Abstract

The Relationship between Regional Anesthesia and Breastfeeding Smoothness in Post Partum Sectio Caesarea Mothers at Muhammadiyah Hospital GresikBackground: The birth rate by caesarean section method is getting higher. Risk data for 2013 shows the method of birth with the operation method of 9.8 percent of the total 49,603 births during 2010 to 2013. Being in practice the mother must be given anesthetic before the surgery begins. This anesthesia will later affect the pain that will occur after SC.Objective: The purpose of this study was to determine the correlation between regional anesthetic drugs and the smoothness of breast milk in women born in sectio caesarea at Muhammadiyah Gresik Hospital.Method: Method with Cross Sectional approach. The population in this study mothers who gave birth in a caesarean section at Muhammadiyah Hospital Gresik in December 2019 to January 2020. The sampling technique in this study is probability / random simple sampling. The sample in this study was a portion of mothers who gave birth in a caesarean section at Muhammadiyah Gresik Hospital. The instrument used was primary data collection in the form of questionnaires and secondary data in the form of patient medical records.Result: The data obtained in this study were processed using spearman correlation statistics. From the statistical test the Correlation coefficient value was 0.807, and obtained P-Value equal to 0,000 this value is less than 0.05.Conclusion: The conclusion of this study is that there is a correlation between the administration of a regional anesthetics and the smoothness of breast milk in mothers of post partum caesarea at Muhammadiyah Gresik Hospital.

Abstract

The Relationship between Ventilation Area and Natural Lighting of the House on AFB Sputum Positive Rate in Patients with Pulmonary TB at Tlogosadang Health Center Background: Tuberculosis (TB) is an infectious disease caused by Mycobacterium tuberculosis. Indonesia is ranked third in the world, contributing 8% of TB cases. The phenomenon that occurs in the working area of the Tlogosadang Health Center is an increase of positive smear TB cases. This is thought to be due to the physical condition of the patient's house with pulmonary tuberculosis, especially the area of ventilation and natural lighting which is not standardized.Objective: To analyze the correlation between the area of ventilation and natural lighting of the house to the level of ARB sputum positivity in patients with pulmonary tuberculosis at Tlogosadang Health Center.Methods: This study used a retrospective cohort design. Research respondents were taken by simple random sampling method on 38 patients with smear-positive pulmonary TB in 2016-2018. Observation of ventilation area (minimum 10% of floor area), while natural lighting (minimum 60 lux). Observation of medical records on the level of ARB positivity (Scanty, +1, +2, and +3) was carried out at the Tlogosadang Health Center Laboratory, then recorded in the observation checklist.Results: The re were 3 respondents with scanty ARB, 20 respondents (+1), 14 respondents (+2), and 1 respondent (+3). More than 65% of the house's natural lighting and ventilation area is not standardized. The results of the analysis of the contingency coefficient test showed the correlation between the area of ventilation (p = 0.60) and natural lighting (p = 0.24) on the level of ARB sputum positivity.Conclusion: There is no correlation between the area of ventilation and natural lighting of the house on the level of ARB sputum positivity in patients with pulmonary tuberculosis at Tlogosadang Health Center. Latar Belakang: Tuberkulosis (TB) adalah penyakit menular yang disebabkan oleh Mycobacterium tuberculosis. Indonesia di peringkat ketiga di dunia menyumbang 8% untuk kasus TB. Fenomena yang terjadi di wilayah kerja Puskesmas Tlogosadang yaitu adanya peningkatan kasus TB BTA positif. Hal ini diduga disebabkan oleh faktor kondisi fisik rumah pasien TB paru khususnya luas ventilasi dan pencahayaan alaminya yang tidak terstandar.Tujuan: Menganalisis hubungan luas ventilasi dan pencahayaan alami rumah terhadap tingkat kepositifan sputum BTA pada penderita TB paru di Puskesmas Tlogosadang.Metode: Penelitian ini menggunakan desain kohort retrospektif. Responden penelitian diambil dengan metode simple random sampling pada 38 penderita TB paru BTA positif pada tahun 2016 - 2018. Pengamatan luas ventilasi (minimal 10% dari luas lantai), sedangkan pencahayaan alami (minimal 60 lux). Pengamatan rekam medis tingkat kepositifan BTA (Scanty, +1, +2, dan +3) dilakukan di Laboratorium Puskesmas Tlogosadang, kemudian dicatat dalam check list observasi.Hasil: Didapatkan 3 responden dengan BTA scanty, 20 responden (+1), 14 responden (+2), dan 1 responden (+3). Lebih dari 65 % luas ventilasi dan pencahayaan alami rumah tidak terstandar. Hasil analisis uji koefisien kontingensi menunjukkan hubungan luas ventilasi (p = 0,60) dan pencahayaan alami (p = 0,24) terhadap tingkat kepositifan sputum BTA.Kesimpulan: Tidak terdapat hubungan luas ventilasi dan pencahayaan alami rumah terhadap tingkat kepositifan sputum BTA pada penderita TB paru di Puskesmas Tlogosadang.

