Abstract

Abstract

Background:  Recurrent coronary heart disease (CHD) increases the risk of complications and mortality among hospitalized patients. Previous studies report a 58% mortality rate within one year of treatment and a 30% recurrence rate in CHD patients. Purpose:  This study aimed to examine the relationship between patient characteristics and CHD recurrence. Methods:  A retrospective quantitative study was conducted at Kediri City Regional Hospital. A total of 123 CHD patients were selected using proportional random sampling. Independent variables included age, gender, education, income, and knowledge, while the dependent variable was CHD recurrence. Data were analyzed using Chi-square tests and logistic regression. Results:  Bivariate analysis showed significant associations between age (p = 0.000) and knowledge (p = 0.01) with CHD recurrence. No significant associations were found for income (p = 0.373), gender (p = 0.194), or education level (p = 0.983). Multivariate analysis confirmed age (p = 0.000, OR = 8.426, 95% CI: 3.266–21.736) and knowledge (p = 0.017, OR = 2.984, 95% CI: 1.215–7.33) as significant predictors of CHD recurrence. Conclusions:  Older patients are at a significantly higher risk of CHD recurrence compared to younger ones. Additionally, patients with better knowledge about CHD are less likely to experience recurrence. These findings highlight the importance of age-specific and educational interventions in preventing CHD relapse.

Abstract

Background: The American Society of Anesthesiologists (ASA) Physical Status classification is used to assess a patient’s overall health and predict potential complications related to spinal anesthesia. The Bromage score is commonly used to evaluate motor block recovery following spinal anesthesia, with the achievement of Bromage score 2 indicating partial motor function return. Purpose: This study aimed to analyze the relationship between ASA physical status and the time required to achieve Bromage score 2 in patients undergoing spinal anesthesia in the recovery room of Dr. Iskak Tulungagung Hospital. Methods: This research employed a cross-sectional design. The study population included all spinal anesthesia patients in the hospital’s recovery room, totaling 250 individuals. A sample of 38 respondents was selected using simple random sampling. Data were collected using observation sheets and analyzed with the Spearman Rho test at a 95% confidence level (α = 0.05). Results: The results showed that the majority of respondents (57.9%) had an ASA status of 2, and most (60.5%) achieved Bromage score 2 within 120–149 minutes. Statistical analysis revealed a p-value of 0.017 (<0.05), indicating a significant relationship between ASA physical status and time to achieve Bromage score 2. Conclusion: Higher ASA classifications are associated with more severe systemic conditions, which may slow the body's response to anesthetic drugs, resulting in a longer time to reach motor recovery.

Abstract

Introduction: The lack of knowledge and skills among family members in caring for post-stroke patients can prolong the recovery time and increase the risk of complications. Health education is essential to enhance understanding and optimize caregiver skills. This study examines the effect of structured health education on the knowledge and caregiving skills of families caring for post-stroke patients at the Poncokusumo Community Health Center, Malang. Methodology: This research employed a pre-experimental design with a total population of 35 families with the post stroke patients at Poncokusumo Community Health Center , from which 32 participants were selected using purposive sampling. The research instrument consisted of a questionnaire, and data were analyzed using the Wilcoxon test. Results: The results show that before the health education intervention, half of the participants had a moderate level of knowledge (16 people, 50.0%). After the intervention, nearly all participants demonstrated good knowledge (28 people, 87.5%). In terms of skills, before the intervention, the majority of participants had poor caregiving skills (19 people, 59.4%). After the intervention, most participants improved to a moderate skill level (19 people, 59.4%). There was a significant effect of health education on family knowledge regarding post-stroke care at Poncokusumo Community Health Center, with a p-value of 0.000 (<0.05). Conclusion: Health education is effective in improving family knowledge and skills in caring for post-stroke patients. With structured education, families understand how to care for patients, from mobilization and complication prevention to psychosocial support. Direct training and educational media help them apply skills with more confidence. This increased understanding contributes to better quality of care, accelerates patient recovery, and reduces the risk of complications.

Abstract

Background: Fatigue is a common problem in both formal and informal workplaces. World Health Organization (WHO) states that mental disorders in workers such as feelings of excessive fatigue lead to depression and are the second most common disease after heart disease. Work fatigue is a common problem in the workplace that is often encountered in the workforce and is a problem that requires attention. This study aims to Analyze the influence of workload, work shifts and work duration to work fatigue of inpatient nurses at Gambiran Regional Hospital, Kediri City. Methods: This research is quantitative with cross-sectional design, population in this research were all inpatient nurses total 174 nurses at Gambiran Hospital Kediri City. Based on slovin formula, sample in this research was 103 respondents. Data collection using questionnaire distributed directly to inpatient nurse, then analysis using multiple linier regression. Results: The regression analysis shows that workload and work duration significantly affect work fatigue among inpatient nurses at Gambiran Hospital, while work shifts do not. Work duration has the greatest influence, indicating that longer working hours increase fatigue more than workload. These findings align with previous studies showing that heavy workload and prolonged working hours contribute to nurse fatigue, whereas well-managed shift systems may mitigate its impact. Conclusions: The regression test results show that work duration has the greatest influence followed by workload, suggesting the need for further research on work fatigue with different variables among nurses in the Emergency Installation Room or Intensive Care Unit.
