Abstract

Genetic mutations in the androgen receptor (AR) gene have been identified as the cause of androgen insensitivity syndrome. These mutations are linked to inconsistent development of the Wolffian duct and may result in conditions such as micropenis, hypospadias, and cryptorchidism. The androgen receptor has two polymorphic sites located in exon 1, which consists of varying amounts of CAG and GGN repeats. These repetitions lead to the formation of polyglutamine and polyglycine stretches of varied lengths. Increased CAG repeats lead to a decrease in androgen receptor transcriptional activity, but the impact of GGN triplets is less well understood. This research examined the CAG and GGN repeat lengths in males who had a past medical record of cryptorchidism. Prospective and retrospective observational studies from PubMed, Science Direct, and Embase were systematically searched up to 15th November 2020. Primary outcomes were analyzed using a fixed or random effect model regarding its heterogeneity and continued with multilevel modeling of each polymorphism and ethnicity. CAG and GGN repeat polymorphism was found to be significantly different compared to control in contributing to cryptorchidism (CAG: 0.55 [CI 95%=0.19-0.91]; p-value=0.003 and GGN 0.90 [CI95%=0.65-1.15]; p value<0.000).  In conclusion, CAG and GGN repeat polymorphism have an essential role in the incidence of cryptorchidism.

Abstract

Atrioventricular septal defects (AVSD) constitute a spectrum of anomalies caused by abnormal endocardial cushion defects. Many classifications have been used to describe AVSD. There is generally subclassified into complete and partial forms. Complete AVSD is characterized by a primum atrial septal defect (ASD) contiguous with an inlet ventricular septal defect (VSD) and a common AV valve. Intermediate AVSD is a subtype of complete AVSD that has distinct right and left atrioventricular valve orifices by a bridging tongue despite having only one common annulus. In some children or adults, we may see unoperated partial or intermediate AVSD, which may be asymptomatic or may present with congestive heart failure, exertional limitation, pulmonary hypertension, infective endocarditis, or heart rhythm disorder. In some cases of AVSD has atrioventricular (AV) conduction disorder, mostly on first-degree AV block. We report a case of a 13-year-old girl with complete AVSD (intermediate subtype) and pulmonary hypertension who is concomitant with total AV block. The patient has undergone implantation of a permanent pacemaker for total AV block and conservative therapy for complete AVSD. Anatomical assessment by trans-thoracal echocardiography (TTE) was essential for diagnostic and detailed morphological characterization of AVSD.

Abstract

Habitual patellar dislocation occurs when the knee is bent and returns to its usual position when straightened. Habitual patella dislocation is 13.5 per 100,000 in children under 13 and 147.7 per 100,000 in children 14–18. This review aims to present a comprehensive and complete literature on the management of habitual dislocation of the patella so that appropriate therapy can be obtained. A complete literature review was used in this investigation. Source search databases are PubMed, Science Direct, and Google Scholar. The literature study shows that habitual patellar dislocation in children is infrequent. Stability requires bone and connective tissue that match the patellofemoral joint. Patella alta, torsional, angular, trochlear dysplasia, muscular anomalies, ligamentous laxity, or ligament damage can produce lateral patellar habitual instability. The quadriceps muscle drags laterally to the limb's mechanical axis. Clinical children with recurrent patellar dislocation may experience a sudden dislocation that "disappeared somewhere." If this condition returns, patients frequently develop unusual knee edema and inflammation. MRI, CT, and plain radiography can diagnose this condition. A CT scan can detect trochlear dysplasia. Joint effusion, osteochondral lesions, medial retinaculum tears, and medial patella and lateral femoral condyle cartilaginous discolorations can be seen by MRI. Conservative or surgical treatment of habitual patellar dislocation depends on the deformity and its effects.

