Abstract

Anemia during pregnancy remains one of the most common nutritional and public health problems worldwide and is associated with increased maternal and neonatal morbidity. It is clinically defined as a hemoglobin (Hb) level below 11 g/dL during pregnancy, indicating an insufficient number of red blood cells to meet the oxygen demands of both the mother and the fetus. Based on hemoglobin concentration, anemia is classified into three categories: mild anemia (Hb 10.0–10.9 g/dL), moderate anemia (Hb 7.0–9.9 g/dL), and severe anemia (Hb <7.0 g/dL). Pregnant women with anemia are at greater risk of fatigue, dizziness, infection, postpartum hemorrhage, preterm birth, low birth weight, and other adverse pregnancy outcomes. According to the World Health Organization, anemia continues to affect a substantial proportion of pregnant women globally, making early detection and appropriate management essential. Continuity of Midwifery Care (CoMC) is a comprehensive, woman-centered approach that provides continuous and integrated care throughout pregnancy, childbirth, the postpartum period, newborn care, and family planning. This model promotes early identification of maternal health problems, timely interventions, and ongoing health education to improve maternal and neonatal outcomes. Therefore, implementing Continuity of Midwifery Care for pregnant women with mild anemia is expected to optimize maternal health status, reduce pregnancy-related complications, and ensure safe and high-quality maternity care.

Abstract

Background: Prolonged preoperative fasting can trigger surgical stress. Preoperative oral carbohydrate loading may reduce insulin resistance, improve patient comfort, and help preserve postoperative muscle function. Objective: This study aimed to compare changes in cortisol and blood glucose levels between patients receiving oral glucose 2 hours before surgery and those receiving placebo during laparoscopic cholecystectomy. Methods: This study used a single-blind randomized clinical trial involving patients undergoing elective laparoscopic cholecystectomy at Wahidin Sudirohusodo Hospital and its network hospitals in Makassar. Results: Mean cortisol levels at T2 were lower in the glucose group than in the placebo group, at 4.51 mcg/dl and 10.19 mcg/dl, respectively (p < 0.05). Blood glucose levels at T1 and T2 were also lower in the glucose group than in the placebo group (p < 0.05). A significant decrease in cortisol levels from T0 to T2 was also observed (p < 0.05). Conclusion: Oral glucose administration 2 hours before surgery was effective in reducing cortisol levels and maintaining blood glucose stability up to 24 hours postoperatively compared with placebo.

Abstract

Cardiac arrest causes global ischemia followed by reperfusion injury, resulting in diffuse myocardial damage and cardiac troponin release. This condition is commonly associated with type 2 myocardial injury caused by an imbalance between myocardial oxygen supply and demand rather than acute coronary occlusion. This article summarizes evidence on the cellular and molecular mechanisms of type 2 myocardial injury and troponin release after cardiac arrest. A narrative literature review was conducted using articles published between 2015 and 2025 from PubMed and Google Scholar. The findings indicate that ischemia causes ATP depletion, ionic imbalance, intracellular calcium overload, and mitochondrial dysfunction. Reperfusion further aggravates myocardial injury through reactive oxygen species generation, inflammasome activation, pro-inflammatory cytokine release, apoptosis, and pyroptosis. These processes impair cardiomyocyte membrane integrity and cause structural damage, leading to troponin release. Troponin elevation may reflect reversible membrane permeability changes or irreversible cardiomyocyte injury. Therefore, high-sensitivity cardiac troponin levels should be interpreted according to pathophysiological mechanisms and clinical context to distinguish type 2 myocardial injury from myocardial infarction caused by acute coronary occlusion.

Abstract

Patient satisfaction is an important indicator of hospital service quality. Data from Nganjuk Regional General Hospital indicated a decline in inpatient visits in 2025, an increase in patient complaints, and relatively low scores for several service elements, particularly service completion time, facilities, and staff behavior. This study aimed to analyze the effects of service quality, service procedures, and tariffs or healthcare financing on inpatient satisfaction. This quantitative study used an observational cross-sectional design. The population consisted of 195 inpatients, and 132 respondents were selected through proportional stratified random sampling based on inpatient class. Data were collected using a Likert-scale questionnaire and analyzed using descriptive statistics, Pearson correlation, and multiple linear regression. The mean scores were 3.959 for service quality, 4.054 for service procedures, 3.964 for tariffs or financing, and 3.951 for patient satisfaction. Service quality had a positive and significant effect on patient satisfaction (B=0.629; β=0.527; p<0.001). Service procedures also had a positive and significant effect (B=0.245; β=0.225; p=0.001), as did tariffs or financing (B=0.161; β=0.150; p=0.022). The overall regression model was significant (F=43.563; p<0.001), with R²=0.505. MService quality was the dominant factor because it had the largest standardized beta coefficient. Improving inpatient satisfaction should prioritize physical facilities, procedural consistency, staff communication, and transparent financing information for both self-paying and national health insurance patients.

