Abstract

Background: Nearly half of all patients with acute myocardial infarction (AMI) have left ventricular systolic dysfunction and one-third have symptoms of heart failure (HF). In patients with AMI the resistin level correlated inversely with left ventricular ejection fraction (LVEF). Increased levels of high sensitive (hs) troponin I are associated with poorer prognosis. This study aimed to determine the association between levels of resistin and hs troponin I in ST elevation myocardial infarction (STEMI) patients with systolic HF.Subjects and Method: This was a cross-sectional study was conducted at Dr. Moewardi General Hospital, Surakarta, from April 1 to May 31, 2018. A sample of 32 patients who admitted which diagnosed with STEMI was selected for this study. They were divided into two group according to result of LVEF measurement, LVEF < 40% and LVEF ≥ 40%. Blood examination and transthoracic echo-cardiography were performed to all patients. Correlation test using partial and multiple correlation test. To different 2 mean using Mann Whitney test. Results: Mean of patient age was 59.5 years old. Resistin decreased LVEF (r= -0.41; p= 0.009), and it was statistically significant. Hs troponin I decreased LVEF (r= -0.25; p= 0.081), but it was marginally significant. Resis-tin level and hs troponin I increased LVEF (r= 0.47; p= 0.025), and it statistically significant. Conclusion: There was an association between resistin and hs troponin I level together in STEMI patients with systolic HF. There was an association of resistin levels in STEM I patients with systolic HF. There was no association of hs troponin I levels in STEMI patients with systolic HF.Keywords: Resistin, hs troponin I, STEMI, systolic heart failure.Correspondence: Trisulo Wasyanto. Abstract

Background: Management of Gartland type III supracondylar humerus fractures is conducted by open and closed repositioning. An adequate reposition and a stable and accurate fixation are desperately needed to prevent fixation failure, deformity, and complication. The study aims to compare the clinical and radiological results between crossed and lateral fixation techniques.Subjects and Method: The study was a retrospective study toward Gartland type III SCHF children in Dr. Soetomo Hospital, Surabaya, Indonesia from 2013–2016. The dependent variable is Supracondylar humerus fracture. Independent variables were the type of fixation option, clinical functional test, degrees of satisfactory, and radiology evaluation. The radiology parameter used was Skaggs criteria. An observation was conducted for the occurrence of complications in the form of infection and peripheral nerve injury. All data were analyzed using Kolmogorov Smirnov and Fischer exact test. Results: The study discovered 28 patients consisted of 20 males and 8 females with an age range from 3 – 13 years old with an average age in crossed fixation group was 7.6 years and in lateral fixation was 4.7 years. The injury sides were 46.4% right elbow and 53.5 % left elbow. Among the crossed fixation group, there were 54.5 % left elbow and 45.5 % right elbow. Among lateral fixation group, there were 50% left side and 50% right side. There was no significant difference in clinical functions, radiology as well as complication in the form of infection and peripheral nerves injury. Conclusion: There is no difference of functional clinical, radiology result as well as post-surgery complication in the form of infection and peripheral nerves injury between crossed fixation technique and lateral fixation technique.Correspondence: Komang Agung Irianto. Abstract

Background: Biloma is loculated collection of bile that may develop due to iatrogenic causes, traumatically or spontaneously with biliary tree disruption. Hepatic bilateral subcapsular biloma is a rare complication of laparoscopic cholecystectomy and an even more scarce when it occurs spontaneously.Case Presentation: A 65 years old man came to our hospital with abdominal pain and en-larged abdomen. Six weeks earlier he underwent laparoscopic cholecystectomy in a private hospital, because of stone in the gall bladder and cholecystitis. The physical examination obtained no icteric and distended abdomen with pain on palpation. Laboratory findings were within normal limit with negative viral infection markers. Abdominal ultrasonography revealed chronic liver disease with giant liver cyst and ascites. Contrast abdominal multi-slice computed monography (MSCT) demonstrated bilateral hepatic subcapsular biloma. Laparatomy and drainage were then performed and he was discharged from the hospital several days later in good condition.Discussion: Biloma was first introduced by Gould and Patel in 1979. The incidence of past laparoscopic cholecystectomy biloma is very low, between 0.6% and 1.5%. Early accurate diagnosis is very important to determine the proper management. In our case, the biloma was found by using USG and MSCT.  Usually it presents with right upper quadrant or epigastric pain, abdominal distention, fever and leukocytosis, but our patient did not have either fever or leukocytosis. Actually the first treatment choice is percutaneous catheter drainage but in our case laparotomy drainage was performed because of subcapsular, biloma in both right and left hepatic lobes. Conclusion: Bilateral hepatic subcapsular biloma is a rare case. One of its diagnostic tools is MSCT. Biloma drainage is the first choice of treatment. Keywords: Biloma, post laparoscopic cholecystectomy, drainageCorrespondence: Triyanta Yuli Pramana. Abstract

