Abstract

Increasing age is generally followed by increasing chronic diseases so that the elderly needs much therapy using drugs for therapy of numerous diseases they have. Hyper-tension is one of diseases of which its prevalence increases along with increasing age. Most of the elderly diagnosed as having hypertension finally take therapy using anti-hypertension drugs. Physiological changes that happen to the elderly lead to use of drugs for side effect diseases of which their consumption should be considered while having anti-hypertension drugs. Changes in biological system to the elderly will af-fect the process of drug molecular interaction, which finally affects clinical efficacy and pharmacotherapeutic safety. Meanwhile, minimizing the problem of drug use insecurity is one of good prescription demands in the implementation of clinical pharmacy. To identify and study safety of anti-hypertension drug use at the elderly polyclinic of Dr. Sardjito Hospital Outpatient Installation by identifying and evalu-ating contraindication, interaction and side effect of anti-hypertension drugs. This was a descriptive study with data taken prospectivelly from 80 patients of the elderly polyclinic of Dr. Sardjito Hospital Outpatients Installation from November 14 to December 13, 2005. Data were obtained from medical records, interviews and ques-tionnaires. The study on contraindication, interaction and side effect of drugs which happened was based on a guideline book. There was 27.5% of patients took anti-hypertension drugs which were not beneficial to patients’ clinical condition, so the that use of antihypertension drugs needs monitoring. As much as 41.3% of patients took combined drugs potential to cause interaction, 8.7% of which had clinical symp-toms presumed to be related with possibility of increasing drug interaction effect. As much as 33.8% of patients had at least one drug side effect considered to be related or possibly related with anti-hypertension medication. Use of anti-hypertension drugs among the elderly had not been relatively safe because there was drug combination potential of causing interaction, light drug side effect and the possibility of contrain-dication due to use of anti-hypertension drugs which was not beneficial to patients’ clinical condition so that monitoring to use of anti-hypertension drugs was needed.

Abstract

The market of skin lightening product grows very fast. This is especially true in almost all Asian countries. Martha Tilaar Innovation Center has found natural ingredients which function as anti oxidant and anti tyrosinase in vitro. These ingre-dients are extracted from Indonesian plants namely Lansium domesticum (LE)*) and Phyllantus niruri (PE) *). Several aspects should be considered to utilize these ingredients in cosmetic, such as, the stability of the plant materials and their safety and efficacy on human skin. The objective of this research is to study whether these ingredients in cosmetic preparations have skin lightening and skin moisturizing effects clinically. This article will describe the safety and efficacy evaluation on natu-ral ingredients extracted with ethanol from the above Indonesian plants. The safety evaluation is conducted clinically using Repeated Opened Patch Test (ROPT) and followed by Single Closed Patch Test (SCPT) in more than 50 selected healthy volun-teers. Their irritation properties on mucous membrane were evaluated using HETCAM method. Then, their efficacies on lightening and moisturizing were conducted in more than 26 volunteers, using Mexameter MX 16 and Corneometer CM 820. These results revealed that both extracts are safe on human skin and mucous membrane and can function as skin lightening agent. Lansium domesticum extract also has skin moisturizing effect.

Abstract

A research on the antibacterial activity of n-hexane extract and the ethyl acetate Garcinia rigida Miq. Bark against Salmonella typhosa ATCC 14028, Staphylo-coccus aureus ATCC 29213 dan Bacillus subtilis ATCC 6633 has been carried out. The research was included the determination of the growth inhibition zona with the cylinder diffusion method and the minimum inhibitory concentration with the petri dish dilution method. The result of this study showed that the n-hexane extract of Garcinia rigida Miq.bark did not give the growth inhibition zona to Salmonella typhosa ATCC 14028, Staphylococcus aureus ATCC 29213 and Bacillus subtilis ATCC 6633, but gave the minimum inhibitory concentration at 500 mg/ml for Sal-monella typhosa ATCC 14028, 250 mg/ml for Staphylococcus aureus ATCC 29213 and 125 mg/ml for Bacillus subtilis ATCC 6633. Whereas the ethyl acetate extract of Garcinia rigida Miq. bark gave the growth inhibition zona of concentra-tion 500, 250 and 125 mg/ml with average diameter to Salmonella typhosa ATCC 14028 were about 11.15, 9.05, 7.55 mm, to Staphylococcus aureus ATCC 29213 were about 14.25, 11.10, 8.95 mm and to Bacillus subtilis ATCC 6633 were about 20.97, 15.00, 10.07 mm. The minimum inhibitory concentration to ethyl acetate extract to Salmonella typhosa ATCC 14028, Staphylococcus aureus ATCC 29213 and Bacillus subtilis ATCC 6633 were about 250, 62,5 and 31,25 mg/ml respec-tively. As a conclusion, the ethyl acetate extract of Garcinia rigida Miq. bark had more better antibacterial activity than the n-hexane extract of Garcinia rigida Miq. Bark.

