Abstract

Dengue infections, such as Dengue Fever (DF) and Dengue Hemorrhagic Fever (DHF), remain significant public health issues in Indonesia, including Bali Province. The high incidence and mortality rates due to DHF have substantial impacts, particularly concerning treatment costs and length of hospitalization. This study aims to analyze the correlation between hematocrit levels and hospital length of stay among DHF patients at RSUD Wangaya in 2023. The variable studied was hematocrit levels. This research employed an analytical observational method with a retrospective cross-sectional design, using medical record data from DHF patients at RSUD Wangaya from January 1, 2023, to December 31, 2023. Data analysis was conducted using Pearson's correlation test to assess the relationship between hematocrit levels and hospital length of stay. The study results showed that the majority of patients were male (58.3%) and that most were under 50 years old (66.7%). The most common clinical manifestation was fever (98.1%), and most patients had platelet counts below 100,000/µL (77.8%). The average hematocrit was 40.6% in the group with levels <50% and 52.1% in the group with levels ?50%. Bivariate analysis using Spearman's correlation test showed no significant association between hematocrit levels and hospital length of stay among dengue patients (p = 0.835). This study concludes that although hematocrit levels are critical clinical indicators in DHF patients, no significant correlation was found between hematocrit levels and hospital length of stay.

Abstract

Background: Dengue infection remains a major public health problem and is frequently accompanied by thrombocytopenia. The lowest platelet level during illness (thrombocyte nadir) may reflect clinical severity and assist in early risk stratification. Objective: To compare the mean thrombocyte nadir between patients with dengue fever (DF) and dengue hemorrhagic fever (DHF). Methods: A cross-sectional study was conducted using medical record data of 150 hospitalized dengue patients at Sanjiwani Hospital (2022–2024). Dengue was confirmed by IgM anti-dengue testing or NS1 antigen detection. DHF was defined as a ≥20% increase in hematocrit at admission. Patients with a history of idiopathic thrombocytopenic purpura were excluded. Demographic and clinical data were summarized descriptively, and differences in thrombocyte nadir between DF and DHF were analyzed statistically. Results: Of 150 patients, 119 (79.3%) had DF and 31 (20.7%) had DHF. The thrombocyte nadir differed significantly between groups, with DHF demonstrating a lower nadir than DF (p = 0.001). Conclusion: Thrombocyte nadir is significantly lower in DHF compared with DF, supporting the usefulness of serial platelet monitoring to aid severity assessment and clinical surveillance in hospitalized dengue patients.

Abstract

Multidrug-resistant tuberculosis (MDR-TB) remains a public health crisis and threat globally, although the incidence of TB is falling at about 2% per year. The main cause of MDR-TB is the low patient adherence to the lifelong TB regimen. High pill burden with long-course treatment simultaneously induces low adherence of the patient to continue the TB regimen. The patient should take four types of TB drugs for at least 6 months. Family support in the management of TB is absolutely needed to alleviate the incidence of TB multidrug resistance. Drug resistance emerges when anti-TB medicines are used inappropriately, through incorrect prescription by health care providers, poor quality drugs, and patients stopping treatment prematurely. The objective of this program is to educate the TB patient's family in monitoring and assisting the patient in taking the TB drug every day and reporting any side effects of the TB drug as soon as possible to prevent low adherence to the treatment. The planned methods are to train the patient's family to monitor the patient's adherence and to assist with treatment at home.

Abstract

Students who manage school health units often have limited knowledge of infectious diseases and health literacy. This program aims to enhance students' understanding of infectious diseases and health literacy skills at Payangan 1 State Senior High School, who oversee the school's health unit. To achieve this, the community service program at Payangan 1 State Senior High School uses an outreach approach and health literacy-focused demonstrations. The partners in this initiative include students managing the school's health unit, supportive teachers, and health professionals. The PKM consists of several stages, including material preparation. Counselling sessions on infectious diseases (such as HIV, dengue fever, and tuberculosis) for the school's health unit management students, utilizing presentations and focus group discussions.  Improvement of the school's health unit facilities and distribution of personal protective equipment (PPE). Counselling on materials related to adolescent entrepreneurship. To evaluate the program's effectiveness, pretests and posttests were administered to measure changes in knowledge of infectious diseases and health literacy. The results showed a 65.1% increase in knowledge about infectious diseases and a 50% increase in health literacy. It is anticipated that this program will positively impact students' physical, mental, and social health, create a healthier school environment, and empower students to become agents of change within their communities.
