Abstract

Depression is a mental disorder that is often found in hemodialysis patients. Depression had bad effects on patients. The study that determines the relationship between depression and all-cause mortality risk in hemodialysis patients is debatable. Objectives: To determine the role of depression as a predictor factor of all-cause mortality within one year in hemodialysis patients. Methods: This cohort study was conducted from February to August 2020 at the hemodialysis unit of Roemani Muhammadiyah Hospital, Semarang. The inclusion criteria were undergoing hemodialysis ≥3 months, hemodialysis frequency twice a week, being willing to participate in this study, being able to communicate well, and there was no history of previous mental disorders. Meanwhile, those whose data were incomplete, transferred to another hemodialysis unit, HBSAg +, and had other stressors unrelated to hemodialysis or Chronic Kidney Disease (CKD) were excluded from this study. Depression was assessed using the Beck Depression Inventory-II questionnaire. The collected data was then analyzed using Kaplan Meier and Cox regression. Results: From 82 respondents, 20 (24,4%) respondents categorized into depression group and 62 (75,6%) respondents into non depression group. Cox Regression analysis showed that depression plays a role as a predictor factor of all-cause mortality within one year in hemodialysis patients (p value=0,007, and Hazard ratio=3,587). Conclusion: Depression plays a role as a predictor factor of all causes of mortality within one year in hemodialysis patients.Keywords: depression, hemodialysis, mortality, end-stage renal disease

Abstract

Basic complete immunization coverage has decreased during the COVID-19 pandemic in Pesisir Selatan Regency. Objectives: To analyzed related factors of acceptance of basic complete immunization programs during the COVID-19 pandemic in Pesisir Selatan Regency. Methods: This research used qualitative methods with explanative design in three clusters of health center areas with 25 informants. The data were analyzed using the triangulation method through in-depth interviews and Focus Group Discussions (FGD). Results: The complete basic immunization has decreased in coverage during the COVID-19 pandemic because of the implementation of un maximum implementation due to lack of personnel accompanied by limited information media in remote areas causing lack of socialization, budget efficiency and no cooperation in each cluster, perceptions of halalness in rural areas, busyness parents in urban areas, non-compliance with health protocols in rural and remote areas, perceptions of no benefit in urban and remote areas, worry and fear of contamination COVID-19 in each cluster. Conclusion: The diversity of perceptions of each cluster causes a coverage has decreased of complete basic immunization during the COVID-19 pandemic in Pesisir Selatan Regency so that clustering on the implementation of complete basic immunization is needed.Keywords: complete basic immunization, coverage achievements, COVID-19

Abstract

Miliary tuberculosis occurs due to the hematogenous spread of Mycobacterium Tuberculosis from the primary complex.  The use of steroid and cytotoxic drugs increases the incidence of miliary tuberculosis.  Typical manifestations of miliary tuberculosis is snowstorm appearance seen on chest x-ray and evidence of tuberculosis microorganism from the microbiological examination.  It has been reported a 33 years old male patient was admitted to the hospital due to breathlessness, chronic coughing, fever, anorexia, weight loss and night sweating with a "damp shadow sign".  Due to chronic myelogenous leukemia and autoimmune hemolytic anemia, the patient is known under imatinib and steroid therapy.  There was no specific sign found from the physical examination.  Chest x-ray showed snowstorm appearance.  The patient underwent GeneXpert MTB/RIF, with the result low MTB detected.  The patient was treated with a 2(HRZE)/4(HR) tuberculosis drugs regimen.  Imatinib and steroid therapy was discontinued.  2-4 weeks of steroid usage with a daily dose equivalent to prednisone 15 mg increases the risk of activating a latent tuberculosis infection.  Imatinib affects the response of T cells to Mycobacterium, thereby triggering tuberculosis reactivation.  In this case, the diagnosis has been made from typical manifestations of tuberculosis, snowstorm appearance from the chest x-ray, and low MTB detected from GeneXpert MTB/RIF.  It has been recommended to detect latent tuberculosis infection before using steroid and cytotoxic drugs.Keywords:  imatinib, miliary tuberculosis,  steroid

