Abstract

Usaha global untuk memberantas TB semakin diperkuat, termasuk di Indonesia sebagai salah satu negara dengan beban tinggi TB. Untuk mewujudkannya tentu dibutuhkan jaringan program yang kuat di semua daerah. Dinas Kesehatan Propinsi Sumatera Barat ikut memberikan kontribusi dalam hal ini. Terlihat dari penemuan kasus menular (BTA+) di antara seluruh kasus TB paru yang diobati, convertion ratedan cure rateyang telah mencapai target nasional. Namun bila convertion rate dan cure rate yang tinggi diikuti dengan penemuan kasus (CDR) yang tinggi pula, tentu saja TB dapat ditanggulangi segera. Banyak cara dan strategi yang dapat dilakukan sebagai upaya akselerasi pemberantasan TB melalui penjaringan penderita. Pemerintah sebagai decision maker perlu memilih rencana yang paling efektif dan efisien untuk dilaksanakan di daerahnya. Selain itu dibutuhkan komitmen semua pihak untuk menjamin keberhasilan program TB. Sehingga visi a world free of TB bisa terwujud.

Abstract

The Covid-19 pandemic has had a comprehensive impact on all aspects of people's lives. The government has launched the Disaster Resilient Village program 2014 in empowering rural communities to face disasters. When the Covid-19 pandemic struck in West Sumatra, this program was echoed again in the form of the Nagari Tageh Program. However, the government and the Nagari community still need strengthening in planning and implementing this program. This activity aims to assist the community and the Sumaniak Nagari Government in preparing a Nagari development plan that is responsive to the impact of the Covid-19 pandemic through strengthening community empowerment. This activity was carried out in 2020 with the stages of identifying problems and priority needs of the Nagari community in reducing the impact of the Covid-19 pandemic, identifying programs and activities to resolve these problems, and discussing program financing through the Nagari government budget. This activity has resulted in programs and activities that have been agreed by the community and the Nagari government to be implemented in 2021. The resulting development plan reflects a development plan that is responsive to community problems and needs and is participatory because it involves the community in its preparation. Through partnership activities with Unand, the team will become Nagari partners starting from the planning, implementation, monitoring, and evaluation stages of the Nagari Tageh development program so that Nagari Sumaniak is ready to face pandemics and other disasters in the future and become an Independent Nagari in all aspects of development.

Abstract

Nagari Sumaniak has so much potential in various aspects that it can be developed. This potential has been utilized well, but there is still some potential that has not been fully utilized. In the aspect of governance, it was identified that the Sumaniak government had difficulties in drafting nagari regulations due to the lack of information obtained by the nagari governments related to the preparation of nagari regulations. Meanwhile, the readiness of human resources (governments) is needed as actors in the regions in carrying out government affairs. For this reason, it is necessary to increase the capacity of the nagari governments in drafting nagari regulations through legal drafting training for the nagari governments. The aim of this activity is to increase the understanding and ability of the nagari governments through drafting regulations in the nagari. The activity was carried out in 2020-2021 at Nagari Sumaniak. The target of this activity is the entire Nagari government in Sumaniak. The method used in this activity is the PRA (Participatory Rural Appraisal) method to prepare a CAP (Community Action Plan). Activities are carried out through various stages, such as FGD / brainstorming with nagari governments, revision and drafting of nagari regulations, as well as training related to legal drafting for nagari governments. From the various activities that have been carried out, it can be seen that there is motivation throughout the nagari governments to make a real contribution in providing ideas and support in the preparation of the nagari. Then, increase the capacity of the nagari governments in preparing the nagari after the training. All resources are committed to being able to assist in the development of the nagari. It is recommended for the nagari government to always carry out regular training and improve its quality and capacity in carrying out work professionally. Then, it is hoped that the local governments can provide further understanding to the community, especially regarding the nagari governments in making a nagari regulations.

Abstract

The rapid and uncontrolled transmission of COVID-19, causes the high morbidity and mortality rates of COVID-19 throughout the world, including West Sumatra (CFR 2.27%). This study aims to analyze the mortality of COVID-19 in West Sumatra and the risk factors that influence it. The research design retrospective cohort using secondary data on COVID-19 cases in West Sumatra from March 26, 2020 to June 7, 2021. The sampling technique is total sampling provided that the sample criteria are met. Data analysis used logistic regression method. 44,693 confirmed cases of COVID-19, 968 (2.17%) died and 43,725 (97.83%) did not die. Factors associated with COVID-19 mortality were age (RR 9.79; 95% CI 8.65-11.09), gender (RR 1.51; 95% CI 1.33-1.71), diabetes (RR 10.21; 95% CI 8.91-11.69), hypertension (RR 6.49; 95% CI 5.59-7.54), cardiovascular diseases (RR 9.57; 95% CI 8.17-11.21), COPD (RR 10.51; 95% CI 7.57-14.57), asthma (RR 2.03; 95% CI 1.23-3.34), and kidney diseases (RR 19.87; 95% CI 16.54-23.86). Kidney diseases are the most dominant factor associated with COVID-19 mortality (RR 16.07; 95% CI 11.34-22.78). COVID-19 patients with kidney disease have the highest risk of death. Treatment of patients with comorbidities must be prioritized and improve health promotion in at-risk groups.

