Abstract

Background: Spontaneous abortion is a traumatic experience that not only has physical consequences but also affects the mother's psychological and emotional well-being. Emotional responses, such as sadness, anxiety, anger, and loss of meaning in life, are often overlooked in health care. Objective: This study aims to explore the emotional experiences of mothers who experience spontaneous abortion and understand how they cope with and interpret the event. Methods: This study used a qualitative approach with a phenomenological design. Six mothers who experienced spontaneous abortion within the past year participated. Data were collected through in-depth interviews and analyzed using thematic analysis techniques. Results: Four main themes emerged in this study: (1) a profound sense of loss, (2) feelings of guilt and self-blame, (3) limited emotional support from the surrounding environment, and (4) the process of acceptance and spiritual meaning. The majority of participants indicated a need for psychological support after the event. Conclusion: Spontaneous abortion causes significant emotional distress. Empathy-based interventions and structured psychological support within health care are needed to help mothers cope with the emotional impact after the event.

Abstract

Background: Medical errors are a serious challenge in the healthcare system that can negatively impact patient safety and the hospital's reputation. The implementation of risk management is a strategic approach to identifying, analyzing, and controlling potential medical errors. Objective: To explain and analyze the role of risk management in preventing medical errors in hospitals. Methods: This study used a descriptive qualitative method with a case study approach in a type B hospital in Indonesia. Data were obtained through in-depth interviews with the risk management team, observations, and a review of internal hospital documents. Data analysis used triangulation techniques. Results: The results indicate that risk management in the hospital has been implemented in the form of clinical risk identification, incident reporting, root cause analysis, and the implementation of prevention policies. However, challenges remain, including a weak reporting culture and a lack of regular training for healthcare workers. Conclusion: Risk management plays a critical role in preventing medical errors. Strengthening a patient safety culture, routine training, and a safe and non-punitive incident reporting system are needed to encourage continuous improvement.

Abstract

Background: Spontaneous abortion is a common pregnancy complication in the first trimester. The causes are multifactorial, one of which is maternal nutritional status. Poor nutrition can affect embryo implantation and growth, increasing the risk of miscarriage. Objective: To determine the relationship between maternal nutritional status and the incidence of spontaneous abortion in the first trimester. Methods: This is an observational analytical study with a case-control approach. A purposive sample of 100 respondents was selected, consisting of 50 cases of spontaneous abortion and 50 women without abortion in the first trimester. Nutritional status was measured based on BMI and MUAC. Data were analyzed using the chi-square test with a significance level of 0.05. Results: There was a significant relationship between maternal nutritional status and the incidence of spontaneous abortion (p = 0.004). Mothers with poor nutritional status had a 3.2-fold higher risk of spontaneous abortion compared to mothers with normal nutritional status. Conclusion: Poor maternal nutritional status is positively correlated with an increased incidence of spontaneous abortion.

Abstract

Health promotion campaigns are an important strategy for increasing public awareness, knowledge, and behavioral change toward a healthy lifestyle. This study aims to evaluate the effectiveness of health promotion campaigns in increasing knowledge and changing healthy behaviors among the public. A quantitative method was used with a pretest-posttest design without a control group. A total of 150 respondents from three sub-districts were selected purposively. The results showed a significant increase in health knowledge (p<0.05) and behavioral changes such as handwashing, regular exercise, and nutritious food consumption. These findings indicate that health promotion campaigns are effective as a public education tool. It is recommended that campaigns be conducted sustainably and involve various media and local cultural approaches.

Abstract

Background: Evidence-Based Nursing Practice (EBNP) is a nursing care approach that integrates current research evidence, clinical experience, and patient preferences to improve the quality of care. Its implementation in hospitals is believed to improve service quality and patient satisfaction, but still faces various challenges in the field. Objective: To evaluate the implementation of an evidence-based nursing model in improving the quality of nursing care in hospitals. Methods: This study used a descriptive evaluative design with a mixed methods approach. The sample consisted of 50 nurses and 100 inpatients at Hospital X, selected using purposive sampling. Instruments included a nurse knowledge and attitude questionnaire, a nursing practice observation checklist, a patient satisfaction questionnaire, and in-depth interviews. Quantitative data were analyzed using a paired t-test, while qualitative data were analyzed using thematic analysis. Results: The average nurse knowledge score increased from 65.4 to 84.6 (p < 0.001). Compliance with SOPs increased from 74% to 93%. Patient satisfaction increased from 78.2 to 90.1 (p < 0.001). Supporting factors for implementation included management support, training, and practice guidelines, while barriers included time constraints, high workload, and limited access to journals. Conclusion: EBNP implementation significantly improved knowledge, skills, procedural compliance, and patient satisfaction. It is recommended that hospitals provide ongoing training, facilitate access to scientific resources, and establish EBNP support teams.

