Abstract

Background: Suruhan herb (Peperomia pellucida (L.) Kunth) had great potential as a natural antioxidant source due to its secondary metabolite content. Objective: This study aimed to determine the effect of solvent polarity (n-hexane, ethyl acetate, and distilled water) on the secondary metabolite profiles and antioxidant activity of suruhan herb extracts using the DPPH method. Method: Samples were extracted via maceration at a 1:10 ratio. Ethyl acetate and n-hexane extracts were concentrated using a rotary evaporator, while the distilled water extract was dried using a freeze dryer. Results: Phytochemical screening showed that all extracts contained alkaloids, flavonoids, tannins, and phenols. N-hexane and ethyl acetate extracts contained steroids/terpenoids and essential oils, while the distilled water extract contained saponins. One-way ANOVA showed highly significant differences in antioxidant activity. All extracts were categorized as very strong, with the lowest IC50 produced by ethyl acetate (29.37 µg/mL), followed by n-hexane (30.60 µg/mL) and distilled water (30.91 µg/mL). For comparison, Vitamin C had an IC50 of 4.08 µg/mL. Conclusion: Solvent polarity directly influenced the secondary metabolite profiles and antioxidant activity of the extracts. All variations exhibited very strong antioxidant activity (IC50 < 50 µg/mL), with statistically significant differences among the groups and the positive control. Each solvent yielded a significantly different antioxidant potency, ranking: ethyl acetate, n-hexane, and distilled water. Statistical analysis confirmed that ethyl acetate was the most effective solvent for extracting antioxidant compounds.

Abstract

Background: Hypertension remains a chronic disease that requires long-term therapy; however, poor patient adherence to medication frequently limits treatment effectiveness. Single Pill Combination (SPC) was developed to simplify treatment by combining two or more antihypertensive agents into one tablet. Objective: To analyze the effect of SPC antihypertensive therapy on medication adherence and its impact on clinical outcomes. Method: This study used a narrative review approach through literature searches in PubMed, ScienceDirect, and Google Scholar. Articles published from 2010–2024 were selected based on predefined inclusion and exclusion criteria. And than 15 studies met the eligibility criteria and were included. Results: The review demonstrated that SPC consistently improved medication adherence and treatment persistence compared with free-combination therapy. Adherence increased by approximately 20–30% among SPC users. SPC was also associated with better blood pressure control, reduced treatment discontinuation, and potential reductions in cardiovascular complications and healthcare costs. Conclusion: SPC antihypertensive therapy is an effective strategy to improve medication adherence and optimize hypertension treatment outcomes. Further cost-effectiveness analysis studies are recommended to evaluate its economic value. Abstract

Background: Tuberculosis (TB) remains a global health problem with high incidence and mortality rates. The success of TB therapy is greatly influenced by patient adherence to long-term treatment. Medication Therapy Management (MTM) has been developed as a pharmaceutical care approach focused on optimizing medication use and improving patient outcomes. Objective: This review aims to analyze the impact of Medication Therapy Management implementation on treatment outcomes in tuberculosis patients and identify opportunities and challenges for its implementation in healthcare practice. Method: This study was conducted using a narrative review method by searching scientific articles from the PubMed, Google Scholar, Frontiers, and ScienceDirect databases. Articles published between 2016 and 2026 that discussed the implementation of MTM in tuberculosis patients were selected based on predetermined inclusion criteria. A total of 15 articles met the criteria and were analyzed descriptively. Results: The review indicates that the implementation of MTM has contributed to improving treatment adherence, identifying and addressing drug-related problems, improving monitoring of drug side effects, and supporting the success of TB patient therapy. Pharmacist interventions through MTM components have also been reported to have the potential to improve patient quality of life and strengthen collaboration among healthcare professionals. However, MTM implementation still faces various obstacles, such as limited human resources, suboptimal documentation systems, and poor patient understanding of therapy. Conclusion: Medication Therapy Management has demonstrated benefits in supporting successful tuberculosis treatment by improving adherence, safety, and effectiveness of therapy. Strengthening the role of pharmacists and providing support from the health system is needed to optimize MTM implementation in TB patients. Abstract

