Abstract

Adolescence is characterized by physical and psychological changes that often cause emotional distress, so some adolescents express stress through self-harm behavior. This condition can have an impact on the appearance of shame, depression, and anxiety. This study aims to analyze the effectiveness of dhikr therapy in reducing the risk of self-harm in adolescents at SMPN 30 Banjarmasin. The study used a quantitative method with a pre-experimental one group pretest-posttest design. The sample consisted of 35 respondents who were selected through purposive sampling techniques. Data collection was carried out using the Self-Harm Inventory (SHI) questionnaire and analyzed using the Wilcoxon test. The results showed a significant difference in the level of self-harm before and after dhikr therapy (p=0.00), with all respondents (100%) experiencing a decrease in the level of self-harm to the mild category. These findings prove that dhikr therapy is effective in reducing the risk of self-harm in adolescents. This therapy can be a simple, independent intervention that is beneficial for improving adolescent mental health and emotional control

Abstract

The involvement of individuals with stroke in self-management is a key factor in recovery, empowerment, and improved health outcomes, which can ultimately influence their quality of life. The aim of this study was to analyze the effect of self-management adherence on the quality of life among stroke patients. This research was a quantitative study using a cross-sectional design, with purposive sampling and a total sample of 120 stroke patients. Data were collected using the Out-of-Hospital Adherence Questionnaire for Stroke Patients (OHAQ-SP) and the Stroke Impact Scale 3.0 (SIS), and analyzed using the Spearman rank test. The results showed that most respondents were adherent to self-management (76.9%) and had a high quality of life (62.3%), with a significant correlation (p = 0.000; r = 0.833). These findings indicate that self-management adherence has a positive, strong, and significant relationship with the quality of life of stroke patients. The higher the patient’s adherence to self-management behaviors, the higher their quality of life

Abstract

Femoral fracture is a major orthopedic injury requiring operative management to prevent complications and restore limb function. Anesthetic technique plays a crucial role in the success of surgical intervention. This case report employed a descriptive observational method with a case study approach on a 36-year-old male patient with a complete fracture of the middle third of the right femur who underwent Open Reduction and Internal Fixation (ORIF). Data were collected from medical records, physical examination, laboratory tests, radiological findings, as well as intraoperative and post-anesthesia monitoring. Anesthetic management was performed using spinal anesthesia with 15 mg of intrathecal bupivacaine, supported by multimodal analgesia and fluid therapy to maintain hemodynamic stability. During surgery, the patient remained stable, although shivering occurred and was successfully managed with intravenous pethidine and external warming. Post-anesthesia, the patient demonstrated gradual motor recovery, stable vital signs, and no serious complications. In conclusion, spinal anesthesia proved effective in providing adequate analgesia, maintaining hemodynamic stability, and reducing intraoperative complications in patients undergoing ORIF for femoral fracture. This case highlights the importance of selecting an appropriate anesthetic technique, close monitoring, and prompt intervention to ensure patient safety and favorable clinical outcomes

Abstract

Encephalitis is an inflammatory condition of the brain parenchyma that often causes serious neurological disorders, as well as muscle weakness, motor dysfunction, impaired coordination, and decreased independence in children's daily activities. The broad clinical impact of encephalitis makes it not only an acute infection but also a condition with long-term implications for children's motor development and quality of life. Therefore, nursing interventions should focus on two main aspects: medical stabilization during the acute phase and structured rehabilitation during the recovery phase. This study aimed to evaluate the effectiveness of nursing interventions in improving muscle strength in children with encephalitis by reviewing the latest literature. This research employed a literature review approach, searching publications from 2020 to 2025 and identifying six articles that met the inclusion criteria. The analysis shows that 83.3% (n=5) of the articles focused on rehabilitative interventions such as muscle strengthening exercises, gait correction, balance exercises, range of motion (ROM), and functional activities, which have been shown to improve motor function and accelerate neurological adaptation. Meanwhile, 16.6% (n=1) of the articles emphasized the importance of close monitoring during the acute phase, focusing on neurological observation, complication prevention, maintaining respiratory stability, and monitoring the risk of increased intracranial pressure. These findings confirm that nursing interventions, which include comprehensive observation, family education, structured therapeutic measures, and multidisciplinary collaboration, significantly optimize neuroplasticity, improve postural control, and maximize muscle strength recovery. Overall, these evidence-based nursing interventions are crucial elements in enhancing the quality of life and independence of children with encephalitis.

