Abstract

Low back pain (LBP) is one of the most common musculoskeletal complaints and its risk increases in patients with neurological disorders such as ischemic stroke. This case study aimed to analyze the management of a patient with low back pain accompanied by ischemic stroke and hypokalemia, focusing on clinical and pharmaceutical aspects. The method used was a descriptive case study of Mrs. A, a 52-year-old female patient treated at the Neurology Ward of Dr. Drs. Moh. Hatta Brain Hospital, Bukittinggi, with data obtained from medical records, physical and laboratory examinations, and daily therapy monitoring. The results showed lumbar degenerative changes on radiography, severe hypokalemia with a serum potassium of 1.9 mmol/L, and dyslipidemia. Therapy using gabapentin, eperisone, ketorolac, and potassium supplementation successfully reduced the pain score from 3 to 0 and corrected potassium to 4.7 mmol/L. Pharmaceutical evaluation identified a Drug Related Problems (DRPs) in the form of the need for additional secondary stroke prevention therapy (antiplatelet and statin) that had not been administered. The implication of this study emphasizes the important role of pharmacists in identifying drug related problems and optimizing rational, safe, and effective therapy to improve the patient's quality of life. Abstrak. Low back pain (LBP) atau nyeri punggung bawah merupakan salah satu keluhan muskuloskeletal yang paling sering dijumpai dan risikonya meningkat pada pasien dengan gangguan neurologis seperti stroke iskemik. Studi kasus ini bertujuan menganalisis penatalaksanaan pasien low back pain dengan penyakit penyerta stroke iskemik dan hipokalemia berdasarkan aspek klinis dan farmasi. Metode yang digunakan adalah studi kasus deskriptif terhadap Ny. A, pasien perempuan berusia 52 tahun yang dirawat di Bangsal Neurologi Rumah Sakit Otak Dr. Drs. Moh. Hatta Bukittinggi, dengan data yang diperoleh dari rekam medis, pemeriksaan fisik, laboratorium, serta pemantauan terapi harian. Hasil menunjukkan adanya perubahan degeneratif lumbal pada radiografi, hipokalemia berat dengan kalium serum 1,9 mmol/L, serta dislipidemia. Terapi menggunakan gabapentin, eperison, ketorolak, dan suplementasi kalium berhasil menurunkan skala nyeri dari 3 menjadi 0 dan mengoreksi kalium menjadi 4,7 mmol/L. Evaluasi kefarmasian menemukan adanya Drug Related Problems (DRPs) berupa kebutuhan terapi tambahan pencegahan sekunder stroke (antiplatelet dan statin) yang belum diberikan. Implikasi studi ini menekankan peran penting apoteker dalam mengidentifikasi drug related problems (DRPs) dan mengoptimalkan terapi yang rasional, aman, dan efektif untuk meningkatkan kualitas hidup pasien.

Abstract

Hypertension is a chronic disease requiring long-term treatment, making adherence to antihypertensive medication essential for blood pressure control and the prevention of complications. Pharmacist counseling, as part of clinical pharmacy services, is one approach that may support patients' adherence to medication therapy. This study aimed to analyze changes in medication adherence among patients with hypertension following pharmacist counseling at Apotek Nusantara, Palembang, Indonesia. A quasi-experimental study using a one-group pretest–posttest design was conducted from April to June 2025. A total of 73 patients with hypertension who met the inclusion criteria were enrolled. Medication adherence was assessed before and after pharmacist counseling using an educational booklet and the Morisky Medication Adherence Scale-4 (MMAS-4). Data were analyzed using the Wilcoxon Signed-Rank test with a 95% confidence level. At baseline, all participants (100%) demonstrated low medication adherence. Following pharmacist counseling, 45 participants (62%) achieved moderate medication adherence, while 28 participants (38%) remained in the low-adherence category. The Wilcoxon Signed-Rank test demonstrated a statistically significant difference in medication adherence before and after counseling (p < 0.001). These findings indicate a change in medication adherence following pharmacist counseling among patients with hypertension. Further studies employing controlled study designs are needed to provide stronger evidence regarding the effect of pharmacist counseling on medication adherence.

Abstract

Stunting is a chronic nutritional condition characterized by impaired linear growth and a low height for-age measurement. Animal-source protein supplies essential amino acids and various micronutrients that contribute to tissue formation and bone growth. This study aimed to examine the relationship between animal protein consumption and height status among toddlers in the working area of UPTD Puskesmas Rajagaluh in 2026. This quantitative analytical observational study employed a cross-sectional design. The study population consisted of 1,769 children aged 0–59 months recorded in the working area, with 95 children selected using Slovin’s formula and simple random sampling. Animal protein consumption was assessed using a semi quantitative Food Frequency Questionnaire (SQ-FFQ), while height status was assessed using the Height-for Age (TB/U) indicator based on WHO Z-scores. Of the 95 respondents, 47 (49.5%) had insufficient animal protein consumption, whereas 48 (50.5%) had good consumption. Based on the TB/U indicator, 30 children (31.6%) were classified as severely short, 36 (37.9%) as short, and 29 (30.5%) as normal. Pearson Chi-Square analysis demonstrated a statistically significant relationship between animal protein consumption and height status (p = 0.000; α < 0.05). These findings highlight the importance of strengthening nutrition education and developing affordable animal-source protein interventions at the community health-service level.

