Abstract

BackgroundOne health indicator assessed for achieving the Sustainable Development Goals (SDGs) is the nutritional status of toddlers. Stunting, indicating chronic malnutrition in children, remains a pressing concern globally, especially in low‑ and middle‑income countries. Stunting is still prevalent in Jakarta as the capital city of Indonesia. The study's objective was to evaluate the implementation of the family medicine approach on stunting in toddlers in Koja Sub-District, North Jakarta. MethodsA pre- and post-test design study was conducted involving 40 toddlers who were diagnosed with stunting. Outcome variables were body weight, height, and stunting while the independent variable was the family medicine approach (home visits, family education, and monitoring for 3 months). The Friedman test was used to determine the difference in height and weight after 3 months of the family medicine approach. ResultsAfter 3 months of the family medicine approach, consisting of holistic history taking, performing a thorough physical examination, educating parents about toddlers' nutritional issues, and conducting routine monthly home visits, the toddlers’ body weight increased by 0.68 kg and their height increased by 1.37 cm (p<0.001). The total number of cases of stunting after the family medicine approach decreased by 30% (12/40) from 40 to 28. ConclusionsThere is a decrease in the number of stunting cases after the family medicine approach, but further research is needed. The study findings will help to improve policy measures focusing on the family medicine approach to reduce childhood stunting.

Abstract

Background Hospitalization disrupts children’s lives and can produce feelings of anxiety, fear, or pain. Playing is an important part of children’s lives.It is necessary for nurses to provide holistic and quality care, including therapeutic play, which contributes to the recovery of children by relaxing them both physically and emotionally. The aim of this study was to determine the level of knowledge of pediatric nurses about therapeutic play and find out the relations of therapeutic play with nurses’age, educational level, marriage status, and length of employment. MethodsThis cross-sectional study was conducted with 126 pediatric nurses working in the pediatric service of a training and research hospital. The data of the study were collected with the “Nurse Introductory Information Form” and “Questionnaire for Determining the Level of Knowledge of Therapeutic Play”. Descriptive statistical tests and nonparametric tests were used to analyze the data. Statistical significance was set at p<0.05. ResultsIn this study, 88.1% of the nurses stated that they did not receive training in therapeutic play, and 79.4% stated that they wanted to receive training in therapeutic play. The nurses’ level of knowledge about therapeutic play significantly decreased with age and increased with educational level (p<0.05). ConclusIonsThe knowledge level of the nurses participating in the study about therapeutic play is above average, but is low regarding the integration of therapeutic play into care. It is recommended that nurses receive in-service training in therapeutic play.

Abstract

The global prevalence of diabetes is rising at an alarming rate, with nearly 11% of adults currently affected. Projections estimate that by 2045, approximately 800 million individuals—1 in every 8 people—will have diabetes, representing a 46% increase in cases. Effective dietary management, however, offers the potential to delay or reduce diabetes-related complications. Gamma-oryzanol (γ-ORZ), a bioactive compound in brown rice (BR), has shown promising effects on type 2 diabetes mellitus (T2DM) and its complications, as evidenced by various scientific studies. Despite its potential, γ-ORZ's mechanisms of action remain underexplored, and BR consumption is less prevalent compared to white rice (WR). This review aimed to examine the effects of γ-ORZ on diabetes, promoting further research and encouraging the adoption of BR in dietary practices. Relevant studies were identified through a systematic search of PubMed, ScienceDirect, and Google Scholar for articles published between January 2013 and December 2023 using the keywords “(Gamma-oryzanol OR γ-oryzanol OR γ-ORZ) AND (Diabetes Mellitus OR Type-2 diabetes mellitus OR T2DM OR Hyperglycemia OR Insulin Resistance).” From an initial pool of 1,912 articles, 15 studies meeting the inclusion criteria were reviewed. Findings revealed that γ-ORZ exhibits antihyperglycemic, antidyslipidemic, anti-inflammatory, and antioxidant properties. It mitigates β-cell dysfunction, improves adipocyte function, enhances insulin secretion and sensitivity, and alleviates diabetic cardiomyopathy. This review underscores γ-ORZ's therapeutic potential in managing diabetes and its complications, while highlighting the need for more robust studies to validate its efficacy and compare it with standard treatments.

Abstract

BackgroundQuincke's disease, or isolated uvular angioedema, is a rare clinical condition and reports on its etiology and management remain limited. Most cases are linked to idiopathic conditions, food allergies, airborne allergens, medications (ACE inhibitors, NSAIDs), hereditary angioneurotic edema, oropharyngeal trauma, and infections. The disease may be encountered in any emergency setup and must be dealt with rapidly and with utmost vigilance to prevent progression and complications. Case descriptionA 24-year-old man visited the primary care clinic complaining of sudden throat discomfort upon waking. He described it as the sensation of a round object touching “the sides of his throat”. The sensation persisted despite swallowing. The patient denied having a sore throat, hoarseness, fever, or cough, and said that he had never experienced similar symptoms. The patient did not consume anything before he went to sleep and woke up with the sensation that something was 'stuck' in the back of his throat. Examination revealed an edematous and hyperemic uvula. Initial management ensured that there was no airway obstruction, with antihistamines and corticosteroids administered to relieve the uvular edema and discomfort. The patient said that the symptoms subsided within 72 hours and that there was no recurrence. ConclusionAwareness about this rare but acute condition, even in the background of unknown etiology, in all medical personnel is essential. Early diagnosis with appropriate management can prevent life-threatening airway obstruction and hypoxemia. Treatment depends on airway symptom severity. Thorough examination of each case is crucial, focusing on etiology and definitive management.

