Abstract

The present study reports the first comprehensive study on the coordination of benefits program in healthcare insurance in Indonesia. Every individual Indonesian citizen who works is required to have health insurance as a fulfillment of the obligation to be physically and comprehensively healthy. Ownership of health insurance can be through government programs which are mandatory government policies for every employee or health insurance managed by the services of a commercial insurance company. So that every Indonesian employee or citizen generally has 2 health insurance memberships, one is commercial insurance and the other is mandatory insurance from the government, BPJS health insurance. The use of health insurance from commercial insurance is the first choice, while BPJS health insurance is used as a backup if the coverage limit on commercial insurance has expired. With limited coverage limits provided by private insurance based on premium payments for each class registered by the company, the government facilitates a program to use the benefits of the two health insurances simultaneously, known as the Coordination of Benefits between BPJS Health and commercial insurance. This study uses a qualitative method to analyze how far this program has progressed, as well as the development of an additional health insurance benefit program in Indonesia. The end goal of this research it can be concluded that highlighting the benefits of this program that can be maximized by health insurance participants, what obstacles are faced by participants, as well as health services when using the coordination of benefits program and its development in the future.

Abstract

The health insurance in Indonesia is something new for most Indonesians because the term health insurance itself, until now after the pandemic, has not become a word that is commonly understood. The understanding of what is called health insurance is still very varied so it is not surprising, for example, in the past, many people said that the Community Health Maintenance Guarantee (JPKM) in Indonesia was not health insurance but only the name was used without using the word insurance. The Indonesian population is generally a risk taker in terms of health and death. The economic condition of the Indonesian population from independence until now has been able to have a per capita income per year, which has not allowed the Indonesian population to set aside funds to buy health insurance or life insurance. Meanwhile, from the supply side, not many insurance companies offer health insurance products. The Indonesian government has been relatively slow in introducing the concept of insurance to the public through ease of licensing and legal certainty in the insurance business ordeveloping social health insurance for the wider community. This study uses a qualitative method that aims to analyze the relationship between the development of knowledge about the basic history of health insurance related to the basic principles of health insurance in Indonesia which is expected to increase knowledge about the history of health insurance and the basic principles of health insurance. The goal of this research is to find out things that can provide compensation for losses originating from ignorance of health insurance knowledge.

Abstract

Turmeric (Curcuma domestica Val.) is a traditional Indonesian plant that has many benefits. Curcuminoids are turmeric compounds known to have effects as immunomodulators and have been proven in several in vivo tests. Immunosuppressive drugs such as cyclophosphamide are widely given to improve clinical symptoms in autoimmune patients and cancer treatment which can cause susceptibility to infection. Therefore, this study aimed to determine the immunostimulating effect of curcuminoid extract from turmeric rhizome on cyclophosphamide-induced white rats. Proof of the immunostimulating effect of curcuminoid extracts was by testing the total leukocytes and leukocytesdifferential. The test results showed that curcuminoid extracts of 1 mL, 0.5 mL, and 0.25 mL increased total leukocytes and leukocyte differential (lymphocytes, neutrophils, monocytes, basophils, eosinophils) with a significant difference (p<0.05) to the Na group CMC 0.5% as a negative control. Therefore, curcuminoid extract had the potential to have an effect as an immunostimulant. Abstract

Indonesia has a variety of plants that can be used to cure various diseases. Natural therapies offer little negative effect, especially in degenerative diseases, due to decreased function of body cells damaged by oxidation. Antioxidants have a vital role in counteracting free radicals to prevent degenerative diseases. One of the most abundant sources of antioxidant compounds is flavonoids. Kedondong leaf (Spondias dulcis) is one of the plants which is also suspected to contain flavonoids. Spondias species contain triterpenoids, namely oleanolic acid and flavonoids such as quercetin, kaempferol, kaempferida and rhamnetin. Examination of simplicia characteristics, phytochemical screening, examination of quercetin with Thin Layer Chromatography, and determination of antioxidant activity and IC50 value of Kedondong leaves using the DPPH method were carried out in this study. The results of the characteristic examination showed that simplicia met the requirements with a water content of 8.63%, a water-soluble extract content of 19.42%, an ethanol-soluble extract content of 5.17%, a total ash content of 12.43%, an acid-insoluble ash content of 7 .81%. Phytochemical screening showed that the bright kedondong leaves contained flavonoids, alkaloids, glycosides, saponins, tannins, and steroids/triterpenes. Examination of quercetin thin layer chromatography with chloroform: methanol (70: 30) as eluent produced six stains with Rf values (0.5; 1; 1.2; 3; 4.3; 6.6), and compared with quercetin raw material on the sixth stain with an Rf value of 6.6 has the same stain as the ethanol extract of kendondong leaves. The results of antioxidant testing at a 400 – 800 nm wavelength using UV-Vis spectrophotometry obtained a maximum wavelength of 515 nm. IC50 value obtained an IC50 value of 68.6260 ppm, so it can be concluded that the ethanol extract of kendondong leaves has antioxidant activity in the strong category.
