Abstract

Although anesthesia nurses play a crucial role in managing perianesthetic emergencies, various reports continue to indicate delays and inaccuracies in clinical interventions due to limited clinical experience and suboptimal training methods. This study aimed to determine the effectiveness of perianesthetic emergency scenario-based simulation training on the preparedness and accuracy of anesthesia nurses’ clinical interventions This study employed a quasi-experimental design with a one-group pretest–posttest approach and was conducted at Siti Rahmah Islamic Hospital from January to June 2026. The study population consisted of all anesthesia nurses working in the operating room, and a total sampling technique was used to recruit 12 respondents. Preparedness was assessed using a questionnaire yielding a Cronbach’s alpha coefficient of 0.841, while the accuracy of anesthesia nurses’ interventions was evaluated using an observation checklist. Data were analyzed using univariate statistics and a paired samples t-test for bivariate analysis. The results showed that most respondents were aged 25–35 years (66.7%), male (58.3%), held a Diploma IV degree in Nurse Anesthesiology (66.7%), and had 0–5 years of work experience (75.0%). The mean preparedness score increased from 38.91 ± 8.20 during the pre-test to 44.25 ± 3.69 during the post-test. Similarly, the mean score for intervention accuracy increased from 12.00 ± 1.47 at pre-test to 12.75 ± 1.05 after receiving perianesthetic emergency scenario-based simulation training. Statistical analysis revealed that scenario-based simulation training had a significant effect on improving anesthesia nurses’ preparedness (p = 0.015) and the accuracy of their clinical interventions (p = 0.032). It can be concluded that scenario-based simulation training significantly improves anesthesia nurses’ preparedness and the accuracy of their clinical interventions.

Abstract

Malnutrition remains a challenge affecting the health and development of toddlers and pregnant women. The development of MORIJAWA-based functional food containing Moringa oleifera, corn, and swallow’s nest is important as an innovative nutritional intervention using local resources. This study aims to develop and evaluate MORIJAWA functional food to improve the nutritional status of toddlers and pregnant women. Metode penelitian yang digunakan adalah Research and Development (R&D). This study involved 200 participants, consisting of 100 toddlers and 100 pregnant women. Data were collected through an organoleptic test of MORIJAWA Cookies and an assessment of nutritional status using anthropometric measurements. The organoleptic data were analyzed using descriptive statistics, while the nutritional status data and the factors influencing improvements in nutritional status were analyzed using Structural Equation Modeling (SEM) with the SmartPLS approach. The results of the paired t-test indicated a significant difference (p<0.05) in anthropometric indicators before and after the intervention. Among toddlers, there was an average weight gain of 1.3 kg, an increase in the proportion with normal nutritional status from 59% to 78%, and a decrease in the prevalence of stunting from 25% to 15%. Among pregnant women, there was an increase in mid-upper arm circumference (MUAC) of 1.7 cm, an increase in the proportion with normal nutritional status from 64% to 86%, as well as a reduction in the risk of KEK from 30% to 12%. The PLS-SEM analysis showed that MORIJAWA’s product characteristics had a strong influence on organoleptic quality (β=0.886), while organoleptic quality influenced consumption intention (β=0.863) and perceived benefits (β=0.855). These findings indicate that MORIJAWA has the potential to serve as a locally sourced functional food to support improved nutritional status among toddlers and pregnant women.

Abstract

Prolonged labor in primigravida mothers is often caused by tension in the pelvic and cervical muscles that have never experienced stretching. Deep squat exercises are one of the non-pharmacological interventions based on an upright position that are utilized to assist in the descent of the fetal head and accelerate cervical dilation. Objective: To analyze the effect of deep squat exercises on the acceleration of cervical dilation in primigravida mothers during the active phase of the first stage of labor. Method: Quasi-experimental research with a two-group posttest-only control group design. The sample consisted of 30 primigravida parturients selected using purposive sampling in the active phase of the first stage of labor, divided into an intervention group (deep squat, n=15) and a control group (standard labor care/left lateral position, n=15). Analysis showed a significant difference in the number of mothers experiencing a fast labor duration during the active phase of the first stage of labor between the experimental group (n=11) and the control group (n=4). The results of the analysis using the nonparametric Kolmogorov-Smirnov test showed ρ=0.009 (ρ≤0.05), indicating that there is a significant effect of the deep squat position on the duration of the first stage of active labor. Conclusion: Deep squat exercises have a significant effect in accelerating the cervical dilation process in primigravida mothers during the active first stage of labor.

