Abstract

Background: : Diabetic foot ulcers are a chronic complication of Diabetes Mellitus (DM) that often leads to disability and amputation. The incidence of foot ulcers is closely related to patient self-care behavior, including diet, physical activity, medication adherence, blood sugar monitoring, and foot care Objective: The purpose of this study was to determine the determinants of self-care on the incidence of diabetic foot ulcers in Diabetes Mellitus patients. Methods: This article used a cross-sectional design with a population of 91 with a sample of 73 DM patients selected by purposive sampling. The instruments used were the Summary of Diabetes Self-Care Activities (SDSCA) questionnaire to assess self-care and an observation sheet for the incidence of diabetic foot ulcers. Data analysis was performed using the Chi-Square test and logistic regression to determine the determinant factors. Results: The results showed a significant relationship between self-care and the incidence of diabetic foot ulcers (p < 0.05). Foot care was the most dominant factor (OR = 0.0004; CI95%), followed by blood sugar monitoring and medication adherence. Conclusion: Self-care significantly influences the incidence of diabetic foot ulcers, with foot care being the primary determinant. Intensive education regarding self-care, particularly foot care and blood sugar monitoring, is needed to prevent further complications.

Abstract

Objective: Tuberculosis is a contagious disease with a high incidence in the community. Tuberculosis can be transmitted through airborne droplets containing tuberculosis bacilli when an infected person coughs, sneezes, or speaks. Tuberculosis transmission can be prevented by involving families in health care. The purpose of this study was to explore families' experiences in preventing the transmission of pulmonary tuberculosis within families at the Kalijudan Community Health Center in Surabaya. Methods: A descriptive phenomenological approach was used to collect. The participants were 10 families with Pulmonary Tuberculosis at the Kalijudan Community Health Center in Surabaya, who were selected by purposive sampling. Data collection was conducted through in-depth interviews based on semi-structured interview guidelines. The data analysis model used was interactive Miles, reduction, data display, analysis, conclusions, and data verification. Result: The research obtained five themes: family knowledge in recognizing family health problems, making treatment decisions, behavior in caring for family members, environmental modification, and utilization of health facilities. Conclusion: The family's ability to carry out health functions is essential in preventing the transmission of pulmonary tuberculosis within the family.

Abstract

Background: Treatment coverage (TC) of Tuberculosis (TB) as indicator of Tuberculosis is the main cause of death from infectious agents throughout the world. WHO has set 2030 as the end of TB, but treatment coverage in several countries is still low. Of all provinces in Indonesia, East Nusa Tenggara is the third lowest province with 40.1% coverage. On the Timor mainland, there are 3 districts with treatment coverage achievements below the national indicator (90%), namely South Central Timor District (43%), North Central East District (46%), and Belu District (71%). This condition will have an impact on achieving end TB by 2030. Several factors that may be the cause of the low contribution of treatment coverage include poverty, the ratio of community health centers/auxiliary health centers/pustu to sub-districts/population, motivation of TB program managers, availability of health insurance, cross-sectoral collaboration and TB management at the border. Objective: This research aims to determine the factors that contribute to the low achievement of treatment coverage. Method: the research used was quantitative study with cross sectional design. Result: the average age of respondents was 38,83 years (SD = 5,84), the average length of work of respondents was 10,87 (SD = 7.55). More than half, namely 66,3%, had a Diploma Nursing education. More of the three quarters of respondents, 75,9% were married. Nearly all patients were civil servants/P3K (94%). Most of the TB patients were from poor families (68,7%); all managers did not feel that there was cross-sectoral collaboration (94,0%). Only motivation that had significant relationship with TC.

Abstract

The prevention of mother-to-child transmission (PMTCT) of HIV remains a major public health challenge in Indonesia, particularly in regions where stigma, limited knowledge, and weak social support hinder optimal program implementation. This study aimed to identify determinants influencing PMTCT efforts among HIV-positive pregnant women in Jombang Regency. An analytical observational design with a cross-sectional approach was employed, involving 30 pregnant women selected through systematic sampling. Data were collected using structured questionnaires and analyzed with univariate and bivariate methods using Spearman-Rho correlation. The results showed that 60% of respondents demonstrated adequate prevention efforts, while 40% were inadequate. Attitude (p = 0.006), parity (p = 0.010), peer support (p = 0.000), husband support (p = 0.010), healthcare worker support (p = 0.010), and accessibility (p = 0.028) were significantly associated with prevention behaviors. Peer support emerged as the strongest determinant, reflecting the importance of social networks and interpersonal influences. The findings indicate that PMTCT success requires more than clinical interventions; it depends on strengthening social support systems, improving counseling quality, and enhancing access to maternal health services. Integrated, community-based strategies are essential for reducing HIV transmission and achieving PMTCT targets.

