Abstract

Maxillofacial trauma frequently requires open reduction and internal fixation (ORIF) using various implant systems that may increase operative costs, particularly in complex fractures. This study aimed to analyze the relationship between fracture complexity and implant costs in maxillofacial trauma patients undergoing ORIF. A retrospective cross-sectional study was conducted using retrospective medical records from 35 patients treated at PKU Muhammadiyah Karanganyar Hospital during 2025–2026. Statistical analysis was performed using the Shapiro–Wilk test and the Independent Samples T-Test. Most patients were male (80.0%) with a mean age of 36.86 ± 18.44 years. Simple fractures accounted for 57.1% of cases, whereas complex fractures accounted for 42.9%. A total of 66 implant components were used during ORIF procedures, with AT-DRV screws accounting for the largest proportion (69.7%). while multiple-point fixation was the most common fixation strategy (40.0%). The mean implant cost was 6.80 ± 3.76 million rupiahs. Statistical analysis showed no statistically significant association between fracture complexity and implant cost (p = 0.845). However, interpretation is limited by the retrospective design and small sample size (n = 35) may have limited the statistical power to detect a true association. Furthermore, the retrospective study design limits causal inference and depends on the completeness and accuracy of the available medical records.

Abstract

Chronic diseases are a significant burden on healthcare systems, especially in remote areas where access to healthcare is limited. Tele-nursing, an emerging solution involving remote nursing care, has the potential to improve patient outcomes by providing continuous monitoring and support. This study aims to evaluate the effectiveness of tele-nursing in improving the quality of life for chronic disease patients in rural settings. A mixed-methods approach was utilized, incorporating quantitative assessments of patient quality of life through the WHOQOL-BREF questionnaire and qualitative interviews with patients and healthcare providers. Results indicated that patients who received tele-nursing interventions showed a 20% improvement in overall health outcomes and a 25% increase in engagement compared to those receiving traditional care. Additionally, patients reported enhanced satisfaction with their care, feeling more connected to their healthcare providers and empowered to manage their conditions. The study concludes that tele-nursing is an effective tool in improving the quality of life for chronic disease patients in remote areas by providing personalized, accessible care. It highlights the need for further research to explore the long-term sustainability of tele-nursing and its integration into healthcare systems globally.

Abstract

The acceleration of globalization compels hospitals to enhance patient loyalty through high-quality services. This study aims to analyze the effect of prescription waiting time and service quality on patient loyalty mediated by patient satisfaction at the Outpatient Pharmacy Department of RSUD Sanjiwani Gianyar. This research design is an explanatory quantitative study rooted in the positivistic paradigm with a developmental research approach. The population comprised all outpatients, averaging 7,836 visits over a six month baseline. A sample of 381 respondents was selected using a non probability purposive sampling technique. Data were collected via structured questionnaires and analyzed using Variance-Based Structural Equation Modeling (SEM) via SmartPLS 4.0. The structural path analysis revealed that waiting time had no significant effect on patient satisfaction or patient loyalty thus, the mediating role of satisfaction on this path was rejected . Conversely, service quality exerted an exceptionally strong, positive, and significant effect on patient satisfaction and directly driven patient loyalty . Furthermore, patient satisfaction significantly and strongly mediated the relationship between service quality and loyalty. Establishing a partial mediation model. The empathy dimension emerged as the primary catalyst that psychologically buffered technical waiting time deficits. Hospital management should heavily focus on emotional care and interpersonal staff interactions to secure sustainable, long-term patient loyalty.

