Abstract

Adverse Drug Reaction (ADR) in the treatment of tuberculosis can reduce the quality of life and compliance of patients, long-term treatment has a risk of ADR, to prevent dangerous ADRs need to be monitored properly, the predictors that affect ADR are very important to be sought as an effort to prevent the incidence ADR. This study was a cross-sectional study conducted on pulmonary tuberculosis patients at the Public Health Center (PKM) of Bantul Regency Yogyakarta, inclusion criteria were patients who received fixed-dose combination tuberculosis (KDT), age> 17 years old, ADR monitoring was carried out since the month of two treatments up to the 6th month by interview method for each control patient to the PKM. Data processing was performed by multinomial regression multivariate tests. The results showed that ADR occurred in 58% of the old age group; 51.8% of the middle age group and 50% of the young age group, there was no significant effect of age on ADR in this study. Based on sex, ADR occurred in 58.5% of men, and 47.3% of women, with no significant effect of gender on ADR. Based on BMI, ADR occurred in the underweight, normal, and overweight groups with a percentage of 78%, 26%, and 40% there was a significant influence between BMI and the incidence of ADR, p-value 0.01 OR = 7,650 (2,350-24,898) CI = 95 %. Based on the BTA ADR group occurred in the positive and negative BTA groups with a percentage of 52% and 62%, there was no significant effect between the BTA and the ADR incidence Key words : Adverse Drug Reaction, Tuberculosis, Fixed-dosed combination-Antituberculosis Drug (FDC)

Abstract

The problem that often occurs in diabetes patients is the lack of knowledge related to treatment and illness. This problem can be overcome by providing education to patients collaboratively and continuously. One intervention that can be done is the Medication Therapy Management (MTM) service. MTM is a service that aims to optimize patient therapy outcomes. This recently implemented service has quite a several elements. So, this study modifies MTM service referenced from the MTM BPJS (Social Security Administering Body). This study aims to determine the effect of MTM services to the level of knowledge and quality of life on diabetes mellitus patients in Yogyakarta Public Health Center. The employed research method was quasi-experimental one-group design with pretest-posttest design and experiments were conducted in March to April 2020. The sampling method uses a purposive sampling technique. This study uses a questionnaire as a research instrument. Data were analyzed using SPSS. Results show a slight increase in the patients’ knowledge from 8.9 ± 2.3 to 11.6 ± 1.5 after MTM services were given with p-value of 0.000, and a slight increase in the quality of life of patients from 50.56 ± 3.9 to 52.84 ± 4.3 with a p-value of 0.003. In conclusion, the MTM-based services from pharmacists can significantly increase the knowledge and quality of life of diabetic patients.

Abstract

Patients compliance is commonly occured in short-course antibiotics therapy. Patients compliance not only contributed to the therapeutics effect but also to prevent antibiotics resistance. Qualitatively, uncompliance in using antibiotics is affected by patients knowledge about antibiotics, improving symptomps, false in interpretating drug labels, activities, the lack of family suppor, and the lack of information given by healthcare provider. This study was aimed to observe patients compliance in using antibiotics assessed in how and howmany times they take their dosage, how they give interval between the doses, and whether they complete their course or not. The reasons behind their behaviour also investigated in this study. The survey was conducted at three public health centres in Surabaya. Patients who received short-course (3-5 days) antibiotics therapy were asked to participate in this study. The compliance was assessed using online developed self-assessment questionnaire containing 17 questions asking how they use their antibiotics and how many pills left sent in the last dosage day. The antibiotics prescription data was recorded as a refference in evaluating patients compliance. During research periods, a total of 100 patients were enrolled in this study. Of that, there were only 12.0% categoryzed as compliant. Most of patients did not give the same time interval between their doses (63,0%) and did not finished or prolonged their course (46,0%). The majority reasons behind their behaviour was feelings better or improving symptomps and simply forgot to take their antibiotics. A strengtnen in counselling and monitoring during their antibiotics course is needed in order to increase their compliance.

