Malaria is still become disease the infection that has the biggest impact to health humans all over the world. When parasites Plasmodium enter into blood can influence physiology general body host, so that can cause disruption of hematological parameters which can cause a number of manifestation clinical such as anemia and thrombocytopenia so that can make things worse Health conditions. Identification in a way accurate infection parasite plasmodium is very important for determine therapy that will given with right. Technology intelligence artificial intelligence (AI) offers solution innovative For identify parasite Plasmodium Malaria with more efficient and accurate. AI, especially that based on deep learning and computer vision, can analyze picture microscopic with high speed and accuracy so that help speed up diagnosis and treatment in Medical decision. For develop and test system based intelligence artificial that can in a way automatic classify type parasite Plasmodium Malaria based on picture microscopic. Type study This covering image data set collection parasite Plasmodium malaria from laboratory medical, data labeling, and training of deep learning models based on convolutional neural networks (CNN). This model will evaluated based on accuracy, sensitivity, and specificity in differentiate various type Plasmodium. Type study is combination between experimental, quantitative and developmental AI technology in microbiology medical. Malaria Level (Mild, Moderate, Severe) and Duration of AI Use (Sig = 0.004). While Malaria Level and Duration Inspection Microscopic Conventional show significant and relevant results, which strengthen urgency innovation AI-based. With mark significance (Sig) 0.006, AI (Artificial Intelligence) is effectively superior in the diagnosis of Severe Malaria in endemic areas.

Abstract

Stunting remains a major public health problem in Indonesia, including Banjar Regency, South Kalimantan. Effective prevention requires an educational approach that is not only evidence-based but also culturally sensitive to ensure community acceptance. Integrating local wisdom, such as the oral art of madihin and traditional taboos, is considered effective because it aligns with the socio-cultural values of the Banjar community. This study aimed to evaluate the effectiveness of stunting education based on Banjar culture through madihin videos (traditional oral art), live madihin performances, and the reinforcement of traditional taboos as pillars of healthy family parenting. A quasi-experimental design with a pre-test–post-test control group was employed. The sample consisted of 100 families with toddlers in Banjar Regency, divided into 50 intervention and 50 control participants. Sampling technique used was purposive sampling. The intervention provided education on stunting, exclusive breastfeeding, complementary feeding, traditional taboos, and parenting through videos and live madihin performances, which included health messages and discussions on Banjarese traditional taboos related to diet, pregnancy, and childcare. Data were analyzed using Wilcoxon test to compare pre-test and post-test scores within groups, and Independent t-test to compare differences between intervention and control groups. The intervention group showed a significant improvement in knowledge and attitudes. The mean knowledge score increased from 26.94 to 31.58 (p=0.001), while the attitude score rose from 38.68 to 47.78 (p=0.001). In contrast, the control group showed only a small increase in knowledge (24.80 to 25.20; p=0.001) and no significant change in attitude (20.21 to 29.79; p=0.108). These findings indicate that a culture-based approach is more effective in improving knowledge and fostering positive family attitudes than conventional education. Health education that incorporates madihin and traditional taboos can serve as a strategic innovation in health promotion, particularly in stunting prevention through strengthening family parenting practices in Banjar Regency.

Abstract

The practice of home care medicine in Indonesia has experienced rapid growth in line with the increasing demand for home-based health services. However, to date, there are no specific regulations that explicitly govern this practice within the national legal system. This study aims to analyze the legal vacuum surrounding home care and its implications for legal certainty, medical professional accountability, and patient protection. The method used is normative legal research with a legislative and conceptual approach, The data was obtained through the collection and analysis of legal materials, namely: primary legal materials (laws and regulations), secondary legal materials (research journals and books), and tertiary legal materials (legal dictionaries). The results of the study show that home care practices are carried out without a specific legal basis, thereby creating legal uncertainty and the potential for medical ethics violations. The absence of regulations also hinders the integration of these services into the JKN financing system and widens the gap in access to health services. The implications of these findings emphasize the urgency of formulating a comprehensive, adaptive, and inclusive regulatory framework to ensure the quality of home care services, protect patient rights, and strengthen the national health system. The designed regulations need to consider legal, ethical, technological, and social justice aspects in the implementation of home-based medical services.

