaterapi diberikan sebanyak 3 kali dengan durasi 15 menit. Evaluasi dilakukan sebelum dan sesudah intervensi diberikan. Evaluasi pemberian foot massage dan aromaterapi dilakukan dengan mengukur kualitas tidur menggunakan kuesioner PSQI. Hemodinamik dievaluasi dengan mengukur tekanan darah, MAP, dan nadi. Hasil: Setelah dilakukan intervensi didapatkan peningkatkan kualitas tidur dari kualitas tidur buruk (skor 11) menjadi kualitas tidur baik (skorv4) dan hemodinamik yang mengalami kestabilan ditunjukkan adanya penurunan sistole dengan rentang 13-32 mmHg dan penurunan diastole dengan rentang 9-13 mmHg. Kesimpulan: Foot massage dan aromaterapi dapat menjadi intervensi mandiri keperawatan yang efektif untuk meningkatkan kualitas tidur dan menstabilkan hemodinamik khususnya pada pasien kanker.

Abstract

AbstrakProsedur kraniektomi dekompresi merupakan salah satu penatalaksanaan cedera kepalauntuk menurunkan tekanan intrakranial. Tindakan ini dapat menimbulkan komplikasisalah satunya berdampak pada penurunan kesadaran dan nyeri. Pemberian intervensiFamilier auditory sensoric training (FAST) dapat dijadikan sebagai salah satu intervensiyang dapat meningkatkan kesadaran dan menurunkan skala nyeri pada pasien post opkraniektomi. Tujuan penelitian ini adalah mengetahui efektivitas pemberian intervensiFAST untuk meningkatkan kesadaran dan menurunkan skala nyeri pada pasien post opkraniektomi dekompresi hari ke 4 di RSUD dr. Soebandi Jember. Metode yang digunakandalam penelitian ini adalah metode case report. Intervensi dilakukan selama enam haripada pasien post op kraniektomi dekompresi hari ke 4. Pengumpulan data tingkatkesadaran menggunakan nilai Glasgow Coma Scale (GCS) dan pengukuran skala nyerimenggunakan Behaviour Pain Scale (BPS). Hasil penelitian menunjukan terdapatperubahan pada tingkat kesadaran dari nilai awal GCS 3 (Coma) menjadi GCS 6 (Supor)dan penurunan skala nyeri dari nilai awal 6 (nyeri) menjadi 5 (tidak nyeri) pada pasienpost op craniotomy dekompresi hari ke 4. Pemberian intervensi FAST pada pasien postop craniotomy hari ke 4 yang dilakukan selama 6 hari dan durasi pemberian selama 10menit terbukti efektif unuk meningkatkan kesadaran dan menurunkan skala nyeri.Keywords: Familiar Auditory Sensory Training, Tingkat Kesadaran, Skala Nyeri AbstractDecompression craniectomy procedure is one of the management of head injuries todecrease intracranial pressure. Soebandi Jember. The method used in this study is case report. The interventionconducted for six days in patients post craniotomy decompression. Level of consciusness wasmeasured using Glasglow Coma Scale (GCS) and pain scale using Behaviour Pain Scale(BPS). The results showed improvement level of consciousness from GCS 3 (Coma) to GCS 6(Suppor) and a decrease in the pain scale from 6 (indicate an unacceptable amount of pain)to 5 (mild pain). Abstract

Introduction: Predicting the length of care becomes important for intervention in the care system. However, it still seems that the ability to predict the length of treatment is not optimal, so that the length of treatment is prolonged. If the ability to predict the length of treatment can be done optimally, it will improve the patient's return time. Methods: This study uses a retrospective study approach. Result: There was no relationship between the predictor of length of stay with a length of treatment. Conclusion: The predictor of length of stay in this study were patients with tibia fractures at RSD dr. Soebandi. Most of the patients with tibial fractures were in the early elderly, with the gender of patients with tibia fractures being mostly male. Most of the patients with tibial fractures have no complications and the type of fracture in patients with tibia fractures is mostly in the fracture of the shaft of the tibia. There was no correlation between the predictors of length of stay and length of stay in patients with tibia fractures.

Abstract

This community service aims to increase students' knowledge and skills in first aid for fractures at Al-Ghozali Integrated Islamic Middle School Jember. The method used in carrying out community service activities regarding first aid to fracture victims involved lectures, discussions, and simulations related to handling fracture victims. The fracture Knowledge Questionnaire evaluated students' knowledge about fracture first aid. The results of the Wilcoxon test from the pre-test and post-test had a significance value of < 0.05, meaning there are differences in students' knowledge before and after health education about first aid trauma management fractures. Fracture education and simulation improved students' ability to identify and perform appropriate fracture first aid.

