Abstract

Cognitive dysfunction is one of the complications of type 2 DM caused by insulin resistance, hyperglycemia, and hypoglycemia. Cognitive impairment in patients results in problems with memory, psychomotor speed, and frontal or executive lobe function. It affects patients' ability to manage self-care, including blood glucose monitoring and foot care. This study aims to identify the cognitive function of patients with type 2 DM. The research used descriptive design with quantitative methods and purposive sampling techniques. The population in this study was 46 patients with type 2 DM in RSUD Sumedang with a sample of 41 people. The instrument used is MoCA-INA to measure the cognitive function of patients with type 2 DM. Data analysis used univariate and frequency distribution The results of the study showed that type 2 DM patients had mild cognitive impairment of 26 (63.4%), severe cognitive impairments of 9 (22.0%), and normal cognitive function of 6 (14.6%). Cognitive impairment in diabetic patients has the potential to affect self-care management, so if not properly maintained, it can worsen the condition of diabetes patients and even lead to death. Therefore, nursing care should enhance education regarding cognitive impairments, regular monitoring and assistance, as well as the use of technology to remind patients of the timing of treatment and self-care.

Abstract

Patients with end-stage renal disease, especially those receiving HD treatment, often show characteristics in the body's protein and energy storage, called protein energy wasting (PEW). HD is the mainstay of treatment for end-stage renal disease and the number of patients undergoing HD is increasing every year. Appropriate nutritional therapy is needed to improve nutritional status and improve the patient's quality of life. Malnutrition is a common condition in patients undergoing haemodialysis (HD) and is associated with increased morbidity and mortality. The main aim of this study was to evaluate the relationship between energy and macronutrient intake and nutritional status in HD patients at at Haemodialysis Unit Dr. H. Abdul Moeloek General Hospital Bandar Lampung. This research is an observational analytical study with a cross sectional design with 30 respondent with chronic kidney failure who underwent HD at at Haemodialysis Unit Dr. H. Abdul Moeloek General Hospital Bandar Lampung from April to May 2024, sampling technique using consecutive sampling technique. Information regarding eating patterns was collected using the 1 x 24 hour food recall method, mini mental status examination (MMSE) used to evaluate dementia in elderly respondents and the nutritional status index was based on an assessment using the mid-upper arm circumference percentile, the patient's height was obtained by measuring the patient's knee height which was then calculated using the Chumlea equation to obtain ideal body weight. The nutritional status index uses mid-upper arm circumference to categorize patients with good and poor nutritional status. Data were analyzed using Fisher's test. The results showed that the majority of respondents had inadequate energy intake as many as 20 patients (66.7%), inadequate protein intake as many as 24 patients (80%), inadequate fat intake as many as 26 patients (86.7%) and inadequate carbohydrate intake as many as 23 patients (76.7%). So it can be concluded that there is no relationship between energy intake (p = 0.633), protein (p = 0.656), fat (p = 0.557) and carbohydrates (p = 0.567) with nutritional status in HD patients at at Haemodialysis Unit Dr. H. Abdul Moeloek General Hospital Bandar Lampung. Further research needs to be carried out to determine the intake of other nutrients such as fluids, sodium and potassium as well as the duration of HD which is a common problem in HD patients.

Abstract

Mental health is a fundamental human right for individual, community, and socio-economic development. There are external and internal factors that can cause a person to experience mental problems. There are various factors contributing to the decline in the mental health of children and adolescents, such as economic problems, education, and the health crisis. The system used to help improve mental health is currently conventional, namely face-to-face between medical personnel and patients. This research aims to develop a "Design of a Website-Based Adolescent Mental Health Disorder Screening Application". Therefore, a web-based adolescent mental health screening application will be developed using the Self Reporting Questionnaire 29 (SRQ 29) method. This application was designed using the PHP programming language, MySQL database, and a decision-making system using the Self Reporting Questionnaire 29 (SRQ 29) method. The research method is development research. The research sampling was accidental sampling, The population and sample in this study consisted of 5 research subjects including: (1) Information systems media expert (2) Nursing subject matter expert (Nursing specialist) (3) Public health center (4) Headmaster (5) UKS supervisor/coordinator. Namely taking samples based on what was found by the researcher. This type of data collection uses primary data which is processed using interview observation techniques to obtain results related to adolescent mental health. Modeling using various existing modeling tools is the next step, if the modeling has been declared complete and by what you want to achieve, then proceed to planning until the main target is implementation. The assessment results from material experts obtained an average score of 4.8 in the "Very feasible" category and Media experts received an average score of 5 in the "very appropriate" category. Conclusion: The design of the Website-Based Adolescent Mental Health Disorder Screening Application that was developed has gone through a process of assessment, revision, and improvement from experts and has been declared suitable to be used as an application that helps simplify the process of screening for Adolescent Mental Health Disorders.