Background: Decreased endometrial receptivity is suspected to be one of the factors causing infertility in women with Polycystic Ovary Syndrome (PCOS). Endometrial abnormalities in PCOS due to impaired balance include estrogen, progesterone, growth factors, cytokines and cell attachment modulators including leptin. This study aims to determine the expression of leptin in the endometrium which is suspected to be more common in PCOS patients and related factors.Subjects and Method: A retrospective-case control study was conducted at Dr. Moewardi general hospital, Surakarta. A total of 60 women were involved; 30 infertile women with PCOS based on Rotterdam criteria (if found two of the three symptoms, including hyperandrogenism, ovulation dysfunction and/or polycystic ovaries from ultrasonography) and 30 normal women as control group. Demographic data such as age, family history of PCOS, menarche, BMI, occupation, and education were recorded. Leptin expression was acquired from endometrium biopsy with IHC examination on day 19-24 of the menstrual cycle; under the condition that inclusion and exclusion criteria are fulfilled. The statistical analysis was performed using the Mann-Whitney test. Results: Mean of leptin expression was found significantly higher in PCOS group (Mean= 59.16; SD= 49.34) compared to control group (Mean= 6.00; SD= 22.98, with p= 0.001. PCOS (b= 0.42; 95% CI= 0.10 to 0.73; p= 0.010) and obesity (b= 0.31; 95% CI= 0.41 to 0.58; p= 0.025) increased leptin expression. Conclusion: There is a difference in leptin expression in the endometrium of PCOS compared to normal.  Leptin expression is higher in the endometrium of women with PCOS and obesity compared to normal. Keywords: Leptin expressions, endometrium, polycystic ovarian syndrome.Correspondence: Dwi Sakti Berlian Besar. Abstract

Background: Tuberous Sclerosis Complex (Bourneville’s disease) is genetic disorder with autosomal pattern of heritence that affecting differentiation sellular in many organ such as brain, lungs, cardiac with varible severity. Birth incidence is estimated to be 1:6000. The purpose of this study was to reported a case of patients with Bourneville’s disease (tuberous sclerosis complex) in Moewardi Hospital. Also reported surveillance and complication in this patient.Case Presentation: A male, 18 yearsold, with seizures, onset from infancy, tonic secondary generalized seizure pattern simultaneously throughout the body. Neurologic status examination found right hemiparese and mental retardation, Encountered in patients with skin disorders such as butterfly appearance with facial angiofibroma, forehead fibrous plaque dan shargreen patch. Abnormalities also appeared in the oral mucosa as multiple papules. When psychological tests patient had difficulty following instructions. Psychiatric examination showed moderate lower intelegence. EEG results slow waves at left parietal, there is severe electrofisiologic abnormality considered structural lesion in left parietal. CT showed multiple calcified nodules in subependymal right and left lateral ventricle. MRI showed  atrophy and left sclerotic hippocampus with glosis in sub cortex occipito left parietal. To overcome the seizure patients given Carbamazepine. There was increasing of transaminase enzim, liver suspect liver angiomyolipoma. Conclusion: Based on clinical criteria for definite tuberous sclerosis complex patients met the criteria. The patients responded positively to carbamazepine that reduced seizure frequency. But somehow, patient's prognosis is worst. Presence of intellectual impairment and neuropsychiatric problems is a potential poor epilepsy control, and liver disfunction so that the necessary comprehensive management and surveillance involves other disciplines. Keywords:Tuberous sclerosis complex, congenital disorder, epilepsy, mental retardationCorrespondence: Baarid Luqman Hamidi.Department of Neurology, Dr. Moewardi Hospital/Faculty of Medicine, Universitas Sebelas Maret. Abstract

