Abstract

Lateral inguinal hernia, an indirect type of inguinal hernia, occurs due to a persistent processus vaginalis or weakness of the abdominal wall and is commonly found in males, requiring proper management to prevent complications. This paper is a literature review analyzing epidemiology, clinical presentation, diagnostic methods, and surgical management based on current scientific sources. The review shows that diagnosis is primarily established through clinical examination, with ultrasonography used as supportive imaging when needed, while surgical repair using open or laparoscopic techniques is effective with low recurrence rates, and laparoscopy offers faster recovery. It is concluded that lateral inguinal hernia requires early detection and operative intervention to prevent incarceration or strangulation, and that modern surgical techniques provide an excellent prognosis.

Abstract

Tuberculosis (TB) in children remains a significant global health problem, particularly in countries with high transmission rates. Children are more susceptible to rapid progression into severe forms of TB, such as TB meningitis and miliary TB, due to their immature immune systems. Diagnosing TB in children is challenging because symptoms are often nonspecific and bacterial loads are low, leading to frequent negative bacteriological results. Common clinical manifestations include persistent cough, prolonged fever, weight loss, and failure to thrive. Major risk factors include close contact with adult TB cases, malnutrition, and immunocompromised conditions. Diagnostic approaches require a combination of clinical evaluation, radiological examination, and immunological tests such as the tuberculin skin test or IGRA. Treatment follows standard antituberculosis drug regimens according to national guidelines. Preventive measures, including BCG vaccination and routine screening of household contacts, play a crucial role in reducing the incidence and complications of TB in children. Overall, early detection and comprehensive management are essential to decrease morbidity and mortality associated with pediatric TB.

Abstract

Insomnia is a common sleep disorder characterized by difficulty initiating sleep, maintaining sleep, or achieving restorative sleep, leading to impaired daytime functioning. Pharmacological therapy is often considered when non-pharmacological interventions fail to provide adequate relief. Several classes of medications are used to manage insomnia, including benzodiazepines, non-benzodiazepine receptor agonists (Z-drugs), melatonin receptor agonists, orexin receptor antagonists, antidepressants with sedative properties, and antihistamines. Each class varies in its mechanism of action, onset, duration of effect, and risk profile. While pharmacological therapy can be effective for short-term symptom relief, inappropriate or prolonged use may lead to tolerance, dependence, cognitive impairment, or residual daytime sedation. Therefore, medication selection should be tailored to the patient’s clinical profile, comorbidities, and sleep patterns. Integrating pharmacological therapy with behavioral approaches such as cognitive behavioral therapy for insomnia (CBT-I) is recommended to achieve sustainable long-term outcomes.

Abstract

Preseptal cellulitis and orbital cellulitis are soft tissue infections surrounding the eye that may lead to serious complications if not properly managed. Preseptal cellulitis is confined to the tissues anterior to the orbital septum, whereas orbital cellulitis involves structures posterior to the septum and carries a higher risk of visual impairment and intracranial complications. These conditions are commonly caused by the spread of infection from the paranasal sinuses, trauma, insect bites, or local eyelid infections. Clinically, preseptal cellulitis presents with eyelid edema and erythema without impairment of ocular motility or visual acuity, while orbital cellulitis is characterized by more severe symptoms such as pain with eye movement, proptosis, ophthalmoplegia, and decreased visual acuity. Diagnosis is based on clinical evaluation and supported by imaging studies, particularly orbital computed tomography (CT) scans, to differentiate between the two conditions. Management of preseptal cellulitis typically involves oral antibiotic therapy, whereas orbital cellulitis requires intravenous antibiotics, close monitoring, and possibly surgical intervention. Early and accurate diagnosis followed by prompt treatment is essential to prevent serious complications and preserve visual function.

Abstract

Sharia accounting is a process of accounting activities based on sharia principles (Islam) and is accountable to Allah SWT. This accounting is needed to support activities that must be carried out in accordance with sharia, because it is impossible to implement sharia-compliant accounting if the accounting process is not sharia (conventional). The BMT AL ISHLAH Cooperative in Jambi City is one of the sharia-based microfinance institutions. This cooperative is under the supervision of the Minister of Cooperatives and SMEs. including the Sharia Savings and Loans and Financing Cooperative (KSPPS). The researcher uses a qualitative research method where qualitative research is research that requires data and information directly to the field or research location. Qualitative research makes the researcher a key (main) instrument where the researcher will collect data, check the validity of the data, and analyze the data. The data source used by the author uses secondary data. secondary data is data obtained or collected by other people or certain institutions that are more informative. Therefore, researchers want to see how the application of sharia accounting standards in financial reports is based on PSAK 101 and murabahah financing based on PSAK 102.

Abstract

The Medical Professional Competency Examination (UKMPPD) is a national assessment designed to ensure the quality and competence of medical graduates in Indonesia. The high proportion of retakers in the Computer-Based Test (CBT) UKMPPD at several institutions, including the Faculty of Medicine, University of Lampung, highlights the need to identify academic factors associated with examination success, such as undergraduate Grade Point Average (GPA) and AIPKI Try-Out scores. This study aimed to analyze the correlation between undergraduate GPA and AIPKI Try-Out scores with CBT UKMPPD results among medical professional program students at the Faculty of Medicine, University of Lampung, during the February 2024–August 2025 period. This study employed a descriptive-analytic design with a cross-sectional approach. Secondary data were obtained from the academic office of the Faculty of Medicine, University of Lampung. A total of 361 students who met the inclusion and exclusion criteria were included in the analysis. Data were analyzed using descriptive and bivariate methods with correlation tests. The results showed that the mean undergraduate GPA was 3.26, the mean AIPKI Try-Out score was 56.77, and the mean CBT UKMPPD score was 78.77. Statistical analysis demonstrated a strong positive correlation between undergraduate GPA and CBT UKMPPD scores (r = 0.637; p = 0.001), as well as between AIPKI Try-Out scores and CBT UKMPPD scores (r = 0.741; p = 0.001).

