Abstract

Diabetes mellitus (DM) is one of the biggest health problems in the world because every year there is an increase in cases. In addition to the increasing number, DM is also known to cause many complications which are divided into two, namely acute complications and chronic complications. The emergence of complications and leading to death in DM patients is due to the low self-efficacy and self-care behavior of DM patients themselves. Self-efficacy is an important factor that is defined as a patient's confidence in maintaining and improving his medical condition. Seeing the importance of self-efficacy in the care of DM patients, increasing patient self-efficacy is needed, namely through the Diabetes Self Management Education (DSME) program. The purpose of this study was to determine the effect of the WhatsApp-based DSME application on the self-efficacy of Type 2 DM patients. This type of research is a Quasy Experiment and the sampling technique used in this study is purposive sampling (Non-Probability sampling). The research design used was Pre-Experiment With Only one Group Pretest - Postest Design. The number of samples was 22 DM type 2 patients. Demographic data collection through questionnaires distributed via google form. The statistical test used was the Paired t test. The results of statistical tests found that there was an effect of WhatsApp-based DSME on the self-efficacy of Type 2 DM patients, with a p value of 0.000 (p <0.05). This shows that there is an effect of WhatsApp-based DSME on the self-efficacy of Type 2 DM patients at the Hamparan Perak Community Health Center.

Abstract

Background: The global prevalence of older adults with diabetes has increased, and family caregivers in Indonesia play a critical role in managing diabetes and providing personal care. However, caregiving can be complex and challenging, often negatively affecting caregivers' quality of life (QoL). Objective: This study aimed to develop and test a hypothesized causal model of QoL among Indonesian family caregivers who care for dependent older persons with type 2 diabetes mellitus (T2DM) in 2024. Methods: A cross-sectional, correlational study was conducted with 270 family caregivers recruited from five Community Health Centers. Data were collected using various scales: the Center for Epidemiologic Studies Depression Scale, the Zarit Burden Interview, the Duke University Religion Index, the Perceived Knowledge on T2DM Care Scale, the Family-Carer Diabetes Management Self-Efficacy Scale, the Multidimensional Scale of Perceived Social Support, and the Quality-of-Life Index. Descriptive statistics and Partial Least Squares Structural Equation Modeling (PLS-SEM) were used for analysis. Results: The final model explained 89.1% of the variance in the quality of life (QoL) of family caregivers (R² = 0.893, Adjusted R² = 0.891), with 66% predictive relevance. Depression symptoms had the strongest negative direct effect on QoL, followed by caregiver burden. Self-efficacy and perceived knowledge had positive direct effects, while social support showed no significant direct effect. Indirect effects revealed that social support and self-efficacy positively influenced QoL through depression symptoms. The total effect (TE) analysis confirmed that depression symptoms had the strongest negative effect on QoL (TE = -0.744, p <0.001), while social support (TE = 0.443, p <0.001) and self-efficacy (TE = 0.413, p <0.001) had positive effects. Conclusion: Reducing depression symptoms and caregiver burden, strengthening social support, and promoting self-efficacy could significantly improve the QoL of family caregivers who care for older persons with T2DM. Nursing practice should address caregivers' physical and emotional needs, provide education, foster social support, and support caregiver mental health.

Abstract

Hipertensi merupakan penyakit serius dan merupakan penyebab utama kematian dini diseluruh dunia Jika tidak ditangani dengan serius maka dapat menyebabkan kematian dini. Pengabdian kepada masyarakat ini bertujuan untuk memberikan air rebusan daun sirsak untuk menurunkan tekanan darah tinggi pada lansia penderita hipertensi di Panti Jompo Taman Boedhi Asri Medan Sumatera Utara. Kegiatan ini dilakukan pada tanggal 17  s/d 24 November 2024 selama tujuh hari diberikan air rebusan daun sirsak sebanyak satu gelas dua kali sehari (pagi dan sore hari) sesudah makan. Jumlah lansia yang berpartisipasi dalam kegiatan ini adalah 15 orang. Tekanan darah diukur sebelum mengkonsumsi rebusan daun sirsak dan sesudah tujuh hari mengkonsumsi rebusan daun sirsak dengan menggunakan tensi meter digital. Hasil pengabdian kepada masyarakat diperoleh bahwa rata-rata tekanan darah sistolik lansia 170.6 mmHg dan diastolik  rata-rata 91.4 mmHg sebelum diberikan air rebusan daun sirsak. Sedangkan stetelah minum rebusan daun sirkas selama 7 hari berturut-turut tekanan darah menurun menjadi rata-rata Sistolik 159.1 mmHg, dan diastolik rata-trata 86.9 mmHg. Hasil selisih tekanan darah adalah 11.5 mmHg pada Sistolik, dan 4.5 mmHg pada Diastolik. Dapat disimpulkan bahwa konsumsi rebusan daun sirsak dua kali sehari selama tujuh hari berturut-turut dapat menunrunkan tekanan darah pada lansia dengan hipertensi. Disarankan kepada petugas kesehatan di Panti Jompo Taman Boedhi Asri Medan Sumatera Utara agar memberikan terapi herbal daun sirsak sebagai terapi non farmakologik untuk menurunkan tekanan darah pada penderita hipertensi.