Abstract

Patient rights are part of human rights guaranteed by the constitution and regulated in various laws and regulations in Indonesia. This study aims to analyze the legal framework governing patients' rights to access healthcare services, including legal protection, enforcement mechanisms, and barriers to implementation in the field. The research method used is normative legal research with a statute approach and qualitative analysis of secondary data in the form of regulations, legal literature, and previous research findings. The results indicate that patient rights are guaranteed in Law Number 36 of 2009 concerning Health, Law Number 44 of 2009 concerning Hospitals, and Minister of Health Regulation Number 4 of 2018 concerning Hospital Obligations and Patient Rights. However, obstacles persist, such as limited access in remote areas, lack of public understanding, and suboptimal complaint mechanisms and sanctions for violators. In conclusion, strengthening regulations, oversight, and public education are necessary to ensure effective fulfillment of patient rights.

Abstract

Traditional leaders play a crucial role in the social structure of remote communities, particularly in influencing social behavior and norms, including health behaviors. This study aims to analyze the role of traditional leaders in community health promotion in remote areas. The method used was a descriptive qualitative approach using in-depth interviews with traditional leaders, health workers, and the community in District X. The results indicate that traditional leaders act as facilitators, messengers, and guardians of cultural values, increasing community awareness and acceptance of health programs. The active involvement of traditional leaders has been shown to increase the effectiveness of health promotion, particularly in immunization programs, environmental sanitation, and maternal and child care. It is concluded that integrating the role of traditional leaders into health promotion strategies can be an effective approach to improving community health status in remote areas.

Abstract

Background: Maternal emergencies are life-threatening conditions during pregnancy, childbirth, and the postpartum period. Prompt and appropriate midwifery intervention can reduce maternal morbidity and mortality. Objective: To analyze the effectiveness of various midwifery interventions in managing maternal emergencies based on a literature review. Methods: This study used a literature review method by reviewing scientific articles published between 2015 and 2025. Searches were conducted through the PubMed, ScienceDirect, Google Scholar, and DOAJ databases using the keywords "maternal emergency," "midwifery care," and "emergency obstetric intervention." Inclusion criteria included studies discussing midwifery interventions in maternal emergencies, published in English or Indonesian, and available in full-text. Results: Of the 25 articles reviewed, midwifery interventions such as postpartum hemorrhage management with active management of the third stage of labor, the use of magnesium sulfate in preeclampsia/eclampsia, fluid resuscitation in shock, and the partograph technique for labor monitoring were proven effective in reducing complications. Rapid response, clinical skills, and coordination with the medical team were key factors for success. Conclusion: Evidence-based midwifery interventions, implemented quickly and in a coordinated manner, play a significant role in reducing maternal morbidity and mortality. Routine training and capacity building for midwives are needed to strengthen the response to maternal emergencies.

Abstract

Background: Postoperative pain is a major problem for patients undergoing surgery. Suboptimal pain management can lead to physiological and psychological complications, delay recovery, and reduce the patient's quality of life. One method of pain management is the use of regional anesthesia, which has the advantage of reducing pain intensity and reducing the need for additional analgesics. Objective: This study aimed to determine the effectiveness of regional anesthesia in reducing postoperative pain compared to general anesthesia. Methods: This study used a quasi-experimental design with a post-test only control group design. A sample of 60 elective surgery patients was divided into two groups: regional anesthesia (n=30) and general anesthesia (n=30). Pain intensity was measured using a Visual Analog Scale (VAS) at 6 and 12 hours postoperatively. Data were analyzed using an independent t-test. Results: The study showed that the average VAS score in the regional anesthesia group was lower than that in the general anesthesia group, both at 6 hours (2.8 ±1.1 vs. 5.4 ±1.5) and 12 hours (3.1 ±1.2 vs. 5.9 ±1.7) postoperatively. A t-test showed a significant difference (p < 0.05) between the two groups. Conclusion: Regional anesthesia has been shown to be effective in reducing postoperative pain intensity. This method can be considered as a primary strategy for perioperative pain management to accelerate patient recovery.

Abstract

Background: Pre-anesthesia anxiety is a common phenomenon that can affect a patient's physiological and psychological state and impact the perioperative process. Identifying factors influencing patient anxiety is important for planning educational interventions and psychosocial support. Objective: This study aimed to analyze factors influencing patient anxiety before anesthesia, including demographic factors, previous experience, family support, and preoperative information. Methods: The study used a descriptive analytical design with a cross-sectional approach. The sample consisted of 100 elective patients undergoing general or regional anesthesia at Hospital X, selected using purposive sampling. The instruments used were a demographic questionnaire and the Hamilton Anxiety Rating Scale (HARS). Data were analyzed using the chi-square test and logistic regression. Results: The results showed that patient anxiety levels were associated with young age (p = 0.032), low education (p = 0.018), first-time surgical experience (p = 0.001), minimal family support (p = 0.005), and lack of preoperative information (p = 0.001). The dominant factor influencing anxiety was a lack of preoperative information, with an OR of 3.5. Conclusion: Patient anxiety levels before anesthesia are influenced by age, education, surgical experience, family support, and preoperative information. Preoperative educational interventions and family support are important strategies for reducing patient anxiety.
