Abstract

Bacterial infections are a significant global health challenge because they can attack various body tissues and cause illnesses ranging from mild to severe. Staphylococcus epidermidis is a bacterium that often causes various infections. Infection by Staphylococcus epidermidis can slow down the wound healing process and carry the risk of serious complications if not properly managed. This study aims to formulate and optimise an antibacterial gel from the ethyl acetate fraction of purple sweet potato leaves (Ipomoea batatas L.) as a natural alternative for treating skin infections caused by the bacterium Staphylococcus epidermidis. Using the I-optimal mixture design method in Design-Expert software, the optimization analysis was performed by evaluating several responses, including pH, viscosity, spreadability, and adhesion of the gel formulation. The experimental data were fitted to an appropriate polynomial model and analyzed using ANOVA to determine the significance of the model and mixture components. Numerical optimization was then carried out by setting the desired criteria for each response to obtain the optimum formulation. The optimal formulation consisted of 2% HPMC and 13% propylene glycol, with a desirability value of 0.964. The results showed that this formulation met all the criteria for a good and stable gel, including organoleptic properties (semi-solid, brownish-green, characteristic odour, homogeneous), pH (5.81), viscosity (2316.52 cPs), spreadability (6.4 cm), and adhesion time (2.18 seconds). The optimal formulation for an antibacterial gel containing the ethyl acetate fraction of purple sweet potato leaves (Ipomoea batatas L.) was achieved using a 2% HPMC concentration and 13% propylene glycol, with a desirability value of 0.964, , this gel formulation is also effective and falls into the category of being highly potent in inhibiting the growth of Staphylococcus epidermidis, as evidenced by the formation of an inhibition zone of 23 mm.

Abstract

Rheumatoid arthritis is a chronic autoimmune disease characterised by persistent inflammation and functional impairment. Methotrexate remains the cornerstone therapy; however, its effects on physical function have not been consistently evaluated. To evaluate the effectiveness of methotrexate-based therapy on inflammatory control and physical function in patients with rheumatoid arthritis. This systematic review followed PRISMA guidelines. Literature searches were conducted in PubMed, Scopus, the Cochrane Library, SpringerLink, and ScienceDirect for studies published between 2016 and 2025. Eligible studies were randomised controlled trials involving adult patients with rheumatoid arthritis receiving methotrexate as monotherapy or combination therapy. The primary outcome was physical function assessed using the Health Assessment Questionnaire Disability Index (HAQ-DI), while secondary outcomes included disease activity measures. Eight randomised controlled trials were included. Methotrexate-based therapy consistently reduced disease activity across studies, with combination regimens generally demonstrating greater inflammatory suppression than monotherapy. Improvements in physical function were observed in several studies, particularly in combination therapy groups; however, these improvements were not consistently proportional to reductions in inflammatory activity. Functional outcomes were also reported heterogeneously across studies. Methotrexate-based therapy is effective in controlling inflammatory activity in rheumatoid arthritis but does not consistently translate into proportional improvements in physical function. Persistent impairment may also be influenced by structural joint damage, chronic pain, fatigue, and psychosocial factors.

Abstract

Schizophrenia is a severe mental disorder often accompanied by symptoms of social isolation, leading to a decline in interaction abilities and negatively impacting patients' quality of life. At Soeharto Heerdjan Hospital, social isolation is one of the most common nursing diagnoses. Social Skill Training (SST) has been proven effective in improving patients' socialization skills; however, its specific impact on quality of life has not been extensively studied. This study aims to analyze the effect of Social Skill Training on the quality of life of clients with schizophrenia. The study employed a quasi-experimental design—specifically a "before-and-after with control" design—by administering SST to respondents. The sample consisted of 60 clients with schizophrenia: 30 received generalist therapy, while the other 30 received both generalist therapy and Social Skill Training. Quality of life was measured using the WHOQOL instrument and analyzed using the independent sample t-test, and and is a standard instrument.  An improvement in quality of life was observed following the administration of Social Skill Training. Analysis results indicated a significant effect of SST on the quality of life of clients with schizophrenia (p < 0.05). Social Skill Training influences the improvement of quality of life in clients with schizophrenia.