Background: Trichuris trichiura worm infection is still a major public health problem in Indonesia, especially in peripheral areas. Until now, the search for antihelmintic combinations that have a high healing ability against T. trichiura infections and in the search for truly effective worming is still being done. Albendazole and Mebendazole single doses still show limited efficacy in the treatment of T. trichuria worms. The combination of Albendazole with Levamisol can increase effectiveness, but this combination still needs further investigation. Based on that fact, it is necessary to study the effectiveness comparison of the degree of infection intensity between the treatment of Albendazol-Levamisol and Mebendazol-Levamisol against T. trichiura.Subjects and Method: A double blind randomized control trial was conducted at the State Elementary School in Medan, North Sumatera, from April to October 2019. A sample of 60 children was selected for this study. The dependent variable was the number of eggs decreased, the cure rate, and side effects. The independent variables were Albendazole 400 mg-Levamisol 50 mg/ 100 mg and Mebendazole 500 mg-Levamisol 50 mg/100 mg. The stools of elementary school children were examined using the Kato-Katz method to find eggs of T. trichiura infection. Data analysis was performed using the Chi square test. Results: Samples in the Albendazol-Levamisol and Mebendazol-Levamisol groups were 30 children, respectively. Children infected with T. trichiura consisted of 58 children of mild intensity (96.7%) and 2 children of moderate intensity (3.3%). On the results of stool examination, a significant difference in the number of eggs decreased in the two groups was obtained (p <0.001) and 96.7% was the cure rate for T. trichiura infection in the two groups of Albendazol-Levamisol and Mebendazol-Levamisol. In this study, there were no significant differences in side effects in the two treatment groups. Conclusion: Treatment of T. trichiura infection with single doses of Albendazol-Levamisol and Mebendazol-Levamisol showed that there were significant differences in the Number of Decreased Eggs. Meanwhile, for the cure rate, there was no difference between the Albendazol-Levamisol and Mebendazol-Levamisol groups. Keywords: T. trichiura, Albendazole-Levamisol, Mebendazole-LevamisolCorrespondence: Endy Juli Anto. Abstract

Background: Multiple hereditary exostosis of the forearm is typically formed in the distal ulna causing disturbances in the growth of the ulna and functional disability. Multiple hereditary exostosis inhibits the growth of the ulna leading to an acquisition of a varus deformity in the radius which sometimes leads to dislocation of the radial head, the development of limitations in the pronation-supination of the forearm, and cosmetic problems. This study aimed to report the multiple hereditary exostosis of distal ulna with dislocation of radial head.Case Presentation: We present a case of a 14 year-old girl with main complaint of a bony prominence on her right elbow which limits the elbow flexion range of motion. 3D CT-Scan revealed a dislocation of radial head, shortening and posterior bowing of ulna. We performed excision of the prominent radial head, reconstruction, and temporary arthrodesis of the elbow. Results: The forearm deformity in patients with multiple hereditary exostosis related to the cross-sectional area of the distal ulnar physis was only one-quarter of the distal radius, the distal ulna is more involved in the condition. There was more longitudinal growth at the distal ulnar physis than at the distal radial physis, the distal ulnar physis contributed more total Ulnar Length than the distal radial physis did to Radial length. Known surgical interventions including simple excision, ulnar lengthening, corrective radial osteotomy, hemi-epiphyseal stapling of the distal radius, the Sauve-Kapandji procedure. Conclusion: Simple excision could improve the range of movement of the forearm but would not halt the progression of disease, particularly in younger patients. It was not effective in controlling progression of the deformity. Mature patients did not recur, but in patients that had been excised before puberty the results were varied and unpredictable. Keywords: multiple hereditary exostosis, radial head dislocation, excisionCorrespondence: Pamudji Utomo.
