Abstract

Latar belakang : disease-19 (COVID-19) telah ditetapkan oleh WHO sebagai pandemi dunia dikarenakan penyebaran infeksi yang menyebar dengan cepat di berbagai belahan dunia. Virus ini menyebabkan berbagai infkesi saluran pernapasan pada manusia yang bervariasi mulai dari flu ringan hingga sindrom gangguang pernapasan berat. Banyak penelitian menyebutkan bahwa hipertensi merupakan komorbid umum pada pasien positif COVID-19. Hipertensi pada pasien COVID-19 diyakini dapat memperparah keadaan pasien dikarenakan terapi pasien positif COVID-19 dengan hipertensi lebih sulit dibanding pasien positif COVID-19 tanpa hipertensi. Tujuan : mengetahui hubungan antara derajat hipertensi dengan tingkat keparahan COVID-19 di Rumah Sakit Siti Khodijah Muhammadiyah Cabang Sepanjang. Metode : Penelitian ini menggunakan jenis penelitian analitik observasional dengan pendekatan cross sectional. Sampel dipilih dengan Teknik total sampling. Sampel yang digunakan dalam penelitian ini berjumlah 86 orang dan sampel merupakan pasien terkonfirmasi COVID-19 di Rumah Sakit Siti Khodijah Muhammadiyah Cabang Sepanjang yang masuk dalam kriteria inklusi. Data dianaliis menggunakan uji rank spearman. Hasil : Berdasarkan hasil uji rank spearman hubungan derajat komorbid hipertensi dengan tingkat keparahan pasien COVID-19 didapatkan p=0,268 (p>0,05) dan koefisien korelasi rank spearman= 0.121. Kesimpulan : Tidak terdapat hubungann antara komorbid hipertensi dengan tingkat keparahan COVID-19. Kata kunci: COVID-19, komorbid hipertensi, tingkat keparahan COVID-19

Abstract

AbstractPhysical activity has proven to be beneficial in improving the clinical conditions that are mostfrequently associated with severe COVID-19. Physical activity contributes to the reduction ofoverall cardiovascular risks, lowering both systolic and diastolic blood pressure andremodeling left ventricular hypertrophy. The immune system is highly responsive to physicalactivity, with the extent of the response related to both duration and intensity of exercise.Moderate-intensity exercise training is associated with a reduced incidence. Social isolation/distancing (‘‘stay home’’) can increase sedentary behavior and decrease the level of physicalactivity, which has already been linked to poor survival rates The current review summarizesharmful effects of limited physical activity on mental and physical health due to socialdistancing and quarantine and highlights the effects of simple physical activity regimescounteracting these detrimental effects.Keyword : COVID-19, Physical activity, Home exercise program

Abstract

Hypertension is defined as an increase in systolic and diastolic blood pressure >140/90 mmHg in adults over the age of 18 years with at least 2 examinations. Obesity is overweight which is characterized by the results of a body mass index that is above normal. Measurement of body mass index can be done by measuring height, weight, upper arm circumference, as well as waist and hip circumference. The results of measurements of excess body mass index (obesity) are one of the factors that can trigger hypertension. Based on this, it is important to conduct research related to the relationship between obesity and hypertension. Objectives: To determine the relationship between obesity and hypertension at the Legung Health Center, Batang-Batang District, Sumenep Regency. Methods: Using an observational analytic method with a cross sectional approach. From 133 research populations, 100 samples were selected using the Slovin formula calculation. Simple random sampling technique was used for sampling to be used in a random sample of hypertensive patients. Research Results: The number of respondents who are obese is 75 people. A total of 87 respondents had grade II hypertension and 13 respondents had grade I hypertension. Statistical analysis using the Chi-Square method obtained p value = 0.626 (p>0.05) so it can be interpreted that there is no significant relationship between body mass index and hypertension. Conclusion: From the results of Chi-Square analysis, it is known that there is no significant relationship between body mass index and hypertension. Respondents with hypertension patients who are not obese may be due to the fact that patients always control their blood pressure by maintaining their diet by avoiding coconut milk and not eating fried foods.Keywords: hypertension, obesity, body mass index, chi-square

Abstract

For all patients with heart failure with reduced ejection fraction (HFrEF), a multimodal approach is necessary, with a combination of numerous medications serving as the cornerstone for symptomatic and prognosis improvement. Newer treatment consist of Angiotensin-converting enzyme (ACE) inhibitors, angiotensin receptor/neprilysin inhibitors (ARNI), beta-blockers mineralocorticoid receptor antagonists (MRAs), and sodium-glucose co-transporter 2 (SGLT2) inhibitors demonstrated a highly significant and clinically relevant reduction in mortality and heart failure hospitalizations, as well as an improvement in quality of life.Keywords : heart failure, ejection fraction, ARNI, SGLT-2 inhibitor, ACE inhibitor, beta blocker, MRA

Abstract

Gagal jantung merupakan masalah kesehatan masyarakat dengan angka morbiditas dan mortalitas yang cukup besar, serta risiko kematian yang tinggi pada negara maju maupun negara berkembang, sehingga membutuhkan manajemen terapi yang tepat untuk mengurangi morbiditas dan mortalitas. ACEI / ARB adalah standar perawatan yang biasa digunakan selama ini, namun sacubitril / valsartan banyak dibicarakan dan memberikan resiko kematian yang lebih rendah serta mengurangi rehospitalisasi pada pasien gagal jantung dibandingkan dengan penggunaan ACEI. Oleh karena itu, sacubitril / valsartan cenderung lebih hemat biaya dibandingkan dengan ACEI (standar perawatan saat ini). Tujuan penulisan ini untuk mengetahui efektivitas biaya sacubitril / valsartan dibandingkan dengan enalapril (ACEI) dalam pengobatan gagal jantung dengan fraksi ejeksi berkurang.