Abstract

One of the non-communicable diseases is a cardiovascular disease that causes the 5th highest death in Indonesia, characterized by a person's condition of having an abnormal increase in blood pressure in the arteries. This study was to determine the administration of celery leaf decoction water in lowering blood pressure in the elderly with essential hypertension in Sidoluhur village, the working area of the Godean I Health Center, Sleman Regency. The type of quantitative with a quasi-experimental approach of pretest-posttest control group design 2 groups on February 6-March 02, 2024 for the elderly with essential hypertension. The data collection techniques carried out by the researcher using primary data and secondary data totaling 110 respondents and data analysis using the wilcoxon sign rank test. The pre-test intervention group had morning systolic blood pressure 154.40 mmHg, morning diastolic blood pressure 79.86 mmHg. The average blood pressure of the elderly on the post-test 145.98 mmHg, afternoon systolic blood pressure and afternoon diastolic blood pressure 89.91 mmHg. The control group pre-test 152.32 mmHg morning systolic blood pressure, morning diastolic blood pressure 81.73 mmHg. The average blood pressure of the elderly in the post-test 144.78 mmHg, afternoon systolic blood pressure and afternoon diastolic blood pressure 85.80 mmHg. The results of bivariate analysis of the intervention group with a p-value of <0.05 and a control group with a p-value of <0.05. There is an effect of giving celery leaf boiled water on lowering blood pressure in the elderly with essential hypertension.

Abstract

Child development can be influenced by postnatal factors such as stimulation mother, which plays an important role, in addition to factors nutritional status and chronic diseases in children. The existence of the COVID-19 pandemic risks increasing the child's pain rate. This study aims to determine the postnatal factors that influence and predominantly affect the development of children aged 12–60 months during the COVID-19 pandemic in the South Cilacap Region. The design used is survey-analytic, with a cross-sectional study. A large sample of 106 mothers and children aged 12–60 months with the cluster random sampling technique. Data was collected based on the results of interviews using a questionnaire via a Google Form link and and measuring child development using the SDIDTK application. Bivariate analysis used chi-square, while multivariate analysis used multiple logistic regression. The results showed that there is a significant influence of, maternal attitudes, knowledge about developmental stimulation and chronic diseases in children on the development of children aged 12–60 months during the COVID-19 pandemic in the South Cilacap Region (pv= 0,005; pv= 0,005; pv= 0,042). There is no influence of nutritional status factors on the development of children aged 12–60 months during the COVID-19 pandemic in the South Cilacap Region (pv= 0,385). The mother's attitude about developmental stimulation is the dominant factor that affects the development of children aged 12–60 months during the COVID-19 pandemic in the South Cilacap Region, Mothers who have a positive attitude towards development have a 69,249 times higher chance of having children with appropriate development compared children's chronic diseases and mothers' knowledge about stimulating children's development (pv= 0,005; α= 0,05; OR= 69,249). Mother's attitude about stimulation is important in child development. Education and knowledge can shape a mother's attitude so that more education is needed to achieve appropriate child development.

Abstract

Diabetes mellitus patients are included in a vulnerable group with a higher risk of developing other diseases, thus requiring dynamic and continuous treatment Treatment options for diabetes mellitus include insulin therapy, diabetes medications, trying alternative treatments, undergoing surgery, and improving lifestyle (healthy eating habits and exercise) by consuming nutritious or healthy foods and engaging in physical activity. Self-management of diabetes refers to individual actions taken to control the condition, including treatment and prevention of complications. Some aspects of diabetes self-management include dietary regulation, physical activity/exercise, blood sugar monitoring, medication adherence, as well as self-care/foot care. Objective to determine the Characteristics of Diabetes Mellitus Patients in the Simpang Periuk Community Health Center area. The type of this research is quantitative with a descriptive design approach. The research design utilized in this study is a cross-sectional research design, which is a method involving measurements or observations at the same time and the sample size in this study consists of 36 respondents with Type 2 Diabetes Mellitus. Data was collected through interviews using a questionnaire to gather research variable data, including gender, age, education level, employment status, duration of diabetes, BMI/body mass index, and self-management. The data was then analyzed using univariate analysis. The processed data was subsequently presented in tabular form, accompanied by explanations. The research results show that more than half of the respondents are male (72.2%), and female (27.8%). The most common age group among respondents (41.7%) is 46-55 years old. Education levels also vary, with the majority (52.6%) having completed high school. In terms of occupation, (36.1%) are self-employed. The duration of having type 2 diabetes mellitus for most respondents (77.8%) is less than three years. The majority (69.4%) have a normal body mass index. More than (72.2%) of the respondents have poor self-management.