Abstract

Hemodialysis is a complex and long-term renal replacement therapy that requires strict adherence to multiple medical recommendations, including fluid restriction, medication adherence, dietary regulation, and scheduled therapy sessions. Non-adherence to the hemodialysis regimen can worsen clinical conditions and increase the risk of complications, morbidity, and mortality. Therefore, effective and sustainable intervention strategies are needed to improve adherence among hemodialysis patients. This narrative review aims to integrate recent findings from studies that examine various adherence-enhancing interventions, including interactive education, psychosocial support, behavioral interventions, and physiological monitoring. A narrative synthesis was conducted through a literature review of ten recent studies evaluating the effectiveness of different approaches, such as teach-back methods, nurse-led follow-up calls, behavioral self-regulation strategies, psychosocial support, and physiological monitoring, in improving adherence to fluid restriction and therapeutic regimens. The findings indicate that interactive educational interventions, particularly teach-back techniques and nurse follow-up calls, significantly improve patient understanding and adherence to treatment protocols. Behavioral interventions, including self-regulation and positive coping skills training, support patients’ psychological adaptation to long-term therapy. In addition, psychosocial support from family and healthcare professionals enhances motivation and promotes sustained adherence. Routine physiological monitoring, such as assessment of interdialytic weight gain, further supports adherence to prescribed regimens. Overall effectiveness

Abstract

Intravenous fluid management is a critical component of perioperative care in patients with chronic kidney disease (CKD). The type of fluid administered may influence acid–base balance, electrolyte status, and the risk of acute kidney injury (AKI). This article aims to review the latest scientific evidence comparing the effects of 0.9% saline and balanced crystalloids in CKD patients in the perioperative setting. A narrative review was conducted by searching PubMed, Scopus, ScienceDirect, and Google Scholar using keywords related to saline, balanced crystalloids, CKD, perioperative care, and AKI. Eligible articles included randomized controlled trials, meta-analyses, and systematic reviews published between 2015–2024. Results: Evidence from multiple studies indicates that balanced crystalloids are superior in maintaining acid–base stability, reducing chloride levels, and lowering the incidence of AKI compared to saline. Large trials such as SMART, SALT-ED, and BEST-Fluids demonstrated reduced major adverse kidney events and delayed graft function in the balanced group, although mortality outcomes remain inconsistent. Balanced crystalloids provide greater physiological advantages and renal protection compared to saline in CKD patients undergoing surgery. While mortality reduction has not been consistently observed, current evidence supports the use of balanced crystalloids as the preferred perioperative fluid, particularly in patients at high risk of AKI.

Abstract

Diabetes mellitus is a chronic disease with a rapidly increasing prevalence among older adults. Poor glycemic control in the elderly increases the risk of complications, reduces quality of life, and leads to greater health care utilization. Beyond pharmacological treatment, family support is an important non-pharmacological factor that may influence diabetes self-management and blood glucose control, particularly in primary health care settings. This study aimed to examine the role of family support in controlling blood glucose levels among older adults with diabetes mellitus. A quasi-experimental study with a pre-test and post-test control group design was conducted among 40 older adults with diabetes mellitus in the working area of Tadoy Primary Health Care Center, North Sulawesi, Indonesia. Participants were assigned to an intervention group (n = 20) receiving structured family support in addition to routine care, or a control group (n = 20) receiving routine care only. Blood glucose levels were measured before and after the intervention and analyzed using paired and independent t-tests. The intervention group showed a significant reduction in blood glucose levels (p = 0.000), whereas the control group showed no significant change (p = 0.087). The reduction was significantly greater in the intervention group than in the control group (p = 0.000). Family support significantly improves blood glucose control among older adults with diabetes mellitus. Integrating family-based approaches into primary health care is recommended to enhance diabetes management in elderly populations.

Abstract

Hypertension is a chronic health problem with a high prevalence and risk of serious complications if not optimally managed, making medication adherence a crucial factor in blood pressure control. This study aims to analyze the relationship between adherence to antihypertensive medication and blood pressure status in hypertensive patients at the Mangaran Community Health Center. The study used a quantitative observational design with a correlational approach and a cross-sectional design. The study population was all hypertensive patients undergoing routine treatment, with a total sampling technique and a total of 30 respondents. Adherence data were collected using a standardized Morisky Medication Adherence Scale (MMAS-8) questionnaire, while blood pressure was measured directly according to clinical procedures. Data were analyzed univariately and bivariately using the Chi-Square Test. The results showed a significant relationship between medication adherence and blood pressure status, where higher adherence is associated with better blood pressure control. These findings provide practical implications for primary health care in strengthening education and monitoring adherence, and contribute theoretically to strengthening the concept of adherence as a major determinant of the success of hypertension therapy

Abstract

Hypertension is a non-communicable disease with an incidence rate that increases annually, while low medication adherence indicates that hypertension management remains suboptimal, particularly among patients who are still actively working. One psychological factor that influences health behavior is health locus of control (HLC), which refers to an individual’s belief regarding control over their health condition. This study aimed to determine the relationship between HLC and medication adherence among hypertensive patients who were still actively working in Taman Village, within the working area of Abiansemal IV Primary Health Center. The study used a descriptive correlational design with a cross-sectional approach and involved 87 respondents selected through purposive sampling. The instruments were the Multidimensional Health Locus of Control (MHLC) questionnaire and the Hill-Bone Compliance to High Blood Pressure Therapy Scale. Data were analyzed using the Spearman Rank correlation test. The results showed that most respondents had a high level of HLC (72.4%) and good medication adherence (74.7%). The Spearman Rank test produced a p-value of 0.000 with a correlation coefficient of 0.410, indicating a statistically significant relationship with moderate strength and a positive direction. These findings indicated that higher HLC was associated with better medication adherence, supported by internal HLC and external factors such as family support and the role of health services, which motivated patients to adhere to treatment consistently