Abstract

Diabetic neuropathy is a chronic complication of diabetes mellitus that often remains undetected in its early stages and may progress to foot ulcers and amputation. Early detection at the primary healthcare level is essential to prevent further complications. This study aimed to analyze the effectiveness of the Comprehensive Diabetic Neuropathy Screening Algorithm (CDNSA) compared with a biothesiometer as the reference standard in detecting diabetic neuropathy among patients with diabetes mellitus at Soromandi Primary Healthcare Center, Indonesia. This quantitative diagnostic test accuracy study used a cross-sectional approach. A total of 64 patients with diabetes mellitus who met the inclusion and exclusion criteria were recruited using consecutive sampling. Data were collected in 2025 through direct clinical assessments using both the CDNSA and a biothesiometer. The CDNSA assessed large-fiber function using a 10-g monofilament and a 128-Hz tuning fork and small-fiber function using pinprick and cold sensation tests, whereas the biothesiometer measured the Vibration Perception Threshold (VPT). Diagnostic performance was evaluated using sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), overall accuracy, and Receiver Operating Characteristic (ROC) curve analysis. CDNSA demonstrated a sensitivity of 94%, specificity of 100%, PPV of 100%, NPV of 80%, and overall diagnostic accuracy of 100%. The Area Under the Curve (AUC) was 97.1%, indicating excellent discriminatory ability in distinguishing patients with and without diabetic neuropathy. The CDNSA demonstrated excellent diagnostic performance and may serve as a practical and comprehensive screening method for the early detection of diabetic neuropathy in primary healthcare settings.

Abstract

Hypertension It is not merely a matter of blood pressure control, but also of chronic diseases that can affect a patient's ability to manage their own care and their quality of life. To determine the relationship between self-efficacy and quality ogf life in hypertession patient. This cross-sectional study involved 261 hypertensive patients in selabatu sukabumi, who were selected using stratified random sampling. Data were collected using validated and reliable questionnaires, with a validity index p-value < 0.05 and reliability > 0.70, namely the WHOQOL-BREF, quality of life based on the Likert scale. Data were analyzed using somers’D statistical test,  test to identify the relationship between variables. Results: The results of the analysis showed a significant relationship between self-efficacy (p = < 0.001) and quality of life  with the self-efficacy of hypertension patients. There was a significant relationship between self-efficacy and quality of life  with hypertension patients in selabatu sukabumi Regency.

Abstract

Diabetes mellitus (DM) requires adequate patient knowledge to support effective self-management and prevent complications. Video-based education provides an audiovisual learning method that may improve patients' understanding of diabetes management. To examine the effect of video-based education on diabetes mellitus management knowledge among patients attending the Cempaka Putih Community Health Center, Central Jakarta. A quantitative quasi-experimental study with a pretest–posttest control group design was conducted from September to October 2025. A total of 68 patients with DM were recruited using purposive sampling and allocated into intervention (n=34) and control (n=34) groups. The intervention group received standardized video-based education on diabetes management, whereas the control group received routine health education. Knowledge was measured using a 20-item questionnaire before and after the intervention. Data were analyzed using paired-samples t-tests, independent-samples t-tests, and multiple linear regression. Baseline knowledge scores were comparable between the intervention (9.68 ± 2.47) and control groups (9.71 ± 3.79). Following the intervention, the mean knowledge score increased significantly to 14.09 ± 1.54 in the intervention group (p < 0.001), whereas no significant change was observed in the control group (9.76 ± 3.39; p = 0.737). The mean knowledge improvement differed significantly between groups (4.41 ± 2.84 vs. 0.06 ± 1.01; p < 0.001). Age significantly influenced knowledge improvement (p = 0.001), while sex, education, and duration of DM were not significant predictors. Video-based education effectively improved diabetes management knowledge and may be integrated into primary healthcare education programs for patients with DM.

Abstract

Maternal and neonatal emergencies such as postpartum hemorrhage, severe preeclampsia/eclampsia, and neonatal asphyxia require prompt and accurate pharmacological management. However, midwifery students' pharmacology competence in managing emergencies is often limited to theoretical classroom learning, so a more applicable learning method such as case-based simulation is needed. This study aimed to analyze the effect of case-based maternal and neonatal emergency simulation on the pharmacology competence of midwifery students at STIKes Yahya Bima. This study used a quantitative quasi-experimental design with a one-group pretest-posttest approach. The sample consisted of 42 third-year Diploma III Midwifery students selected using a total sampling technique. Data were collected using a 30-item maternal and neonatal emergency pharmacology competence test, which had been tested for validity using the Pearson correlation test and was declared reliable with a Cronbach's alpha value of 0.842. Data were analyzed using the Shapiro-Wilk normality test and the Paired t-test. The results showed that the mean pharmacology competence score before the simulation was 58.4 (SD=9.2), increasing to 82.7 (SD=7.5) after the simulation, with a mean difference of 24.3 points (p=0.000). The proportion of students in the good competence category increased from 9.5% to 57.1% after the intervention. Further analysis showed that the increase in scores was significantly higher among students with a grade point average (GPA) ≥3.00 (p=0.031), while age and prior clinical practice experience showed no significant difference. Conclusion: Case-based maternal and neonatal emergency simulation has a significant effect on improving the pharmacology competence of midwifery students.