Abstract

Research Objective: This study intends to investigate and explain the impact of service quality and medicine availability on patient satisfaction, both directly and indirectly, using patient trust as a mediating variable at RSUD Natuna’s Pharmacy Installation. Research Type: This study is quantitative explanatory research. Data were collected through questionnaires distributed to 100 outpatient respondents. The data analysis technique used was Structural Equation Modeling (SEM) based on Partial Least Squares (PLS), implemented in SmartPLS. Research Findings: The results indicate that service quality positively impacts patient trust and satisfaction. The availability of drugs has been demonstrated to boost patient trust, although it has no significant direct impact on patient satisfaction. Furthermore, patient trust was found to positively affect satisfaction and to successfully mediate the relationship between service quality and satisfaction. However, trust cannot mediate the relationship between medicine availability and patient satisfaction, as low medicine stock acts as a physical barrier that cannot be compensated for by the emotional factor of trust alone.

Abstract

The way a hospital looks plays a positive part in forming patients’ views and affecting their choices to seek medical care. This study seeks to investigate the impact of service quality on hospital image, as mediated by patient satisfaction and trust. This study’s population consisted of 351 outpatients from Rumah Sakit Permata Bekasi. Questionnaires were used to data collect, which was then analyzed using SEM-PLS software’s Partial Least Squares (PLS) approach. The results indicate that service quality has a substantial effect on patient satisfaction, service quality substantial affects patient trust, good service quality substantial impacts the hospital image, patient satisfaction does not substantial influence hospital image, patient trust substantial affects hospital image, patient satisfaction does not mediate the relationship between service quality and hospital image, and patient trust substantial mediates the Based on these findings, the study suggests that hospitals should boost broader factors such as communication management and hospital image management on social media, which can improve public opinion of the hospital while also optimizing service quality and overall hospital image.

Abstract

The emergence of nano-medicine has introduced transformative approaches for the treatment of degenerative diseases, including Parkinson’s, Alzheimer’s, and osteoarthritis. These technologies enable targeted drug delivery, enhanced bioavailability, and improved therapeutic precision. Despite clinical potential, ethical challenges related to patient comprehension, informed consent, and long-term safety remain underexplored, particularly for vulnerable populations affected by cognitive decline. Understanding these challenges is critical for aligning technological innovation with bioethical principles. This study aims to investigate the bioethical issues arising from the use of nano-medicine in degenerative disease therapy and to identify strategies that enhance ethical implementation in clinical practice. The research further seeks to develop a conceptual framework to guide ethical decision-making in emerging medical technologies. A qualitative exploratory design was employed, including fifteen participants comprising clinicians, patients, and regulatory authorities. Data were collected through semi-structured interviews, direct observation of clinical interactions, and document analysis of protocols and consent forms. Thematic coding and triangulation were applied to elucidate patterns in ethical practices, communication strategies, and institutional oversight. Findings indicate that patient comprehension varies significantly with age and cognitive status, while clinician strategies and institutional protocols influence ethical adherence. Iterative consent processes and technological support enhance transparency, autonomy, and responsible clinical practice.

Abstract

This study aims to examine the relationship between coping strategies and anxiety levels among third-trimester pregnant women at Baitussalam Public Health Center, Aceh Besar Regency. This research employed a quantitative analytic design with a cross-sectional approach, selected for its efficiency in measuring associations between variables at a single point in time. The population consisted of 67 pregnant women, and a sample of 40 respondents was selected using the Slovin formula with a 10% margin of error, through purposive sampling. Data were collected using the Brief COPE questionnaire to assess coping strategies and the Hamilton Anxiety Rating Scale (HARS) to measure anxiety levels, both of which have demonstrated good validity and reliability in prior studies. The results showed that 65.0% of respondents used adaptive coping strategies, while 35.0% applied maladaptive coping. In terms of anxiety, 45.0% of respondents reported no anxiety, whereas 55.0% experienced varying levels of anxiety, predominantly mild. Statistical analysis using the Chi-square test revealed a significant relationship between coping strategies and anxiety levels (p = 0.009). The novelty of this study lies in highlighting the role of coping strategies as a key psychological determinant in maternal mental health during late pregnancy in a local Indonesian setting. The findings imply that strengthening adaptive coping through antenatal education and psychosocial support can effectively reduce anxiety and improve maternal well-being. This study contributes to the development of maternal health science, particularly in integrating psychological aspects into comprehensive antenatal care services.