Abstract

Rapid and accurate measurement of capillary glucose level using point-of-care testing (POCT) is needed to maintain the patient’s normoglycemic status in obtaining adequate management. The glucose POCT method should be evaluated to determine analytical performance by comparing with the hexokinase method as a reference method to provide accurate and reliable results. Objectives: To determined the correlation between capillary glucose using glucose dehydrogenase-nicotinamide adenine dinucleotide (GDH-NAD)  and serum glucose hexokinase methods. Methods:  This analytic cross-sectional study on 42 outpatients who underwent fasting blood glucose examination at Dr. M Djamil Padang General Hospital. This study was conducted from February until September 2020. Capillary fasting blood glucose was measured using glucose POCT GDH-NAD method and serum glucose with hexokinase method. The Spearman correlation test was used to analyze data, significant if p<0.05. Results: The subjects were 28 male (66.7%),14 female (33.3%) with mean age and hematocrit level was 56.12±12.97 years and 40.90±2.42%, respectively. The median capillary glucose GDH-NAD method and serum glucose hexokinase method were 100.00 mg/dL each, with a median difference was 3.00 mg/dL. Spearman correlation test showed very strong positive correlation and statistically significant (r=0,961;p=<0,001). Conclusion: Capillary glucose GDH-NAD method had a very strong positive correlation with serum glucose hexokinase method.Keywords:  hexokinase, fasting glucose level, GDH-NAD, glucose POCT

Abstract

The timeliness of Measles-Rubella (MR) immunization is still low among other immunization services for infants, including at the Tempuran Health Center, Magelang Regency. Attitude is something that influences a person's actions. Objectives: To analyzed the mother's attitude towards MR immunization services on time during the covid-19 pandemic in the working area of the Tempuran Health Center, Magelang Regency. Methods: It was quantitative research with a cross-sectional approach. The research population was mothers who have babies aged 9-12 months with a sample of 100 mothers using cluster sampling technique in each village. The research instrument was a questionnaire. The data were analyzed univariately and bivariate with the Chi-square test. Results: The timeliness of MR immunization was still low. 74% of mothers were not punctual in immunizing MR to their children even though most mothers had a good attitude (65%). The Chi-square test showed a relationship between the mother's attitude and timeliness of MR immunization (p=0.004). Conclusion: The mother's attitude was not optimal because there were various problems with the mother's attitude, so it is hoped that the mother will increase knowledge, share experiences with peer groups, and if unable to attend, ask for family assistance, while health center is expected to carry out promotions, education about measles-rubella immunization and also coordinate with health office regarding vaccine stockKeywords:  immunization, measles-rubella, timeliness

Abstract

Preoperative anxiety is frequently correlated with anesthesia procedures, resulting in the anesthesiologist being the most suitable person to decrease the patient's anxiety through effective doctor–patient communication during pre–anesthesia visits. During anxiety, one of the body responses is the release of epinephrine to the blood vessel resulting in increased blood pressure. Objectives: To knew the correlation between pre-anesthesia visit anxiety and blood pressure before anesthesia procedure. Methods: This study used a cross-sectional design that was conducted at Universitas Sumatera Utara General Hospital from September until October 2019. The sample was collected by consecutive sampling techniques, which meet inclusion and exclusion criteria. This study used primary data then was analyzed using Fisher’s Exact Test. Results: From 67 patients going through elective surgery, there are 41 patients (61,2%) with mild anxiety, 17 patients (25,4%) with moderate anxiety, and nine people (13,4%) with severe anxiety. There is no significant correlation (p>0,05) between anxiety with sex, types of anesthesia, PS ASA and heart rate. There is a significant correlation (p<0,05) between anxiety with blood pressure and types of surgery. Conclusion: There is a correlation between anxiety during the pre-anesthesia visit with blood pressure before anesthesia procedures.Keywords:  anesthesia, anxiety, blood pressure, pre-anesthesia visit

Abstract

Breast cancer is a condition in which cells have undergone normal repairs, resulting in abnormal growth. In Indonesia, patients with metastatic cancer (stage IV) tend to have a lot of complaints and complications due to cancer they experience. Pain is the main complaint that is often complained of. Objectives: To determined the relationship between the degree of pain in BPI (Brief Pain Inventory) and the stage of breast cancer. Methods:  Research conducted is analytic with cross-sectional design. The study sample was patients diagnosed with breast cancer in RSUP HAM who had met the inclusion and exclusion criteria based on the total sampling method. Research data are primary data taken using interview techniques. The research instrument was the BPI questionnaire. BPI is a measure of pain experience that measures the severity of pain and disruption of daily functioning due to the pain experienced by patients. Measured using a ratio scale where the score of 0 is the lowest and the ten highest scores have the worst pain intensity. Results: The analysis of the Chi-square test showed a significant relationship between the degree of pain and the stage of breast cancer in RSUP HAM with a p= 0,01 (p<0,05. Conclusion: There is a relationship between the degree of pain and the stage of breast cancer.Keywords:  BPI, breast cancer, pain, stage