Abstract

Risk management is a preventive measure for service-sector companies, particularly hospitals, which are vulnerable to errors and complaints. Mapping risks according to work units is essential to minimize accidents or errors affecting patients, visitors, and staff. This study aims to examine and identify the implementation of Occupational Health and Safety (OHS) risk management at Hospital X in Jakarta. A quantitative cross-sectional design was employed to describe OHS risk management implementation across units and workplaces by identifying risks, analyzing hazards, and determining appropriate control measures. The study examined hazards that could cause accidents among employees, visitors, and patients, as well as efforts to minimize incidents and accidents. Statistical analysis showed that all examined variables significantly influenced OHS risk management implementation in 2025. The p-values were 0.028 for Knowledge, 0.001 for Attitude, 0.000 for Practice/Behavior, 0.000 for Organizational Structure and Management Support, 0.018 for Risk Identification and Assessment, and 0.009 for Training, Competence, and Preparedness. All variables had p-values ≤ 0.05. The study concludes that Knowledge, Attitude, Practice/Behavior, Organizational Structure and Management Support, Risk Identification and Assessment, and Training, Competence, and Preparedness significantly influence OHS risk management implementation at Hospital X in 2025.

Abstract

Nurses' performance is one of the primary indicators of hospital service quality and is influenced by various physiological and psychological factors. Biometric indicators, including blood pressure, heart rate, and oxygen saturation, reflect nurses' physical health conditions that may affect work stress and service performance. This study aimed to analyze the relationship between biometric indicators and nurses' service performance through work stress as an intervening variable at Teuku Umar Aceh Jaya Regional General Hospital. This study employed a quantitative research design with a cross-sectional approach. The study population consisted of all nurses at Teuku Umar Regional General Hospital, Aceh Jaya, with a total sample of 137 respondents selected using a purposive sampling technique.Results: showed that most respondents were female (77.4%), worked morning shifts (44.5%), experienced normal levels of work stress (88.3%), and demonstrated good service performance (98.5%). The mean systolic blood pressure was 114.77 ± 9.24 mmHg, diastolic blood pressure was 77.29 ± 7.43 mmHg, heart rate was 81.91 ± 11.53 beats per minute, and the mean oxygen saturation was 97.45%, with a range of 95–100%. The findings indicated that biometric indicators were associated with work stress, which subsequently influenced nurses' service performance. Nurses' biometric conditions contribute to work stress, which subsequently affects service performance. Therefore, regular monitoring of nurses' physical health is recommended to improve the quality of nursing services in hospitals.

Abstract

The Occupational Health and Safety Management System (SMK3) is a crucial element in ensuring a safe and healthy work environment, particularly at PT X, where workers face high health and safety risks. Regulatory compliance, employee training, risk management, and management support are key factors in the successful implementation of SMK3. The use of adequate technology and infrastructure also supports the system's effectiveness. Furthermore, a strong safety culture and periodic evaluations contribute significantly to improving workplace safety. This study aims to adopt a comprehensive approach and provide ongoing support for SMK3 implementation at PT X. A cross-sectional method employing both qualitative and quantitative research designs was used. The sample consists of 164 respondents at PT X. The study results indicate the following: 1) Project commitments and policies were established directly based on company standards. 2) OHS planning for PT X construction projects involved hazard identification, risk assessment, and the determination of risk controls; this planning began with the creation of a safety plan based on initial assessments as well as relevant OHS laws and requirements. 3) Regarding implementation, workers and new employees received safety inductions and health checks. Personal Protective Equipment (PPE) was fully provided according to potential risks, and workers were required to use it while working in the field; routine SHE talks and toolbox meetings were conducted to facilitate OHS communication among workers. 4) OHS monitoring and evaluation involved periodic inspections to identify potential hazards arising from workplace conditions, equipment, materials, and worker actions; this process facilitated necessary corrective and preventive measures and ensured the provision of emergency equipment including emergency warning systems that were regularly inspected, tested, and maintained in accordance with regulations. 5) Review and improvement were carried out by compiling accident risk statistics and audit findings from the project, followed by addressing any identified deviations. Statistical analysis showed significant overall results (p ≤ 0.05), indicating a relationship between OHS implementation specifically regarding knowledge, attitude, training, and promotion and the occurrence of workplace accidents. The analysis results indicate that the variable most dominantly associated with the occurrence of workplace accidents is worker attitude, with an OR of 22.392; this means that workers with a negative attitude are 22 times more likely to experience a workplace accident compared to those with a positive attitude (95% CI: 4.091–122.559).