Abstract

The Central Sterile Supply Department (CSSD) is a vital hospital support unit that ensures the availability of sterile medical instruments to support clinical services and prevent nosocomial infections. However, many hospitals in Indonesia, including Dr. Abdul Aziz General Hospital, Singkawang, have not established cost-based tariffs for CSSD services. This study aimed to calculate the unit cost of CSSD services at Dr. Abdul Aziz General Hospital, Singkawang, using the Activity-Based Costing (ABC) method. A descriptive qualitative study was conducted. Data were collected through observation, interviews with CSSD staff and management, and document review of financial and operational reports for the CSSD in 2024, specifically focusing on cost components, resource consumption, and activity volumes. Data Analysis used the Activity-Based Costing (ABC) method to meticulously trace overhead costs to services. This analysis followed six systematic steps: (1) identifying all cost components, (2) defining main CSSD activities, (3) determining appropriate cost drivers for each activity, (4) allocating total costs to each activity center, (5) calculating the unit cost per activity, and finally (6) determining the total cost for each instrument set sterilized. The total CSSD operational cost in 2024 was IDR 2,335,001,386, with a unit cost per sterilization cycle of IDR 2,470,901. Costs per instrument set were: major surgery set IDR 1,235,450; medium surgery set IDR 412,640; minor surgery set IDR 247,090; and single instrument IDR 123,545. Activity costs were dominated by sterilization (30%) and packing (25%). The ABC method produces more accurate cost calculations compared to traditional methods and can be used as a basis for tariff setting, cost control, and hospital financial planning. Future studies are recommended to expand the analysis to other hospital support units to benchmark efficiency and strengthen tariff policies.

Abstract

Breast milk is the ideal food for babies. Besides boosting the baby's autoimmune system, making them less susceptible to illness, breastfeeding can also prevent irregular and low milk production. A preliminary study found that some mothers still do not exclusively breastfeed due to irregular or low milk production. To address this issue, an innovation was created in the form of boiled ginger and turmeric water, which is believed to facilitate milk production and increase milk production due to its lactogogum content. The purpose of this study was to identify differences in breast milk production before and after the intervention. This study used a quasi-experimental design with a one-group pre-test-post-test. This study used a checklist data collection technique with direct observation of the administration of boiled ginger and turmeric water on breast milk production in postpartum mothers. The population was 15 postpartum mothers on days 5-7 using purposive sampling. The results of the paired t-test analysis showed an increase in breast milk production, proving that boiled ginger and turmeric water can increase breast milk production and facilitate breast milk production.

Abstract

Determining appropriate healthcare service tariffs is essential for hospital management to ensure financial sustainability, accessibility, and quality of care. Accurate tariff setting requires precise unit cost calculations. This study reviews and compares two widely used methods in healthcare cost analysis: Activity-Based Costing (ABC) and Double Distribution (DD). Using a scoping review guided by the PRISMA framework, literature was searched across Google Scholar, PubMed, Scopus, Web of Science, and the Cochrane Library with keywords “Activity-Based Costing,” “Clinical Pathway,” “Double Distribution,” “Financing,” and “Hospital,” limited to publications from 2019 to 2024. Eleven relevant studies were identified. The findings indicate that the DD method often underestimates costs, as it does not fully account for all healthcare activities, though it is easier to implement and widely used in Indonesia. In contrast, the ABC method provides a more detailed and accurate calculation by linking costs to actual activities, offering greater transparency for decision-making, though it requires comprehensive data and resources. Four studies applied ABC integrated with Clinical Pathways (CP), showing improved cost accuracy, quality control, and efficiency through standardized care processes. Overall, ABC, particularly when combined with CP, demonstrates stronger potential in supporting hospitals to set fairer tariffs and enhance service efficiency compared to the DD method.

Abstract

Electronic documentation of nursing care is an important component in ensuring service quality and patient safety. However, its implementation often faces obstacles such as high workload, limited application features, unstable internet network, and organizational policies that do not provide optimal support. This study aims to analyze the relationship between workload, nurse compliance, application features, internet network, and hospital policies with the completeness of electronic documentation of nursing care at Hospital X. This study used a quantitative approach with a cross-sectional design. The sample consisted of 76 nurses consisting of 26 outpatient nurses and 50 inpatient nurses, with an accidental sampling technique. The instruments consisted of a questionnaire and an observation sheet for completeness of documentation. Validity test results > 0.361 and reliability 0.89. Data analysis used the chi-square test and binary logistic regression. The results showed a significant relationship between completeness of documentation with the variables of workload (p = 0.001), nurse compliance (p = 0.001), application features (p = 0.001), and internet network (p = 0.001), while hospital policies were not significant (p = 0.071). Multivariate analysis showed that internet connection was the most dominant factor with an odds ratio of 29.7, followed by workload with an odds ratio of 23.7. Workload and internet connection quality are the dominant factors associated with the completeness of electronic nursing care documentation. Managerial intervention and infrastructure strengthening are needed to support the implementation of an effective documentation system.