Abstract

Corona Virus Disease-19 (COVID-19) has been declared a pandemic by the World Health Organization (WHO) on March 11, 2020, previously on March 2, 2020 Indonesia has reported 2 confirmed cases of COVID-19. Immunomodulators that can increase the body's resistance to the COVID-19 virus by stimulating the immune system. The purpose of this study was to determine the effect of COVID-19 on the sale of health supplement products in a pharmacy in Surakarta.This research is an analytical observational research. Data was collected from sales records at pharmacies in the 3 months before the COVID-19 pandemic and 3 months at the beginning of the COVID-19 pandemic. Data analysis using statistical tests.Sales of health supplement products for 6 months were 17843 pcs. There were 4773 health supplement products sold before the pandemic, while 13070 pcs were sold during the pandemic. The results of the normality test showed that the data were not normally distributed. The Wilcoxon test results show that the significant value is 0.028 (<0.05), meaning that there is a difference in sales before and after the determination of the pandemic. The conclusion of this study is that COVID-19 has an effect on the sale of health supplement products in a pharmacy in Surakarta.

Abstract

Black rice bran (BBH) which contains anthocyanins has the ability to lower blood sugar. In therapy diabetes mellitus (DM) it is possible that BBH is used together with the oral antidiabetic Glibenclamide (Gli). Glibenclamide, a weak acid, is well absorbed in the gastrointestinal tract. The absorption of BBH is also good under acidic conditions. So, it is necessary to study the pharmacokinetic interactions between Gli and BBH. The study was designed using a one-way completely randomized design of ten (10) hyperglycemic rats, divided into two groups. Group I rats received Gli 5 mg / kgBW treatment, and group II received Gli 5 mg/kgBW along with BBH 50 mg / kgBW. Sampling was carried out at 1, 2, 4,6, 8 and 24 hours after treatment. Gli levels in the blood were determined by Liquid Chromatography/Mass Spectroscopy (LC/MS). The results showed that in the absorption phase, giving BBH with Gli did not affect the Gli profile in terms of parameters Tmax, Cmax and AUC0-inf. Likewise, in the distribution and elimination phases in terms of parameters VD, ClT and elimination half-time (t1/2). So that the provision of BBH does not affect the pharmacokinetic parameters of Gli in hyperglycemic rats.

Abstract

Purification extract ethanol 96% of Moringa dry leaves was carried out to increase the active substance content and to obtain a more attractive color. The purpose of this study was to determine the type of solvent in the fractionation which provides a higher active substance content than the extract. This research begins with maceration of dried Moringa leaves with 96% ethanol. The filtrate that has been dried then purified. Purification begins with dissolving the extract in hot distilled water with a ratio of 1:10, after that ethyl acetate or n-hexane is added to the same amount as hot distilled water in a separating funnel, shaking it. The ethyl acetate or n-hexane phase is taken then the filtrate obtained is combined. The 96% ethanol extract (E1), ethyl acetate (E2) and n-hexane (E3) were then evaluated with parameters of organoleptic, % yield, total phenol, and total flavonoids. Data were analyzed statistically one way ANOVA with confidence level of 95%. The measurement results for E1, E2, and E3 in the yield were 15.81%;22.18%;20%, the total phenol was 28.94±0.54mgGAE/g; 29.44±0.93mgGAE/g; 11,41±0,12mgGAE/g, and total flavonoids 3,19±0.13%; 9,92±0.06%; 5.81±0.18% respectively. The consistency, smell and taste, all are identical, while in color, E2 is more yellowish green and brighter, making it more attractive. Statistically, there were significant differences between E1, E2, and E3 in total phenols and flavonoids. Thus it can be concluded that E2 significantly higher total phenol and flavonoids than E1 and E3.

