Abstract

The management of public health services currenly relies heavily on the integration of digital technology to minimize information fragmentation and support accurate managerial decision-making. This study aims to synthesize national data on the utilization of Health Information Systems (HIS) and evaluate the role of digital maturity in supporting the efficient management of public health service. The research method employs a qualitative approach with a literature review design through a systematic search of the Google Scholar and PubMed databases for literature published over the past 10 years. The results indicate that effective HIS imlementation positively correlates with improved healthcare worker perfomance and accelerated clinical administration, where accurate data can systematically optimize operational workflows. However, challenges were identified in the form of technical system constraits that cause long queues and the potential for increased administrative workload for nurses if the system is not holistically integrated. It is recommended that future EHR development focus on strengthening cross-system interoperability, simplifying reporting variables, and enhancing human resource capacity through continuous training.

Abstract

The surge in National Health Insurance (JKN) participants to 267.3 million by the end of 2023 has significantly increased hospital operational workloads, leading to prolonged patient waiting times. Excessive waiting time reflects low process efficiency driven by non-value-added activities (waste). This issue severely diminishes patient satisfaction and contributes to global healthcare financial inefficiencies estimated at 20–40%. This study aims to synthesize empirical evidence regarding the effectiveness of DMAIC-based Lean Six Sigma (LSS) in reducing patient waiting times in hospitals, identify implementation challenges, and formulate strategic management recommendations. A systematic literature review was conducted by analyzing nine original research articles sourced from Google Scholar, PubMed, and PMC databases. Data were extracted based on methodology (DMAIC, VSM, pre-post trial), key findings, and field challenges, followed by a narrative synthesis. The synthesis demonstrates that LSS consistently and significantly reduces patient waiting times without requiring substantial capital investment. Primary barriers include Hospital Management Information System (SIMRS) instability, unergonomic facility layouts, manual prescribing practices, and slow adaptation to statistical competencies among staff. Effective improvement strategies focus on waste elimination, Lean tools application (5S, Kanban), and digitalizing administrative workflows. The implementation of DMAIC-based LSS proves to be an effective, measurable, and sustainable approach to reducing patient waiting times. Hospital management is highly recommended to integrate LSS as an internal quality regulation, enhance integrated IT infrastructure, and conduct periodic training on DMAIC methodologies for healthcare personnel.

Abstract

Advances in digital technology in the healthcare sector are driving increased use of wearable data across various services, including health insurance. Data generated by wearable devices such as physical activity, heart rate, and sleep patterns offer opportunities to support more dynamic health risk assessments, the development of more personalized premiums, and the strengthening of preventive approaches within the health insurance system. This trend indicates a shift in the insurance system from a reactive approach toward a data-driven and preventive approach. This study employs a literature review approach by examining relevant scientific articles from databases such as Google Scholar, PubMed, and Garuda. The analysis focuses on literature regarding the utilization of wearable data in health insurance as well as the accompanying ethical considerations. The findings indicate that wearable data holds significant potential in supporting the transformation of a data-driven health insurance system; however, its implementation still faces limitations regarding privacy, data security, and the readiness of regulations specifically governing the use of digital health data. Additionally, the use of wearable data raises ethical challenges, such as the potential for misuse of health data and the possibility of unequal treatment of insurance participants based on their health conditions. In Indonesia, regulations regarding the protection of digital health data are still evolving and do not yet fully address the technical aspects of using wearable data in health insurance systems. Therefore, there is a need to strengthen regulations and policies grounded in digital ethics to ensure that the use of wearable data is safe, fair, and responsible.

Abstract

Community participation is a crucial element in health development efforts because the success of health programs depends heavily on the active involvement of the community, whether in the form of labor, ideas, or other forms of support. Community participation is necessary to assist the government in achieving its goals of improving health outcomes. Such participation is not merely defined by physical presence but also encompasses the community’s mental and emotional engagement in supporting development activities. Therefore, community involvement needs to be enhanced at various stages of activities, including in the implementation and monitoring of health programs at the local level. A literature review method was employed by searching for scientific articles through the Google Scholar, Garuda, and PubMed databases. Inclusion criteria focused on qualitative and quantitative studies regarding the evaluation of community participation published within the last five years. The selected articles were those relevant to the topic and aligned with the research objectives. An evaluation of community participation policies in the planning, implementation, and monitoring of health programs at the local level indicates that community involvement remains suboptimal. This is due to a lack of knowledge and awareness among the public, limited communication, and a lack of community involvement in health decision-making. During the implementation phase, a participatory approach through health education, outreach, community-based communication, and the empowerment of health volunteers can increase community involvement in health programs. However, during the monitoring phase, the community still does not play a significant role in evaluating health policies.