Abstract

Abstract Background: Adequate health and well-being is a human right that has been recognized throughout the world. Indonesia has agreed to this as evidenced by the existence of Law No.36 of 2009. Hospital is one of the public health services that has pharmaceutical installation facilities, outpatient care, and so on. Objective: To determine the characteristics of pediatric specialist clinic patients and to determine the relationship between internal pharmaceutical service factors and the satisfaction level of pediatric specialist clinic patients at Aisyiyah Siti Fatimah Tulangan Hospital. Methods: This study is a descriptive analytic study with a cross sectional approach. The technique used is total sampling. The sample size was taken from all parents of pediatric specialist clinic patients who took medicine at the Pharmacy Installation at Aisyiyah Siti Fatimah Tulangan Hospital and met the inclusion criteria. The instrument used was satisfaction questionnaire that was collected from November to December 2019. The data obtained in this study were analyzed using the Chi Square method. Results: Based on the Chi Square test between Human Resources, Medicine, and Process, it shows p> 0.05, so there is no significant correlation between these variables and patient satisfaction. The results of the Chi Square test between the facilities and infrastructure variables showed p <0.05, which means that there is a significant correlation between the variable facilities and infrastructure for the pharmaceutical installation of Aisyiyah Siti Fatimah Tulangan Hospital and patient satisfaction. Conclusion: Internal service factor of pharmacy that has influence is infrastructure, while other factors in the form of human resources and processes have no relationship

Abstract

Background: Satisfaction is a feeling that the consumers feel when the service they receive meets or surpass their expectations. Public satisfaction with health care services is still a significant issue for health institutions in improving the service quality. Despite various efforts that health care providers have made, some residents in an area were unsatisfied with the health services in their environment.Objective: To determine the effect of the dimensions of health service quality on patient satisfaction at the Primary Health Care 1 Melaya, Jembrana, BaliMethod: Analytical observational study, a cross-sectional design. The total sample was 70 respondents. Data collection was performed using questionnaires. Multiple linear regression was used to analyze the average value of patient satisfaction based on the five dimensions of quality of health care.Result: The dimensions of tangibility, responsiveness, assurance, and empathy have a significance value (p<0.05), while the reliability dimension has no significance value (p>0.05). So the formula used to predict patient satisfaction with health services received is as follows:y = 0,352+0,295*X1–0,104*X2–0,201*X3+0,334*X4+0,180*X5The dimension of health services that has the largest influence on the satisfaction of inpatients is assurance. Patients feel assured and satisfied when the health workers communicate their expertise and competencies before doing their work.Conclusion: Tangibility, reliability, responsiveness, and assurance were four of five dimensions of health services that have significantly influenced the satisfaction of inpatients.

Abstract

Bloodstream infections (BSIs) caused by extended-spectrum beta-lactamase (ESBL)-producing Enterobacteriaceae represent a growing concern in healthcare settings, particularly in tertiary hospitals. These infections are associated with limited therapeutic options, increased mortality, and rising healthcare costs. This study aimed to describe the clinical, microbiological, and economic characteristics of adult inpatients diagnosed with hospital-acquired (HA) BSIs due to ESBL-producing Enterobacteriaceae at Dr. Soetomo General Academic Hospital from January 1 to December 31, 2024. A cross-sectional study was conducted using retrospective data from adult inpatients (≥18 years) with confirmed HA-BSIs caused by ESBL-producing Escherichia coli, Klebsiella pneumoniae, or Proteus mirabilis. Only the first positive blood culture per patient was analyzed. Community-acquired infections and non-Enterobacteriaceae organisms were excluded. The most frequent pathogen was Escherichia coli ESBL, especially among patients from medical wards. BSIs occurred predominantly in medical and intensive care units. Notably, microbiological confirmation of ESBL-producing organisms led to higher claim reimbursements through the national health insurance system, yielding a positive cost margin despite longer hospital stays and expensive treatment regimens. In conclusion, ESBL-producing Enterobacteriaceae remain a major challenge in managing HA-BSIs at Dr. Soetomo General Academic Hospital. While clinically burdensome, accurate microbiological diagnosis also supports optimal resource utilization and insurance claim efficiency in the hospital setting.

