Abstract

Hypertension becomes a problem in old age, more than 50% of people with hypertension are over 60 years old. The application of nursing theory in the provision of nursing care needs to be continuously developed. One of the theories of nursing that can be applied to the elderly who have hypertension is the Levine conservation model. Nursing actions are based on four principles: energy conservation, conservation of structural integrity, conservation of personal integrity and conservation of social integrity. The purpose of this study was to identify the effect of Levine's conceptual model-based nursing interventions on the elderly.The design of this study used quasy experiments with pre and post control groups with a large sample of 20 respondents in each group. The sampling technique used is consecutive sampling with independent variables, namely Levine model-based nursing interventions (benson relaxation techniqued, deep breath relaxation techniqued, progressive muscle relaxation technique and health education), Dependent variable in this study were sleep disorders, pain, anxiety and family support. The research instruments used are sleep quality scale (SQS), Numeric rating scale, Hamilton anxiety rating scale and family support scale. The research was carried out at the Budi Sejahtera Banjarbaru Elderly Social Protection and Rehabilitation Institution (PPRSLU). The statistical test used in this study was man whitney test. The test results found that Levine-based interventions had an effect on sleep disorders (p=0.016), but had no effect on pain (p=0.67), anxiety (p=1.00), family support (p=0.63). In elderly with hypertension, nurses need to help the elderly so that the problem of hypertension is resolved and able to fight disability. Levine-based interventions can be applied for the recovery of symptoms of hypertension.

Abstract

The World Health Organization (WHO) states that hyperuricemia sufferers are increasing every year in the world. The Cikeusal Health Center in Serang Regency shows that cases of hyperuricemia are quite high, namely as many as 189 in 2020, increasing in 2021, namely 340, even in 2022 in the January-September period there were 208 cases. One of the pharmacological treatments in the treatment of hyperuricemia is coffee because it contains polyphenolic compounds. Purpose of Writing was to knowing the analysis of uric acid levels in pre-elderly patients in terms of coffee drinking behavior in the working area of the Cikeusal Health Center, Serang Regency, in 2022. Research Methods on this research is a quantitative analytic study with a cross sectional design. The sample in this study were pre-elderly patients who visited the Cikeusal Health Center in Serang Regency in August-October 2022 with a total of 81 respondents using a purposive sampling technique. The research instrument used questionnaires and observation sheets. The data is primary data analyzed using the one way ANOVA test which previously had been carried out the normality test with normal data distribution results. The results of univariate analysis revealed that the average uric acid level was 4.786, coffee drinking behavior was low, medium and high respectively 33.3% with an average of 21.07. The results of the bivariate analysis revealed that there was an effect of coffee drinking behavior on uric acid levels with a p value = 0.021. Coffee drinking behavior affects uric acid levels in pre-elderly patients. It is hoped that the community can carry out routine checks and maintain food intake, one of which is drinking coffee regularly between 1-2 times a day.

Abstract

Marketing mix is a set of strategies used by service organizations, including hospitals, to create value and influence consumer decisions through seven main elements, namely Product, Price, Place, Promotion, People, process, and physical evidence. In the context of healthcare, the implementation of an effective marketing mix is believed to be able to increase satisfaction and retain patients. This study aims to analyze the influence of marketing mix on outpatient retention with electronic word of mouth (e-WOM) as a mediating variable. The research method used is quantitative with a cross sectional approach. Sampling in this study used purposive sampling of 100 respondents. Data analysis was carried out using multiple linear regression as well as the Sobel test to test the influence of mediation. The results showed that Product, Price, Place, Promotion, People, process, physical evidence and patient satisfaction had a significant effect on patient retention with a sig value (0.000<0.05). The conclusion of this study is that there is an influence between the 7P marketing mix and patient satisfaction on outpatient retention with a value (sig 0.000<0.05), and there is no influence between Promotion, Place, and process on patient retention through e-WOM (sig >0.05).