Abstract

Background: the application of complementary therapies in hospitals is still not optimal. The limitations of the application of nursing interventions are caused by the factor of applying therapy by nurses to patients and education related to therapy to patients is still rarely done. Nurses are still focusing on conventional treatment with pharmacological therapies such as analgesic drugs to relieve pain, while warm compress therapy is still not prioritized. The application of warm compress therapy in the inpatient room of Gardena dr. Soebandi General Hospital Jember to overcome acute pain nursing problems in patients is still rare. Aims: evaluate warm compress interventions in patients with acute pain nursing problems. Method: type of research case study using nursing care method. The sample was a patient treated in the Gardena room of dr. Soebandi General Hospital Jember with acute pain nursing problems. Patients are given nursing intervention in the form of warm compress therapy for 15-20 minutes for 3 days, from January 16 to 18, 2023. Nursing intervention is given through the nursing process, which is teaching directly to patients and families with the right and appropriate techniques to adjust the patient's condition. Results: the provision of nursing interventions in the form of warm compresses can overcome acute pain nursing problems. This is shown by the improvement of the patient's condition that complaints of pain, grimacing and restlessness decrease and sleep patterns improve. Patients can do the therapy independently properly and correctly. Conclusion: warm compress therapy is effective in overcoming acute pain nursing problems.

Abstract

In patients with decreased consciousness and a respirator installed in intensive care unit room, it causes obstruction to discharge of secretions. Excessive accumulation of secretions in the airways can obstruct the airways and cause the patient to experience dyspnea. These conditions can cause ineffective airway hygiene problems. Ineffective airway clearance problems may occur in patients with pneumonia caused by excessive accumulation of secretions. The purpose of this studi was to determine the effectiveness of implementing chest physiotherapy interventions in overcoming the problem of ineffective airway clearance nursing in patients in the ICU. Case study with a nursing care approach to patients in the intensive care unit. The sample used was 1 patient with chest physiotherapy nursing intervention for 3x24 hours in the form of chest physiotherapy. The results showed that effectiveness of implementing chest physiotherapy interventions in overcoming the problem of ineffective airway clearance nursing in patients in the ICU which is carried out in two sessions each shift is effective for reducing additional rhonchi breath sounds, reducing respiratory frequency, and helping to remove secretions. Based on the results, researcher conclude that nursing care for patients with ineffective airway clearance problems can be resolved with chest physiotherapy.

Abstract

Background: Management of secretions is one of the main problems faced in the medical condition of respiratory disorders. The use of complementary therapy techniques to treat airway clearance such as the active cycle of breathing technique is still low when compared with other interventions in treating respiratory problems. Aims: The research aims to determine the effectiveness of providing active cycle of breathing technique therapy to overcome nursing problems ineffectiveness airway clearance in nursing care. Method: This type of research is a case study using nursing care methods. The research sample was patients treated in the Catleya room at Dr. Soebandi Jember Hospital. Patients are given nursing intervention in the form of therapy active cycle of breathing technique for 4 consecutive days 2 times a day for 15-20 minutes with 4 cycles. Results: providing active cycle of breathing technique therapy can reduce nursing problems ineffectiveness airway clearance. Providing active cycle of breathing technique therapy effectively shows improvements in conditions such as improved respiratory frequency, reduced shortness of breath, and decreased sputum production. Conclusion: providing active cycle of breathing technique therapy effectively solve nursing problems ineffectiveness airway clearance.

Abstract

Patients with diabetes can affect cardiac output, such as blood pressure dysfunction. Changes in insulin resistance and hyperglycemia can lead to blood pressure dysfunction and increased cardiac output problems. Warm compresses affect macrovascular function and impact blood pressure in patients. This study analyzed the effectiveness of warm compresses in reducing blood pressure in type 2 DM patients with decreased cardiac output problems. The study used a case report method. Warm compresses were applied for three days, for 2 hours per day, using a warm water sack placed on the abdomen at 40-42°C. Blood pressure was measured before and after the intervention. Results showed that after 3 days, there was a reduction in blood pressure, with an average decrease of 16.667 mmHg in systolic and 6.667 mmHg in diastolic pressure. Warm compresses effectively reduce blood pressure in patients with cardiac output problems. Nurses can apply warm compress therapy as a simple intervention to improve blood pressure in patients with decreased cardiac output.