Abstract

Sleep is one form of adaptation of babies to their environment. Shortly after birth, babies sleep for 16-20 hours a day. At 2 months of age, babies begin to sleep more at night than during the day. At 3 months of age, newborns will spend about 15-17 hours sleeping, with 8 hours for naps and 9 hours for nighttime sleep. As the baby gets older, their sleep hours also decrease. At 3-6 months of age, the number of naps decreases, approximately 3 times. The total sleep time of babies aged 0-6 months ranges from 13-15 hours/day. At 6 months of age, their sleep patterns begin to appear similar to adults. This study aims to determine the effectiveness of the use of swings and massage on the quality of infant sleep at the Rantepao Community Health Center. The research method used a one-group pretest-posttest design. The population of all infants aged 0-6 months was 30 people. Statistical tests used a one-sample t test. The results showed that the sleep quality of babies aged 0-6 months who were rocked was predominantly in the good category, namely (53%) and those with poor sleep quality (27%). The sleep quality of babies aged 0-6 months after baby massage was predominantly in the good category (54%) and those with poor sleep quality (13%). Swinging and baby massage are effective in improving the quality of baby sleep with a p value of 0.000 <0.05. So it is necessary to develop promotions and education to the community, especially parents, to improve the quality of baby sleep.

Abstract

The number of women experiencing menopause worldwide is expected to increase to 1.2 billion by 2030. By 2025, the number of premenopausal and menopausal women is expected to reach 1.2 billion worldwide, with an average age of 40 to 50 years. Menopausal women experience physical and psychological changes. Common psychological changes during menopause include depression, fatigue, lack of energy, and insomnia. Lack of sleep can negatively impact physical and mental health and can sometimes lead to cognitive problems. To find out the application of yoga to improve sleep quality in menopausal women. The method used for nursing care for female clients with disturbed sleep quality through the application of yoga to improve sleep quality in menopausal women is to use one group pretest and posttest, namely the group is given treatment after the pretest and is observed again through the posttest and there is no comparison group. The results of applying yoga for 3 days on menopausal women who experience sleep pattern disorders are that they experience changes, although complaints of sleep pattern disorders are still felt, but there are changes compared to before doing yoga. Practicing yoga can improve the quality of sleep in menopausal women if done regularly.

Abstract

Invasive therapy is one of the procedures that causes fear and anxiety in preschool children during hospitalization. This anxiety can lead children to refuse medical procedures, thereby hindering the healing process, worsening their condition, and prolonging the length of hospital stay. Nursing interventions that can be implemented to reduce children’s anxiety include play therapy, one of which is storytelling. Storytelling play therapy is a play activity conducted through narration, utilizing the power of stories to convey messages, provide entertainment, and help children understand and process their experiences, thereby reducing anxiety during invasive procedures such as intravenous (IV) insertion. Applying hand puppet storytelling to preschool children with anxiety using an evidence-based practice approach. The design used in this Final Nursing Scientific Project was a case study with an intervention in the form of storytelling using hand puppet media for 10 minutes. The number of respondents was three children. The intervention was conducted on October 4, October 5, and November 29, 2025, in the Theresia Ward at Charitas Hospital Palembang. After the implementation of storytelling play therapy using hand puppets, changes in the children’s anxiety levels were observed. Child E’s anxiety decreased from a scale of 5 to 3, Child R’s from 5 to 3, and Child N’s from 4 to 2. There was a reduction in anxiety levels among all three respondents after receiving the storytelling intervention. The application of evidence-based storytelling play therapy using hand puppets can be utilized to reduce anxiety levels in preschool children who are about to undergo invasive therapy.
