Abstract

Hboost  is an innovative hemoglobin booster that accompanies blood-boosting tablets that can help optimize and reduce the side effects of blood-boosting tablets. Objective: To determined the effect of giving Hboost in helping to increase hemoglobin levels during pregnancy in the working area of the Mantrijeron Health Center, Yogyakarta City. Methods: A Quasi-Experimental Design with plans Pretest-Posttest with a Control Group design. The population of this research study was all pregnant women in the working area of the Mantrijeron Health Center, Yogyakarta City. The sampling technique used Purposive Sampling to get a total sample of 30 respondents. Data analysis using analysis independent sample t-test. Results: The average hemoglobin level increase in the experimental group Hboost + iron tablets is 1.693 gr/dL. In comparison, the average hemoglobin level increase in the control group of iron tablets is only  0.520 gr/dL. Based on that data, it was found that the average of hemoglobin level increase in the experimental group was higher at 1.173 gr/dL than in the control group. Conclusion: Hboost (Hb Booster) effectively complements blood supplement tablets in increasing hemoglobin levels during pregnancy in the working area of the Mantrijeron Health Center, Yogyakarta City.Keywords:  anemia, blood supplement tablets, Hboost, hemoglobin levels, pregnant women

Abstract

Physiological and psychological problems can arise from sleep disorders. The majority of older people experience a decrease in sleep quality with age, so it is necessary to do morning physical activity such as walking to improve sleep quality. Objective: To determine whether walking in the morning exercise can improve older people's sleep quality. Methods: It was a quantitative research with a quasi-experimental type using a one-group pre and post-test design. The sample consisted of 28 respondents selected by purposive sampling. Measurement of sleep quality using the Pittsburgh Sleep Quality Index (PSQI) questionnaire to evaluate a person's sleep quality. The walking morning exercise intervention was carried out thrice a week for up to 4 weeks. Results: Wilcoxon test for bivariate analysis showed a significance level of 0.008 (p < 0.05). Conclusion: Walking in the morning exercise improves sleep quality in older people.Keywords:  older people, sleep quality, walking morning exercise

Abstract

Urinary tract infection (UTI) characterized by positivity of leukocyte esterase and nitrite can affect the Neutrophil Lymphocyte Ratio (NLR) value as a reflection of the degree of infection and inflammation by pathogenic microorganisms. Objective: To analyzed the effect of positivity leukocyte esterase and nitrite urine on NLR values in UTI patients. Methods: This study used an observational analytic method with a cross-sectional approach. UTI patients will be subjected to urinalysis and complete blood cell examination. The data obtained will be subjected to the Pearson correlation and linear regression tests. Results: This study used 36 samples of UTI patients with an abnormal number of positive leukocyte esterase samples in 36 samples (100%), positive urine nitrite in 20 samples (55,6%), and abnormal NLR values >3,5 are 27 samples (75%) with a mean of 10,21. Conclusion: There is a significant influence and relationship between the positivity of leukocyte esterase and urine nitrite on NLR value in UTI patients with a simultaneous value of 45,1% and p 0,000.Keywords:  UTI, leukocyte esterase, urine nitric, NLR

Abstract

Percutaneous Coronary Intervention (PCI) is one of the essential methods for Acute Coronary Syndrome (ACS) treatment, but this procedure has serious complications. Early loading dose statin has a favorable prognostic impact on patients undergoing PCI. However, this effect is still unclear.Objective: To evaluated the effect of loading high-intensity statin on Major Adverse Cardiac Events (MACE) before PCI in patients with ACS. Methods: Randomized controlled trials were conducted using databases from  CENTRAL, PubMed, Science Direct, and British Medical Journal. Then, titles, abstracts, and full texts were screened using the inclusion criteria to filter out irrelevant studies. Quality assessments were undertaken using the Jadad scale, and the results were narratively synthesized and presented in tables. Results: Seven studies were included in this systematic review after screening 1037 records. Four studies demonstrated a statistically significant reduction in MACE in the groups that received loading dose atorvastatin or rosuvastatin before PCI, compared with the comparator groups (p < 0.05). Conclusion: Loading high-intensity statin before PCI seems to be highly effective in reducing MACE in patients with ACS.Keywords:  acute coronary syndrome, major adverse cardiac events, percutaneous coronary intervention