Abstract

Upaya memutus rantai penularan melalui pelacakan kontak memiliki tantangan tersendiri dalam penanggulangan pandemi COVID-19 di negara berkembang, dengan laju penularan yang sangat cepat di tengah terbatasnya sumber daya yang tersedia. Penelitian ini bertujuan untuk mengekplorasi hambatan dan tantangan pelaksanaan pelacakan kontak penderita COVID-19 di Kecamatan Kuranji, salah satu kecamatan dengan jumlah penderita terbanyak di Kota Padang. Penelitian ini dilakukan dengan metode kualitatif. Pengumpulan informasi dilakukan pada bulan Januari hingga Juli 2021. Informan berjumlah tujuh orang yang dipilih secara purposif. Pengumpulan informasi dilakukan melalui wawancara mendalam dan telaah dokumen. Analisis konten dilakukan terhadap informasi yang diperoleh dengan mengacu pada komponen sistem (input, proses, dan output). Hasil penelitian ini menemukan bahwa pelaksanaan pelacakan kontak penderita COVID-19 di Kecamatan Kuranji dilaksanakan sesuai dengan panduan pelacakan kontak Kemenkes RI. Beberapa kendala dalam pelacakan kontak adalah terkait dengan kapasitas tracer, insentif, alat pelindung diri (APD), ketersediaan data alamat atau nomor telepon kasus, dan kesediaan kasus dalam memberi informasi.

Abstract

The COVID-19 pandemic in West Sumatra Province indicates a greater number of cases and mortality. The spread of COVID-19 is related to the mobility of the population, so the potential for transmission between regions is difficult to control. This study aims to determine the local index used in evaluating the tendency for local spatial groupings and can show some form of spatial relationship. Spatial analysis were conducted on 2020 to analyze spatial distribution of Covid-19 in West Sumatera Province. Spatial relationship was assessed by Local Indicator of Spatial Association (LISA). Mapping can be done with the LISA cluster map. The data used is COVID-19 incidence data based on reports from district or city in West Sumatra Province in 4 June 2020. We used Open Geoda Software to analyze the spatial distribution. There is positive spatial autocorrelation and classification in hot spots, cold spots, and outliers in the spread of COVID-19 cases in West Sumatra Province. Hot spots were found in Padang City, Bukittinggi City, and Padang Panjang City. Cold spots also detected in several districts, that is West Pasaman, Pasaman, Payakumbuh, Solok, Padang Pariaman, Pariaman, Sawahlunto, and Sijunjung. The transmission of the COVID-19 case does not recognize regional boundaries, but the grouping of districts or cities based on regional vulnerability is important as part of local control efforts to allocate resources. Coordination and collaboration among local governments need to be strengthened in preventing transmission between regions and reducing the number of cases in vulnerable areas based on hot spots and cold spots from LISA cluster map. Abstract

 Beberapa penyebab komplikasi dan kematian ibu dan bayi baru lahir adalah karena kondisi dari masyarakat itu sendiri yang kurang menyadari pentingnya melaksanakan kunjungan Antenatal care lengkap selama kehamilan. Tujuan review ini adalah untuk menganalisis dan mengkaji apa saja yang berkaitan dengan Faktor-faktor yang berhubungan dengan kelengkapan pemeriksaan kehamilan Antenatal Care K4. Metode Penelitian ini menggunakan desain tinjauan Pustaka literatur review dengan Sumber kajian literatur berasal dari artikel yang ditelusuri  melalui Google Scholer, PubMed, dan dipilih berdasarkan kriteria pencarian dari tahun 2018–2022. Kesimpulan review ini adalah Pada umumnya ibu hamil akan melakukan kunjungan Antenatal Care adalah ibu yang memiliki sikap positif serta memiliki kesadaran penu, dan mengetahui akan bahaya yang mungkin akan datang jika tidak melaksanakan kunjungan Antenatal care K4 secara rutin sesuai dengan standar yang sudah ditetapkan.

