Abstract

The correlation between visceral fat levels (VF) or muscle mass and insulin resistance or type-2 diabetes mellitus has been studied extensively elsewhere, particularly in the urban-dwelling populations. The current study aimed to evaluate the association of VF or muscle mass with random blood glucose (RBG) among the rural population. A cross-sectional study was conducted in Buleleng Regency. A total of 105 samples were enrolled in the study and obtained through a systematic interview, random blood glucose measurement, anthropometry, and bioelectrical impedance analysis. Spearman’s correlation test was used to determine the correlation between visceral fat levels (VF) or muscle mass with RBG levels. A partial correlation test was used to control confounding variables (age, muscle mass, body mass index, body weight, waist circumference, hip circumference). Most of the samples (61.9%) were female, with a mean age was 53,48 ± 13,03. The statistical analysis results show VF had a significant correlation with RBG (r= 0.363, p<0.001 but neither as role of muscle mass in which the r= -0.073, p=0.461). There was no significant correlation found on the partial correlation test, between VF and RBG. In conclusion, although the correlation between VF levels and RBG was weak, the clinician should aware of the role of VF in the development of insulin resistance or diabetes mellitus type-2.

Abstract

Dengue infection is one of the significant global health problems, especially in children. This paper aims to identify factors that affect the severity of dengue infection in the child population. The factors analysed included age, infection status, obesity, viral genome and Ns1 protein titers of dengue. Children who are younger and have an obese nutritional status tend to experience more severe symptoms. In addition, high NS1 titers, viral genomes, and secondary infections also contribute to a higher risk of infection. Detecting warning signs is important because it can be used as a predictor of more severe dengue infection.

Abstract

Management of intradialytic nonthrombotic complications in patients with chronic kidney disease is an essential aspect of hemodialysis care. These complications, such as hypotension, hypertension, DDS, muscle cramps, nausea and itching, can have a significant effect on the patient's well-being. Effective treatment requires a multidisciplinary approach that involves the medical team, patient education, and close monitoring during dialysis sessions. Effective protocols, including fluid volume regulation and the proper use of medications, play a crucial role in reducing the frequency and severity of complications. With the right management strategy, it is hoped that the quality of life of patients can be improved, and the morbidity rate associated with intradialytic nonthrombotic complications can be minimised. Further research is needed to optimise clinical practice and enhance the understanding of these complications.

Abstract

Stunting masih menjadi salah satu permasalahan kesehatan global yang perlu mendapat perhatian serius. Kondisi ini dapat bermula sejak masa kehamilan, berlanjut pada periode pertumbuhan anak, khususnya dalam 1000 hari pertama kehidupan. Penanggulangan stunting memerlukan pendekatan komprehensif yang menyasar berbagai faktor penyebabnya. Kegiatan pengabdian masyarakat ini bertujuan menekan prevalensi stunting di Desa Ban, Kecamatan Kubu, Kabupaten Karangasem, Bali. Metode yang diterapkan meliputi edukasi mengenai pencegahan dan penanganan stunting, praktik penyusunan menu bergizi seimbang melalui konsep “Isi Piringku”, serta penyaluran bantuan pangan berupa ayam buras, telur ayam, dan beras organik. Selain aspek kesehatan, kegiatan ini juga mencakup penyuluhan terkait perlindungan hukum bagi perempuan, pelatihan singkat Bahasa Inggris, serta pelatihan pembuatan pakan ternak ayam. Hasil kegiatan menunjukkan adanya peningkatan keterampilan ibu dalam menyiapkan makanan bergizi seimbang berbasis “Isi Piringku” sebesar 49,8%. Oleh karena itu, dukungan dan partisipasi masyarakat secara berkelanjutan sangat diperlukan untuk menurunkan angka kejadian stunting.