Abstract

Tuberculosis remains the infectious disease with the highest global burden, and its control is complicated by drug-resistant cases; resistance to the two first-line drugs is largely attributable to the katG S315T and rpoB S531L mutations. Quassinoids of Brucea javanica (L.) Merr. are reported to be antimicrobial, but comparative studies of their interaction with wild-type and mutant target proteins remain limited. To evaluate B. javanica quassinoids as early antitubercular drug candidates through molecular docking and ADMET prediction against KatG and RpoB in both wild-type and mutant forms. An exploratory and predictive in silico study with a comparative design. Twenty-five quassinoids and two controls (isoniazid, rifampicin) were retrieved from PubChem and ChEMBL; target structures were 2CCA (wild-type KatG), 2CCD (KatG-S315T), 5UAC (wild-type RpoB) and 5UAL (RpoB-S531L). Ligands were minimised with the UFF force field and docked with AutoDock Vina in PyRx (exhaustiveness 8, nine poses per combination). The RpoB search space was centred on the co-crystallised rifampicin and validated by redocking, whereas the KatG search space covered the whole enzyme. ADMET was predicted with ADMET-AI against the ATC J04 reference set. On wild-type RpoB all 25 quassinoids bound between −6.7 and −8.6 kcal/mol (mean −7.61 ± 0.52) and 15 matched or exceeded re-prepared rifampicin (−7.5 kcal/mol), although none surpassed the native ligand (−9.2 kcal/mol). S531L weakened every compound (mean ΔE +0.57 kcal/mol), the largest loss occurring in the native ligand itself (+1.20 kcal/mol). On wild-type KatG affinities were weaker and widely dispersed (mean −3.34 ± 3.02 kcal/mol) and none exceeded isoniazid (−6.1 kcal/mol); S315T improved scores (mean ΔE −2.04 kcal/mol) but abolished every contact with the distal heme pocket. Only bruceine D, bruceine H and bruceine I met all four Lipinski criteria, with intestinal absorption ≥ 0.96 and low hERG and cytochrome P450 liability, whereas all sixteen glycosides violated three criteria simultaneously. RpoB is the more promising route for this scaffold; bruceine I is the priority candidate and bruceoside C the glycoside lead. All values are predictive and await experimental confirmation.

Abstract

Oxidative stress is implicated in ageing and a range of degenerative disorders, and quercetin is a flavonol with well-documented radical-scavenging activity. Its pharmaceutical use is nevertheless limited because it belongs to class II of the Biopharmaceutics Classification System, with low aqueous solubility. A self-nanoemulsifying drug delivery system (SNEDDS) keeps such a compound molecularly dissolved in an isotropic oil–surfactant–cosurfactant mixture that disperses spontaneously into a nanoemulsion. To optimise the proportions of oleic acid, Tween 80 and PEG 400 in a quercetin SNEDDS and to verify the physical characteristics and antioxidant activity of the optimum formula. A laboratory experimental study using a D-optimal mixture design in Design-Expert® version 13, with oleic acid 10–20%, Tween 80 70–80% and PEG 400 10–20% constrained to a total of 100%, generating 16 runs. Preconcentrates were sonicated, homogenised, saturated with quercetin and centrifuged. Responses were drug load, emulsification time, particle size, polydispersity index, transmittance, zeta potential and DPPH IC₅₀. Quercetin was quantified by a validated UV-Vis method at 373 nm. Numerical optimisation minimised particle size, emulsification time, polydispersity index and IC₅₀ while maximising drug load, and the optimum formula was verified in triplicate. Responses varied widely across the 16 runs: drug load 1.043–16.755 mg/mL, emulsification time 18.6–34.8 s, particle size 90–220 nm, polydispersity index 0.138–0.245, transmittance 90.106–99.379%, zeta potential −20.5 to −38.0 mV and IC₅₀ 40.49–146.13 ppm. All five optimisation models were significant (p < 0.05) with non-significant lack of fit. Raising the oleic acid proportion enlarged droplets, lengthened emulsification and widened the size distribution, whereas Tween 80 acted in the opposite direction. The optimum formula was oleic acid 10%, Tween 80 80% and PEG 400 10%, with a desirability of 0.898. Verification gave a particle size of 90 ± 5 nm, polydispersity index 0.138 ± 0.006, drug load 13.377 ± 0.554 mg/mL and IC₅₀ 67.637 ± 0.957 ppm, none differing significantly from prediction (p > 0.05; prediction errors ≤ 0.85%). The optimised quercetin SNEDDS forms a nanometric, homogeneous and rapidly self-emulsifying system with strong antioxidant activity, and the mixture model is sufficiently predictive to guide further development.