Abstract

One of the most common problems in preschoolers is emotional mental health issues. Emotional mental health disorders are a condition that indicates an individual is experiencing an emotional change that can develop into a pathological state if it continues. Therefore, to prevent the occurrence of more severe mental and emotional problems in preschoolers, early detection of emotional mental health is needed. The delivery of health education is more interesting by using animated videos is one of the effective methods because respondents can see and listen directly, so that information is easier to understand and remember. The results of this research can be the basis for development (penkes) through animated videos and encourage the public to increase education about early detection of emotional mental health in preschoolers. In the long term, this research is expected to increase parental awareness in recognizing the signs of children's mental-emotional disorders and disseminate effective information dissemination and build positive behavioral changes in parenting. This study aims to determine the influence of "emotic" health education on parents' knowledge about early detection of emotional mental health in preschool children in Kandangmas Village, Dawe District, Kudus Regency. This study used a quasi-experimental method with a pre-test post-test control group design, which compared the differences before and after intervention in the intervention and control groups. The study respondents amounted to 50 parents who had preschool-age children, who were selected based on the inclusion criteria and the exclusion criteria that were determined to be willing to be respondents.

Abstract

Malnutrition is a common health problem among older adults and has the potential to increase the risk of falls, one of the leading causes of morbidity and mortality in this age group. This systematic review aims to analyze the relationship between malnutrition and the risk of falls in older adults by synthesizing findings from eight observational studies. Literature searches were conducted in the PubMed, EBSCO, and Cochrane databases until August 2025. Nutritional status in these studies was assessed using tools such as the Mini Nutritional Assessment (MNA), MUST, and BMI, while the risk of falling was measured using instruments such as the Morse Fall Scale, Johns Hopkins Fall Risk Assessment Tool, Timed Up and Go Test (TUG), and fall history. The results showed a consistent and significant association between malnutrition and an increased risk of falling, with odds ratios ranging from 1.42 to 4.94. These findings were reinforced by biological mechanisms such as sarcopenia and micronutrient deficiencies that impair muscle strength, balance, and neuromuscular function. Based on the GRADE assessment, the level of certainty of the evidence was moderate. Overall, this review confirms that malnutrition is an important and modifiable risk factor, and that routine nutritional screening should be an integral part of fall prevention strategies for older adults.

Abstract

The prevalence of Diabetes Mellitus in Indonesia is estimated to reach 21.3 million people by 2030. Treatment with oral antidiabetic drugs carries the risk of myocardial infarction and congestive heart failure, while modern therapy of NIDDM has adverse side effects. WHO recommends researching traditional methods, especially plants with hypoglycemic effects. Efficacy claims in herbal medicines, especially herbal medicines, are rarely reported, so it is necessary to test antidiabetic activity to ensure their use. To determine the antidiabetic activity of herbal medicine product X on blood sugar levels in aloxane-induced mice (Mus Musculus), determine the most effective dose, and determine the decrease in GDP after the administration of herbal medicine X on days 7 and 14. Experimental research uses male mice (Mus Musculus) as test animals. Blood glucose levels were measured before and after treatment, with a negative control of CMC-Na of 0.5% and a positive control of glibenclamide of 0.65mg/Kg BB. The optimal dose of herbal product X is determined by a ratio of doses of 52 mg/KgBB, 104 mg/KgBB, and 156 mg/KgBB for 7 to 14 days. Herbal Medicine X at doses of 52mg/KgBB, 104mg/KgBB, and 156mg/KgBB succeeded in reducing aloxan-induced blood sugar levels in mice. The dose of 156mg/KgBB was found to be the optimal dose based on the Mann-Whitney test with Sig<0.05. The most significant decline on day 14 was based on the Kruskal Wallis test with Sig<0.05. Herbal Medicine X was effective in lowering blood sugar levels in mice, with an optimal dose of 156mg/Kg BB and a significant decrease on day 14.

Abstract

Tuberculosis (TB) remains a public health problem, especially in remote areas with limited resources and access to health services. The effectiveness of the TB surveillance system is crucial for successful disease control. This study aimed to assess the effectiveness of the TB surveillance system in Kabawo District, Muna Regency, using a mixed-methods approach. This study employed a mixed-methods design with a concurrent triangulation strategy. Quantitative data were analyzed univariately using SPSS to assess the components of the surveillance system, while qualitative data were obtained through in-depth interviews and analyzed descriptively. The quantitative analysis showed that the availability and capacity of human resources had an average score of 3.28, the TB case reporting system 3.83, the TB treatment success process 3.52, and the availability of facilities and infrastructure 2.89. The TB case detection process had an average score of 0.87. The overall effectiveness of the TB surveillance system was categorized as effective with an average score of 0.85. Qualitative findings revealed barriers such as social stigma, trust in traditional medicine, and limited geographic access that impacted the implementation of TB surveillance. The TB surveillance system in Kabawo District is considered effective, but still requires strengthening in the aspects of active case detection, utilization of facilities and infrastructure, as well as socio-cultural and geographic access-based interventions to increase effectiveness in a sustainable manner.