Abstract

Red fruit oil (Pandanus conoideus) is a natural product typicaly from Papua used as a medication for some disease, as a food supplement, and for beauty skin care. But this advantages are not supported by an appropriate dosage form. Oils are difficult to dissolve in GIT and difficult to penetrate the human skin, slower the absorption then. Microemulsion is a dispersion system which can help to solve the problems by enhanc-ing the oil solubility in GIT and the oil penetration through the skin. The objective of this study is to formulate a clear and stabile microemulsion. Microemulsion will be physically evaluated for 2 months. The material composition is 5% glycerin and 15% sorbitol as the cosolvent, and 20% , 30%, and 40% tween -20 as the surfactants. The result showed that formula of 40% tween-20 gave a good microemulsion which is physically stable as long as 2 months stored at room temperature.

Abstract

Vaccines have been developed for a range of different infectious diseases. The complex-ity of microbial infections requires novel approaches to vaccine design. The first-generation of vaccines were live attenuated pathogens. Because of safety concerns, the second-generation of vaccines, chemically or physically inactivated pathogens were later developed. Purified or synthetic proteins represent a third generation, and recent advances in molecular biology and genetic engineering have led to the development of the fourth vaccine generation, which includes DNA and virus vector-based vaccines. This review discusses on the genetic elements and construction of DNA vaccines, comparison of DNA vaccines and conventional vaccines, the benefits and limitations of DNA vaccines, and the advances of genetic vaccine development over the last decade.

Abstract

Leaves of tamarind (Tamarindus indica L.) has been traditionally used to treat some diseases such as: constipation, dyspepsia, flatulence and urinary tract infection. The leaves extract also reported has antibacterial and antidiabetic activities. It is essential to determine its specific and non specific parameters in assesing the quality and safety. The samples collected from three different perferctures. Grounded samples were macerated with ethanol. The extracts were subjected to determine its characteristic parameters. Parameter of quality for the extracts consisted of water content, ethanol-extractive, water-extractive, loss on drying, total ash, acid-insoluble ash, solvent residue and heavy metal content. Phytochemical identification showed that the ex-tract contains flanovoid, tannin, glycosides, and saponin. TLC chromatogram using chloroform-methanol-water (80:12:2) as mobile phase, exhibited 4 spots after sprayed with iron(III) chloride. Total phenolic was determined pectrophotometrically using Folin Ciocalteu reagent, gave 0.35-8.24% total phenolic as gallic acid.

Abstract

Urinary tract infections (UTI) was an inflammatory response of the urethra when attacked by microorganisms. This disease can happen to people at all ages beginning with asymptomatic to moderately symptomatic infections. Thus far, antibiotic treat-ment was the best for curing the UTI although the chances for being resistance to the antibiotics were also high. Resistance to gram negative bacteri,a in particular, often occur with the antibiotics treatment. Our study at the Faculty of Medicine, University of Indonesia (UI), Bacterial isolation from patient urine sample was performed in the microbiology laboratory of UI. A total of 50 urine samples were collected from X patients volunteered in our study for bacterial isolation, however, only 23 bacterial isolates were successfully obtained. Study was carried out to moni-tor the susceptibility of bacterial isolate towards several types of antibiotics (ofloksazim, amoxyicillin, fosfomisin and sefepim) using Cakram disffusion method. Study re-sults demonstrated that susceptibility of oflokazim to both gram positive and nega-tive bacteria was very low while amoxycillin showed desecding trend of efficacy towards all types of bacteria. Fosfomisin and sefepim, on the other hand, demon-strated strong susceptibility to both gram positive and negative bacteria found in the isolates. Furthermore, it is very interesting to observe ofloksazim was resistant to gram postivie and negative bacteria. Collectively, these research findings strongly illustrated the susceptibility patten and resistance scale of baterial isolates towards various antibiotic tested in the study.