Abstract

An islamic boarding school is a place for shaping the character of adolescents. Students who study in an islamic boarding school are called santri. However, in reality, students still experience various socio-spiritual problems. This study aims to identify the level of spiritual intelligence in adolescent students at the Baiturrahman Islamic Boarding School in Bandung Regency. The quantitative method was used with a total sampling of 118 respondents, who were adolescent students at the Baiturrahman Islamic Boarding School in Bandung Regency at the Senior High School (SMA) level. The Spiritual Intelligence Self-Report Inventory (SISRI-24) scale was used. The validity of The Spiritual Intelligence Self-Report Inventory (SISRI-24) scale was found to have a product moment correlation coefficient greater than 0.388. Additionally, the reliability of the scale was tested using cronbach's alpha, and the resulting value was 0.789. Data analysis using univariate analysis. The results showed that the level of spiritual intelligence was in the high category (58.5%). The dimensions of Personal Meaning Production (PMP), Conscious State Expansion (CSE), and Transcendental Awareness (TA) are classified in the high spiritual intelligence category with percentages of 73.7%, 56.8%, and 55.9% respectively. The Critical Existential Thinking (CET) dimension, on the other hand, is at a moderate level with a percentage of 51.7%. Various efforts that can be made to improve CET can be through resilience training, a sense of honesty, and altruism, which often contributes to the ability to think and find purpose in life, thereby forming spiritual well-being.

Abstract

Thalassemia major necessitates routine treatment that affects the physical, psychological, and social capacities of both the sufferers and their parents. In these circumstances, the way parents perceive their child's illness significantly influences their quality of life, making this a crucial aspect to consider.Objective: This study aims to explore the relationship between illness perception and quality of life in parents of adolescents with thalassemia major at RSUD dr. Slamet Garut. Method: The study employed a cross-sectional approach, including 64 parents of adolescents with thalassemia major (aged 10-18 years) selected through consecutive sampling. The Brief Illness Perception Questionnaire (BIPQ) was used to measure illness perception, and the World Health Organization Quality of Life Brief Version (WHOQoL-BREF) was used to assess quality of life. Both instruments are validated and reliable, with BIPQ having reliability coefficients of r=0.40-0.78 (α=0.74) and WHOQoL-BREF having r=0.39-0.79 (α=0.94). The Spearman correlation test was utilized for bivariate analysis. Results: The findings indicated that 55 respondents (85.9%) had a positive illness perception of thalassemia, and 61 respondents (95.3%) reported a high quality of life. The correlation analysis revealed a significant relationship between illness perception and the quality of life of parents, with a correlation coefficient of 0.548 (p<0.05). Conclusions: This concludes that there is a significant relationship between illness perception and quality of life in parents of adolescents with thalassemia major at RSUD dr. Slamet Garut.

Abstract

Tuberculosis remains a significant problem in Madiun. There are several issues in the TB program, particularly in the low coverage of Tuberculosis Prevention Therapy (TPT). Besides the low coverage of TPT, there are also significant gaps in the delivery of TPT at several health facilities. Based on the analysis of the issues related to the low coverage of the TPT program, not all health facilities have implemented TPT. Objective: Identifying priorities of problems until providing alternative solutions to problem solving. Method: This study is a qualitative, descriptive observational study. The population consists of all program managers at the Madiun City Health Agency, with seven individuals selected as samples. Problem identification was conducted through in-depth interviews. Prioritization of problems was achieved using the Urgency, Seriousness, and Growth (USG) methods. The prioritization of problems was analyzed using a fishbone diagram to determine the root cause of the issues. Results: Identified the priority problem as the low coverage of TPT due to various factors grouped into Man, Process, Material, and Market. Conslusions: Based on the analysis of priority issues using the USG method, the results show that the achievement of Tuberculosis Prevention Therapy is still low and has not reached the target. The analysis of the root causes of the problem using the fishbone method reveals that the limited number of personnel results in double job burdens, the preference of medication adherence monitor for family over cadres, and the difficulty inviting cadres for socialization.

Abstract

Prematurely born babies often face a range of complex issues such as body temperature instability, cardiovascular problems, fluctuations in blood pressure and heart rate, and respiratory disorders. Improper positioning exacerbates these challenges by compromising their underdeveloped homeostatic mechanisms, leading to physiological instability. Purpose: This study aimed to investigate the impact of the quarter-prone position on the hemodynamic status and sleep duration of premature infants.Methods: A quantitative approach was employed using a quasi-experimental research design with a pre-post control group. The study was conducted at one of urban hospitals in Indonesia over a one-month period from February to July 2023. Thirty-two participants were divided into intervention and control groups. Data analysis using univariate and bivariate analysis Independent T-Test. Results: There are no statistically significant differences among the control and intervention groups (p-value > 0.005). that there is significant statistical difference among two group at post-test at all variables (p-value : 0.00) Discussion The quarter-prone position, in conjunction with environmental factors like noise and lighting, influences the respiratory rate, heart rate, oxygen saturation, and sleep duration of premature infants. Nurses are encouraged to implement the quarter-prone position in caring for premature babies to enhance nursing quality and improve their overall quality of life. This approach is supported by its potential impact on the growth and developmental outcomes of premature infants in the long term.