Abstract

Vertigo is a common complaint in emergency departments and neurology clinics. Although most cases are caused by peripheral vestibular disorders, a minority result from central nervous system lesions that may be life threatening and difficult to distinguish based solely on clinical symptoms. Case Presentation: A 51 year old woman presented with persistent rotatory vertigo for two months, worsening over the previous three days, accompanied by recurrent vomiting, intermittent hand tremor, and headache. Examination revealed horizontal left beating nystagmus and a Romberg test with consistent falling to the left, without focal motor, sensory, or cranial nerve deficits. Non contrast cranial CT demonstrated a poorly defined, heterogeneously hyperdense lesion in the left cerebellar hemisphere with surrounding hypodense perifocal edema and no midline shift, suggestive of an intracranial mass. Clinical improvement was observed after four days of antiemetics, corticosteroids, and vestibular suppressants, followed by referral for neurosurgical and oncological evaluation. Conclusion: Persistent vertigo without focal neurological deficits may be an early sign of a cerebellar lesion. Recognition of red flags and timely neuroimaging are essential to avoid delayed diagnosis of potentially life threatening intracranial pathology.

Abstract

Smoking behavior among adolescents is one of the health problems commonly found in society and continues to increase. This study aims to determine the relationship between adolescents' knowledge of the dangers of smoking and smoking behavior in RT 04, Pegadungan Village, West Jakarta. This study used an analytical correlation design with a cross-sectional approach. The research sample consisted of 116 respondents selected using a purposive sampling technique. The research instruments used a knowledge questionnaire on the dangers of smoking consisting of 10 statements and a smoking behavior questionnaire consisting of 30 statements, both of which had met the validity and reliability tests. Data were analyzed using the Chi-Square test. The results showed that out of 116 respondents, the majority of adolescents with good knowledge had low smoking behavior (68.5%), while adolescents with sufficient knowledge (58.8%) and poor knowledge (85.7%) tended to have high smoking behavior. The Chi-Square test results showed a significant relationship between knowledge of the dangers of smoking and smoking behavior (p = 0.000; p < 0.05). In conclusion, the better the level of adolescents' knowledge about the dangers of smoking, the lower their tendency to engage in smoking behavior. Therefore, continuous educational efforts for adolescents, along with the involvement of families and schools in providing understanding about the dangers of smoking, are needed to prevent the formation of smoking behavior among adolescents.

Abstract

Diabetes mellitus is a chronic metabolic disease marked by elevated blood glucose levels due to impaired insulin secretion and/or action, often accompanied by fatigue that lowers patients' quality of life, while non-pharmacological interventions such as Soleus Push Up leg exercise are believed to enhance muscle glucose uptake and improve energy metabolism, though evidence on their effect on fatigue remains limited. This study aimed to determine the effect of Soleus Push Up leg exercise on blood glucose stability and fatigue levels among patients with Diabetes mellitus in the working area of UPTD Puskesmas Kedungmundu, Semarang. A pre-experimental one-group pretest-posttest design was used, involving 55 respondents selected through purposive sampling, with blood glucose levels and fatigue measured before and after the intervention. The results showed a significant decrease in blood glucose levels after the intervention, and fatigue levels also decreased significantly. Most respondents were female, in late elderly age, and had suffered from Diabetes mellitus for more than two years. It can be concluded that Soleus Push Up leg exercise is effective in lowering blood glucose levels and reducing fatigue among patients with Diabetes mellitus. These findings imply that the exercise can be integrated by health workers as a simple, easy, and independently applicable non-pharmacological intervention in the daily management of Diabetes mellitus.