Abstract

Objective: Hypertension is a leading cause of death worldwide, with over one billion people suffering from it. Several factors influence blood pressure, including blood sugar, uric acid, and hemoglobin. This study aimed to analyze the relationship between blood sugar, uric acid, and hemoglobin with blood pressure. Methods: The study design was correlational with a cross-sectional approach. The study population was 46 elderly people in Puton Village, Diwek District, Jombang Regency. Data on uric acid, blood sugar, and hemoglobin levels were collected using a multifunction blood test device through peripheral blood sampling. Blood pressure was measured using a sphygmomanometer according to standard guidelines. Data were analyzed using coding, editing, tabulation, and scoring, and statistical tests were performed using the Spearman Rank test. Results: Most respondents (73.9%) had high blood sugar. Most respondents (71.7%) had high uric acid (33%). Half of respondents (50%) had normal hemoglobin (23%). Nearly half of respondents (34%) had prehypertensive systolic blood pressure and stage 1 hypertension (34.8%) each. Half of the respondents had diastolic blood pressure (50%, 23 people) with grade 1 hypertension. The Spearman rank test for blood sugar and uric acid levels on systolic and diastolic blood pressure using SPSS yielded a p value > 0.05, indicating no correlation. The correlation between hemoglobin and diastolic blood pressure yielded a p value > 0.05, indicating no correlation. The hemoglobin-systolic blood pressure test yielded a p value of 0.017, less than 0.05, with a correlation coefficient of 0.349, indicating a moderate correlation between hemoglobin and systolic blood pressure. Conclusion: Proper control of the intervening variables is essential to ensure a truly homogeneous sample of respondents to achieve optimal results. The number of respondents should be increased.

Abstract

Background: Patients with chronic kidney disease undergoing hemodialysis face substantial symptom burdens that may influence adherence behaviors and overall quality of life. The Common-Sense Model (CSM) proposes that illness perception shapes coping responses, including adherence, which subsequently affect health outcomes. However, the relationships among these variables remain inconsistent, particularly in the Indonesian context. Objective to examine the associations among illness perception, adherence behaviors, and quality of life in patients with chronic kidney disease undergoing hemodialysis from a Common-Sense Model perspective. Methods A cross-sectional study was conducted among adult patients with chronic kidney disease undergoing hemodialysis in two dialysis centers. Illness perception was assessed using the Brief Illness Perception Questionnaire; adherence behaviors using the End-Stage Renal Disease Adherence Questionnaire (ESRD-AQ); and quality of life using the KDQOL-SF. Pearson correlation tests were used to analyze associations among variables. Results Illness perception was not significantly associated with adherence behaviors or any quality-of-life domain. In contrast, specific adherence behaviors demonstrated significant relationships with mental health outcomes. Adherence to fluid restrictions was positively associated with the Mental Component Summary (MCS) (r = 0.302, p < 0.01), and dietary adherence also correlated with MCS (r = 0.238, p < 0.05). No adherence variable was associated with the Physical Component Summary (PCS). Interdialytic Weight Gain (IDWG) showed negative correlations with PCS (r = –0.250, p < 0.05) and the burden-of-kidney-disease domain (r = –0.278, p < 0.05), indicating poorer physical well-being with higher fluid accumulation. Conclusion: Within the CSM framework, illness perception did not predict adherence or quality of life. Adherence to dietary restrictions, adherence to fluid restrictions, and effective fluid management emerged as important correlates of both the mental and physical aspects of quality of life. Interventions that strengthen behavioral adherence and promote optimal fluid control may provide greater improvements in patient well-being compared to approaches that focus only on modifying illness perceptions

Abstract

Objective: This study aims to determine the effect of family psychoeducation on the mental health status of families with children with mental retardation Methods: The study used a pre-experimental design with a one-group pretest–posttest design. The study involved 45 families with children with mental retardation, using a total sampling technique. The independent variable was family psychoeducation, and the dependent variable was family mental health status. The intervention consisted of three sessions of family psychoeducation, each lasting 30–60 minutes. Family mental health status was measured using the Self-Reporting Questionnaire (SRQ-20) before and after the intervention. Data were analyzed using the Wilcoxon Signed Ranks Test. Results: The results showed that before the intervention, the majority of families (62%) were in the mild emotional disturbance category. After psychoeducation, the majority of families (67%) were in the normal category. The statistical analysis showed a p-value of 0.003 <0.005, indicating a significant effect of family psychoeducation on improving family mental health status. Conclusion: This study concluded that family psychoeducation is effective in improving the mental health status of families caring for children with mental retardation. This intervention is recommended for implementation in community psychiatric nursing services to improve family well-being and the quality of care for children with mental retardation.