Abstract

Background: Patients with Coronary Heart Disease (CHD) tend to have a low Quality of Life Purpose: This study aims to identify the relationship between spirituality and the QoL of Patients with CHD. Methods: Descriptive correlational statistics with a cross-sectional design involving inpatients of CHD at the Adventist General Hospital, Medan. There were 89 patients who were selected using a purposive sampling technique. This study used the Daily Spiritual Experience Scale (DSES) and Quality of Life Index (QLI). Spearman Rank Correlation was used to analyze the relationship between the two variables. Results: The results of this study showed that 56.9% of CHD patients had low spirituality, and 52.8% had poor QoL. There is a significant relationship between spirituality and the QoL of CHD patients (r = -0.533; P = 0.001 <0.05). Conclusion: CHD patients with low spirituality have poor QoL. It is very important to provide interventions to improve the spirituality of patients with CHD which can ultimately improve their (QoL), which affects the recovery process. Several QoL. literatures note that the need for spirituality plays an important role in recovery in chronic patients.

Abstract

Background : It is importan to promote physical activity for elderly people, but to improve level of physicalactivity of elderly people is a challange for health workers. Purpose : The purpose of this quasi- experimentalstudy was to determine the effect of exercise consultation program by using Transtheoretical Model (TTM)approach on physical activity level in elderly people at Darussalam’ s Community Health Center. Researchmethod :The two comparison groups for this study were purposively designed for pre-test/post-test procedures;sixty elderly people at Darussalam’s Community Health Center with randomly allocated into the experimental(30 people) and the control (30 people) groups. While the experimental group received the exercise consultationfor 8 weeks, the control group received routine care. Physical Activity Scale for Elderly (PASE) was use toassess the the physical activity of elderly. Comparative assessments on differences in level of physical activityboth within group using Wilcoxson macthed-pairs and between group using Mann Withney U Test. Results :The results of this study found that after receiving exercise consultation program, level of physical activity inelderly people significantly increased in the experimental group (P < 0.05), but there was no significantdifference in the control group. Between groups, level of physical activity significantly increased afterreceiving the exercise consultation while no change was found among those who did not (P <0.05).Conclusion: The exercise consultation program by using TTM approach could increase level of physicalactivity of elderly people.

Abstract

Hypertension is one of the factors of stroke. the number of people with hypertension in the elderly is very high. The purpose of the research was determine the relationship between hypertension and the people who get stroke in the elderly at Taman Bodhi Asri Medan, Kecamatan Medan  Sunggal  in 2024. The method of the research was an analytic survey using a cross – sectional design. The population of the research were all hypertension and stroke as many as 46 people. The technique of sampling used by total sampling with 46 elder people who affected by hypertension and stroke. Data was collected by looking at medical records. The statistical test was used by Fisher’s Exact Test. The result of the study obtained in grade 1 of hypertension (140/90 – 159/99 mmHg) were 36 people (78,3), grade 2 of hypertension (160/100 – 179/109) were 10 people (21,7%). There were 17 people (37%) who had a stroke and 29 people (37%) who did not have a stroke. The result obtained by using Fisher’s Exact test showed that the hypertension in grade 2 correlated with stroke’s patients, have 8 people (80%) with p value 0,003 and having an old ratio = 10,106. This is because of hypertension in grade 2 is 10,106 times more possible to get a stroke. It was concluded that the higher of hypertension’s degree,  will get the stroke faster. Therefore, this study is expected that one who with hypertension must be careful. Do not let the degree of hypertension increase because it will be 10.106 times to get a stroke.

