Abstract

The accuracy of the diagnosis coder reflects the correspondence between the code assigned by the coding officer and the diagnosis in the patient's medical record according to ICD-10 and ICD-9. The problems found included 40% of the writing of diagnoses that was unclear and continuous, 20% of writing that was incomplete and used abbreviations, and 40% of diagnosis codes that were inaccurate. The aim of this research was to determine the clarity and completeness of writing in the accuracy of internal medicine poly diagnosis codes. at Mitra Medika Batanghari Hospital. This descriptive quantitative research was carried out at Mitra Medika Batanghari Hospital on 09-10 July 2022, with a population of 305 medical records of internal medicine patients and a sample of 75, taken using systematic random sampling. Data was collected through observation and checklist tables, and analyzed using univariate techniques. The research results obtained from 75 medical record files found that less than half, namely 27 (36.0%) wrote unclear diagnoses, more than half 61 (81.3%) wrote incomplete diagnoses, less than half 22 (29.3%) %) incorrect diagnosis code. Based on the research results, it can be concluded that there are still unclear and incomplete diagnoses written because they still use abbreviations and there are still incorrect diagnosis codes. It is recommended that the coder confirm again with the doctor concerned if there is an illegible diagnosis written so that there are no errors in assigning the diagnosis code. Keywords: Clarity, Completeness, Accuracy of Diagnosis Codes

Abstract

Pharmaceutical logistics management is a critical aspect of healthcare service management in hospitals, including RSUD dr. Rasidin Padang. This study aims to explore pharmaceutical logistics management, focusing on human resources (HR), infrastructure, and drug storage procedures. A qualitative phenomenological approach was used, involving four key informants: the head of the pharmacy department, the pharmacy warehouse coordinator, and two drug and medical consumable logistics officers. The findings reveal that the existing HR is insufficient, particularly during periods of increased workload. Although the qualifications of the staff meet the requirements, no formal training on drug storage has been conducted. The infrastructure, such as warehouse space and storage equipment, is generally adequate but lacks essential items like ladders for accessing high shelves. Standard operating procedures (SOPs) are available and provide crucial guidance, but their implementation is hindered by workforce limitations. In conclusion, the pharmaceutical logistics management at RSUD dr. Rasidin Padang has a solid foundation but requires additional workforce, specialized training, and facility improvements to enhance effectiveness. Keywords: Pharmaceutical logistics, drug storage

Abstract

The procurement of medical equipment is part of hospital logistics management to meet user needs and improve service quality. RSUD dr. Rasidin Padang has implemented this function, but challenges remain, such as unutilized medical equipment post-procurement. This study aims to analyze the procurement of medical equipment logistics at RSUD dr. Rasidin. In the input component, the IPSRS workforce consists of 6 staff members, but their capacity is limited due to a lack of formal training and the absence of SOPs. In the process component, planning is conducted annually based on user requests, with procurement carried out through e-catalogs or vendors. In the output component, the results are suboptimal due to insufficient coordination and equipment utilization. The study concludes that improvements in staff training, SOP development, and inter-unit coordination are needed to enhance procurement efficiency and hospital services. Keywords: Procurement, Logistics, Medical Equipment

Abstract

The length of time it takes for perineal wounds to heal can be uncomfortable for new mothers and affect the frequency of painful infections. Perineal rupture-related occurrences have been linked to 21.1 million deaths worldwide, with 16.3 million of those deaths occurring in Southeast Asia. This study set out to identify the variables linked to the healing of perineal wounds. Using a cross-sectional design and successive sampling approaches, 30 samples with first and second degree lacerations were used. The REEDA scale instrument, formular record, and SPSS chi square analysis were used during the 30-day trial. The study's findings revealed that 0.003 <0.05 for the maternal age statistical test. Results of the statistical test for the parity variable are 0.030 <0.05. A statistical test of the degree of laceration variable yields 0.000 <0.05. Therefore, among postpartum moms, the degree of laceration, maternal age, and parity are related to the healing of perineal suture wounds. Mothers are advised to consider the condition when preparing for their subsequent pregnancy. Keywords: Postpartum, age, parity, laceration, perineal wound

Abstract

Drug distribution is a crucial aspect of drug management in healthcare facilities. However, this process often faces various issues that may affect the quality and continuity of drug distribution. This study aims to analyze the implementation of risk management in the drug distribution process at the pharmacy installation in healthcare facilities. The method used in this study is a systematic literature review with a traditional review approach. Article searches were conducted through four databases, namely Google Scholar, Springer Link, PubMed, and Elsevier, using the keywords "distribution of drugs AND risk management AND pharmaceutical services AND health care facilities." The results indicate several risks that may occur in drug distribution, such as dead stock, expired drugs, distribution errors, and discrepancies between the received and required quantities of drugs. Furthermore, factors contributing to these risks include storage system errors, human errors, and non-compliance with established procedures. Risk evaluation revealed that dead stock and distribution errors have a very high impact. This study recommends the need for a more systematic and structured risk management implementation in every healthcare facility to mitigate the impacts of identified risks. Keywords: Risk Management, Drug Distribution

Abstract

The storage of medical consumables in the pharmacy warehouse is an integral part of ensuring their quality and suitability before being distributed to patients. Errors in storage can damage the quality of medical consumables and result in losses for the hospital. This study aims to evaluate the management of medical consumables storage at RSUD Padang Pariaman from the perspectives of human resources, procedures, and infrastructure. The results indicate that the available human resources are adequate in terms of quantity and competency but require regular training to maintain work quality. Detailed SOPs have been developed; however, their implementation faces challenges, such as the absence of stock cards and labels on storage racks. From the perspective of infrastructure, several deficiencies were identified, including the lack of room thermometers, fire extinguishers, and storage space dividers. Based on the evaluation, corrective measures are needed, such as improving human resource competencies, enforcing SOP implementation, and providing facilities that meet the standards. With these improvements, the management of medical consumables can support optimal healthcare services. Keywords: storage, BMHP, pharmacy

Abstract

Exclusive breastfeeding is the provision of breast milk for the first 6 months of a baby's life without any additional food. The success of exclusive breastfeeding is influenced by several factors, such as perception, breastmilk fluency, knowledge, and maternal nutritional intake. This study aims to analyze the relationship between age, parity, and education of breastfeeding mothers with breastmilk fluency. The study used a cross-sectional approach with a total sampling technique so that a sample of 30 respondents of breastfeeding mothers aged 0-3 months was obtained. The study was conducted in November-December 2023. The research instrument used a questionnaire about the characteristics of respondents and 9 indicators of breastmilk fluency. The research analysis used chi square and all respondents had signed an informed consent. The results of the study showed that there was a correlation p between age and parity with breastmilk fluency with a p value of 0.026 each, and there was no correlation between education and breastmilk fluency. Based on the results of the study, it is recommended that midwives can provide education during pregnancy regarding several factors that can affect breastmilk fluency so that the coverage of exclusive breastfeeding can increase..Keywords: Factors, Age, education, parity, breastmilk fluency
