Abstract

Medical competence is an important index to prepare students to enter the clinical stage in the education process. One of the competencies of doctors in Indonesia is 3B, which is the ability to deal with emergency cases. Patients who experience emergencies must receive immediate help and proper treatment. If the treatment is not appropriate, it can result in patient disability and even death. The purpose of the study was to analyze the ability of doctor study program to interpret supporting examinations in the form of ECG in emergency cases. This study is a quantitative study with a descriptive analytic method with cross sectional design. The samples of this study were all UNIMUS medical professional program students who participated in the UKMPPD OSCE tryout in May 2023 period which was taken through the total sampling technique. This study uses secondary data consisting of the results of ECG interpretation scores and the final results of the stase assessment given by the examining lecturer. The results of the study with 41 samples showed that the majority of students answered incorrectly on heart rate frequency (53.7%), P wave (65.9%) and PR interval (75.6%). While the majority of students answered correctly on rhythm (56.1%), axis (58.5%), transition zone (63.4%), QRS complex (58.5%), ST segment (51.2%) and conclusion (61%). Based on the Competency Test for Medical Professional Program Students OSCE try-out score, most of the sample did not pass (65.9%). The ability of medical professional study program students to interpret supporting examinations in the form of ECG in emergency cases in cardiovascular staging is categorized as low. Keywords: EKG; emergency; OSCE; try out

Abstract

Dengue is a viral infectious disease with a wide spectrum of clinical severity, ranging from mild dengue to severe forms with serious complications. Hematologic markers such as the neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) have been studied as prognostic indicators in various infections, including dengue. Method This was an observational analytic cross-sectional study involving 128 dengue patients admitted to H. Adam Malik General Hospital, Medan. Demographic data and laboratory parameters were analyzed using correlation and comparison tests across severity levels. The median values of NLR and PLR were 3.18 and 53.4, respectively. A positive correlation was observed between NLR and dengue severity, while PLR showed a decreasing trend as severity increased. The lowest PLR values were found in patients with DHF grade II and DSS. NLR and PLR have potential as prognostic biomarkers for predicting the severity and complications of dengue virus infection. Further studies with larger populations are needed for validation. Keywords: Biomarker, dengue,  severity.

Abstract

Sepsis is the most common cause of Acute Kidney Injury (AKI). Diagnosis of AKI based on serum creatinine (sCr) has limitations, sCr increases only after >50% kidney function is lost. uACR examination is a simple test to assess microalbuminuria which reflects impaired kidney function. Analyzing the correlation between uACR and AKI incidence in sepsis patients treated at Adam Malik Hospital. The study design was observational analytic with prospective cohort design. Samples were taken consecutively. Sepsis diagnosis was based on qSOFA score. uACR and sCr I were checked <24 hours after sepsis was diagnosed, sCr II was rechecked 48 hours later to assess the occurrence of AKI. Data were analyzed using SPSS. There were 52 study subjects, predominantly male (57,7%), with an average age of 52,5±16,42 years, and the most common source of infection was the lungs (82,7%). AKI occurred in 46,2% of study subjects. The AUC area of uACR as a predictor of AKI was 75,2%. The independent variable that significantly influenced AKI was uACR (p<0,001). The uACR cut-off was 225 mg/g, with a sensitivity of 83,3%, a specificity of 67,9% and an accuracy rate of 75%. uACR levels ≥225 mg/g had a 10,556 times risk of AKI. There is a significant correlation between uACR and the incidence of AKI so it can be used as a predictor of AKI in sepsis patients.Keywords: Acute kidney injury, albuminuria, rasio albumin kreatinin, sepsis.

Abstract

Language development in early childhood is a fundamental foundation for the formation of thinking, communication, and social skills. This research aims to explore in-depth and develop effective strategies for stimulating early childhood language development, as well as to identify challenges and solutions through the implementation of Action Research in two Early Childhood Education (ECE) institutions in Bandung City. This study adopts a qualitative approach with a dual case study design combined with Action Research. The study was conducted at PAUD Rumah Bintang and PAUD Alamanda, Bandung City, involving teachers, students, and principals as subjects. The Action Research design was applied in two cycles, encompassing planning, implementation, observation, and reflection, to develop and evaluate the effectiveness of strategies directly in the field. Data collection involved participatory observation, in-depth interviews, and documentation, then analyzed using the interactive model of Miles and Huberman with triangulation techniques for data validity. The research findings indicate that the implementation of Action Research successfully optimized diverse language stimulation strategies, including read-aloud practices and interactive conversations, adapted to the unique context of each ECE institution. Another key finding is the adaptive strategies in addressing challenges such as limited resources and children's diverse language backgrounds, with solutions emerging from the cycle of reflection and field-based improvement. This study recommends the Action Research approach as an effective framework for ECE educators and institutions to continuously develop and tailor language learning strategies. Keywords: Action research, childdDevelopment, early childhood, language development, learning strategies.

Abstract

Periodontitis is a chronic inflammatory disease that progressively damages periodontal supporting tissues as a result of colonization by pathogenic bacteria, especially Porphyromonas gingivalis. This condition is characterized by gingival inflammation, loss of periodontal ligament, and resorption of alveolar bone. Conventional therapy through scaling, root planing, and systemic antibiotics is frequently limited by bacterial resistance, relapse, and potential adverse effects. Therefore, alternative natural agents that are safe and able to support bone regeneration are highly required. This study aimed to evaluate the antibacterial activity as well as the biological effect of pineapple (Ananas comosus) prebiotic extract on osteoblast viability. An in vitro experimental design was applied using disc diffusion assays to measure inhibition zones against P. gingivalis and MTT assays to evaluate osteoblast viability. Fermented extracts at concentrations of 10%, 20%, and 30% were tested, with metronidazole as a positive control. Results showed concentration-dependent increases in inhibition zones, namely 7.9 mm (10%), 10.42 mm (20%), and 13.22 mm (30%). MTT assays demonstrated enhanced osteoblast viability at 10% (0.534) and 20% (0.600), whereas viability decreased at 30% (0.551), suggesting possible cytotoxicity at higher doses. ANOVA indicated significant differences among groups (p < 0.05). In conclusion, pineapple prebiotic extract exhibits dual therapeutic potential: antibacterial activity against P. gingivalis and stimulation of osteoblast viability, supporting its development as a promising adjuvant therapy for periodontitis with infection control and alveolar bone regeneration benefits. Keywords: Antibacterial, bromelain, pineapple, osteoblast, periodontitis.