Abstract

Nasopharyngeal carcinoma (NPC) is a malignancy with a high incidence in Southeast Asia and is frequently diagnosed at an advanced stage. Forkhead box M1 (FOXM1) is a transcription factor involved in cell-cycle progression, proliferation, invasion, migration, and metastasis. Previous studies have reported FOXM1 overexpression in NPC and its association with tumor progression; however, its relationship with clinical stage remains unclear. To determine the association between Forkhead Box M1 (FOXM1) expression and the stage of nasopharyngeal carcinoma. This analytic observational study used a cross-sectional design and was conducted at the Otorhinolaryngology outpatient clinic of Adam Malik Hospital, Medan, from 2022 to 2024. The study population comprised patients with histopathologically confirmed nasopharyngeal carcinoma. A total of 39 patients who fulfilled the inclusion and exclusion criteria were enrolled using a non-probability consecutive sampling technique. Histopathological examination was performed at the Department of Anatomical Pathology, Adam Malik Hospital, Medan, while FOXM1 mRNA expression was assessed at the Integrated Laboratory, Faculty of Medicine, Universitas Sumatera Utara, using real-time reverse transcription polymerase chain reaction (RT-qPCR) and expressed as ΔCt values. Clinical stage was determined according to the American Joint Committee on Cancer (AJCC) 8th edition TNM classification. Data were analyzed using univariate and bivariate analyses, and the Kruskal–Wallis test was performed to assess the relationship between FOXM1 expression and clinical stage. Among the 39 patients, 28 (71.8%) were male, and the most common age group was 41–60 years (64.1%). Most patients had advanced disease, with T4 being the most frequent primary tumor category (46.2%). The mean FOXM1 ΔCt value was 7.05 ± 3.38, with a median of 6.01 and a range of −1.96 to 13.86. Median FOXM1 ΔCt values were 8.78 (4.98–12.52) in patients with stage II–III disease, 6.28 (−1.96–13.86) in stage IVA, and 5.39 (4.92–12.20) in stage IVB. Although ΔCt values decreased progressively with increasing clinical stage, indicating a trend toward higher FOXM1 expression in advanced disease, the difference was not statistically significant (p = 0.140). There was no statistically significant relationship between FOXM1 mRNA expression and clinical stage of nasopharyngeal carcinoma. However, the progressive decrease in ΔCt values from earlier to more advanced stages suggests a potential trend toward increased FOXM1 expression with disease progression.

Abstract

Inadequate breast milk production and output is one of the problems that mothers can experience during the postpartum period and can be an obstacle to optimal breastfeeding. One of the efforts empirically used by the community to help facilitate breast milk production is the consumption of herbal drinks, one of which is turmeric and tamarind. This study aims to determine the effectiveness of administering turmeric and tamarind on breast milk production in postpartum mothers. This study used a quantitative method using a pre-experimental design with an approach (One Group Pre-Post Test Design). The population in this study amounted to 24 postpartum mothers with a sample of 20 respondents. Breast milk production was measured using an observation sheet by recording the volume of breast milk in milliliters (mL) before and after administering turmeric and tamarind. Data were analyzed univariately to describe the distribution of breast milk production and bivariate analysis using the Wilcoxon Signed-Rank test with a significance level of α = 0.05. The results of the study before being given turmeric tamarind on respondents' breast milk production were 10ml-30ml, after being given turmeric tamarind on respondents' breast milk production were 60ml-85ml and there was a significant difference in respondents before being given turmeric tamarind and after being given turmeric tamarind on breast milk production (P Value = 0.000). There was a significant difference in respondents before being given turmeric tamarind and after being given turmeric tamarind on breast milk production.