Abstract

Hypertension in Chronic Kidney Disease (CKD) remains a global health issue, often presenting as resistant hypertension even with multiple antihypertensive medications. This study aims to describe the characteristics of hypertension in CKD patients undergoing hemodialysis at Sanjiwani Regional Hospital. A descriptive observational study with a cross-sectional design was conducted using secondary data from medical records of CKD patients with hypertension who underwent hemodialysis from January 2022 to December 2023, involving 100 patients. The study found that the highest proportion of hypertensive CKD patients were aged 46-65 years (60%), predominantly male (63%), with grade 1 hypertension (69%). The majority had CKD for ≥5 years (60%) and were taking ≤3 antihypertensive medications (72%). The commonly used antihypertensive drug class was calcium channel blockers (68%), with diabetes mellitus identified as the primary etiology (45%). Many CKD patients experience resistant hypertension, primarily affecting males aged 46-65 years.

Abstract

Snakebites are a common occurrence in tropical regions, including Indonesia. This study aims to determine the demographic and clinical characteristics of patients bitten by snakes. It employs a descriptive, cross-sectional approach to analyze cases at Sanjiwani General Hospital. The sample includes all snakebite cases from January 2021 to December 2022, totaling 192 cases. The results indicate that the majority of snakebite incidents occurred in individuals aged 40-49 years (25.0%), predominantly among males (69.3%), and that most cases originated in Gianyar District (26.6%). The most frequently reported clinical symptom was pain (96.9%), with 91.1% of cases graded 2. Most patients did not experience complications (97.9%). Fang marks were the most common type of wound (96.9%), with the upper extremities being the most frequent bite site (51.6%). Laboratory examinations revealed that the average complete blood count was within the normal range, although prothrombin time (PT) and activated partial thromboplastin time (APTT) were prolonged. The treatments administered included antivenom (99.0%), Human Tetanus Immunoglobulin (94.3%), and antibiotics (99.5%). Notably, all patients in the study survived. The findings highlight the urgent need for targeted public health interventions to educate high-risk populations about snakebite prevention and the importance of seeking prompt medical treatment. Additionally, this study emphasizes the need for improved healthcare resources and training for medical personnel to manage snakebite cases effectively, thereby improving outcomes and reducing complications.

Abstract

Kesehatan merupakan kebutuhan dasar setiap individu. Pemeriksaan kesehatan secara rutin sangat penting untuk mendeteksi dini berbagai penyakit dan mencegah komplikasi lebih lanjut. Demam berdarah, japanese ensefalitis, tuberkulosis, diare, COVD-19 masih menjadi masalah kesehatan global. Kementerian Kesehatan Indonesia menunjukkan bahwa penyakit tidak menular seperti hipertensi, diabetes melitus, peyakit ginjal kronis dan penyakit jantung kini menjadi penyebab utama kematian di Indonesia. Melihat pentingnya hal tersebut, kami berinisiatif untuk mengadakan program pemeriksaan kesehatan gratis sebagai upaya meningkatkan kesadaran akan pentingnya hidup sehat dan deteksi dini penyakit. pelaksanaan pemeriksaan kesehatan gratis dilakukan di Desa Sidan, Gianyar dengan tujuan meningkatkan kesadaran akan pentingnya hidup sehat dan deteksi dini penyakit. Program ini meluputi pelayanan pemeriksaan dan pengobatan gratis bagi masyarakat yang membutuhkan. Dalam program ini juga memberikan penyuluhan mengenai gizi, kesehatan, serta infeksi. Secara keseluruhan, kegiatan bakti sosial ini berhasil mencapai tujuan yang telah ditetapkan dan kegiatan ini diharapkan menjadi model bagi kegiatan serupa di daerah lain, dengan penekanan pada pentingnya deteksi dini, edukasi kesehatan, dan pencegahan penyakit.