Abstract

Medical records are essential for clinical decision-making, continuity of care, legal compliance, and accreditation. Despite their importance, incomplete documentation remains a significant challenge in primary health care, particularly in settings that rely on manual record-keeping. This study assessed the completeness of outpatient medical records at the general polyclinic of Klinik UIN Syarif Hidayatullah Jakarta and examined patterns in outpatient service utilisation, including patient demographics, insurance coverage, and referral trends. A mixed-methods design was employed, integrating quantitative analysis of 399 outpatient medical records from January to February 2024 with qualitative data from observations and interviews. The quantitative component evaluated the completeness of SOAP documentation, patient demographics, service utilisation, and referral rates. The qualitative component investigated workflow challenges, clinician perspectives, and factors influencing documentation quality. In January, 201 outpatient visits were recorded, and in February, 198 visits occurred. Each month, more than 64% of patients were female. BPJS beneficiaries accounted for 842% of all visits, indicating they were the predominant users of the service. Referrals increased from 21 in January to 41 in February, for a total of 62. SOAP documentation was largely complete in January but declined in February, with most omissions found in the Assessment and Objective sections. Fourteen incomplete SOAP components were identified over two months, with 12 occurring in February. Although service utilisation remained stable, documentation quality varied substantially. The frequent incompleteness of the Objective and Assessment components underscores the need for improved workflow organisation, enhanced clinician training, and the adoption of electronic medical records. Regular audits and standardisation of SOAP documentation are recommended to strengthen compliance and support accreditation.

Abstract

Idiopathic pulmonary fibrosis (IPF) is a chronic interstitial lung disease characterized by extracellular matrix accumulation, often mimicking pulmonary malignancy in clinical and radiological aspects. Early and accurate diagnosis remains a challenge. Objective: To report a case of IPF presenting with hemoptysis and radiological features suggestive of malignancy, highlighting diagnostic complexities. Methods: This qualitative case study involved detailed clinical, radiological, and histopathological examination of a 60-year-old female patient. Data were collected through medical records, CT imaging, biopsies, and interviews. Thematic content analysis ensured rigorous interpretation. Results: Radiologic findings indicated suspicious masses bilaterally, yet histopathology confirmed pulmonary fibrosis without malignancy. This underscores the overlap in presentation and diagnostic ambiguity between IPF and lung cancer. Conclusion: Multimodal diagnostic approaches combining imaging and biopsy are essential to distinguish IPF from malignancy and to guide treatment. Increased clinician awareness and multidisciplinary collaboration enhance diagnostic accuracy and prognosis. Further research with larger samples is recommended to improve non-invasive diagnostic tools.

Abstract

Poor sleep quality is a common problem in patients with diabetes mellitus which can negatively impact metabolic control and reduce the patient's quality of life. Therefore, this study is important to find alternative non-pharmacological therapies to improve the sleep quality of diabetic patients. The main purpose of this study is to determine the effect of healing touch-dhikr therapy on the sleep quality of patients with diabetes mellitus in the Kaliwungu Health Center Working Area. This study uses a quantitative method, namely quasi experimental with a pretest-posttest with control group design. The sample consisted of 78 respondents who were selected through purposive sampling and divided into two groups: the intervention group (n = 39) and the control group (n = 39). The healing touch-dhikr intervention was given for seven days with a duration of 15–30 minutes per session. The data collection instrument used the Pittsburgh Sleep Quality Index (PSQI). Data analysis was carried out by dependent t-test and independent t-test test. The results showed a significant decrease in PSQI scores in both groups (p<0.001). The average sleep quality score of the intervention group decreased from 11.92 to 6.00 (difference of 5.92), while in the control group it decreased from 12.10 to 10.05 (difference of 2.05). Although both showed significant changes, the larger decline in the intervention group showed that healing touch–dhikr therapy was more clinically and statistically effective in improving the sleep quality of patients with diabetes mellitus. This is supported by an independent t-test showing a significant difference between the two groups (p< 0.001), which indicates the effectiveness of the intervention. Thus, it can be concluded that healing touch-dhikr therapy has a significant and clinically effective effect in improving the quality of sleep in patients with diabetes mellitus. This research supports the application of spiritual-based non-pharmacological interventions in nursing practice as a holistic approach that pays attention to the physical, psychological, and spiritual aspects of patients.