Abstract

Private hospitals in archipelagic regions—specifically Ambon City face challenges regarding limited access and resources, as well as healthcare service competition that necessitates marketing strategies grounded in quality and patient experience. Research mapping patient experience in private hospitals within archipelagic regions remains limited; thus, baseline data is required to develop contextual and sustainable marketing strategies. Objective to analyze factors associated with patient experience, specifically human resources (people), service processes (process), and facilities/infrastructure (physical evidence). A quantitative descriptive-analytic study using a cross-sectional design. The sample consisted of 150 patients who served as respondents and completed a Likert-scale questionnaire. The questionnaire employed a 1–5 Likert scale, with response options tailored to the specific statements, ranging from the lowest to the highest rating. Prior to primary data collection, the instrument underwent validity and reliability testing. Item validity was assessed using Corrected Item-Total Correlation with each statement item deemed valid and reliability was evaluated using Cronbach’s Alpha (threshold ≥0.70). Data were analyzed using  chisquare test. The study revealed that people, process, and physical evidence were significantly associated with patient experience (p = 0.000; p < 0.05). These findings indicate that the quality of human resources, the effectiveness of service processes, and the comfort and completeness of facilities are crucial components in shaping patient experience. These three aspects can serve as an empirical baseline for developing patient experience-based marketing strategies to enhance service quality, satisfaction, loyalty, and the competitiveness of private hospitals in archipelagic regions.

Abstract

Hypertension is a chronic condition requiring long-term therapy and medication adherence. Among older adults, adherence may be affected by physical limitations, regimen complexity, cognitive changes, medication beliefs, and family support. Family self-efficacy and caregiver readiness may therefore play important roles in maintaining antihypertensive medication adherence. This systematic review aimed to identify and synthesize evidence regarding the role of family self-efficacy and caregiver readiness in antihypertensive medication adherence among older adults with hypertension. A systematic review with narrative synthesis was conducted using articles retrieved from PubMed (n = 13), Scopus (n = 12), Google Scholar (n = 50), and ScienceDirect (n = 3). Studies published between [year] and [year] addressing hypertension, older adults, medication adherence, family and caregiver factors, and patient-family partnership were considered. Randomized controlled trials, observational studies, systematic reviews, and meta-analyses were included. Of 78 identified articles, 7 met the inclusion criteria and were included in the final synthesis. Family-based and self-management interventions improved medication adherence, self-efficacy, caregiver knowledge and behaviors, and blood pressure control. Observational evidence also demonstrated associations between self-efficacy, social support, and medication adherence. However, evidence directly examining caregiver readiness as a distinct construct remains limited. Family self-efficacy can strengthen caregivers' capacity to support medication adherence. Caregiver readiness may be a multidimensional construct encompassing knowledge, skills, self-efficacy, emotional readiness, communication abilities, and collaboration with patients.

Abstract

Gout arthritis is an inflammatory joint disease caused by the deposition of monosodium urate crystals resulting from hyperuricemia. It is characterized by joint pain, swelling, redness, stiffness, and limited mobility, which may reduce patients' quality of life. In addition to pharmacological treatment, acupressure is a complementary non-pharmacological therapy that may help relieve pain through stimulation of specific acupressure points. This case study aimed to evaluate the effectiveness of acupressure therapy as a complementary nursing intervention in reducing pain among patients with gout arthritis within the context of family nursing care. This case study involved one family with a member diagnosed with gout arthritis. Acupressure therapy was administered over three sessions by stimulating the SP6 (Sanyinjiao), LV3 (Taichong), LI4 (Hegu), KI1 (Yongquan), and KI3 (Taixi) points. Data were collected through interviews, direct observation, physical assessment, pain measurement using the Numeric Rating Scale (NRS), and serum uric acid measurements. Family caregiving ability and independence in performing acupressure were assessed through direct observation. Data were analyzed descriptively by comparing changes across the three sessions. Pain intensity decreased from 5 (moderate pain) to 2 (mild pain), while serum uric acid levels decreased from 6.8 mg/dL to 3.5 mg/dL. Improvements were also observed in joint swelling, stiffness, mobility, comfort, treatment adherence, and family independence in performing acupressure and providing home-based care. Acupressure therapy may serve as a beneficial complementary nursing intervention for reducing pain, improving physical condition, and enhancing family independence in caring for patients with gout arthritis.