Abstract

The COVID -19 pandemic has affected many aspects of life, one of which is in the field of education. Transformation toward various e-learning process has been conducted in order to prevent transmission of the disease caused by massive gathering. Obstacles appear when trying to deliver and assess practical skills which are usually done face to face with the teachers or instructors. These obstacles are also seen in pharmacy schools where skills such as patient counselling, compounding and dispensing, etc. are important competencies for the students to achieve. In order to overcome these obstacles, we developed a method for conducting the objective structured clinical examination (OSCE) through online process. This study was aimed to evaluate the new online OSCE method for pharmacy students to be conducted in the pandemic era. The evaluation of online OSCE trial was conducted using a simple Google form-based questionnaire which evaluated the platform used, the process of online OSCE and the instruction understandings. Respondents consisted of 52 examinees from the students of Pharmacist Profession Program UMY and 15 examiners from the lecturers or assistants who participated in the online OSCE trial held in June 2020. Data was analyzed descriptively to give a comprehensive picture on respondents’ perception towards the new method. The result showed that 78.8% examinees and 73.3% examiner agreed that the platform was easy to use, 65.4% examinees and 100% examiner agreed that the whole process of the exam was easy to understand, only 15.4% examinees and 26.7% examiner agreed that the time was sufficient, 73.1% examinees and 100% examiner agreed that the instructions of each station was clear. It can be concluded from the study that online OSCE method was feasible to be conducted during pandemic, particularly, for clinical pharmacy skills. Adjustment should be done to ensure that the time allocation for each station is adequate.

Abstract

Myristicin has an alkenyl benzene structure with a double bond that is not conjugated with a benzene ring with good sunscreen activity, the change of the double bond to the conjugated position can be done by an isomerization reaction. This change will result in the molecule having longer delocalized double bonds and is expected to increase its activity as a sunscreen. The purpose of this study was to see the effect of changes in myristicin structure on its activity as a sunscreen. The isomerization reaction was carried out by reflux method using 20% ​​KOH catalyst in ethanol for 5 hours. Sunscreen activity test was carried out in-vitro using a UV spectrophotometer. The results of the isomerization reaction obtained isomyristicin in the form of a clear light yellow liquid that dissolves in ethanol (1: 1) with a% yield of 84% and a purity of 90% E: Z (6: 1). The results of the sunscreen activity test showed changes that were not much different between myristicin and isomyristicin, which indicated that the isomerization reaction had no significant effect on sunscreen activity.

Abstract

The toxicity test is one of the pharmacological testing stages that must be carried out to assess the use of a drug. Research on the activity test of the ethanol extract of Tamarind leaves (Tamrindus indica L) at a dose of 14 mg / 200 g BW as an antihyperglycemia in experimental animals gave results that were not significantly different from the standard drug activity of Glibenclamide as an antidiabetic mellitus, so this study was conducted with the aim of looking at the ethanol of tamarind leaves. (Tamrindus indica L) for 30 days on levels of the enzymes AST, ALT, and creatinine in rats (Rattus norvegicus). This sub-acute toxicity test used 14 test animals divided into 2 groups, each group consisting of 7 rats. Group I was given 0.5% Na CMC treatment as a control and group II was treated with the ethanol extract of tamarind leaves at a dose of 14 mg / 200 g BW for 30 days. On the 31st day, blood serum measurements were taken with AST, ALT and creatinine parameters using a humalyzer. The results of the subacute toxicity test showed that giving tamarind leaves ethanol extract as an antihyperglycemia did not cause an increase in the AST, ALT AND creatinine enzymes beyond normal limits.

Abstract

Moringa leaves have been recognized by the public as a treatment. One such disease is Alzheimer's disease where the way this plant works is to inhibit the damage that occurs in the nervous system in the brain. The purpose of this study was to obtain ethanol extract of Moringa leaves by carrying out a phytochemical screening process to determine the chemical content contained in these plants. The moringa leaves obtained were subjected to a grinding process to obtain simplicia powder. After that, the extraction is done by soaking the Moringa leaves with 96% ethanol for 11 times until the extract is perfect. After that, the concentration is done with a Rotary Evaporator until you get a thick extract of Moringa leaves. After that, the thick extract is followed by the Freeze Drying process to get a thick extract that is stable and can hold in storage. After that, the extract was tested against the inhibition of the acetylcholinesterase enzyme based on the Ellman method, the IC50 yield of Moringa leaf extract was 819.7517 ppm. Meanwhile, Eserin positive control with an IC50 value of 5.010 ppm. Acetylcholinesterase inhibition kinetics were competitive. So it is said that Moringa oleifera extract has an effect on the treatment of Alzheimer's disease.
