Abstract

Background: General anesthesia is a state that changes the physiological condition of the body by removing consciousness, this consists of 3 components namely, hypnotics, analgesics, and muscle relaxation. Patients who have been given anesthetic drugs will recover and regain consciousness. ASA physical status is one of the factors that can determine the length of recovery of consciousness in patients. Methods: The method is a quantitative analytic observational research method with a cross-sectional approach. The sampling technique using consecutive sampling techniques obtained 46 respondents with inclusion and exclusion criteria. Result: The results showed that the most ASA physical status was ASA 2 (47.8%), while the most time to recover consciousness was in the delayed category (56.5%). The correlation test using Kendall's tau showed a significance of p = 0.000 (p < 0.05) with a correlation coefficient of 0.765. Conclusion: There is a strong association between ASA physical status and time to recover consciousness after general anesthesia.

Abstract

Background: The medulla distinguishes nausea and vomiting. Intra-operative nausea and vomiting (IONV) can occur up to 80% of the time following spinal anesthesia for caesarean delivery. Patients are uncomfortable and surgery is halted. Aims: This meta-analysis examines the effects of ondancentron on IONV, hypertension, and pruritus. Methods: The databases examined were CINAHL, PubMed, ScienceDirect, Wiley, and Scopus, with coverage from September 2014 to July 2024. Article text must be complete with Uji Coba Terkendali Acak design and published in English. They used R-Studio 4.3.1 m, Comperhesivif Meta-Analysis 3.3.070, and Review Manager Cochrane 5.4.1 for lunak and risk analysis. Results: The heterogeneity analysis of on- and centron residuals on IONV indicated I-squared (I2) of 99%, hypotension I2=98%, and pruritus I2=97% for 1103 people in thirteen studies. The Random Effect Model revealed a significant positive effect on Standard Mean Difference (SMD) analysis (IONV = 5.77, 95% CI: 0.58, 10.95; p=<0.01), decreased hypotension (SMD = -2.14, 95% CI: -4.80, 0.52= p=<0.01), and pruritus incidence (SMD = 0.67, 95%). CI = -1.48 to 2.83; p < 0.01. The meta regression subgroup revealed a significant decrease in IONV in the first 5 minutes with an Odds Ratio of 0.38 (95%-CI 0.04 to 3.62; p=<0.01) for the 4mg dose. Conclusions: While useful in controlling perioperative and hypotension, ondansentron may exacerbate pruritus. Ondansentron dose and pruritus effects need additional study.

Abstract

The caesarean section method is recognized recently with the concept of ERACS (Enhanced Recovery After Cesarean Surgery). The surgery process is more convenient because there is less pain and the postoperative recovery process is faster than the traditional method. The ERACS concept was developed from the ERAS (Enhanced Recovery After Surgery) concept and applied to gastrointestinal surgery. It has been shown to minimize hospital admissions and complications after surgery, which results in improved patient satisfaction. The design of this study was quantitative non- experimental using descriptive methods conducted in September 2023 with a population of all adult patients undergoing surgery under general anesthesia and a sample of 70 respondents. This method is used to study populations by collecting data to clarify and test hypotheses. This study focuses on describing the types and induction drugs with the ERACS method. in post-cesarean section patients at Hermina General Hospital Yogyakarta. Data were collected using observation techniques recorded in the observation sheet and then analyzed using the frequency distribution test using SPSS version 23. The results showed that most of the respondents in this study were women aged 26-35 years as many as 38 respondents (54.3%). Based on other respondent characteristics, it was also found that the results of blood pressure <130 and <90 mmHg were (52.9%), which is 37 respondents, followed by a pulse of 80-100x/min as many as 40 respondents (57.1%), spo2> 95% as many as 65 respondents (92.9%) ASA 1 as many as 46 respondents (65.7%), and the average use of morphine and fentanyl was (100%) using a combination of morphine drugs with a dose of 0.1mg and fentanyl with a drug dose of 25mcg.