Pendahuluan: Hipertensi ialah penyakit kronis yang membutuhkan terapi jangka panjang. Kepatuhan minum obat menjadi salah satu faktor penting dalam pengendalian tekanan darah dan pencegahan komp;ikasi kardiovaskular. Hubungan antara kepatuhan minum obat dan tekanan darah pada pasien hipertensi di fasilitas kesehatan primer masih menunjukkan hasil yang bervariasi Tujuan: Penelitian ini bertujuan untuk menganalisis hubungan tingkat kepatuhan minum obat dengan tekanan darah pada pasien hipertensi serta mengidentifikasi faktor demografi yang berhubungan dengan kepatuhan. Metode: Penelitian ini menggunakan rancangan deskriptif observasional dengan pendekatan cross-sectional. Data dikumpulkan pada Mei–Juli 2024 dari 194 pasien hipertensi rawat jalan yang memenuhi kriteria inklusi. Tingkat kepatuhan diukur menggunakan kuesioner Medication Adherence Report Scale (MARS-5), sedangkan analisis data dilakukan secara univariat dan bivariat menggunakan uji Chi-Square pada SPSS Hasil: Hasil penelitian menunjukkan bahwa mayoritas responden berusia >60 tahun, berjenis kelamin perempuan, berpendidikan SMA, berstatus menikah, dan telah menderita hipertensi lebih dari 3 tahun. Sebagian besar pasien memiliki tingkat kepatuhan sedang, sedangkan tekanan darah terkontrol lebih banyak ditemukan pada pasien di Puskesmas Pontang dibandingkan Puskesmas Baros. Analisis bivariat menunjukkan bahwa lama menderita hipertensi berhubungan signifikan dengan tingkat kepatuhan minum obat (p=0,002), sedangkan hubungan antara kepatuhan minum obat dan tekanan darah tidak menunjukkan signifikansi statistik (p=0,077) Kesimpulan: Penelitian ini menyimpulkan bahwa lama menderita hipertensi merupakan faktor yang berhubungan dengan kepatuhan minum obat, sedangkan tekanan darah tidak berhubungan secara bermakna dengan tingkat kepatuhan.

Abstract

Pendahuluan: Seledri (Apium graveolens L.) merupakan tanaman yang mengandung metabolit sekunder polifenol dengan berbagai aktivitas biologis. Selain itu, seledri juga kaya akan kalium yang berperan dalam menjaga keseimbangan cairan tubuh dan berpotensi membantu mencegah hipertensi. Tujuan: Penelitian ini bertujuan untuk mengidentifikasi kandungan kalium serta membandingkan kadar flavonoid total pada ekstrak etanol tanaman seledri (EETS), infusa tanaman seledri segar (ITSS), infusa tanaman seledri kering (ITSK), dan sari tanaman seledri segar (STSS). Metode: Identifikasi kalium dilakukan menggunakan uji warna dan uji nyala. Penetapan kadar flavonoid total dilakukan secara spektrofotometri UV-Vis dengan metode kolorimetri dan dinyatakan sebagai ekuivalen kuersetin (QE). Hasil: Hasil identifikasi menunjukkan bahwa tanaman seledri dengan berbagai preparasi, yaitu EETS, ITSS, TSK dan STSS, positif mengandung kalium yang ditunjukkan dengan terbentuknya kristal berbentuk roset pada uji warna dan nyala berwarna ungu pada uji nyala. Kadar flavonoid total yang diperoleh secara berturut-turut adalah 16,7240±0,1898; 0,2662±0,0015; 3,6000±0,0090; dan 0,0019±0,00 mg QE/g. Kesimpulan: EETS, ITSS, TSK dan STSS terbukti mengandung kalium. EETS memiliki kadar flavonoid total tertinggi, sehingga berpotensi menjadi bahan aktif dalam bentuk sediaan yang lebih efektif untuk memperoleh senyawa flavonoid dari seledri.