Abstract

AbstractBackground: Deaths due to TB are still a public health problem today. Monitoring tuberculosis treatment outcomes and examining factors that influence these outcomes is important for evaluation and feedback on national TB control programs. So this research aims to determine the risk factors that influence the survival of TB patients. Method: This study used a Systematic review. The data sources used are Science Direct and PubMed databases published in 2019 – 2023. The research article found 1,420 articles, filtering the abstracts according to the desired research topic, and ensuring that the articles can be accessed in full-text form, resulting in 14 studies that meet the criteria. Results: The results of this study showed that the factors that had the greatest influence on TB patient survival were age and HIV status, in 7 of 14 studies. Meanwhile, a third of the studies reviewed found that TB classification, TB anatomical location, gender, race/ethnicity, and history of DM are factors associated with TB patient survival.Conclusion: The most common variables associated with the survival of TB are age and HIV status. These conditions describe the increasing age and decreased immunity that can affect the survival of TB patients.Keywords: Mortality, Risk factors, Survival, Tuberculosis AbstrakLatar belakang: Kematian akibat TB masih menjadi masalah kesehatan masyarakat hingga saat ini. Pemantauan hasil pengobatan tuberkulosis dan pemeriksaan faktor-faktor yang mempengaruhi penting untuk dilakukan evaluasi dan umpan balik pada program pengendalian TB nasional. Oleh karena itu, penelitian ini bertujuan untuk mengetahui faktor-faktor risiko yang mempengaruhi kelangsungan hidup pasien TB. Metode: Penelitian ini menggunakan tinjauan sistematis. Sumber data yang digunakan adalah database Science Direct dan PubMed yang diterbitkan pada tahun 2019 - 2023. Artikel penelitian ditemukan sebanyak 1.420 artikel, menyaring abstrak sesuai dengan topik penelitian yang diinginkan, dan memastikan bahwa artikel dapat diakses dalam bentuk full-text, sehingga menghasilkan 14 penelitian yang memenuhi kriteria. Hasil: Hasil penelitian ini menunjukkan bahwa faktor yang paling berpengaruh terhadap ketahanan hidup pasien TB adalah usia dan status HIV, yaitu pada 7 dari 14 penelitian. Sementara itu, sepertiga dari penelitian yang direview menemukan bahwa klasifikasi TB, lokasi anatomis TB, jenis kelamin, ras/etnis, dan riwayat DM merupakan faktor yang berhubungan dengan ketahanan hidup pasien TB. Kesimpulan: Variabel yang paling umum berhubungan dengan ketahanan hidup TB adalah usia dan status HIV. Kondisi ini menggambarkan semakin bertambahnya usia dan menurunnya kekebalan tubuh dapat mempengaruhi ketahanan hidup pasien TB. Kata kunci: Faktor risiko, Ketahanan hidup, Mortalitas, Tuberkulosis

Abstract

Background: Nutritional surveillance monitors nutritional status to inform policy and programme decisions to improve community nutrition, including stunting. Chronic malnutrition and recurring illnesses cause stunting, which is when a child's length or height falls below the health minister's guidelines. Challenges include: not all Posyandu are active/covered low, not all targets have been input into the EPPGBM application (Toddlers and Pregnant Women), the use of the EPPGBM application in Community Health Centres has not been optimal, not all health workers and cadres have received height measurement training, coordination at each level, the monitoring system, and supervision to ensure height measurement quality. Due to its impact on the newborn, the first 1,000 days are critical. Permanent and lasting damage. Providing nutrition assistance at community health centre with surveillance can avoid stunting.Methods: The employed approach is participatory in nature, wherein partners are engaged to actively participate in various activities, while being supported by a dedicated staff that serves as a valuable resource and facilitator. The data analysis conducted for training purposes is to facilitate the implementation of the stunting surveillance programme, which is anticipated to encompass many sectors. This programme will primarily focus on monitoring and reporting nutrition-related activities, with a particular emphasis on stunting routines.

Abstract

Problems in the implementation of recording and reporting of tuberculosis control programs in community health centers are still found, there are differences in the data recorded in the Tuberculosis Information System (SITB) with manual recording, which affects the quality of TB data and TB program achievement indicators. This research aims to analyze the implementation of recording and reporting of the tuberculosis program from the input, process and output aspects. This type of research is qualitative and was carried out in January-August 2023 at two health centers, namely Andalas Health Center and Pauh Health Center. Interviews were conducted with eight informants using purposive sampling technique. Recording and reporting at both health centers is carried out manually and electronically (SITB) starting from patients, laboratories and logistics. The difference found was that the recording at the Andalas Community Health Center was consistent in filling out manual and SITB forms, in contrast to the Pauh Community Health Center where there were still inconsistencies that affected the quality of the data produced. In both community health centers, incomplete data was still found. Recording is carried out every day and reporting is sent to DKK Padang in the form of monthly, quarterly and annual reports. The implementation of TB program recording and reporting at both community health centers is in accordance with implementation guidelines but is not yet optimal. It is recommended that both community health centers maximize data entry in the manual register and SITB completely.