Abstract

Cancer is characterized by abnormal, uncontrolled cellular proliferation and its incidence in women continues to rise, including in Central Java, Indonesia. Women undergoing cancer treatment frequently experience psychological distress such as hopelessness, pessimism, and loss of motivation, which can influence their self-efficacy, coping mechanisms, and overall life quality. This study aimed to examine the correlations of self-efficacy and coping mechanisms with the life quality of women with cancer at Dr. Kariadi General Hospital, Semarang. A quantitative correlational design with a cross-sectional approach was applied. The population comprised 1,820 women diagnosed with breast, ovarian, cervical, and endometrial cancer, from which 106 respondents were selected through purposive sampling based on inclusion and exclusion criteria. Data were collected using the Self-Efficacy for Managing Chronic Disease (SEMCD6) questionnaire, the Cancer Coping Questionnaire, and the EORTC QLQ-C30, then analyzed using Spearman's rank correlation test. The results showed that most respondents had high self-efficacy (79.2%), adaptive coping mechanisms (93.4%), and good life quality (90.0%). Spearman's rank correlation revealed no significant correlation between self-efficacy and life quality (p = 0.138; r = 0.145) or between coping mechanisms and life quality (p = 0.402; r = 0.082), both showing a very weak positive correlation. These findings indicate that the life quality of women with cancer is shaped by multiple interacting factors beyond self-efficacy and coping mechanisms alone, such as family and spiritual support, physical symptoms, and cancer stage. Future researchers should explore additional determinants of life quality among women with cancer.

Abstract

Cervical cancer is a malignancy characterized by the uncontrolled growth of abnormal cells in the cervix that can spread to the uterus and vagina. Persistent infection with the Human Papillomavirus (HPV) is the primary cause of this disease. This study aimed to examine the correlations between sexual function, husband support, and the quality of life of patients with cervical cancer undergoing chemotherapy. This quantitative study employed a cross-sectional design. The researchers recruited 121 patients with cervical cancer at Dr. Moewardi Regional General Hospital, Surakarta, using consecutive sampling between May 4 and May 28, 2026. They assessed sexual function using the Female Sexual Function Index (FSFI), measured husband support with a husband support questionnaire, and evaluated quality of life using the Indonesian version of the World Health Organization Quality of Life–BREF (WHOQOL-BREF). Most respondents were 36–45 years old 92 (76.0%), had completed elementary school as their highest level of education 45 (37.2%), were unemployed 110 (90.9%), had stage III cervical cancer 60 (49.9%), had received chemotherapy for less than six months 77 (63.6%), had previously undergone radiotherapy 85 (70.2%), and received platinum- and taxane-based combination chemotherapy 117 (96.7%). In addition, 115 respondents (95.0%) experienced sexual dysfunction, 70 (57.9%) reported moderate husband support, and 75 (62.0%) had poor quality of life. Spearman's rank correlation analysis showed a significant correlation between sexual function and quality of life (p = 0.001). The correlation coefficient (r = 0.292) indicated a weak positive correlation. Husband support also showed a significant correlation with quality of life (p < 0.001), with a weak positive correlation (r = 0.336), indicating that stronger husband support was associated with better quality of life.

Abstract

Medicine inventory management in pharmacies plays a direct role in service quality and patient safety. Improper storage and stock rotation can lead to medicine accumulation, an increase in expired medicines, and losses for pharmacies. This literature review aims to describe the application of the First In First Out (FIFO) and First Expired First Out (FEFO) methods in drug management in various pharmacies in Indonesia and the factors that influence their success. The study was conducted by reviewing ten articles that met the inclusion criteria and summarized in a table containing the location of the study, storage methods, and the main findings of each study. The results of the review show that some pharmacies have implemented FIFO and FEFO in accordance with standards with a high level of compliance, while other pharmacies still face problems with expired drugs, slow-moving drugs, and weaknesses in internal control. The implementation of an FEFO-based stock management information system, including the use of barcodes and expiration date alerts, helps improve stock rotation accuracy and reduce the risk of human error, thereby maintaining the quality of service in pharmacies.

Abstract

Diabetes mellitus (DM) is a non-communicable disease whose prevalence continues to increase, including among adolescents, in relation to unhealthy dietary patterns and low physical activity. This study aimed to identify factors associated with DM incidence as reflected by random blood glucose (RBG) status among adolescents in the working area of Cengkareng Sub-district Community Health Center. A quantitative analytic-correlational study with a cross-sectional approach was conducted among 80 adolescents aged 10–19 years selected using purposive sampling. Data were collected using questionnaires on sugary food and beverage consumption, physical activity, and family history of DM, together with random blood glucose measurements. Data were analyzed using univariate and bivariate analyses with the Pearson Chi-Square test at α = 0.05. Most respondents had high sugary food and beverage consumption (70.0%), low or inactive physical activity (47.5%), no family history of DM (62.5%), and normal RBG status (73.8%). The bivariate analysis reported significant associations between sugary food and beverage consumption (χ² = 5.685; p = 0.017), physical activity (χ² = 9.399; p = 0.009), and family history of DM (χ² = 28.242; p = 0.001) with RBG status. The findings indicate that physical activity and family history were particularly important factors associated with RBG status, while healthy dietary habits remain relevant for long-term prevention. Early screening and health education are recommended, especially for adolescents with a family history of DM.