Abstract

Dysmenorrhea is a medical term used to describe pain experienced during menstruation, particularly in the lower abdominal area, occurring before or during the menstrual period. This condition is commonly known as menstrual pain or menstrual cramps. Limited understanding of dysmenorrhea self-medication is influenced by inadequate access to health information, especially regarding menstrual pain management. Therefore, this study was conducted to describe the level of knowledge of takhasus female students at Hajar Aswad Islamic Boarding School, Yogyakarta, regarding dysmenorrhea self-medication. This research employed a purposive sampling method, in which respondents were selected based on specific criteria. The sample size was determined using the Slovin formula. Data were collected through a questionnaire, and data analysis was conducted using a paired sample t-test with the assistance of Microsoft Excel and SPSS software. The results showed that after receiving educational intervention, there was a significant increase in students’ knowledge, with most respondents achieving a good knowledge category. The paired sample t-test results indicated a significant difference between pre-test and post-test scores (p-value = 0.000), demonstrating that educational intervention significantly improved students’ knowledge regarding dysmenorrhea self-medication.

Abstract

Systemic lupus erythematosus (SLE) is a chronic autoimmune disease that predominantly affects women of reproductive age and poses significant risks during pregnancy. Pregnant women with SLE have a higher incidence of maternal and fetal complications, including recurrent pregnancy loss, preterm birth, and fetal growth restriction. Early recognition and multidisciplinary management are essential to improve pregnancy outcomes. We report a case of a 31-year-old pregnant woman (G4P0A3) at 19–20 weeks of gestation who presented with progressive joint pain, thrombocytopenia, alopecia, and photosensitive facial rash. The patient had a history of three previous pregnancy losses. Immunological examination revealed positive anti–double-stranded DNA antibodies, confirming the diagnosis of systemic lupus erythematosus with mild disease activity. Obstetric ultrasound showed a single live intrauterine fetus consistent with gestational age. The patient was managed collaboratively by obstetrics–gynecology and internal medicine teams. Treatment included systemic corticosteroids and hydroxychloroquine, along with supportive obstetric care. Despite conservative management, the patient developed signs of impending abortion and subsequently experienced a complete spontaneous abortion. Post-abortion care was provided, and the patient remained hemodynamically stable. This case highlights the significant impact of systemic lupus erythematosus on pregnancy outcomes, particularly recurrent pregnancy loss, even in patients with mild disease activity. Early diagnosis, preconception counseling, and close multidisciplinary monitoring throughout pregnancy are crucial to reduce maternal and fetal morbidity in women with SLE.

Abstract

Informal workers dominate Indonesia’s labour market but remain largely excluded from formal pension systems due to low and volatile incomes, raising concerns about old-age income security. The central question addressed is whether an ultra-micro digital pension scheme can generate meaningful retirement benefits for low-income informal workers and whether artificial intelligence (AI) can enhance contribution effectiveness under income uncertainty. A simulation-based framework is employed using official income statistics, combining deterministic and Monte Carlo simulations over a 25-year accumulation period and a 15-year payout phase. Three contribution designs are evaluated: a fixed ultra-micro nominal contribution, a flexible income-based contribution equal to 3% of earnings, and an income-based scheme augmented by an AI-assisted top-up mechanism. Fixed nominal contributions produce limited replacement rates of around 8%, while income-based contributions increase replacement rates to approximately 15%. The integration of AI-assisted contribution optimisation further raises replacement rates to about 19–20% and shifts the distribution of outcomes upward, improving downside protection under income volatility. Although the resulting benefits remain below conventional adequacy benchmarks, the findings demonstrate that ultra-micro digital pensions are financially feasible as complementary retirement instruments and that AI-enabled contribution mechanisms add measurable value in highly informal, low-income labour market settings

Abstract

Unstable Angina Pectoris (UAP) is a form of acute coronary syndrome characterized by new-onset chest pain, worsening angina, or symptoms occurring at rest without elevation of cardiac biomarkers. This condition reflects unstable myocardial ischemia and requires prompt management to prevent progression to myocardial infarction. The diagnosis of UAP is established through a combination of clinical symptoms and electrocardiographic findings such as transient ST-segment depression, T-wave inversion, or other ischemic changes, accompanied by normal troponin levels. Initial management includes the administration of antiplatelet agents, anticoagulants, nitrates, beta-blockers, and statins according to current guidelines, along with risk stratification to determine the need for invasive intervention. Further evaluation through coronary angiography is essential to assess the degree of coronary artery stenosis and guide subsequent treatment strategies, whether through percutaneous coronary intervention or intensive medical therapy. Early identification, comprehensive evaluation, and evidence-based intervention are crucial to reducing morbidity and preventing more severe cardiovascular complications in patients with UAP.
