Abstract

The hospital is one of the institutions engaged in health services. The main indicator of the standard of a health facility and is a measure of the quality of service, low customer satisfaction will have an impact on the number of visits that will affect the profitability of health facilities. This study aims to determine the effect of quality and the dominant variables that affect service on patient satisfaction at the Haji-Medan Hospital in 2020. The quantitative research was conducted through an analytical survey research approach with a cross sectional study approach. Testing through logistic regression on the independent variables, namely; Reliability, Responsiveness, Assurance, Empathy and Tangible from the respondents' questionnaire data as many as 80 samples. The results showed that service quality had a significant effect on patient satisfaction from four variables, namely; reliability p value 0.020 (p < 0.05), responsiveness p value 0.038 (p < 0.05), empathy p value 0.000 (p < 0.05), p value tangible 0.001 (p < 0.05). Assurance variable, the quality of health services has no effect on patient satisfaction with a p value of 0.332 (p> 0.05). The dominant variable affecting patient satisfaction is the empathy variable. It is recommended to improve services from the aspect of assurance in order to increase patient confidence in getting services so that patients can feel satisfied with the services provided.

Abstract

According to Globocan data, in 2022, cervical cancer caused 348,874 deaths among women, and cases of cervical cancer in Indonesia increased to 36,633 people. WHO recommends early detection through the VIA (Visual Inspection with Acetic Acid) examination. However, the VIA examination coverage in Kampar Regency in 2023 was low, at only 1.2%. This study aims to analyze the Acetic Acid Visual Inspection (VIA) program for women of reproductive age (WRA) based on healthcare service accessibility in Kampar Regency, Riau Province, in 2024. This research uses a cross-sectional method, and the results show that information about VIA services is lacking, the acceptance of VIA services is low, service hours are not always available, and community empowerment in supporting VIA services is insufficient. The factors that most influence the participation of women of reproductive age in the VIA examination, based on healthcare service accessibility, are service acceptance (p=0.008 and exp(B)= 19.342) and community empowerment (p=0.017 and exp(B) = 8.272). Variables such as service location distance, knowledge, attitude, transportation ease, and environmental support did not significantly affect the situation. It is recommended that service acceptance be improved through promotion, education, and service socialization, as well as strengthened community empowerment by involving local health workers, the Family Welfare Movement (PKK), and related cross-sector stakeholders in an integrated manner.

Abstract

Type 2 diabetes mellitus (T2DM) is a global health challenge with cardiovascular complications as the leading cause of morbidity and mortality. Monitoring HbA1c levels and lipid profiles are crucial parameters in evaluating cardiovascular risk in T2DM patients. This cross-sectional analytical study was conducted at the Gianyar Police Clinic from January to June 2024. A total of 60 T2DM patients meeting the inclusion criteria were analyzed using Pearson correlation and multiple linear regression tests to assess the relationship between HbA1c levels and total cholesterol. A significant positive correlation was found between HbA1c levels and total cholesterol (r=0.458, p<0.001). The correlation was stronger in patients with diabetes duration >10 years (r=0.512, p<0.001). 73.3% of patients had HbA1c >7% and 56.7% experienced total cholesterol abnormalities. Statin use showed a weaker correlation (r=0.378) compared to non-users (r=0.526). The majority of patients were in the 50-65 years age group (56.7%), with 63.3% having hypertension and 40% experiencing obesity as comorbidities. There is a significant relationship between HbA1c levels and total cholesterol in T2DM patients, with the strength of the relationship influenced by diabetes duration, statin use, and comorbidities. These findings emphasize the importance of simultaneous monitoring of both parameters in T2DM management, considering local socio-cultural factors.

Abstract

Chronic Kidney Disease (CKD) stage 5 is a condition of chronic kidney function decline with GFR less than 15 mL/minute/1.73 m2 . CKD stage 5 requires renal replacement therapy either by transplantation or dialysis. Currently, the most common renal replacement therapy is hemodialysis. Hemodialysis is undergone by patients throughout their lives and is carried out in hospitals or hemodialysis centers. CKD patients stage 5 who undergo hemodialysis have many setbacks and limitations in their lives so that they are susceptible to depression. This study aims to determine the relationship between the length of time patients undergo hemodialysis and the level of depression. The design in this study was descriptive analytical with a cross-sectional approach with 31 respondents who underwent hemodialysis therapy at Ruteng Regional Hospital in September - October 2024. Depression data were taken using the Zung Self-rating Depression Scale questionnaire. Bivariate data analysis used the Spearman Rank test with a significant value at p-value <0.05. The Spearman Rank test showed a negative correlation between the length of time patients underwent hemodialysis and the level of depression with a p value = 0.002 (p <0.05) and a correlation coefficient (r = -.539) which showed that the longer the patient underwent hemodialysis, the lower the level of depression.