Abstract

Inflammation is crucial to cancer development. A complete blood count is standard patient testing. Thus, inflammatory biomarkers like neutrophil-lymphocyte ratio (NLR) and platelet lymphocyte ratio (PLR) may predict lung cancer prognosis. Serum carcinoembryonic antigen (CEA) is the main predictive marker of most studies. Elevated NLR, PLR, and CEA levels are frequently associated with poorer overall survival in NSCLC patients. A retrospective cross-sectional study included 50 NSCLC medical records patients from Ulin Regional South Kalimantan Hospital. NLR, PLR, and CEA baseline peripheral blood individuals were investigated for NSCLC overall survival (OS). Patients are separated by OS mean into two groups. Mann-Whitney compared variables. The ROC curve and AUC were used to evaluate the above indicators' prognostic value. The mean of OS was 6 months. NLR, PLR, and CEA patients in ≤ 6 months had higher median values compared to > 6 months groups (8.73 vs. 4.3; 301.23 vs. 217.81; and 106 vs. 27.87). Survival was significantly associated with NLR and CEA (p-values 0.010 and 0.011). NLR >5.90 with AUC 0.725 (sensitivity 63.6%, specificity 29.4%) and CEA >41.39 ng/mL with AUC 0.722 (sensitivity 72.7%, specificity 29.4%). Parallel tests of NLR and CEA testing increased sensitivity and specificity (75.8%, 70.6%). This study revealed that elevated NLR and CEA are associated with patient survival, and monitoring both markers enhances survival prediction accuracy. It can improve insight into disease progression and adjust the therapeutic approach for NSCLC patients.

Abstract

Implant failure and acetabular protrusion are significant complications following Austin Moore prosthesis insertion, particularly in elderly patients with hip fractures. Total hip arthroplasty (THA) offers superior outcomes compared to hemiarthroplasty (HA) but comes with its own set of challenges. Understanding the risk factors and implications of these complications is crucial for choosing effective management.  In this article, we report a 76-year-old woman who presented with severe left hip pain post-fall, eight years after HA with an Austin Moore prosthesis due to a trochanter fracture. The patient was able to walk with a walker before the fall. Physical examination revealed tenderness and limited range of motion. Radiographic imaging confirmed acetabular protrusion and AMP neck fracture. Conversion to THA using a hybrid system was performed successfully, with the patient exhibiting no post-operative complications. This case report showed that implant failure and acetabular protrusion pose complex challenges, exacerbated by patient demographics, implant design, and bone quality. THA emerges as a preferred option due to its superior outcomes, although conversion from HA is associated with increased risks. Early diagnosis and intervention, as well as a multidisciplinary approach and careful consideration of patient-specific factors, are crucial for optimizing patient outcomes and quality of life.

Abstract

The incidence of pregnancy with obesity in Indonesia reaches 21.8%. This can be more serious, if the pregnant woman is too old (≥ 35 years) then there will be more complications for the mother and fetus. This case study was written based on midwifery management, in the form of subjective, objective documentation, analysis, and management. Based on a case study conducted on GIP0A0 ​​gestational age 36 weeks, 35 years old with obesity and old age. The impact of obesity found is chronic hypertension which will reduce the supply of oxygen and food from the mother to the baby so that the fetus is diagnosed with suspected Intrauterine Growth Restriction (IUGR). The mother is categorized as level II obese, obesity increases the risk of narrowing of the blood vessels by the accumulation of fat, which causes poor blood circulation which transports O2 and nutrients to the baby so that the fetus's weight does not match the gestational age, what is detected from TFU does not match the gestational age, in fact, accompanied by minimal amniotic conditions (oligohydramnios). To prevent complications for the mother and baby, delivery is carried out by cesarean section (SC). The mother's baby was a High Social Value Baby (HSVB), and after SC there was no success in providing exclusive breastfeeding, due to the mother's lack of knowledge about how and the benefits of breastfeeding.  The mother chose IUD contraception which was inserted at the time of delivery. In conclusion, appropriate care provided by midwives during pregnancy, childbirth, newborns, postpartum and family planning is very important for the health of mothers and babies. So that early detection of risk factors can be minimized and possible complications can be prevented.