Abstract

This community service program aims to increase the added value and competitiveness of shallots through the innovation of BRAMA shallot powder (Brambang Mama) as a signature product of Karangsemi Village, Gondang District, Nganjuk Regency. The main problem faced by the community is the fluctuating price of shallots, particularly during harvest seasons, which leads to decreased income for farmers. The method consisted of collecting gender-disaggregated data, coordinating with the village government and the SAPA MAMA group, conducting intensive training on processing shallots into powder, packaging and branding techniques, and providing technical assistance in production technology. The results show that participants were able to produce high-quality shallot powder with a distinctive taste and aroma, as well as attractive packaging. BRAMA was agreed upon as a flagship village product to be marketed through UMKM, village bazaars, and sub-district-level promotion. This program had a positive impact by improving community skills, utilizing local potential, and creating sustainable business opportunities. Further training on packaging design, business management, PIRT and halal certification, as well as digital marketing strategies is recommended to strengthen BRAMA as a culinary identity of Nganjuk and to support the village’s economic resilience.Keywords: Shallot; Community empowerment; Village flagship product; BRAMA powder

Abstract

Waste remains a complex environmental problem, particularly in rural areas that rely heavily on agricultural and livestock activities. This condition requires sustainable technology-based waste management alternatives to prevent deterioration in sanitation quality, contamination of water sources, and the decline of environmental carrying capacity for community life. Compreng Village, located in Subang Regency, is predominantly inhabited by farmers, making it vulnerable to environmental pollution due to inadequate waste management practices.This Community Service Program was implemented by introducing an alternative waste management approach, namely the utilization of a Biodigester prototype for household organic waste and agricultural residue processing. The program was carried out under the theme “Healthy and Productive Village,” in which community members were trained to convert organic waste into a renewable energy source as a complementary alternative to LPG gas, while the resulting bioslurry was processed into compost fertilizer. The implementation methods consisted of four main stages: program socialization, training on waste segregation and biodigester operation, mentoring for organic waste processing practice, and technology transfer through community-based biodigester prototype application. A pre- and post-program knowledge assessment was also conducted using a paired t-test. The evaluation results narratively indicated that community members were able to utilize the biodigester prototype to manage organic waste and use the produced bioslurry as fertilizer. Additionally, the paired t-test results showed a significant increase in knowledge regarding the use of biodigesters for organic waste management (p < .001). As a recommendation, ongoing evaluation and follow-up planning are necessary to ensure the sustainability of this program, so that it may be applied. Keywords: Biodigester, environment, healthy productive village, waste management.

Abstract

Nursing professional organizations in Indonesia have established standardized guidelines for nursing documentation. However, the formulation of nursing diagnoses is still carried out as a routine activity by nursing teams, resulting in cases where diagnoses are not established according to proper standards. This community service program aimed to improve nurses’ understanding of nursing diagnosis formulation in accordance with Indonesian standards. The activity was implemented using a mini-seminar method involving 30 participants from two hospitals in the Bandung area. It was part of an activity based on Kemmis and McTaggart’s theory. In the preliminary phase, evaluation was conducted using a knowledge scale measurement and documentation observation to assess the number of nursing diagnoses implemented. The results showed an improvement in nurses’ ability to formulate nursing diagnoses, marked by an increase in understanding from only 10% before the activity to 83.33% after, with a p-value <0.001. In addition, the average number of nursing diagnosis labels increased from two to five per nurse. These findings indicate that the mini-seminar method is effective in enhancing nurses’ ability to formulate nursing diagnoses in accordance with established standards. Keywords: Nursing diagnosis, partisipatory research.

Abstract

Interprofessional collaboration between health workers is essential to ensure quality and patient-centered healthcare delivery, especially in public health care or in Indonesian called as pusat kesehatan Masyarakat or Puskesmas. Understanding the differences in interprofessional attitudes between doctors and nurses is crucial, as these attitudes directly affect teamwork and the quality of patient care. Objective: To examine the variances of interprofessional attitudes between doctors and nurses within Puskesmas settings. This study used quantitative design and cross-sectional approach. This study was attended by 71 respondents with details of 26 doctors and 45 nurses from five different psukesmas. Data were collected using the Interprofessional Attitude Scale (IPAS) questionnaire distributed via google form media. The data were then analyzed using SPSS software with the Mann-Whitney test. The mean score for all domains was categorized as excellent (>85.06) with the highest score belonging to the “diversity and ethics” domain. While the lowest mean score was owned by the “interprofessional bias” domain with a total of (85.06), but still classified as good. Significant results were found in differences in attitudes based on profession (p = 0.000), but not on gender (p = 0.422) or work experience (p = 0.322).  Interprofessional attitudes between doctors and nurses at Puskesmas are generally well-established. Significant differences in attitudes were seen based on professional roles but not based on gender or length of work experience. Efforts to strengthen the understanding of interprofessional collaboration are recommended to further improve collaboration. Keywords: Doctor, interprofessional attitudes, interprofessional collaboration, nurses, Puskesmas