Abstract

Since the beginning of the case, the human immunodeficiency virus (HIV) has grown dramatically on a global scale. Pregnant women with HIV are one of the groups that receive worse negative impacts due to changes in health and social life due to being infected with HIV. Several studies have revealed coping strategies that pregnant women with HIV carry out to continue their lives. Therefore, this scoping review aims to identify coping strategies carried out by pregnant women in facing difficulties after being diagnosed with HIV. The study was established based on a scoping review framework by Arskey and O'Malley. Article searches were carried out on the MEDLINE and Scopus databases in early 2023. The selection of articles refers to inclusion criteria which include: the results of research on coping strategies carried out by pregnant women with HIV in facing difficulties, having a full text in English, and published in 2014-2023. The results of the analysis of 18 selected studies found two themes related to pregnant women diagnosed with HIV, namely the difficulties faced by pregnant women with HIV and coping strategies carried out by pregnant women with HIV. Almost all pregnant women with HIV have faced difficulties in controlling their negative emotions and stigma about HIV. However, positive coping strategies make pregnant women with HIV improve and live their next lives. The experience of pregnant women with HIV who have successfully faced their difficulties with positive coping strategies needs to be considered by the government and health service providers in managing the reduction of HIV prevalence.

Abstract

Office Syndrome is increasingly common due to sedentary work environments and inadequate ergonomic practices. This condition is characterized by a variety of symptoms, such as musculoskeletal pain, visual fatigue, sensory disturbances, sleep disturbances, and mental stress, which impact workers’ health and reduce productivity. However, the assessment of this condition is often hampered by the lack of comprehensive and standardized measurement tools. This study aims to develop and test the validity and reliability of an office syndrome screening questionnaire that covers seven dimensions: physical pain, muscle fatigue, visual fatigue, sensory disturbances, decreased productivity, sleep disturbances, and mental stress. The study was conducted cross-sectionally involving 47 office workers in the Jababeka area, Cikarang, who met the inclusion criteria. Data were collected using a questionnaire that was filled out independently via Google Form. Validity testing was carried out using Pearson correlation analysis (items are valid if r > 0.3 and p < 0.05), while reliability testing used Cronbach’s Alpha (reliable if α > 0.7). The results showed that all items had a significant correlation to the total score (p < 0.05), with correlation values ranging from 0.416 to 0.697. The highest correlation was found in the items "Decreased work efficiency" (r = 0.697) and "Difficulty concentrating" (r = 0.673), while the item "Muscle tension" had the lowest correlation (r = 0.416) but was still valid. The overall Cronbach's Alpha was 0.745, indicating good reliability. This screening questionnaire was proven to be valid and reliable to measure symptoms of office syndrome comprehensively.

Abstract

Electronic Medical Record (EMR) revolutionize patient information management in healthcare but pose challenges for nurses, who play a crucial role in their utilization. Despite perceived benefits, nurses face significant mental burdens navigating these systems, requiring tailored strategies for successful integration. This article explores nurses' challenges with EMRs and proposes solutions to alleviate their burden while ensuring efficient patient care. This study employed a systematic literature review following the PRISMA method. Conducting searches across databases Scopus and PubMed using keywords “electronic medical record” and “nurse”. Total of 22 selected articles show that Nurses' cognitive challenges with EMRs in hospitals stem from design complexities, impacting workflow and decision-making, exacerbated by factors like experience levels and external events like the COVID-19 pandemic, while usability, system quality, and participatory design approaches can mitigate stressors and enhance nurses' engagement and efficiency. Nurses' EMR experiences impact their well-being and patient care. Addressing these challenges requires optimizing design, usability, training, and support. Supporting nurses in EMR use enhances patient care and well-being, necessitating ongoing research and collaboration for improvement.

Abstract

Measles is an internal disease (VPD) caused by Morbilivirus. This disease is characterized by the appearance of fever, reddish spots, cough, runny nose, red eyes and rashes all over the body which generally occurs in children. Measles, also known as Morbili or Measles, is a disease caused by the measles virus of the Paramyxovirus group. Transmission can occur through air that has been contaminated by droplets (spit) of an infected person. This case tends to attack early childhood and elementary school age children. Once a person has had measles, he or she will have lifelong immunity to the disease The aim of this research is to determine the factors related to the incidence of measles in toddlers in South Halmahera Regency. The research design used is quantitative research using a "case control" approach. The sampling technique used was purposive sampling with a sample of 106 consisting of 53 case groups and 53 control respondents. Data analysis techniques use Chi Square, Multivariate and Logistic Regression tests. The results of the research show that of the 106 respondents studied, the factors that most influence the incidence of measles in toddlers 0 - 59 months based on the beta value and p value, it is found that the environmental variable has a value < α, namely .021 and the beta value is -1.830, so the variable with the most dominant is the environment. Conclusion. The factors that most influence the occurrence of measles are environmental factors, so it is necessary to conduct socialization or education on the importance of environmental cleanliness to prevent the emergence of measles in children under five.

Abstract

Improving effective communication is the second point in patient safety targets. Based on initial data, effective communication at RSU X Padang City in its implementation has not reached the target of 100% and the weakness of this hospital is that risk management has not been implemented properly. The purpose of this study was to analyze the implementation of effective communication by strengthening risk management in the inpatient ward of RSU X Padang City. This study used a qualitative research design with a phenomenological approach. Researchers conducted semi-structured interviews with 16 informants, document reviews, and observations. The results of the study showed that the role of HR factors in effective communication was not optimal, cooperation within the unit was quite good, the role of management and the hospital environment was not optimal, the role of risk management was not optimal, and the impact of effective communication could improve the achievement of IMP-RS, facilitate the delivery of patient conditions, and minimize errors in providing therapy or medication to patients so that patient quality and safety are guaranteed. It is recommended that the implementation of effective communication always be improved with improvement efforts, namely completing facilities and infrastructure, implementing socialization and training, monitoring and evaluation, and implementing risk management programs.