Abstract

Diabetic neuropathy is one of the most common chronic complications in patients with type 2 diabetes mellitus and may lead to foot ulcers and amputation. Peripheral circulation disorders, tissue oxygenation impairment, and poor glycemic control are considered contributing factors. To determine the relationship between oxygen saturation (SpO₂), Ankle–Brachial Index (ABI), and HbA1c levels with diabetic neuropathy among patients with type 2 diabetes mellitus. A quantitative descriptive-analytic cross-sectional study was conducted among 60 patients with type 2 diabetes mellitus at Lantana Medika Clinic, South Jakarta. Participants were recruited using consecutive sampling based on predefined inclusion and exclusion criteria. Data were collected through face-to-face interviews, structured questionnaires, direct clinical assessments, and medical record review. SpO₂ was measured using a pulse oximeter, ABI was determined using a sphygmomanometer and handheld Doppler device, and HbA1c values were obtained from the participants' most recent laboratory results. Diabetic neuropathy was assessed using the Comprehensive Diabetic Neuropathy Screening Algorithm (CDNSA). Data were analyzed using descriptive statistics, Chi-square tests, and binary logistic regression. There was a significant relationship between duration of diabetes and HbA1c levels with diabetic neuropathy (p < 0.05). Multivariate analysis identified HbA1c as the most dominant factor associated with diabetic neuropathy. HbA1c and duration of diabetes are significantly associated with diabetic neuropathy. A comprehensive screening approach incorporating tissue oxygenation, peripheral circulation, and glycemic control assessment may enhance the early detection of diabetic neuropathy in primary healthcare settings.

Abstract

Academic stress is one of the problems that nursing students often experience due to academic demands, clinical practice, and learning loads. Poor stress management can be associated with changes in health behaviors, such as sleep patterns, diet, physical activity, and daily habits that have the potential to affect kidney health. This study aimed to analyze the relationship between academic stress levels and unhealthy behaviors and kidney health risks in nursing students. The study used a quantitative design with a correlational approach. The research was conducted in July 2026 in Blora. Respondents were 160 nursing students who met the research criteria. The sampling technique used simple random sampling. Data was collected using instruments measuring academic stress levels, unhealthy behaviors, and kidney health risks. The instruments were valid and reliable. The results showed that there was variation in academic stress levels, health behaviors, and kidney health risks in nursing students. The analysis showed that academic stress level was significantly related to unhealthy behavior. The findings of the study also show a link between health behavioral factors and kidney health risks. The conclusion is that academic stress and health behaviors are pivotal aspects in nursing students’ health. Promotive and preventive efforts through stress management, the implementation of healthy living behaviors, and education on the maintenance of kidney health need to be developed in a sustainable manner in the nursing education environment.

Abstract

Preterm birth is a maternal-neonatal health problem that contributes to morbidity, mortality, and long-term healthcare needs. This study aimed to develop and conduct preliminary internal validation of a preterm birth prediction model based on chronic energy deficiency (KEK) status and anaemia among pregnant women in Bima City. This quantitative analytical study developed a prediction model for preterm birth using logistic regression. A total of 44 pregnant women were recruited through purposive sampling, based on predetermined inclusion criteria and the availability of complete data on birth outcomes, chronic energy deficiency (CED/KEK) status, anaemia, maternal age, parity, and antenatal care (ANC) visits. The analyses comprised descriptive and bivariate analyses, multivariable logistic regression, discrimination assessment using the receiver operating characteristic-area under the curve (ROC-AUC), calibration assessment using the Brier Score, threshold determination using the Youden Index, and internal validation through bootstrap resampling. Five of 44 respondents (11,36%) experienced preterm birth. The main model yielded the equation Logit(P) = -2,9517 + 0,2041(KEK) + 3,3171(Anemia). KEK yielded an adjusted odds ratio (AOR) of 1,226 (95% CI 0,069-21,715; p=0,889), while anaemia yielded an AOR of 27,579 (95% CI 2,800-271,677; p=0,004). The apparent ROC-AUC of 0,7487 decreased to 0,6725 after optimism correction using bootstrap. The apparent Brier Score was 0,0697. At a threshold of 0,5903, model sensitivity was 60,0% and specificity was 94,9%. The combination of KEK and anaemia can be formulated into a probabilistic model for predicting preterm birth, with anaemia providing the strongest predictive signal in the study sample. However, the limited number of preterm birth events resulted in unstable estimates and performance optimism. The model should be positioned as a pilot model and requires further development in a larger sample and external validation before being used for risk stratification in ANC services.