Abstract

Introduction: type 2 diabetes mellitus is a chronic disease requiring long-them therapy. Geriatric patients are higher. risk of medication non-adherence due to cognitive decline, polypharmacy, and a lack of understanding regarding their therapy. Pharmacist counseling is a form of pharmaceutical service expected to improve patient adherence.Objective: to determine the effect of pharmacist counseling on medication adherence among geriatric patients with type 2 diabetes mellitus at Rowosari Community Health Center.Methods: this study used a one-group pretest-posttest design with 30 geriatric patients with type 2 diabetes mellitus selected by purposive sampling. Adherence was measured using the ARMS questionnaire before and after counseling. Analysis was performed using a paired t-test.Results: Before counseling, all respondents had a moderate level of adherence. After counseling, adherence increased, with 13.3% of respondents achieving high adherence. The mean ARMS score decreased from 16.60 to 14.23, with p = 0.000, indicating a significant difference

Abstract

Physical characterization is essential to ensure the quality, stability, and usability of herbal hair tonic formulations. However, information regarding the effect of pomegranate (Punica granatum L.) extract concentration on the physical characteristics of hair tonic remains limited. Objective: This study aimed to evaluate the physical characteristics of pomegranate extract hair tonic, determine the effect of extract concentration on its physical quality and identify the optimum formulation. Methods: An experimental laboratory study was conducted using six formulations consisting of one control and five formulations containing 2%, 2.5%, 3%, 3.5%, and 4% pomegranate extract. Physical evaluations included organoleptic properties, homogeneity, pH, viscosity, weight, and stability using the cycling test. Data were analyzed using one-way ANOVA. Results: All formulations exhibited good organoleptic properties, homogeneous appearance, and satisfactory physical stability after the cycling test. The pH values ranged from 4.70 to 5.20, while all viscosity values were below 5 cP. Statistical analysis demonstrated that extract concentration significantly affected pH, viscosity, and weight (p < 0.05). The formulation containing 4% pomegranate extract showed the best overall physical characteristics. Conclusion: Pomegranate extract was successfully formulated into a stable hair tonic with acceptable physical characteristics. The 4% extract formulation demonstrated the most favorable physical quality and stability.

Abstract

Introduction: Understanding the mechanisms of action of natural compounds as immunomodulators is important for the prevention and treatment of infections. Objective: Secondary metabolites from Clitoria ternatea L. with potential as immunomodulators were identified through affinity screening of proteins involved in immune system regulation and ADME prediction. Methods: Molecular docking was performed using the Molecular Operating Environment (MOE®), and ADME prediction was conducted using SwissADME. Results: The docking results showed that the proteins IFNβ (1AU1), IL-6 (1ALU), NF-κB (3QXY), TNF-Alpha (2AZ5 & 6X85), and IFN-alpha (3SE3) had RMSD values < 2 Å and were selected as PDB IDs. The three potential compounds from butterfly pea flowers were able to bind to the 3QXY protein through the amino acids ASP248; TYR223; ASN251 (Quercetin-3-rutinoside), GLU154; ALA222 (-(-)-Epicatechin) and LEU146 (Delphinidin-3-O-glucoside). Compounds from butterfly pea flowers that can bind to NF-κB target proteins (PDB ID: 3QXY) are more potent than the natural ligands, namely quercetin-3-rutinoside, (-)-epicatechin, and delphinidin-3-O-glucoside. ADME prediction results showed good lipophilicity with a log P value <5, but low gastrointestinal absorption. Conclusion: Secondary metabolites from butterfly pea flowers have the potential to be developed as immunomodulatory candidates due to their relatively good affinity.