Abstract

Vitamin C, as one of the antioxidants in energy drinks, plays an essential role in physical endurance. Modified Harvard step test is a cardiac endurance test to assess cardiac fitness based on VO2 max, heart rate recovery, and recovery period. Objectives: assessed the relationship between giving vitamin C before exercising with heart-lung fitness in the form of VO2 max, recovery of pulse rate, and recovery period. Methods: The design of this study was a pretest-posttest study with a control group of 40 male students aged 18-22 years old. One week before administrating vitamin C, the VO2 max of all subjects was measured using the modified Harvard step test. The subjects were divided into two groups, the vitamin C group, which had vitamin C drinks prior to the step test and the control group only had plain water. Results: There was no significant difference in VO2max, Heart Rate Recovery (HRR) 1 minute, HRR 2 minute, and recovery time before and after consuming vitamin C between groups with p value = 0.090, 0.181, 0.211, and 0.95, respectively (p > 0.05). This study showed that there was no difference in VO2 max before and after vitamin C consumption. Conclusion: Vitamin C drinks consumption before the step test cannot increase the VO2 max, heart rate recovery, and recovery time.Keywords:  heart rate recovery, recovery time, vitamin C, VO2 max

Abstract

Chronic Kidney Disease (CKD) will cause anemia which can lead to death. CKD patients undergoing hemodialysis therapy can also experience anemia. Therefore, it is necessary to check the erythrocyte index consisting of Mean Corpuscular Volume (MCV), Mean Corpuscular Hemoglobin (MCH), and Mean Corpuscular Hemoglobin Concentration (MCHC) to determine the type of anemia so that appropriate therapy can be given. Objectives: To determined the description of the erythrocyte index in CKD patients undergoing hemodialysis at H. Adam Malik General Hospital Medan. Methods: This research used the descriptive observational method with a cross-sectional design. The study population was all CKD patients who underwent hemodialysis at the Adam Malik General Hospital Medan period July 2018 until June  2019 according to inclusion and exclusion criteria (total sampling) using medical record data. Results: From 149 study samples, 92 people (61.7%) were male, 57 people were female (38.3%), more in the age range of 55-64 years, hemoglobin (Hb) on average in low grades (98.9% men, 94.7% women), and erythrocyte index (MCV, MCH, MCHC) averages in the normal range. Conclusion: The description of patients with chronic kidney disease undergoing hemodialysis at H. Adam Malik General Hospital in July 2018 - June 2019, in general, is normochromic normocytic anemia.Keywords:  CKD, erythrocyte index, Hb, hemodialysis

Abstract

The anatomical variations of the nasal and sinus paranasal can cause headaches. The most common variants have deviated septum, concha bullosa, and agger nasi cells. These variations can cause narrowing of the osteomeatal complex. Computed tomography was the primary modality for evaluating anatomical variations of the nasal and paranasal sinuses in diagnosis and management. Functional endoscopic sinus surgery was generally performed to restore the sinuses' drainage and ventilation in anatomical variations. It has been reported a 45-year-old woman with a congested left nostril and headache that has gotten worse since 2 months ago and computed tomography of the paranasal sinuses show thickening of the bilateral maxillary sinus mucosa, bilateral anterior ethmoid, left septum deviation, concha bullous, and right inferior turbinate hypertrophy. Functional Endoscopy Sinus Surgery was performed, found anatomical variations namely, deviation septum, concha bullosa, agger nasi cells and supra agger cells. The operation gave satisfactory results. Correct evaluation of anatomical variations based on history, physical examination and investigations can assist in patient management and avoid errors during surgery. The operation gave a satisfactory result for managing symptoms on patient with anatomical variation of nasal and paranasal sinuses when compared with medicamentosa.Keywords: anatomical variations,  agger nasi cells , concha bullosa,  deviated septum, headache, supra agger cell