Abstract

Since policy sector pharmaceutical affairs 1993 by government about deregulations sector community pharmacy, competition business community pharmacy in Jakarta was very strict. For win something this competitions, Kimia Farma community pharmacy as the biggest community pharmacy network in Indonesia have propagan-dized vision be the foremost business networking community pharmacy in Indonesia and mission first-rate services for customers with customer satisfaction. The objec-tive of this research is detect satisfaction level of service quality of Kimia Farma community pharmacy Jakarta, in particular Kimia Farma community pharmacy -1 Kemayoran, Kimia Farma community pharmacy-48 Matraman, Kimia Farma com-munity pharmacy-147 Duren Sawit. Beside This research also objective for knowed vision, mission of Kimia Farma community pharmacy was reached. This research using survey method with quisionaire Single Cross Sectional Study approaches. Research using Servqual (Service Quality) model was discovered by Parasuraman, Zeithaml and Berry use five dimensions of service quality, that was tangible, reli-ability, responsiveness, assurance, and emphaty. Satisfaction level measured with Gap analysis that was difference expectation before customer receiving the service and perception after that; and Cartesian Diagram analysis, that was mapping atribut service quality on Cartesian Diagram. Analysis difference on customer satisfaction within community pharmacy used analysis varian (Anova). Result: dimension that highest satisfaction level was emphaty (gap -0.37 or satisfactions levels 91.88 %); attribute that highest satisfaction level was well designed interior/exterior building (gap -0.14 or satisfaction levels 96.4 %). Mapping attribute on Cartesian Diagram majority on quadrat B, means service quality Kimia Farma community pharmacy was sufficient. Based on hypothesis test, was not difference significant satisfactions level Kimia Farma community pharmacy was research (alpha > 0.05). Conclusions: generally satisfactions level Kimia Farma community pharmacy that researched nearly satisfy, with satisfaction levels within community pharmacy was not significant difference.

Abstract

The administration of antibiotic at RSUD dr. H. Abdul Moeloek Lampung is based on sensitivity test result and empirical therapy on diabetic ulcer. Therefore, the objective of the research is to analyze the effectivity of various antibiotics adminis-tration based on the sensitivity test result on clinical outcome reached by consider-ing influenced factors. On the other hand, total evaluation of the resistances of empirical antibiotic used by considering germ and sensitivity test feature was done. The research was conducted at dr. H. Abdul Moeloek Regional Hospital from April - July 2008 by applying cross sectional design retrospectively. Data was taken by the diabetic ulcer patients medical record which were treated from January 1st 2005 to Mei 30, 2009. Requirement of patient inclution are patient with diabetic ulcer infection, receiving antibiotic recipe, antibiotic given is based on sensitivity test result, patient was cured until getting the doctor approval to go home. The research which involved 98 patient inclution showed that clinical outcome of diabetic ulcer patient who was administered antibiotic based on sensitivity test result was influ-enced by chronic level of patient and age.

Abstract

Japanese encephalitis virus (JEV) is a neuropathogenic virus commonly caused cen-tral nervous diseases such as meningitis and severe encephalitis. Although vaccine has been developed, no specific and effective drug is available so far. We previously carried out a screening of inhibitor of JEV RNA helicase, an enzyme that essential for virus replication, from Actinomycetes and found that Streptomyces chartreusis produce the inhibitor of JEV RNA helicase. In this study, an extracellular protein which has inhibition activity on ATPase activity of JEV RNA helicase was purified from supernatant of Streptomyces chartreusis culture by ammonium sulfate pre-cipitation and size exclusion chromatography. SDS-PAGE analysis showed a single band with aproximate molecular mass of 11,4 kDa, suggesting that the inhibitor was successfully purified into a single protein.
