Abstract

Background: Tuberculosis (TB) is a human disease caused by Mycobacterium tuberculosis. The disease primarily attacks the lungs, making pulmonary disease the most common symptom. Other organ systems frequently affected include the respiratory system, gastrointestinal (GI) system, lymphoreticular system, skin, central nervous system, musculoskeletal system, reproductive system, and liver. Scrofuloderma is rarely found in children. Late diagnosis and inappropriate treatment can lead to poor prognosis and complications. Case report: A 12 year old girl was referred from the regional hospital with complaints of shortness of breath since 3 days before admission. Accompanied by gasping, productive cough with difficulty coming out of phlegm, accompanied by an enlarged stomach and swollen legs, and fever fluctuating since 1 week before admission. Starting with a history of lumps on the neck since 6 months and have started tuberculosis therapy for 1 week before admission. Physical examination revealed multiple lymphadenopathy in the right and left cervical regions, decreased sounds in both lungs, and severe malnutrition. The results of a chest x-ray examination showed a massive pleural effusion. Laboratory results showed hypoalbuminemia, positive rivalta test in effusion fluid, and uncompensated respiratory acidosis. Management of this patient consists of oxygen, correction of hypoalbuminemia, antibiotics, anti-tuberculosis drugs, management of malnutrition, and water seal drainage (WSD). Conclusion: Although rare, scrofuloderma can be found in children. Therefore, collection of tissue specimens from specific extrapulmonary locations is necessary. Late diagnosis and inappropriate treatment can lead to poor prognosis and antimicrobial resistance. Delayed diagnosis and treatment will also cause complications such as the spread of infection to the lungs and other organs, severe malnutrition and stunting. Infections in the lungs and pleura can cause respiratory failure.

Abstract

The prevalence of cardiovascular disease in Central Java is high and the heart rate variability (HRV) among medical students is low. Heart rate variability is a non-invasive marker that describes the balance of the autonomic nervous system in the cardiovascular system. Heart rate variability is influenced by several factors. One of the factors that affect HRV is lung function. There is synchronization between the respiratory and cardiovascular systems. However, there is not much of explanation about the correlation between Force Vital Capacity and HRV. This Study aims to determine the correlation of FVC and HRV in female students of FK Universitas Jenderal Soedirman. This study is an analytic observational study with cross sectional approach. The number of subjects in this study were 31 female students who were determined by the consecutive sampling method. Force Vital Capacity was measured by using a spirometer. Heart rate variability measurement was done using a POLAR M400 heart rate monitor connected to the Welltory application by measuring the HRV parameter in the form of SDNN (standard deviation of normal to normal). Force Vital Capacity of respondents was in the range 1.82 – 3.47 Liters and had a mean of 2,722.72 ± 0.42 L. Mean HRV (SDNN) was 57.72 ± 23.77 ms. Bivariate analysis used Pearson's correlative parametric test. The results of the Pearson correlation test between FVC and SDNN showed a value of r = 0.282 and p = 0.124. There is no correlation between heart rate variability and vital lung capacity.

Abstract

Diabetes mellitus (DM) is a long-term disease that occurs when the pancreas does not produce enough insulin or the body cannot use insulin efficiently. People with DM will face various changes in their lives, such as diet, exercise, blood sugar control, and others, which must be carried out continuously. These sudden changes in lifestyle can trigger negative psychological reactions in people with DM, such as irritability, feelings of uselessness, and increased anxiety that can lead to depression. Some steps that can be taken to reduce the level of depression in people with DM include the use of medication, modality therapy, and adequate system support, especially from the family. This study aims to determine the relationship between family support and the level of depression in patients with diabetes mellitus in the Botupingge Health Center working area. The study used a descriptive quantitative method with a cross-sectional approach, data were collected from 72 DM patients in the Botupingge Health Center work area using the Proportional Simple Random Sampling sampling technique. The results showed that there was a significant relationship between family support and the level of depression in DM patients in the Botupingge Health Center work area with the results of the Spearman rank test 0.000 (p-value <0.05). It is hoped that it can be used as a reperence for further researchers to conduct further research by using this method.