Abstract

The family plays a crucial role in maintaining and improving its members’ health, particularly during adolescence, a period characterized by vulnerability to various health behaviors and issues. However, several issues remain prevalent, including inadequate menstrual hygiene and handwashing practices, smoking behavior within the family, and suboptimal management of gastritis. These conditions highlight the importance of a family nursing approach to enhance family independence in managing health. This final scientific paper aimed to describe the process of family nursing care for a family with an adolescent experiencing health problems related to menstrual hygiene, handwashing practices, smoking behavior, and gastritis. Based on the assessment conducted from December 30, 2025 to January 1, 2026, in Gampong Lam Ujong, Darussalam Sub-district, Aceh Besar Regency, three priority nursing diagnoses were established collaboratively with the family: ineffective family health management related to menstrual hygiene and handwashing practices, risk-prone health behavior in the form of smoking, and ineffective health management related to gastritis. Interventions were developed based on the five family health tasks. Interventions for the first diagnosis focused on improving menstrual hygiene practices and the six-step handwashing technique. The second diagnosis focused on education regarding the dangers of smoking, teaching the Spiritual Emotional Freedom Technique (SEFT), and creating a smoke-free environment. The third diagnosis included health education on gastritis, promoting regular eating patterns, reducing consumption of instant foods, and applying deep-breathing relaxation techniques and warm compresses. Evaluation results indicated improvements in knowledge, changes in attitude, and the family’s ability to implement clean, healthy lifestyle behaviors, support smoking cessation efforts, and engage in more structured self-management of gastritis. Community health centers are expected to strengthen family-based education and support programs to promote sustained improvements in health status

Abstract

Household waste management remains one of the environmental health problems commonly found in rural communities. Improper waste management can cause environmental pollution and increase the risk of public health problems. This study aimed to describe household waste management in Sama Subur Village, Motui District, North Konawe Regency. This research was a descriptive study with a quantitative approach. The study was conducted in January 2026 involving 46 household heads selected using a total sampling technique. Data were collected through interviews using structured questionnaires and observation sheets. The variables studied included the availability of household trash bins, separation of organic and inorganic waste, main methods of waste disposal, frequency of waste disposal, barriers to waste management, and disturbances caused by waste. Data were analyzed descriptively and presented in frequency distribution tables and percentages. The results showed that most respondents used open trash bins (78.3%), never separated organic and inorganic waste (76.1%), and disposed of waste by burning (84.8%). Most respondents disposed of waste daily (54.3%). The main barrier to waste management was the absence of waste collection services (60.9%). Most respondents reported no disturbances caused by waste (82.6%). The study concluded that household waste management in Sama Subur Village is still not optimal and requires improved facilities and community education regarding proper and healthy waste management.

Abstract

Musculoskeletal disorders—problems with the musculoskeletal system—cause pain, stiffness, and discomfort in certain parts of the body due to non-ergonomic work practices. Among vendors at the Mandonga Wet Market in Kendari, it is suspected that MSD complaints arise from improper work practices, such as bending over, standing for too long, and repeatedly lifting heavy objects. This study aims to identify factors associated with Musculoskeletal Disorders (MSDs) among vendors at the Mandonga Wet Market in Kendari in 2025. This is an analytical, observational study using a cross-sectional design. The sample size was 209 participants. Data were analyzed by testing the relationship between two variables using the Chi-Square test. The results indicate a significant association between age (p-value = 0.000) and MSD complaints, a significant association between work posture (p-value = 0.000) and MSD complaints, and a significant association between length of service (p-value = 0.000) and MSD complaints. However, there was no significant association between work duration (p-value = 0.320) and MSD complaints. It can be concluded that there is an association between age, work method, and duration of employment with musculoskeletal disorder (MSD) complaints; however, there is no association between duration of employment and musculoskeletal disorder (MSD) complaints among vendors at the Mandonga Wet Market in Kendari in 2025.

Abstract

The digitalization of health insurance in Indonesia, particularly in the implementation of the National Health Insurance (JKN) program, offers significant benefits in improving the efficiency and accessibility of healthcare services. However, this digital transformation also presents serious challenges related to data security and public trust. This study aims to examine the relationship between data security and public trust in the context of health insurance digitalization through a literature review approach. The method used is descriptive-analytical by reviewing relevant scientific articles published between 2020 and 2026, obtained from databases such as Google Scholar, Garuda, and other international sources. The findings indicate that data security risks, including data breaches, cybercrime, and weak data governance, are key factors that may reduce public trust in digital health insurance services. In addition, non-technical factors such as low privacy literacy and human error further increase system vulnerabilities. On the other hand, public trust is influenced not only by technical security but also by perceived benefits, transparency, and ethical governance. Therefore, strengthening data security systems, adopting innovative technologies, and developing trust-based governance are essential to support the sustainability of health insurance digitalization in Indonesia.

Abstract

Audits, as systematic evaluation tools, play a strategic role in ensuring and improving the quality of healthcare services. This literature review aims to analyze the contribution of audits—including clinical audits, operational audits, and information system audits—to service quality standards in healthcare facilities. The method used was a systematic literature review with a narrative synthesis approach of nine selected scientific articles published between 2016 and 2026. The results of the study indicate that ISO 9001:2015-based internal audits, operational audits, clinical coding audits, and Hospital Management Information System (HMIS) audits have a positive impact on service effectiveness, patient safety, and the accuracy of clinical data. Audit implementation still faces various obstacles, such as the high workload of healthcare personnel, limited human resource competencies, the perception of audits as an administrative burden, and limitations in information technology infrastructure. The conclusion of this study emphasizes that optimizing audits requires the integration of human resource capacity building, the development of reliable information systems, and a supportive organizational culture of quality. The application of the Plan-Do-Check-Act (PDCA) cycle is recommended as a framework to drive continuous improvement in service quality.