Abstract

Rheumatic Heart Disease (RHD) has high morbidity and mortality. Cardiac valve disorders after rheumatic fever are the leading cause of heart failure in children and young adults. A comprehensive understanding of RHD can help early diagnosis and provide optimal management in the future. It has been reported that a 55-year-old man came to the heart clinic of Bhayangkara Hospital Denpasar for routine control of heart disease. The patient has been diagnosed with rheumatic heart disease since 2018. The patient complains of shortness of breath when walking long distances and doing strenuous activities. The patient was diagnosed with CHF FC II ec RHD + MS moderate-severe (Wilkins score 8) + AF NVR (CHA2DS2VASc 3) and treated with heart failure and antiplatelet medication. Mitral stenosis due to RHD produces the typical pathological features seen in Transthoracic echocardiography (TTE), including fibrous thickening and calcification of the valve leaflets, fusion of the commissures, and thickening and shortening of chordae tendineae. Chronic stress overload on the LA due to MS can cause LA dilatation and trigger AF. RHD patients with AF are recommended for anticoagulation for stroke prevention. Patients with acute rheumatic fever clinical features should be evaluated further and given long-term therapy to prevent more severe heart problems.Keywords:  atrial fibrillation, mitral stenosis, rheumatic  heart  disease

Abstract

Retinal detachment is a critical ocular condition arising from separating two retina layers, leading to an abrupt decline in visual acuity. Exudative retinal detachment occurs due to fluid accumulation from the inflammatory reaction. Objective: To ascertain the demographic characteristics of individuals diagnosed with retinal detachment at SMEC Eye Clinic in Samarinda from 2019 to 2022. Methods: This study was a cross-sectional descriptive study that utilized data from the available population. The sample comprises seven medical records of patients with exudative retinal detachment at SMEC Eye Clinic in Samarinda between 2019 and 2022. Results: The age range observed was six patients (85.7%) between the ages of 15-65 years and one patient (14.3%) > 65. The mean age of patients was 39,4 years. Among the patients, 6 (85.7%) female and 1 (14.3%) male. Unilateral eye involvement was observed in 6 patients (85.7%), while bilateral involvement was observed in 1 (14.3%). The most commonly observed accompanying eye disease was choroidal disorder. In contrast, the most commonly observed non-eye comorbidity was diabetes mellitus. Conclusion: Most exudative retinal detachment cases observed at SMEC Eye Clinic between 2019 and 2022 were concentrated within the age bracket of 15 to 65 years, with a higher proportion of female patients. The condition was predominantly unilateral, and the presence of comorbidities such as choroidal disorder and diabetes mellitus was also noted.Keywords:  affected eye side, comorbidities, exudative retinal detachment, risk factors

Abstract

The incidence of Internet addiction in the world continues to increase in various circles of society. One of the causes is stress. Mindfulness methods such as the Cognitive Transcendence Strategy (CTS) are strategies for changing behavior through cognitive functions and spiritual values. Objective: To determined the effect of CTS on stress in medical students who suffer from internet addiction. Methods: This research is a quantitative experimental study with the Quasi Eksperimental Nonequivalent Control Group Design. The sample of 16 people was collected through a purposive sampling technique based on inclusion and exclusion criteria. Data were analyzed using the Wilcoxon test. Results: Univariate shows that 60.1% of medical students experience internet addiction, of which 38.4% is in the mild category, 21.4% were moderate, and 0.4% were severe. The stress level of selected respondents based on the HRV analyzer stress index obtained was 62.5% normal, 12.5% good, 18.75% excellent, and 6.25% poor. The results of the Wilcoxon statistical test between CTS and stress were found to be p = 0.261 (p>0.05), which shows that there is no effect of CTS administration on stress statistically. The results of the Wilcoxon test between CTS and internet addiction showed a p = 0.012 (p<0.05). Conclusion: There is the influence of CTS on internet addiction.Keywords: cognitive transcendence strategy, stress, internet addiction, medical student