Abstract

Background: Maternal health during pregnancy is crucial for neonatal outcomes, particularly regarding iron status. This study investigates the impact of maternal weight gain and inflammatory markers on neonatal iron deficiency in a cohort of mothers and their newborns. Methods: A cross-sectional study was conducted involving spontaneously delivered infants from single, full-term pregnancies, with Apgar scores ≥7 at the first minute and normal birth weights (≥2,500 to <4,000 grams). Maternal weight gain was categorized as "appropriate" or "inappropriate" based on the 2009 Institute of Medicine (IOM) recommendations. Maternal blood samples were collected during the third trimester, and infant iron status was assessed through hematological parameters and serum iron levels. Statistical analyses included the Mann-Whitney test and independent t-tests, with significance set at p < 0.05. Results: A total of 59.5% of mothers did not achieve appropriate weight gain, and 57.1% exhibited positive CRP levels, indicating inflammation. Infants born to mothers with appropriate weight gain had significantly better hematological parameters, including higher erythrocyte counts, hemoglobin, and hematocrit levels (p < 0.05). Conversely, infants of mothers with positive CRP levels demonstrated lower erythrocyte counts and hemoglobin levels, indicating a potential negative impact of maternal inflammation on iron transfer (p < 0.05). No significant differences in neonatal iron status were observed between infants born to anemic and non-anemic mothers. Discussion: The findings underscore the importance of monitoring maternal nutritional status and inflammatory markers during pregnancy. Adequate weight gain according to IOM guidelines is associated with improved neonatal iron status, while maternal inflammation negatively impacts iron transfer to the fetus. These results highlight the need for targeted interventions, including nutritional education and management of inflammatory conditions, to enhance maternal and neonatal health outcomes. Conclusion: This study contributes to the understanding of the relationships between maternal weight gain, inflammatory markers, and neonatal iron status. By addressing these factors, healthcare providers can improve outcomes for mothers and their newborns, ultimately promoting healthier populations.

Abstract

This study aimed to evaluate the relationship between blood pressure and blood sugar levels, and how age and gender moderate the relationship. The study involved 105 respondents who underwent health check-ups in Banyumas Regency. Blood pressure was categorized as normal or hypertension, while blood sugar levels were divided into normal, prediabetes, and diabetes. The Chi-Square test showed a significant association between blood pressure and blood sugar levels (p < 0.05), although the strength of the association was relatively weak. Individuals with hypertension have a higher risk of developing blood sugar disorders, especially in women and individuals over 45 years old. Ordinal regression analysis showed that age and gender played an important role in influencing this association. These findings emphasize the importance of an integrated approach in screening and management of hypertension and diabetes, to prevent long-term complications such as cardiovascular disease and renal failure. The practical implications of this study can be used to design more effective public health programs

Abstract

This study aims to analyze the relationship between body weight, height, body mass index (BMI), and blood glucose levels among 105 study subjects . Descriptive analysis results show that the average body weight of subjects is 64.81 kg, height is 1.62 m, and blood glucose level is 107.05 mg/dl, with significant variation in glucose levels. Scatter plots indicate that blood glucose does not have a significant relationship with body weight and height, while there is a tendency for increased blood glucose levels with increasing BMI. Correlation analysis reveals a strong positive relationship between body weight and BMI, but blood glucose does not show significant relationships with other variables. Using quantile regression, the analysis shows no significant relationship between independent variables and blood glucose at the median quantile. These findings highlight the need for further research to understand the factors influencing blood glucose levels and the importance of comprehensive assessment of anthropometric parameters.

Abstract

Scabies is a skin disease caused by an infestation of the mite Sarcoptes scabiei var. hominis. This disease often mimics other skin conditions, earning it the nickname "The Great Imitator." Four main signs (cardinal signs) of scabies infestation are recognized: intense itching at night (pruritus nocturna), group infections affecting multiple individuals, burrows accompanied by papules or vesicles, and the presence of mites on the patient’s lesions. Atypical scabies has several rare and difficult-to-recognize forms, which can lead to misdiagnosis. Accurate diagnosis is crucial to prevent further spread and complications.Several diagnostic methods can be employed to confirm this disease. Two diagnostic techniques commonly used are skin swab and skin scraping; however, the comparative effectiveness of these methods has not been extensively studied. This study aims to compare the effectiveness of these two techniques in detecting scabies. The research design used in this study is observational with a cross-sectional approach and a full-sampling method. The test subjects were scabies-infected rabbit obtained from the Research Laboratory of the Faculty of Medicine, Jenderal Soedirman University.A total of 8 samples were collected: 4 samples using the skin swab technique and 4 samples using the skin scraping technique. Samples were taken from active lesion areas using the skin swab technique with transparent adhesive tape and the skin scraping technique with a sterile scalpel. The collected samples were examined under a microscope to detect mites, eggs, or their feces. The results showed no statistically significant difference between the outcomes of the skin swab and skin scraping methods, with a p-value of 0.448 (p > 0.05). Both techniques were equally effective in diagnosing scabies in rabbit.Additionally, the skin swab technique was more comfortable for patients and minimized the risk of injury. In conclusion, the skin swab technique can serve as a safer and more effective alternative to skin scraping, particularly for diagnosing scabies in sensitive lesion areas. This study is expected to assist medical practitioners in selecting the optimal diagnostic method for scabies.