Abstract

BackgroundPhysiological and immunological changes in the elderly may contribute to the composition and patterns of the gut microbiota, including Escherichia coli, as well as antimicrobial susceptibility patterns. The research was conducted to assess the effect of aging on Escherichia coli growth and susceptibility to several antibiotics. MethodsAn experimental laboratory study using 7 young (6-month-old) (YWR) and 7 old (24-month-old) Wistar rats (OWR) as subjects. Rat stool specimens were used as representing the subjects. Antibiotic susceptibility was determined using the Kirby-Bauer method. The independent t-test and Mann-Whitney test were used to analyse the data. ResultsAll fecal specimens collected from both groups were positive for E. coli growth. The YWR showed a low density of E. coli, while the OWR showed a statistically significant increase in the population of E. coli (p=0.001). Overall, the E. coli isolates showed a high proportion of resistance to erythromycin (100%), ampicillin (86%), and oxytetracycline (58%). The isolates collected from YWR were significantly more resistant to streptomycin (83% vs. 19%, p<0.001) and tetracycline (47% vs. 3%, p<0.001) than the isolates from OWR. Young Wistar rats also had a higher resistance rate than OWR to the most frequently used antibiotics, such as ampicillin (97% vs. 76%, p<0.001). ConclusionOur study demonstrated an increase in the fecal E. coli population with increasing age of the Wistar rats. In addition, the results of susceptibility tests for several antibiotics showed that age may not be associated with an increase in the population of a resistant strain of E. coli.

Abstract

BACKGROUNDCarica papaya is traditionally used to treat malaria. The mechanism of action of the active constituents may be determined by molecular docking. This study therefore examined the in silico antimalarial activity of selected compounds from Carica papaya using Plasmodium falciparum dihydrofolate reductase thymidylate synthase (PfDHFR-TS) as target protein. METHODSAntimalarial activity screening of Carica papaya compounds was done in silico using AutoDock 4.2 software which was equipped with Autodock tools 1.5.6 for preparation. Five compounds contained in Carica papaya leaves, i.e. quercitrin, isoquercitrin, carpaine, caricaxanthin, and violaxanthin were successfully docked with the target protein. The molecular docking method is declared valid if the RMSD obtained is not more than 2 Å. In vitro evaluation of the test compounds as antimalarials was accomplished by determining their inhibitory activity against dihydrofolate reductase thymidylate synthase (PfDHFR-TS) which plays a role in the synthesis of nucleotides needed by Plasmodium falciparum. RESULTSValidation of Plasmodium falciparum DHFR-TS with PDB ID 1J3I showed an RMSD value of 1.58 Å. The docking results showed that quercitrin, isoquercitrin, carpaine, and caricaxanthin showed negative energy values similar to the native ligand. Therefore the four compounds have good affinity for the target protein, while violaxanthin shows a positive energy value, indicating no affinity for the target protein. CONCLUSIONBased on binding affinity values and molecular interactions, isoquercitrin and quercitrin have inhibitory activity against dihydrofolate reductase thymidylate synthase (PfDHFR-TS), such that they have potential as natural antimalarial candidates.

Abstract

BackgroundBreast cancer therapy is mostly influenced by the type and molecular subtype, especially in the case of human epidermal growth factor receptor 2 (HER2) status, where HER2-positive patients will receive anti-HER2 therapy. HER2 status is obtained from HER2 immunohistochemistry examination which can be performed on primary breast tumors or lymph node metastatic lesions. This study aimed to determine the concordance of HER2 status between primary tumors and lymph node metastatic lesions in invasive breast cancer. MethodsA cross-sectional study was conducted involving 30 invasive breast cancer patients. HER2 immunohistochemistry examination was carried out on both the primary tumor and lymph node metastatic lesions. The Cohen κ coefficient was used to analyse the data. Results The concordance rate for HER2 was 86.67%. Thirteen cases were concordantly HER2-negative in primary breast cancer (BC) and nodal metastases, and 13 cases were HER2-positive in both primary and metastatic tumors. Changes in HER2 status between primary BC and corresponding synchronous metastases were observed in four (4.72%) cases. One of the discordant cases was HER2-negative in the primary tumor and HER2-positive in the metastases, while three cases were HER2-positive in the primary breast cancer and HER2- negative in the metastases. Conclusion There is a discordance of HER2 status between the primary tumor and lymph node metastases in invasive breast cancer patients. It is necessary to evaluate the HER2 status of the primary tumor and metastases simultaneously. Such an evaluation is recommended for better prognosis and survival.