Abstract

One of the factors influencing SARS-CoV-2 infection is comorbidity. Diabetes mellitus is a comorbidity with a high mortality rate. Poor glucose control in patients can affect cellular immune responses and increase morbidity and mortality associated with infections. The severity of COVID-19 is classified into 5 levels: asymptomatic, mild, moderate, severe, and critical. HbA1c is a good test for identifying the diabetes status of COVID-19 patients. According to the American Diabetes Association, HbA1c levels are considered controlled if <7% and uncontrolled if ≥7%. This study aims to analyze HbA1c levels in COVID-19 patients with diabetes mellitus from 2020 to 2022. This study uses the productive age category (15-64 years). To determine the relationship between HbA1c levels and the severity of COVID-19 patients with diabetes mellitus at Haji General Hospital, East Java Province, from 2020 to 2022. This research employs a cross-sectional, analytical observational method with a consecutive sampling technique involving 96 hospitalized COVID-19 patients. Data collection was conducted through medical record observation. The majority of the samples were male, with an average age of 53 years. The Fisher Exact Test yielded a p-value of 0.13, which is greater than the significance level (0.05), indicating no significant relationship between HbA1c levels and the severity of COVID-19 in diabetic patients. There is no significant relationship between HbA1c levels and the severity of COVID-19 in diabetic patients at Haji General Hospital, East Java Province.

Abstract

Stroke is the number one cause of disability in Indonesia. Stroke is a condition of loss of brain function caused by stopping the blood supply to the brain. The main risk factor for stroke is hypertension. Hypertension sufferers who have poor preventive behavior have the potential to experience a stroke. This study aimed to identify the dominant factors related to stroke prevention behavior based on the Health Promotion Model (HPM) theory in the Gubeng District area of ​​Surabaya. This research used cross-sectional research with a sample of 108 respondents using purposive sampling. Data were collected using a questionnaire and analyzed using the logistic regression test α < 0.05. Independent factors related to stroke prevention behavior are perceived self-efficacy (0.000), activity-related affect (0.002), personal influences (0.000), and situational influences (0.023). Meanwhile, factors that have no relationship are prior related behavior (0.360), personal factor (motivation) (0.319), perceived benefits to action (0.200), perceived barriers to action (0.243), education level (0.708), socio-economic status (0.105). In conclusion, this research can be used as material for developing health workers' knowledge in intervening related to stroke prevention behavior in hypertension sufferers.

Abstract

First-year medical students tend to experience stress, which can affect how they maintain focus on their work. One alternative treatment that can be done is through relaxation techniques using aromatherapy in the form of essential oils. Crocodile agarwood (Aetoxylon sympetalum) is one of the plants that can provide anxiolytic and antidepressant effects that can be associated with these problems. This study aims to determine the effect of giving Aetoxylon sympetalum essential oil on the level of ability to maintain the focus of first-year students of the medical faculty. This quasi-experiment research method with a nonequivalent control group design was conducted on 34 samples which were divided into control and treatment groups with the same number to see the effect of essential oil on both groups. The research data were analyzed using Spearman's rho test and showed a significance value of 0.004 and a correlation coefficient value of 0.661, meaning that a significant correlation was obtained, the level of correlation between variables was strong and unidirectional. So it can be concluded that the more often inhaling Aetoxylon sympetalum essential oil, the ability to maintain focus will increase.

Abstract

We report a case of a 50-year-old female who presented with rhinosinusitis-like symptoms, which she had been experiencing intermittently for over two decades. Her medical history revealed a nasal surgery performed approximately 22 years ago, after which she continued to experience unresolved symptoms despite multiple treatments. A Computed Tomography (CT) scan of the paranasal sinuses revealed findings with ethmoiditis, prompting the decision for endoscopic sinus surgery to address the underlying pathology. During surgery, a 3 cm gauze was discovered after opening the posterior ethmoid sinus. We highlight an unexpected foreign body inside the ethmoid sinus based on an unidentified picture from the CT scan, which we accidentally discovered during endoscopic surgery. This case underscores the importance of considering a retained foreign body as a potential cause of chronic rhinosinusitis, even many years after the initial surgical intervention. The unexpected identification of the gauze, which was not depicted in the preoperative CT scan, highlights the challenges in diagnosing retained foreign bodies within the complex anatomy of the sinuses. The report discusses the clinical presentation, diagnostic challenges, and management of this unusual case, contributing valuable insights to the literature on chronic rhinosinusitis and retained foreign bodies in the sinonasal region.