Abstract

Inhibiting carbohydrate-digesting enzymes such ?-amylase may help regulate type 2 diabetes mellitus, a long-term metabolic condition marked by continuous high blood sugar levels. The purpose of this research was to examine the phytochemical composition and in vitro ?-amylase inhibitory action of a mixture of yacon leaf (Smallanthus sonchifolius) and karas tulang root (Chloranthus erectus) ethanolic extracts. The plant materials were macerated in 70% ethanol to remove their constituent parts, and then the two extracts were mixed in a weight-for-weight ratio of 1:1. The DNS technique was used to assess the ?-amylase inhibitory activity, with acarbose as the positive control, and thin-layer chromatography was used for phytochemical screening. The yield of yacon leaves was 14.2% and that of karas tulang roots was 11.4% when extracted. There were terpenoids, polyphenols, alkaloids, flavonoids, and polyphenols found in the phytochemical screening. The combined extract exhibited an increase in ?-amylase inhibition ratio of 34.528 percent at 20 ppm to 90.359 percent at 200 ppm, which was concentration dependant. The combined extract had an IC?? value of 63.298 ± 0.162 ppm, while acarbose had an IC?? value of 43.183 ± 1.335 ppm. The inhibitory action of the combined extract was encouraging, even if it was not as strong as acarbose. These results indicate that a possible natural antidiabetic candidate might be created by combining yacon leaves and karas tulang roots via ?-amylase inhibition..

Abstract

Meningitis is a life-threatening neurological infection requiring rapid diagnosis and management. Clinical symptoms often overlap between bacterial, viral, and tuberculous causes, making early differentiation challenging. The Bacterial Meningitis Score (BMS) was developed to support early risk stratification. This study evaluates the diagnostic value and prognostic relevance of BMS in suspected meningitis cases at RSUD Dr. Soetomo Surabaya. A retrospective cross-sectional study was conducted using medical records of patients suspected of meningitis at RSUD Dr. Soetomo from 2023–2025. BMS components and clinical data were analyzed, including cerebrospinal fluid findings, peripheral blood results, and clinical manifestations. Diagnostic accuracy and prognostic outcomes were assessed using descriptive and comparative analysis. Among 43 patients, bacterial meningitis accounted for 25%, while tuberculous meningitis (37.5%), other neurological disorders (31.25%), and viral meningitis (6.25%) were more prevalent. Most patients were classified as low risk based on BMS. The score demonstrated better performance in ruling out bacterial meningitis than confirming it, supported by a high negative predictive value. However, diagnostic accuracy was limited by overlapping clinical features and incomplete cerebrospinal fluid data. Prognosis was more strongly associated with Glasgow Coma Scale (GCS), where lower scores correlated with poorer outcomes and increased mortality risk. BMS is useful for early risk stratification and exclusion of bacterial meningitis but must be combined with clinical and laboratory evaluation to improve diagnostic accuracy and patient outcomes.

Abstract

Universal Health Coverage in Deli Serdang Regency has reached 98.6%. However, JKN participant activity and primary health care utilization in several areas remain below the national target. At UPT Puskesmas Batang Kuis, JKN utilization by participants has reached only 67.2% of total users. This study modifies Andersen’s theoretical model by incorporating contemporary digitalization indicators (use of the Mobile JKN application and digital queue system) within the knowledge assessment instrument. It aims to analyze factors associated with health service utilization among JKN participants. This quantitative study used a cross-sectional design with 96 respondents selected through simple random sampling. Data were collected using a structured questionnaire tested for validity and reliability, then analyzed using the chi-square test and binary logistic regression. Univariate analysis showed that 51.0% of respondents utilized health services. Individual assessment of the health center was the dominant factor (Exp(B)=13.741). Positive attitudes, family support, and good perceptions of facility quality were the main determinants, while knowledge alone was insufficient. Health centers should improve physical service quality, staff responsiveness, and medicine availability.