Abstract

Latar belakang ;Ketidakstabilan kadar glukosa darah dapat menyebabkan berbagai komplikasi serius jika tidak dikontrol dengan baik. Pengelolaan DM tidak hanya melalui pengobatan medis, namun juga melalui pengaturan pola makan, termasuk konsumsi buah-buahan yang kaya antioksidan seperti buah naga (Hylocereus spp.). Buah naga diketahui mengandung serat, vitamin C, dan senyawa aktif yang dapat membantu menstabilkan kadar glukosa darah. Tujuan: Kegiatan ini bertujuan untuk meningkatkan pengetahuan masyarakat, khususnya penderita DM, mengenai manfaat konsumsi jus buah naga sebagai bagian dari pola hidup sehat dalam mengelola kadar glukosa darah. Metode: Kegiatan pengabdian masyarakat ini dilaksanakan di wilayah kerja Puskesmas Helvetia melalui metode ceramah, diskusi interaktif, dan demonstrasi pembuatan jus buah naga. Sebanyak 20 peserta kegiatan adalah penderita DM dan keluarga yang terdaftar dalam program manajemen penyakit kronis (Prolanis). Evaluasi dilakukan melalui pre-test dan post-test untuk mengukur peningkatan pengetahuan peserta. Hasil: Kegiatan ini menunjukkan peningkatan pengetahuan peserta tentang manfaat buah naga terhadap kestabilan kadar glukosa darah. Skor penegtahuan sebelum diberikan intervensi rata-rata 3.5, setelah dibeikan intervesi meningkat menjadi 7.65, P=0.000 < 0.05. Mayoritas peserta menunjukkan minat untuk memasukkan jus buah naga ke dalam menu harian mereka sebagai upaya mandiri dalam mengelola DM. Kesimpulan: Sosialisasi konsumsi jus buah naga memberikan dampak positif dalam meningkatkan pengetahuan dan kesadaran penderita DM tentang pengelolaan penyakit melalui pendekatan alami dan edukatif. Diharapkan kegiatan ini menjadi langkah awal untuk penerapan pola hidup sehat yang berkelanjutan di masyarakat.

Abstract

Background: The quality of life (QoL) of family caregivers supporting older persons with functional dependence due to Type 2 Diabetes Mellitus (T2DM) is a growing concern. Accurately assessing caregiver well-being requires culturally appropriate, valid, and reliable instruments. However, such tools remain limited in the Indonesian context. Objective: This study aimed to translate the Quality of Life Index (QLI) into Indonesian and evaluate the psychometric properties of the Indonesian version (QLI-I) among family caregivers of dependent older persons with T2DM. Methods: Brislin’s back-translation method was used to ensure linguistic and conceptual equivalence in the translation process. Content validity was assessed by a panel of five experts. Psychometric evaluation was conducted on a purposive sample of 394 family caregivers recruited from eleven community health centers in Medan, Indonesia. Construct validity was examined using confirmatory factor analysis (CFA), while convergent validity and construct reliability (CR) were analyzed to assess measurement accuracy. Results: The Indonesian version of the Quality of Life Index (QLI-I) demonstrated strong construct validity, with standardized factor loadings ranging from 0.707 to 0.870. CFA indicated an acceptable model fit based on the following indices: Root Mean Square Error of Approximation (RMSEA) = 0.067, ratio of chi-square to degrees of freedom (CMIN/df) = 2.779, Comparative Fit Index (CFI) = 0.906, Incremental Fit Index (IFI) = 0.906, Goodness of Fit Index (GFI) = 0.814, and Tucker-Lewis Index (TLI) = 0.898. Composite Reliability (CR) values were high across all domains: Health/Functioning (HF) = 0.957, Family (FA) = 0.876, Social/Economic (SE) = 0.924, and Psychological/Spiritual (PS) = 0.922. Average Variance Extracted (AVE) values met the recommended threshold for each domain: HF = 0.633, FA = 0.586, SE = 0.605, and PS = 0.627. All constructs demonstrated acceptable discriminant validity, confirming the distinctiveness of each domain. Conclusion: The QLI-I is a valid and reliable instrument for assessing the QoL of family caregivers of dependent older persons with T2DM. Its use in clinical and community settings can support nurses and healthcare professionals in identifying caregiver needs and developing culturally appropriate, evidence-based interventions to enhance caregiver well-being.

Data analysis involved descriptive statistics and chi-square tests. The study found that the caregivers aged between 60 and 74 years reported a significantly higher prevalence of poor QoL (78.9%). Male caregivers were more likely to report poor QoL (84.7%) compared to female caregivers (45.0%). Unmarried caregivers have good QoL (100%) compared to widowed and married status (24% and 48.7%). Low level of education has poor QoL 83.1%. Spouse caregiver has poorer QoL compared to child and other relative’s relationships. Those caring for older persons with T2DM for less than 5 years (68.7% for 1-2 years and 71.9% for 3-5 years) reported poorer QoL compared to those caregiving for more than 6 years (44%). Caregivers who look after older adults with two and more than three chronic illnesses and complications tend to have a lower QoL. This study highlights that older age, male gender, married/widowed, elementary school, caregiver spouse, shorter caregiving duration (less than 5 years), and having comorbid and complication tend to have poorer QoL.