Abstract

Background Acquired immunodeficiency syndrome (AIDS) remains a global health issue. Antiretroviral therapy (ART) controls HIV progression, but widespread use had led to drug resistance mutations (DRMs) such as M184V and K103N, compromising treatment efficacy. This study assessed the prevalence of these mutations and identified associated risk factors in patients with first-line nucleoside reverse transcriptase inhibitor (NRTI)-based ART. Methods A cross-sectional study was conducted involving 80 HIV patients aged > 18 years who had been on NRTI and non-NRTI (NNRTI) therapy for >6 months. Data included sociodemographic characteristics, ART adherence, opportunistic infections, viral load, CD4 count, treatment duration, ART regimen, and presence of M184V and/or K103N mutations. Genetic analysis was performed and statistical associations were assessed using simple and multivariate logistic regression. Results M184V and/or K103N mutations were detected in 8 patients (10%), significantly associated with poor ART adherence (p<0.01), detectable viral load (p<0.01) and male gender (p=0.048), but not with age (p=0.653), body mass index (p=0.661), opportunistic infections (p=0.938), CD4 count (p=0.265), and treatment duration (p=0.365). Multivariate logistic regression analysis showed that poor ART adherence was associated with a decreased risk of mutations compared to good adherence (OR = 0.73, 95% CI:0.012-0.427) and male patients had a 10-fold higher risk compared to females (OR = 10.03, 95% CI:1.033-97.350). Conclusion This study demonstrated that male gender was significantly associated with an increased risk of mutations, while poor ART adherence showed an unexpected inverse association. Strengthening adherence support programs remains essential to preventing drug resistance mutations and ensuring treatment efficacy.

Abstract

BACKGROUNDSepsis is a heterogeneous syndrome characterized by a variety of clinical features. Multiple studies have identified sepsis as the leading cause of death in hospitalized patients. A comprehensive report on the incidence, clinical characteristics, and predictors of sepsis is important. This study aimed to determine the relative importance of predictors of in-hospital mortality in sepsis. METHODSA retrospective cohort study at Dr. M. Djamil Central General Hospital focused on sepsis patients. A total of 200 participants, aged 18 and older, were included based on specific criteria and recruited through consecutive sampling. Data was gathered from medical records and laboratory results to identify factors influencing mortality in sepsis patients. These factors were classified into sociodemographic, intrinsic, and extrinsic categories. Statistical analysis utilized simple and multiple logistic regression. A p-value of less than 0.05 indicated statistical significance for predicting in-hospital mortality in sepsis. RESULTSThe sepsis patient mortality rate was 69.50%. Hospital-acquired pneumonia (HAP) emerged as the most common infectious diagnosis, impacting 47.50% of the patients. Type 2 diabetes mellitus (Type 2 DM) was identified as the most frequent comorbidity, present in 36.50% of cases. Multivariate analysis indicated that HAP (adjusted odds ratio [aOR] 2.32; 95% confidence interval [CI] 1.19–4.49; p=0.013) and hyperlactatemia (aOR 2.11; 95% CI 1.06–4.18; p=0.032) significantly increased the risk of mortality in sepsis patients. CONCLUSIONHospital-acquired pneumonia was the primary predictor of mortality in sepsis patients. Timely prediction and evaluation of sepsis outcomes are essential for developing strategies to reduce mortality rates.

Abstract

Background Mesenchymal stem cells (MSCs) are valued in regenerative medicine for their multipotency, proliferative capacity, and immunomodulatory properties. Wharton’s jelly-derived MSCs (WJ-MSCs) from the umbilical cord offer a non-invasive, promising source for clinical applications, because easy isolation, lack of ethical concerns, and the presence of both embryonic and adult stem cells have made them a valuable source for use in therapeutic applications and regenerative medicine. This study aimed to optimize WJ-MSC isolation and characterization methods. Methods Human umbilical cords from three healthy donors were collected post-cesarean under strict inclusion criteria. WJ-MSCs were isolated using the explant culture method, with cells adhering to T75 flasks pre-coated with 2% gelatin. Cultures were maintained in Dulbecco’s Modified Eagle Medium (DMEM) supplemented with 10% freshly prepared Human AB serum and monitored for 21 days. Flow cytometry (BD FACSAria) was performed at passages 1 and 5 to assess MSC markers CD105, CD73, CD90, and CD44, alongside the exclusion marker CD45. Results WJ-MSCs exhibited fibroblast-like morphology by passage 1 and showed robust proliferation. Flow cytometry revealed high CD44 expression (~60%) at passage 1, while CD105, CD73, and CD90 became prominent by passage 5. CD45 remained low, suggesting minimal hematopoietic contamination. Conclusion This study confirms the feasibility of isolating and expanding WJ-MSCs using DMEM with 10% human AB serum. While consistent cell growth was achieved, the 21-day culture period may require optimization for scalability, including serum concentration, substrate coatings, and oxygen levels. CPJ-MSCs may be preferable for applications demanding rapid expansion and early marker expression.