Abstract

Uveitic glaucoma is a complication of uveitis that can occur due to severe intraocular inflammation. Objective: To determined the clinical characteristics of uveitic glaucoma based on clinical features and management at the Cicendo Hospital National Eye Center from 2021 until 2022. Methods: This study used a retrospective descriptive method and a cross-sectional design with secondary data as subjects in the form of electronic medical records of patients at the National Eye Center Cicendo Hospital Bandung who had a diagnosis of uveitic glaucoma in the period January 2021 until December 2022. Results: 162 patients (228 eyes) met the inclusion criteria. Most of the patients were women, with a percentage of 66%. Anterior uveitis (44.3%) was the most common type of uveitis. The mean IOP at the initial visit was 30.1 mmHg. One hundred fourteen eyes (50%) were below 3/60 visual acuity. Infection (41.4%) was the most common cause of uveitis. A total of 107 eyes (46,9%) underwent surgery. There was a decrease in the average IOP in each follow-up period. Conclusions: Glaucoma uveitis is commonly found in women with an average age of 48.6 years. Anterior uveitis with an infectious cause is the most common type and cause of uveitis. The most common surgery is a combination of trabeculectomy, lens extraction, and IOL implantation.Keywords:  management of glaucoma,  secondary glaucoma, uveitic glaucoma

Abstract

COVID-19 (coronavirus disease 2019) is a pulmonary infection disease caused by SARS-CoV-2. Chest radiographic examination of COVID-19 cases performed to make a diagnosis and monitor therapy. Objective: To    evaluated the results of chest radiography examination in COVID-19 cases as a predictor of patient outcome after treatment at the Sultan Agung Islamic Hospital in Semarang. Methods: This research was an observational analytic study with a retrospective design using medical record data from January until December 2021. The results of chest radiographs were analyzed using the Brixia Score system with a maximum score of 18. Outcomes of COVID-19 patients were described as recovered patients or died. Data analysis uses the ROC curve to determine the cut-off point. Results: This study showed that out of 47 patients (43±14 years old), 39 patients (81%) recovered and nine patients died (19%). The levels of leukocytes, D-dimer, Neutrophil-lymphocyte ratio (NLR), and Brixia scores were significantly higher in dead patients (p<0.05). Sensitivity and specificity values predictor of leukocyte count (83%, 61%), D-dimer (83%, 84%), NLR (83%, 82%), and Brixia score (83%, 79%), respectively. Conclusion: Increased levels of leukocytes, D-dimer, NLR, and Brixia Score can be used as a predictor of patient mortality after hospitalization.Keywords: Brixia score, COVID-19, thoracic radiograph

Abstract

Pulmonary tuberculosis (TB) can be made worse by a history of Diabetes Mellitus (DM), which can increase the risk of failure treatment, relapse, and longer duration of treatment.  Objective: To find out the prevalence of Pulmonary TB comorbid DM at Sultan Syarif Mohamad Alkadrie General Hospital Pontianak. Methods: This research was a descriptive observational study with a cross-sectional design. The population in this study were all patients diagnosed with pulmonary tuberculosis at the Sultan Syarif Mohamad Alkadrie Hospital Pontianak from 1 January to December 31st, 2019. Data collection was carried out by taking secondary data on patients diagnosed with pulmonary TB with a history of DM in outpatient and inpatient installations at Sultan Syarif Mohamad Alkadrie General Hospital Pontianak.Results: There were 1311 pulmonary TB patients in Sultan Syarif Mohamad Alkadrie General Hospital Pontianak in 2019; pulmonary TB patients with a history of DM as many as 98 patients obtained from 73 outpatient installations and inpatients hospitalized as many as 25 patients. The results showed that the prevalence of pulmonary TB comorbid DM was 7.47%. Conclusion: The prevalence of pulmonary TB comorbid DM in RSUD Sultan Syarif Mohamad Alkadrie Pontianak is relatively low.Keywords:  ages, diabetes  mellitus, pulmonary TB
