Abstract

Dyspnea is a clinical manifestation of congestive heart failure (CHF) due to lack of oxygen supply because of accumulation of fluid in the alveoli. This is an important factor that affects the quality of life of patients. The accumulation makes the heart unable to pump up to the maximum. The effect in respiratory muscle increases the sensation of dyspnea. Non-pharmacological management is measures aimed to maintain physical stability, avoid behaviors that can aggravate the condition, and detect early symptoms of worsening heart failure. The purpose of this study was to evaluate the influence of deep breathing exercise and active range of motion on

Abstract

Coronary heart disease is one of the main causes of mortality rate in the world. This disease is affected by several risk factors. People who has high or moderate risk factors for coronary heart disease should have good preventive behavior, but this also requires a good level of self-efficacy as well, so that the expected behavior can be performed. This study aimed to determine the level of community self-efficacy of coronary heart disease based on characteristic risk factors. This research used a descriptive quantitative approach. Participant consisted of 70 people in Desa Limusgede, West Java, Indonesia acquired through a non-probability technique of purposive sampling. Self-efficacy was measured using a self-efficacy questionnaire (validity value in the range of 0.484 to 0.773 and reliability value 0.862) while risk factor data were determined by age, body mass index, blood pressure, smoking behavior, diabetes mellitus, and physical activity which were referencedby Jakarta Cardiovascular Score. Data were analyzed using median and frequencydistribution. The results showed that median (minimum-maximum score) of self-efficacy is 26.00 (11-41), most of the respondent (62.86%) had high self-efficacy of coronary heart disease and more than a half respondent (47.14%) had moderate and high risk factors for cardiovascular disease. Further, almost half the respondent who had high self-efficacy also had moderate and high risk factors for cardiovascular disease. So, health professional must concern in activities to decrease the level of cardiovascular risk factors, such as health education, health promotion, and disease prevention.

Abstract

Patients with end-stage kidney disease (ESRD) undergoing hemodialysis (HD) tend to have multiple comorbidities, especially cardiovascular disease, mineral and bone disorders which are important factors and contribute to the morbidity and mortalityof HD patients. So it is important to know the types of intra-dialysis physical exercises that are safe and effective for HD patients. The type of research analyzed in this journal writing is a randomized controlled trial (RCT) that focuses on interdialysis physical exercise interventions both at home and HD center. With a literature search using PubMed, ScienceDirect, Willey, Proquest and Google Scholar databases. And those who meet the relevant requirements and reviews are: 8. Based on several reviews of intradialytic exercise studies in HD patients, intra-dialysis low intensity strength training is found safe and effective in adults and older. Research shows that doing 30 minutes of intradialytic exercise per session 3 times a week for at least >= 8 weeks is beneficial for HD patients. Regular exercise should be considered as an important therapeutic modality in the care of patients undergoing HD. Strength training improves physical performance, nutritional status and physical activity. But this study needs further research, so that it can contribute to the provision of evidence-based information in applying safe intradialytic exercises and become part of routine dialysis treatment units.

Abstract

The incidence of the fell is one of the hospital serious problems, especially in hospitalized patient. The number of fall incidence is one of the main indicators in patient safety and hospital quality. Qualitative study with phenomenology approach was conducted to explore factors affecting the risk of fall in pediatric patients. Four nurses consist of two team leaders and two associate nurses participated indepth interviews. The findings from the interviews were triangulated in some process including an interview with the head of an inpatient installation, supervisor, and patient family, participative observation, documentation of form humpty dumpty and operational standard prevent risk of fall. The result data was analyzed with the content analysis method and concluded with a fish bone. Four themes emerged from analysis: knowledge, human resources, environment, and facilitation. Badimplementation of operational standard procedure, heavy workload of a nurse with unbalance ratio between patient and nurses, the slippery floor, unfixed bedside rails, and unavailability of patient bell have a role in affecting the risk of fall. This study suggests to held fall prevention education, make a review of nurse's workload with recalculate the ratio of patient and nurse and upgrade the hospital facilitation and environment situation.