Abstract

Background The coronavirus disease (COVID-19) pandemic still happening and when it’s going to be resolved is not known. In this COVID-19 era, physicians need to better understand the risk and purpose of giving drugs that patients do not need. Proton pump inhibitors (PPI) are sometimes easily prescribed and misused by physicians. The study objective is to find out whether PPI use is associated with better or worse outcomes in patients with COVID-19. Method We searched retrospective studies in various publication libraries like PubMed, Embase, and CENTRAL from 2020 to 2022. Inclusion criteria were studied which differentiated patients with COVID-19 who regularly used PPI and control which is COVID-19 patients who did not use PPI. That study also needs to report the outcomes. The outcome was then divided into two categories which are good outcomes and worse outcomes consisting of severe COVID-19 needing oxygen therapy, admission to intensive care unit (ICU), acute respiratory distress syndrome (ARDS), shock or mortality, to get each study and total odd ratio (OR), 95% confidence interval, and weight. Studies that did not report the outcomes were excluded. We also analyze the data using a fixed or random effect model accordingly and asses the possibility of publication bias using Egger’s test. Case Seven of 11 studies with more than 30.000 COVID-19 patients were analyzed in this study. These patients were divided into 2 groups: patients with COVID-19 who were using PPI up to 30 days before being infected and COVID-19 patients who didn’t use PPI before. The total number of patients in the first group is 3531 patients and the second group is 38138 patients. After statistical analysis, we found that the data is heterogenous with p <0,05, I2 94,22% (95%CI 90,44-96,51%) suggesting the OR needs to be determined in the random effect model. We found that pooled OR is 1.99 (p 0.01, 95% CI, 1.18-3.38). Egger’s test for the possibility of publication bias is 0,64 (95%CI -7,24-4,93). Conclusion COVID-19 patients who use PPI are twice as likely to have a worse outcome than COVID-19 patients who don’t use PPI. This study is statistically significant with a low possibility of publication bias.

Abstract

Traveler’s diarrhea (TD) remains a prevalent condition affecting individuals traveling to regions with varying sanitation standards. The diagnosis of TD has evolved with advancements in diagnostic tools and a better understanding of its etiology. Traditionally diagnosed based on clinical presentation and exposure history, current approaches increasingly incorporate molecular techniques to identify causative pathogens more accurately. Recent updates highlight the integration of polymerase chain reaction (PCR) and other molecular diagnostic methods that offer rapid, sensitive, and specific detection of bacterial, viral, and parasitic pathogens. These advances enable differentiation between infectious and non-infectious causes of diarrhea, which is crucial for effective management and treatment. Additionally, the role of stool cultures, although less commonly used in rapid diagnostics, remains important for comprehensive pathogen identification. Emerging research emphasizes the need for a tailored diagnostic approach considering geographic region, patient history, and symptom profile. The use of multiplex assays and metagenomic sequencing is showing promise in identifying a broad spectrum of pathogens and understanding the complex microbiological landscape of TD. This update underscores the importance of adopting a multifaceted diagnostic strategy to improve the accuracy of TD diagnosis, thereby enhancing patient care and contributing to the development of targeted treatment and preventive measures.

Abstract

Dengue infection remains a major global health problem. This study aimed to determine factors associated with the severity of dengue infection among adult patients treated at Sanjiwani Gianyar Regional General Hospital in 2023. An analytical observational study with a cross-sectional design was conducted using consecutive sampling. A total of 100 adult patients with confirmed dengue infection were included. Severity was classified as Dengue Fever (DF) and Dengue Hemorrhagic Fever (DHF) based on WHO criteria. Chi-square analysis and crude odds ratio (OR) estimation were performed. Age >60 years was significantly associated with severe dengue (OR = 2.94; 95% CI: 1.25–6.92; p=0.014). Female gender was also associated with higher odds of severe dengue compared to males (OR = 2.33; 95% CI: 1.05–5.15; p=0.039). Secondary infection status showed the strongest association with severity (OR = 5.40; 95% CI: 2.24–13.04; p=0.001). In conclusion, age, gender, and infection status were significantly associated with dengue severity. These findings may assist clinicians in identifying patients who require closer clinical monitoring.