Abstract

The Muhammadiyah Hospital network (RSM) is a religious non-profit oriented facilities in Indonesia which emphasizing ethical or faith-based considerations, in approach to providing services. RSM faces the challenge of excelling compared to its competitors, including Regional General Hospitals (RSUD) and other private competitor hospitals. This study aims to examine the relationship between hospital ownership status and Google star ratings among hospitals in East Java.  A cross-sectional study was conducted involving 31 RSM, 19 RSUD, and 20 private competitor hospitals, which were compared in their respective cities. Kruskal-Wallis test was performed to compare the Google rating between each ownership, and simple logistic regression was performed to examine the association between hospital ownership and Google star rating.  Of 31 regions, private hospitals (both competitor and RSM) had the highest rating in 22 regions when compared in their respective region. Private competitor hospital had the highest median (4.2 (3.9-4.7)), followed by RSM (4.1 (3.1-4.7)) and RSUD (4.0 (2.8-4.8)). Generally, a significant difference was found between RSUD and private competitor hospital (p<0.05), while between RSM and private competitor or RSUD were not significant. Logistic regression showed a significant association between hospital ownership and Google rating, with being a private competitor being likely to have a higher rating compared to RSUD (OR (95% CI): 8.10 (1.46-45.06)). In conclusion, our findings suggest hospital ownership had significant association with Google ratings, specifically between private competitor and public hospitals. While RSM didn’t have any significant association although they tend to have higher ratings to RSUD.

Abstract

Penelitian ini bertujuan untuk menganalisis pengaruh sistem remunerasi terhadap kepuasan kerja dan kinerja karyawan di Klinik PPK1 PT Petrokimia Gresik. Latar belakang penelitian ini didasari oleh pentingnya sistem kompensasi yang adil dan berbasis kinerja untuk mempertahankan motivasi serta meningkatkan produktivitas tenaga kesehatan dalam lingkungan klinik perusahaan. Metode penelitian yang digunakan adalah kuantitatif observasional dengan pendekatan cross-sectional. Instrumen penelitian berupa kuesioner yang disusun berdasarkan Job Descriptive Index (JDI) dan Individual Work Performance Questionnaire (IWPQ), serta diuji validitas dan reliabilitasnya. Seluruh populasi sebanyak 31 orang dijadikan sampel penelitian. Hasil uji validitas dan reliabilitas menunjukkan bahwa instrumen yang digunakan dinyatakan layak dan konsisten. Namun, hasil analisis regresi dan koefisien determinasi menunjukkan bahwa remunerasi tidak berpengaruh secara signifikan terhadap kepuasan kerja maupun kinerja karyawan, dengan nilai signifikansi masing-masing di atas 0,05 dan nilai R² yang sangat rendah (0,4% dan 0,8%). Hal ini menunjukkan bahwa terdapat faktor-faktor lain yang lebih dominan memengaruhi kepuasan dan kinerja karyawan di klinik tersebut, seperti lingkungan kerja, kepemimpinan, dan aspek non-finansial lainnya. Penelitian ini memberikan kontribusi penting dalam memahami kompleksitas hubungan antara sistem remunerasi dan perilaku kerja dalam konteks klinik internal perusahaan industri, serta menjadi masukan bagi manajemen untuk merancang strategi pengelolaan SDM yang lebih efektif dan kontekstual.

Abstract

The implementation of Electronic Medical Records (EMR) at Fatimah Islamic Hospital Banyuwangi (RSI Fatimah Banyuwangi) is part of an important digital transformation in improving the quality of health services. This study aims to analyze the factors that influence the implementation of EMR, as well as to identify challenges and potential improvements in its implementation. This study uses a quantitative approach with an explanatory research design and multiple linear regression analysis to test the influence of factors such as acceptability, adoption, feasibility, fidelity, implementation cost, coverage, and sustainability on the implementation of EMR. The results show that all factors tested have a significant effect on the successful implementation of RME. Among these factors, coverage has the greatest influence with a regression coefficient of 0.995, followed by sustainability and feasibility. In addition, acceptability and adoption are also proven to contribute positively to the effectiveness of RME implementation. Although the implementation of EMR at RSI Fatimah Banyuwangi has been running well, challenges related to technical support, system sustainability, and expansion of EMR usage throughout all hospital units still need to be addressed. This study provides practical recommendations to improve technological infrastructure, enhance staff training, and ensure the maintenance and continuous updating of the EMR system.