Abstract

Juvenile idiopathic arthritis (JIA) is a heterogeneous group of chronic arthritides in children under 16 years old, characterized by persistent arthritis of unknown etiology with variable systemic and extra-articular manifestations. Systemic juvenile idiopathic arthritis (sJIA) represents a distinct subtype with clinical features including intermittent high fever, arthritis, and cutaneous eruptions, which often pose diagnostic challenges due to overlapping manifestations with other autoimmune or vasculitic diseases. The purpose of reporting this case is to highlight the diagnostic challenges of sJIA with atypical cutaneous manifestations, which may mimic other autoimmune disorders, and to emphasize the importance of careful clinical evaluation for early recognition and management. We report the case of an 8-year-old girl who presented with generalized blackish maculopapular rash, butterfly-shaped erythematous plaques on the face, high intermittent fever, and symmetrical arthritis of both knees. Laboratory examinations revealed elevated inflammatory markers and liver enzymes, with negative antinuclear antibody and anti double stranded DNA results. The data were collected through patient follow-up during hospitalization and regular outpatient visits at the Dermatology, Venereology, and Aesthetics Polyclinic of H. Adam Malik Hospital Medan over a 3-month period. Histopathological findings showed vasculitis. Differential diagnoses considered were systemic lupus erythematosus (SLE) and Henoch-Schönlein purpura (HSP), which were excluded based on clinical, laboratory, and histopathological evaluations. The final diagnosis was systemic juvenile idiopathic arthritis. The patient was treated with methotrexate, folic acid, topical corticosteroids, and emollients, with gradual clinical improvement observed during follow-up. This case highlights the importance of recognizing cutaneous manifestations in JIA, which may mimic other systemic diseases, and emphasizes the role of a multidisciplinary approach for early diagnosis and management to prevent long-term complications.

Abstract

Speech Delay is a condition where the quality of children's speech development is not appropriate or is below their age, children have difficulty expressing their feelings and lack of vocabulary mastery in children. According to the Indonesian Ministry of Health, it is explained that the toddler period is an important period in the growth and development of the next child, speech and language delays (Speech Delay) in Indonesia as much as 5%-10% in preschool children. Speech Delay is estimated to occur by several factors, namely intelligence, language use, speech style/model imitated, family relationships, health, genetics, physical disability, neurological malfunctions, gender, maternal education, socioeconomic status, family function, and number of children. This study aims to determine the effect and risk of speech delay in children with a history of being born with normal weight and those born with abnormal weight. By knowing the risk factors for speech delay in children, it is hoped that prevention efforts can be made so as to reduce the incidence of speech delay in children in the community. This type of research is analytic observational using a cross-sectional design, with a quantitative approach. The population in this study were parents who had children aged 2-5 years. The sample in this study were parents who had children aged 2-5 years experiencing speech delay disorders and who did not experience speech disorders as controls. Selection of research subjects using purposive sampling. The analysis test used was chis square test. Quantitative research data collection was carried out by filling out questionnaires through questionnaires and google form. Research results shows there is a relationship between birth weight and the incidence of speech delay (p value=0.045). The results of the OR calculation show that respondents who have a history of birth weight <2500 grams and > 4000 grams are at risk of 2.286 experiencing speech delay in children compared to respondents who have a history of birth weight 2500gram - 4000gram.

Abstract

Breadfruit leaves (Artocarpus altilis) contain various secondary metabolites such as flavonoids, tannins, terpenoids, and alkaloids which are potential bioactive compounds for therapeutic purposes. This study is an experimental laboratory research aimed at identifying bioactive compound groups and evaluating the antibacterial and antioxidant activities of breadfruit leaf extract. Extraction was carried out using the maceration method with 96% ethanol p.a as solvent. Identification of bioactive compounds was analyzed by Gas Chromatography Mass Spectrometry (GC-MS) method, antibacterial testing by the disc diffusion method, and antioxidant evaluation using a modified Ferric Reducing Antioxidant Power (FRAP) assay. The test bacteria used were Klebsiella pneumoniae and Staphylococcus aureus, gentamicin and chloramphenicol as positive controls. Ascorbic acid was used as the positive control for the antioxidant assay. The GC-MS analysis revealed the presence of active compounds such as Tricyclo [4.3.0.0(7,9)] nonane, trans-11-tetradecenyl acetate, ethyl tridecanoate, α-farnesol isomer, vitamin E, and β-sitosterol. The extract showed antibacterial activity against both tested bacteria, with the smallest effective concentrations of 50 mg/mL for K. pneumoniae and 25 mg/mL for S. aureus, resulting in inhibition zones of 0.3125 mm and 1.5375 mm, respectively, although these values did not meet the sensitivity criteria. The antioxidant activity test showed a potential antioxidant value of 9.84 mg AAE/g extract, using ascorbic acid as the standard curve.
